The efficacy of lumateperone across symptoms of a major depressive episode with mixed features in patients with major depressive disorder and bipolar disorder: Post hoc analyses.

INTRODUCTION: Current treatments for major depressive episodes may not resolve all symptoms, with residual symptoms predicting disease relapse, recurrence, and functional impairment. This post hoc analysis of Study 403 (NCT04285515), a randomized, double-blind, placebo-controlled trial in patients with major depressive disorder (MDD) or bipolar disorder presenting with a major depressive episode with mixed features, evaluated the breadth of efficacy of lumateperone 42 mg across depressive symptoms assessed by Montgomery-Åsberg Depression Rating Scale (MADRS) single item scores. METHODS: Adult

Efficacy and safety of pharmacological interventions for the maintenance of bipolar disorder: A systematic review and dose-related network meta-analysis across different age groups.

BACKGROUND: Bipolar disorder (BD) is a chronic mental illness that requires lifelong treatment. We investigated the comparative efficacy and safety of pharmacological interventions for bipolar maintenance, considering dose effects across different age groups. METHODS: We conducted a network meta-analysis (NMA) of randomized controlled trials (RCTs) comparing pharmacological interventions against each other or placebo for the maintenance of BD, searching for records indexed in PubMed/MEDLINE, Embase, Web of Science, and Scopus (from inception through 2026.1.12). Aripiprazole once-monthly for patients diagnosed with bipolar I disorder: Number needed to treat, number needed to harm, and likelihood to be helped or harmed.

BACKGROUND: This post hoc analysis assessed the benefit-risk profile of aripiprazole once-monthly 400 mg (AOM 400) as a long-term maintenance treatment in patients diagnosed with bipolar I disorder (BP-I), using the evidence-based metrics of number needed to treat (NNT), number needed to harm (NNH), and the likelihood to be helped or harmed (LHH). METHODS: Data were derived from a double-blind, randomized withdrawal study that evaluated the efficacy and safety/tolerability of AOM 400 versus placebo over 52 weeks (NCT01567527). The main efficacy outcome was the proportion of patients free from

Symptom networks in manic and mixed presentations of bipolar disorder: Mapping the crossroads.

BACKGROUND: Mixed states in bipolar disorder (BD) remain a diagnostic and nosological challenge, with current classifications differing in their ability to capture the overlap between manic and depressive symptomatology. METHODS: We used baseline data from 1384 patients in a manic or mixed episode of BD according to DSM-IV criteria across five randomized clinical trials. We modelled mood symptom networks using the Young Mania Rating Scale (YMRS) and Montgomery-Åsberg Depression Rating Scale (MADRS). We identified mood symptom clusters and compared symptom networks between DSM-IV manic and mix

Comparative effectiveness of mood stabiliser monotherapy versus second-generation antipsychotic monotherapy versus combination therapy for continuation and maintenance treatment after a first episode of psychotic mania in North America, Chile, and Spain: a target trial emulation using data from the BD-CAUSAL Collaboration.

BACKGROUND: The early stage of bipolar I disorder is considered a crucial period for improving long-term outcomes, yet evidence guiding treatment selection after a first manic episode is scarce. Using data from the BD-CAUSAL Collaboration, we aimed to emulate a pragmatic target trial among individuals aged 16 years or older presenting with a first manic episode with psychotic features. METHODS: For the target trial, individuals were eligible if they were aged 16 years or older, treated at a first-episode psychosis clinic with a diagnosis of bipolar I disorder between Jan 1, 2006, and Dec 31,

Non-Invasive Brain Stimulation in Older Inpatients with Depression: A Real-World Comparison of Repetitive Transcranial Magnetic Stimulation (rTMS) and Transcranial Direct Current Stimulation (tDCS) on Depressive Symptoms and Functional Recovery.

Background: Non-invasive brain stimulation (NIBS) is increasingly used as an adjunctive option in late-life depression (≥60 years), a condition frequently complicated by multimorbidity and incomplete response to standard treatments. Comparative real-world evidence between repetitive Transcranial Magnetic Stimulation (rTMS) and transcranial Direct Current Stimulation (tDCS), particularly including functional outcomes, remains limited. Methods: We conducted a retrospective, naturalistic comparative study of 104 depressed inpatients (≥60 years), either unipolar or bipolar, treated with rTMS (n =

A Retrospective Analysis of the Impact of Electroconvulsive Therapy on Anxiety Symptoms in Patients With Treatment-Resistant Depression.

BACKGROUND: Electroconvulsive therapy (ECT) is an effective treatment for treatment-resistant depression (TRD). There are limited data on the improvement of anxiety symptoms in patients receiving ECT for TRD. OBJECTIVE: The aim of the study was to examine the extent to which anxiety symptom severity improves, relative to improvements in depressive symptoms, in TRD patients receiving an acute course of ECT. METHODS: A retrospective chart review of 117 TRD patients who received an acute ECT course in a naturalistic outpatient setting was conducted. Symptomatic response was measured using the B

Symptom modulation and tolerability of intravenous ketamine in treatment-resistant bipolar depression: A retrospective study.

Ketamine's use in treating bipolar depression must account for risks, such as switching to manic episodes or worsening symptoms. This study examines ketamine's impact on depressive symptoms, focusing on 'inner tension,' 'sleep reduction,' and 'suicidal ideation' over four weeks in treatment-resistant bipolar disorder (TR-BD) patients. Fifty-nine patients with TR-BD were treated consecutively with ketamine (avg dose 0.8 mg/kg). Results showed significant reductions in MADRS scores without manic switches. Ketamine was well-tolerated despite polypharmacy. Antidepressant treatment of bipolar depre

Clinical and cost-effectiveness of lithium versus quetiapine augmentation for treatment-resistant depression: a pragmatic, open-label, parallel-group, randomised controlled superiority trial in the UK.

BACKGROUND: Lithium and quetiapine are first-line augmentation options for treatment-resistant depression; however, few studies have compared them directly, and none for longer than 8 weeks. We aimed to assess whether quetiapine augmentation therapy is more clinically effective and cost-effective than lithium for patients with treatment-resistant depression over 12 months. METHODS: We did this pragmatic, open-label, parallel-group, randomised controlled superiority trial at six National Health Service trusts in England. Eligible participants were adults (aged ≥18 years) with a current episode

Sex differences in pediatric major depressive episodes: a cross-sectional study on psychiatric symptoms in early-onset mood disorders.

BACKGROUND: Sex differences in psychiatric symptoms among children and adolescents with a major depressive episode (MDE) are less studied than among adults. Previous non-recent studies reported a greater severity in adolescent girls and small differences between sexes in specific symptoms. We aim to explore the differences between male and female patients in the diagnoses, comorbidities, and psychiatric symptoms in a large cohort of pediatric patients referred to a tertiary center for child and adolescent psychiatry. METHODS: We collected cross-sectional data on 382 consecutively referred pat

Bipolar II disorder: a state-of-the-art review.

Bipolar II disorder (BD-II) is currently identified by both the DSM-5 and ICD-11 as a distinct subtype of bipolar disorder, defined by at least one depressive episode and at least one hypomanic episode, with no history of mania. Despite its prevalence and impact, the literature on BD-II remains relatively sparse. This paper provides a comprehensive overview of the available research and current debate on the disorder, including its diagnostic criteria, clinical presentations, comorbidities, epidemiology, risk factors, and treatment strategies. Patients with BD-II often present with recurrent d

Treatment journey clustering with a novel kernel k-means machine learning algorithm: a retrospective analysis of insurance claims in bipolar I disorder.

In real-world psychiatric practice, patients may experience complex treatment journeys, including various diagnoses and lines of therapy. Insurance claims databases could potentially provide insight into outcomes of psychiatric treatment processes, but the diversity of event sequences restricts analyses with currently available methods. Here, we developed a novel kernel k-means clustering algorithm for event sequences that can accommodate highly diverse event types and sequence lengths. The approach, Divisive Optimized Clustering using Kernel K-means for Event Sequences (DOCKKES), also leverag

Efficacy and Safety of Lithium for Behavioral and Cognitive Symptoms in Alzheimer's Disease Dementia: A Systematic Review With Frequentist and Bayesian Meta-Analysis.

BACKGROUND: Alzheimer's disease (AD) dementia is the leading cause of cognitive decline in late life, yet treatment options remain limited. Lithium, widely used in bipolar disorder, has been suggested to exert neuroprotective effects through inhibition of GSK-3β and modulation of amyloid and tau pathology. We aimed to evaluate the efficacy and safety of lithium in AD dementia. METHODS: This systematic review and meta-analysis was prospectively registered in PROSPERO and conducted following PRISMA guidelines. We searched PubMed, Embase, and Cochrane Library through April 2025 for randomized co
