This evidence-to-recommendation (EtR) framework underpins the Agency for Care Effectiveness Clinical Guideline (ACG) for bacterial skin and soft tissue infections (SSTIs), providing rationale and justifications for its recommendations on diagnosis and antibiotic prescribing in primary and community care. Based on the Grading of Recommendations Assessment, Development and Evaluation framework, five evidence-based recommendations were developed to guide practitioners in making appropriate decisions on the diagnosis, assessment and management of SSTIs, including cellulitis, erysipelas, impetigo,

BACKGROUND: Bacterial infections of the skin and soft tissues (SSTIs) are common and should be considered a key differential diagnosis in patients presenting with redness and swelling in these areas. They range from mild superficial cases to severe, complicated forms requiring prompt antimicrobial therapy and, often, surgical intervention. We aimed to describe clinical pathways, diagnostic evolution, treatment strategies, and outcomes in a hospitalized orthopedic/trauma surgery cohort.
METHODS: We performed a retrospective single-center analysis of all inpatients treated for extremity SSTIs b

Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis characterized by rapidly progressive painful ulcers, and increasing evidence supports a role for the interleukin-36 pathway in its pathogenesis. We report the case of a 59-year-old woman with idiopathic refractory PG that evolved over 8 years. The patient had an insufficient response to prolonged systemic corticosteroids and azathioprine, with persistent steroid dependency. Spesolimab was initiated as a single 900-mg intravenous infusion in combination with low-dose prednisone (10 mg daily). This led to pain reduction, re-epithelializ

UNLABELLED: Familial Mediterranean fever (FMF) is classically inherited as an autosomal recessive disease, yet some clinically diagnosed patients are heterozygous for pathogenic MEFV variants. We evaluated long-term clinical course, cumulative damage, and colchicine outcomes in children with FMF and heterozygous pathogenic MEFV exon 10 variants. Of 3187 patients screened, 274 with heterozygous pathogenic MEFV exon 10 variants fulfilling Yalçınkaya-Özen or EUROFEVER/PRINTO criteria and ≥ 5 years of follow-up were included. Clinical phenotype, disease severity, renal findings, ADDI-defined damag

BACKGROUND: Children with clinical levels of conduct problems are at high risk of developing mental health problems such as persistent antisocial behavior or emotional problems in adolescence. Serious conduct problems in childhood also predict poor functioning across other areas of life in early adulthood such as overweight, heavy drinking, social isolation and not in employment or education. It is important to capture those children who are most at risk, early in their development. The Diagnostic and Statistical Manual of Mental Disorders (DSM-5) is commonly used in clinical settings, to iden

BACKGROUND: Due to their immature immune system, neonates are at high risk of infection. This vulnerability when combined with limited resources and health education in developing countries can lead to sepsis, resulting in high global neonatal mortality rates. Many of these deaths are preventable. The BabyGel pilot trial tested the feasibility of conducting the main randomised trial, with the provision of alcohol handgel to postpartum mothers for prevention of neonatal infective morbidity in the rural community. This secondary analysis sought to evaluate the methods of detecting infections in

BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin disease characterized by barrier dysfunction and a Type 2 T helper cell-dominant immune response. Probiotics have emerged as promising candidates for the management of AD by modulating intestinal and systemic immune responses.
OBJECTIVE: This study aimed to investigate the effects of Lacticaseibacillus paracasei MG4693 and Lactococcus lactis MG5474 on 2,4-dinitrochlorobenzene-induced AD mice.
DESIGN: AD was induced in Bagg Albino/c (BALB/c) mice by repeated topical 2,4-dinitrochlorobenzene applications, followed by the daily o

Erysipelas is an acute bacterial infection of the skin and lymphatic vessels, predominantly caused by Streptococcus pyogenes. Atypical localizations such as the trunk, as well as rare clinical variants including bullous or multifocal erysipelas, may complicate the diagnostic process and require differentiation from other blistering or widespread erythematous conditions, such as staphylococcal scalded skin syndrome (SSSS), toxic epidermal necrolysis (TEN) or cellulitis. In immunocompromised or oncologically pretreated patients, erysipelas should be considered even in unusual presentations.

BACKGROUND: Erysipelas, caused by streptococci, should be treated with penicillin, while uncomplicated cellulitis (phlegmon), often caused by Staphylococcus aureus, requires penicillinase-resistant beta-lactam antibiotics, which have a higher risk of adverse effects. Distinguishing between these infections is important. Constitutional symptoms like chills and fever may help in diagnosis. Therefore, we have compared retrospectively how frequently patients with erysipelas versus patients with cellulitis have experienced constitutional symptoms and whether they responded to penicillin.

OBJECTIVE: This study evaluated colchicine adherence in children and adolescents with familial Mediterranean fever (FMF) using the Medication Adherence Scale in FMF Patients (MASIF), and examined the influence of patient-related factors and family history.
METHOD: A cross-sectional study was conducted among 324 paediatric FMF patients (1-18 years old). Colchicine adherence was measured via MASIF, and demographic, clinical, genetic, and socioeconomic variables were analysed.
RESULTS: Of 324 patients (mean age 11.8 ± 3.8 years), 88 (27.2%) showed good adherence and 236 (72.8%) poor adherence.

Cutaneous lesions of the lower extremities represent a frequent diagnostic challenge in daily clinical practice, as a wide range of infectious, inflammatory, vascular, neoplastic, and metabolic disorders may present with similar clinical features. Hypodermitis as a manifestation of chronic venous insufficiency, for example, is often misdiagnosed as erysipelas, which may result in delayed or inadequate treatment and potentially irreversible fibrotic changes. This article addresses this issue by presenting an illustrative case and provides a structured overview of relevant differential diagnoses

BACKGROUND: In Africa, pig significantly contributes to animal protein supply to enhance food security. Pig farming in Cameroon is subject to perpetual health threats, with the spread of several diseases which strongly impact the production. This study aimed to evaluate the biosecurity compliance in pig farms in the West Region of Cameroon and assess its relationship with production performance. During on farm visit and face to face interview, data about biosecurity, veterinary drug usage and production parameters were collected in 78 randomly selected farms from April and October 2024.
