BACKGROUND: Both naturalistic studies and treatment research on bipolar disorder are reviewed to describe its clinical course, the need for maintenance therapy, the efficacy of current pharmacologic prophylaxis, and the empirical basis for more comprehensive approaches to treatment.
METHOD: Articles were identified through computerized literature searches and from bibliographies of published studies, review articles, and textbooks.
RESULTS: Bipolar disorder is marked by multiple relapses and recurrences, as well as significant interepisode psychopathology. Within 1 year of recovery from a mo

Although epidemiologic figures place the lifetime prevalence of bipolar disorders at about 1%, current evidence indicates that the spectrum of bipolar disorders may account for 3-6% of the general population. This summarizes two decades of research conducted by the author and other investigators that supports the existence of such a spectrum that at the one extreme merges with severe psychotic disorder and at the other with temperamental dysregulation. The enlargement of the territory of bipolar disorders is due to new data that have validated mixed, rapid cycling, and "soft" bipolar condition

Since 1993, with ICD-10 (WHO), and 1994, with DSM IV (APA), practitioners have had at their disposal two (practically compatible) classifications of mental disorders containing operational criteria for diagnosis, and helpful in guiding clinical and therapeutic approach. Moreover, the use of one of these classifications (ICD-10) is compulsory in French state psychiatric institutions. We shall try to convince that these manuals are useful, and indeed unavoidable, henceforth, not only for researchers but also for practitioners, for the following reasons: "Any form of order is preferable to chaos"

Members of the National Depressive and Manic-Depressive Association who have bipolar disorder were surveyed. 59% of respondents had their first symptoms during childhood or adolescence. Long delays between symptom onset, treatment-seeking, and receipt of a bipolar diagnosis were common. 45% of respondents currently experience frequent recurrences. Child/adolescent onset was associated with a positive family history, depressive or mixed initial symptoms, and frequent recurrence, with predominantly depressive symptoms. Frequent recurrences were associated with depressive or mixed initial symptom

We found significant differences in time to recovery and rates of chronicity in 155 patients with bipolar illness when the episodes were subtyped into those with manic symptoms alone (pure manic), depressive symptoms alone (pure depressed), or symptoms of depression and mania (mixed or cycling) up to the time of entry into a clinical research study. Most of the patients in all three groups who did not recover received levels of somatotherapy that were generally consistent with current recommendations for intensity of treatment appropriate to each condition. Based on a median follow-up of 18 mo
