Nutraceuticals and phytoceuticals in the treatment of bipolar disorder: A systematic review and network meta-analysis.

Nutraceuticals and phytoceuticals are increasingly investigated for bipolar disorder (BD). We conducted a systematic review and network meta-analysis of randomized controlled trials (RCTs). PubMed/MEDLINE, EMBASE, Scopus, PsycINFO, and ClinicalTrials.gov were searched from inception to July 1, 2025. Co-primary outcomes were changes in depressive or manic symptoms (standardized mean difference [SMD]), treatment response, and acceptability (risk ratio [RR]); tolerability and remission (RR) were secondary outcomes. Heterogeneity, transitivity, inconsistency, small-study effects, risk of bias, and

Cortical thickness associated with past year mood episode in major depressive and bipolar disorders.

BACKGROUND: Major depressive disorder and bipolar disorder are affective disorders that carry substantial disease burdens, yet the structural brain alterations due to recent mood episodes remain unclear. To identify acute effects on brain structure, we compared cortical thickness in adults with and without a mood episode in the past year. METHODS: Participants were 30 adults who met lifetime DSM-5 criteria for MDD (n=21) or BD (n=9), divided into a Past Year Mood Episode (PYME) group (n=18), and no-PYME group (n=12). Participants completed the Hamilton Rating Scale for Depression (HAM-D), You

Molecular and Neuroimaging Correlates of Bipolar Disorder: Linking Inflammation, Mitochondria, and Brain Circuitry.

Bipolar affective disorder (BD) is a severe mental illness characterized by recurrent episodes of mania, hypomania, and depression, accompanied by progressive neurobiological changes that go beyond the classical concepts of neurotransmitter dysregulation. Increasing evidence points to the key role of the interaction between inflammatory processes, mitochondrial dysfunction, and disturbances within neural networks in the pathogenesis, course, and treatment response of BD. Neuroinflammatory processes, including elevated levels of pro-inflammatory cytokines, chemokines, and microglial activation,

Cortisol-Responsive regulation of RASGRP1 in lymphoblastoid cells from individuals with bipolar disorder.

OBJECTIVE: Bipolar Disorder (BD) is a complex psychiatric disorder, characterised by recurrent episodes of mania and depression, and is associated with the dysregulation of stress hormone signalling, immune and inflammatory pathways. However, limited information is currently available on the effects of stress hormones on immune and inflammatory pathways in BD. Ras guanyl releasing protein 1 (RASGRP1) is a pro-inflammatory guanine nucleotide exchange factor involved in Ras-MAPK signalling. Cortisol exerts well-known anti-inflammatory effects; we therefore hypothesized that cortisol exposure wou

Predicting Affective Episodes in Bipolar Disorder Using Statistical Process Control Analysis of GPS-Based Mobility Patterns: Quantitative Study.

BACKGROUND: Bipolar disorders (BDs) represent a significant global health challenge, with frequent and severe affective episodes that impair quality of life. Accurate, early prediction of these episodes remains difficult. Recent advances in mobile sensing offer new possibilities to detect prodromal changes via smart digital phenotypes, such as geolocation data. OBJECTIVE: This study aimed to examine whether spatial exploratory behavior, assessed via passive GPS data, can predict depressive and manic episodes in individuals with BD. Specifically, we evaluated the predictive value of unique pla

A 3-Month Follow-Up Pilot Study on Accelerated Intermittent Theta Burst Stimulation for Bipolar Depression.

BACKGROUND: Approximately 25% of patients with bipolar disorder are reported to have treatment-resistant bipolar depression (TRBD). Accelerated intermittent Theta Burst Stimulation (aiTBS) is an innovative form of repetitive transcranial magnetic stimulation (rTMS), delivering bursts of stimulation at theta wave frequencies, which are believed to enhance synaptic plasticity. This pilot study aimed to explore the safety, tolerability, and preliminary efficacy of aiTBS in individuals with treatment-resistant bipolar depression (TRBD). METHODS: This open-label pilot study (registered at Overview

Acceptability and safety of two sequential doses of psilocybin in bipolar II depression: protocol for an open-label single-arm feasibility study.

INTRODUCTION: Bipolar II disorder (BD-II) is characterised by hypomanic and depressive episodes with the latter tending to dominate the clinical course of the disorder, and they are often associated with suicidality. Despite available guideline-based recommendations for first-line pharmacological treatments, patient outcomes remain suboptimal, especially for those with chronic depression and suicidal ideation. The dearth of viable treatment options for this population is evident, which is where psychedelics propose an additional alternative path. Psilocybin has recently shown promise in treati

Efficacy and safety of memantine as adjunctive treatment for manic episodes in bipolar disorder (BD): a randomized, double-blind, placebo-controlled clinical trial.

BACKGROUND: Bipolar disorder (BD) is a chronic and recurrent psychiatric illness, and acute manic episodes are associated with marked functional impairment. Mood stabilizers and antipsychotics remain the mainstay of treatment. Dysregulation of glutamatergic neurotransmission and N-methyl-D-aspartate (NMDA) receptor signaling has been implicated in bipolar disorder (BD), providing a rationale for evaluating memantine as an adjunctive treatment for manic episodes in bipolar I disorder (BD-I). METHODS: In this 6-week randomized, double-blind, placebo-controlled trial, adults aged 18-65 years wit

Beta-Caryophyllene Prevents Ouabain-Induced Neurodegeneration and Behavioral Alterations Through PKA/GSK-3β Pathway.

Bipolar disorder (BD) is a severe psychiatric condition characterized by recurrent mood episodes and progressive neurobiological alterations associated with oxidative stress, mitochondrial dysfunction, and neuronal damage. Current pharmacological treatments remain limited by incomplete efficacy and significant adverse effects, highlighting the need for novel therapeutic strategies. The present study investigated the neuroprotective effects of beta-caryophyllene (BCP), a natural sesquiterpene and selective cannabinoid receptor type 2 (CB2R) agonist, in a rat model of mania induced by intracereb

DNA methylation in bipolar disorder: mechanistic insights and translational challenges.

Bipolar disorder (BD) is a severe and recurrent psychiatric disorder characterized by alternating manic, hypomanic, and depressive episodes, frequent comorbidity, high relapse rates, and an increased risk of suicide. Although the pathophysiology of BD remains incompletely understood, increasing evidence suggests that DNA methylation may represent an important epigenetic regulatory layer involved in BD-related biological heterogeneity. DNA methylation alterations have been reported in genes related to dopamine, serotonin, glutamate, and gamma-aminobutyric acid (GABA) systems, suggesting a poten

Symptom networks in manic and mixed presentations of bipolar disorder: Mapping the crossroads.

BACKGROUND: Mixed states in bipolar disorder (BD) remain a diagnostic and nosological challenge, with current classifications differing in their ability to capture the overlap between manic and depressive symptomatology. METHODS: We used baseline data from 1384 patients in a manic or mixed episode of BD according to DSM-IV criteria across five randomized clinical trials. We modelled mood symptom networks using the Young Mania Rating Scale (YMRS) and Montgomery-Åsberg Depression Rating Scale (MADRS). We identified mood symptom clusters and compared symptom networks between DSM-IV manic and mix

Mood switching in 1629 antidepressant-treated bipolar disorder patients.

Mood-switching from depression into [hypo]mania is prevalent in bipolar disorder (BD) patients, particularly during treatment with an antidepressant, but many characteristics associated with such responses require clarification. Using standard bivariate and multivariate measures we compared characteristics of 1629 prospectively evaluated BD patients who did or did not become [hypo]manic during antidepressant treatment. Mood-switching occurred in 27.6% of BD patients (type II [BD2]: 31.4%; type I [BD1]: 24.5%) observed prospectively for 4.98 years, most often (65.5%) with new hypomania. Factors

Comparative effectiveness of mood stabiliser monotherapy versus second-generation antipsychotic monotherapy versus combination therapy for continuation and maintenance treatment after a first episode of psychotic mania in North America, Chile, and Spain: a target trial emulation using data from the BD-CAUSAL Collaboration.

BACKGROUND: The early stage of bipolar I disorder is considered a crucial period for improving long-term outcomes, yet evidence guiding treatment selection after a first manic episode is scarce. Using data from the BD-CAUSAL Collaboration, we aimed to emulate a pragmatic target trial among individuals aged 16 years or older presenting with a first manic episode with psychotic features. METHODS: For the target trial, individuals were eligible if they were aged 16 years or older, treated at a first-episode psychosis clinic with a diagnosis of bipolar I disorder between Jan 1, 2006, and Dec 31,

Cognitive Impairment and Non-Invasive Neuromodulation Interventions in Bipolar Disorder: A Narrative Review of Current Status and Future Directions.

Bipolar disorder (BD) is a severe mental illness characterized by recurrent episodes of mania and depression. The disorder is associated with high rates of relapse and disability, significantly affecting patients' social functioning and quality of life. It is estimated that 30-50% of individuals with BD do not regain their premorbid level of social functioning, primarily due to persistent cognitive impairments. These cognitive deficits are prominent not only during acute episodes but also persist throughout remission, even when emotional symptoms have stabilized. Multiple studies have demonstr

Efficacy and safety of pharmacological and non-invasive brain stimulation for adolescent bipolar depression: a systematic review and network meta-analysis.

This network meta-analysis evaluates the efficacy and safety of pharmacological and physical treatments for depressive episodes in adolescents with bipolar disorder (BD), incorporating evidence from six randomized controlled trials (RCTs), total N = 879. Due to the absence of shared comparators, pharmacological treatments and non-invasive brain stimulation (NIBS) were analyzed in separate evidence networks rather than being directly compared.Pharmacological treatments included lurasidone, olanzapine-fluoxetine combination (OFC), and quetiapine, while NIBS consisted of transcranial direct curre
