Is ageism among prescribers an influential factor in the management of depression in the elderly? A vignette-based study.

BACKGROUND: The decision-making process, from diagnosing depression to treatment proposal, involves many interrelated factors. Patient age has been identified as a factor that influences care proposals. Our aim is to investigate the association between how prescribers perceive ageing and their decision-making when treating depression. METHODS: A cross-sectional survey of 57 physicians was conducted. The questionnaire collected socio-demographic data, and ageing semantic differential (ASD) scale to assess ageism. It also examined the reactions to treatment proposals in a clinical vignette abou

Depression and Anxiety in Stroke-Related and Alzheimer Disease-Related Pseudobulbar Affect: A TriNetX Retrospective Cohort Study.

INTRODUCTION: Pseudobulbar affect (PBA) is a disorder of emotional expression associated with neurological diseases, including stroke and Alzheimer disease. Depression and anxiety frequently co-occur, but whether psychiatric outcomes differ by neurological etiology remains unclear. OBJECTIVE: To compare the risk and timing of depressive episodes and anxiety disorders in stroke-associated versus Alzheimer disease-associated PBA. METHODS: We conducted a retrospective cohort study using the TriNetX Global Health Research Network (2020 to 2025). Two mutually exclusive cohorts were identified: PB

Long-Term Psychiatric Outcomes in Adult Patients With Migraine on Prophylactic Adjunctive Amitriptyline vs. Duloxetine: A Retrospective Cohort TriNetX Study.

Migraine, a primary headache disorder, is a prevalent and debilitating chronic neurologic disorder. Migraine headaches are bidirectionally related to psychiatric conditions, such as anxiety and depression. Given these associations, further investigation into how medications used for migraine prophylaxis are associated with long-term psychiatric outcomes is warranted. Our study aims to examine, among patients with migraine, whether the use of adjunctive antidepressants (duloxetine or amitriptyline) in combination with topiramate is associated with differences in the incidence of long-term psych

Public thresholds for antidepressant effectiveness and adverse drug reaction rates: a cross-sectional study.

The effectiveness of antidepressants (ADs) in the treatment of depression is continuously discussed regarding its clinical relevance. The objective of this study was to explore thresholds of members of the public for effectiveness of ADs relative to the frequency of adverse drug reactions (ADRs) deemed acceptable. We conducted a cross-sectional online survey in Germany that included individuals with and without experiences of depression and ADs. Participants were presented a case scenario describing a moderate depressive episode, operationalized with the Montgomery Åsberg Depression Rating Sca

Late-life difficult-to-treat depression and dementia subtypes: a naturalistic cohort study using electronic health records.

INTRODUCTION: Late-life difficult-to-treat depression (LL-DTD) and dementia frequently coexist in later life, but it remains unclear whether clinical and sociodemographic characteristics, as well as medication exposure patterns, differ across dementia subtypes among older adults with both conditions. METHODS: We analysed anonymised electronic health records from a south London catchment area. We included patients aged ≥60 years at first recorded depression diagnosis with a dementia diagnosis. LL-DTD was defined as inadequate response to ≥2 antidepressant trials. Dementia diagnoses were classi

Suicide Attempts in First Episodes of Major Psychiatric Disorders With Psychotic Features.

Suicidal behavior is prevalent with first psychotic episodes, but reports of associated factors involve inconsistent findings and emphasis on schizophrenia. We evaluated suicide attempt rates and associated risk factors in 395 first-episode patients with various DSM-5-TR diagnoses with psychotic features, comparing 83 suicidal patients to others. Suicide attempt risk averaged 21.0%, with the final diagnosis ranked as follows: major depression, bipolar I depression, bipolar I mixed features, schizoaffective-depressed, unspecified psychosis, schizophrenia, schizoaffective-bipolar, bipolar I mani

Bringing Attention to a Forgotten Age Group: A Discussion on Late-Onset Bipolar Disorder.

Affecting millions worldwide, bipolar disorder is a chronic condition most commonly diagnosed before the age of 50. However, diagnosing bipolar disorder beyond this age bracket presents unique challenges and is referred to as late-onset bipolar disorder. Due to the limited availability of data, this diagnosis remains complex. Although several efforts have been made to understand the impact of age of onset and neurologic etiologies on the condition, extensive research on this subject is unavailable, which will definitely facilitate enhancing services to this group. We present the case of a 75-y

Management of Acute Onset Catatonia Postpartum in a Patient With a History of Major Depressive Disorder With Psychotic Features: A Case Report.

Catatonia is a rare but potentially life-threatening neuropsychiatric syndrome characterized by a range of motor, behavioral, and affective abnormalities. Catatonia is broadly categorized into three clinical presentations: associated with a psychiatric disorder, secondary to a medical condition, or of unspecified origin. Postpartum catatonia remains poorly understood, particularly in cases where there is no active psychiatric or medical illness at the time of onset. Most documented cases occur in the context of severe peripartum mood or psychotic episodes, highlighting the unique nature of cat

Gender Differences in Depressive Disorders in Children and Adolescents: Results from a Clinical Sample.

Objectives: Adolescence is a unique period of significant neurodevelopmental changes that can increase vulnerability to mental health problems. We are currently witnessing an increase in depressive symptoms among adolescents, with suicide being among the top three causes of death for young people aged 10-25 years. In depressive disorders (DD) there are significant differences between genders in terms of symptomatology, risk factors, evolution and prognosis. Thus, the objective of our study is to investigate gender differences among children and adolescent patients admitted for evaluation of de

Developing and Implementing a Web-Based Branching Logic Survey to Support Psychiatric Crisis Evaluations of Individuals With Developmental Disabilities: Qualitative Study and Evaluation of Validity.

BACKGROUND: Individuals with developmental disabilities (DD) experience increased rates of emotional and behavioral crises that necessitate assessment and intervention. Psychiatric disorders can contribute to crises; however, screening measures developed for the general population are inadequate for those with DD. Medical conditions can exacerbate crises and merit evaluation. Screening tools using checklist formats, even when designed for DD, are too limited in depth and scope for crisis assessments. The Sources of Distress survey implements a web-based branching logic format to screen for com
