Pan-genomic insights into resistance, virulence, and stress adaptation in Clostridium perfringens from the Tibetan Plateau.

Clostridium perfringens is a ubiquitous foodborne and zoonotic pathogen responsible for necrotizing enteritis, food poisoning, and gas gangrene in humans and livestock, with mortality reaching 100% in specific subtypes. The Qinghai-Tibetan Plateau, with its high altitude, hypoxia, large temperature fluctuations, and strong UV radiation, imposes a strong selective regime that drives distinctive, plateau-specific microbial evolution. We isolated and sequenced eight C. perfringens isolates from yaks and Tibetan pigs and analyzed them with 129 public genomes. Multilocus sequence typing (MLST) reve

Ultrasonographic features of Lucilia sericata-induced wound myiasis with foot gas gangrene: A case report.

Myiasis is the tissue infestation by fly larvae, mainly Dermatobia hominis or Lucilia sericata. Sonographic features of D. hominis myiasis are well described but not those of L. sericata with concurrent gas gangrene. A 57-year-old man presented with a 1-month left foot injury, swelling, and functional decline. Examination revealed left foot gangrene with the loss of the fifth toe, maggot infestation, and systemic inflammation. Laboratory tests revealed diabetes mellitus. Point-of-care ultrasound showed subcutaneous air, fluid-debris layers, and hyperechoic, moving structures consistent with la

A diabetic mouse model of early stages of Clostridium perfringens -gas gangrene.

INTRODUCTION: Since Clostridium perfringens gas gangrene is strongly associated with human diabetes, this study compared gas gangrene severity in db/db (a model for type 2 diabetes) vs wild-type (WT) mice after a 4 h C. perfringens challenge. MATERIAL AND METHODS: Before C. perfringens challenge, glucose and branched chain amino acids (BCAA) concentrations were significantly higher in db/db vs. WT mice. All mice were then inoculated intramuscularly in, i) the left thigh with DPBS containing 106, 107 or 108 washed cells of C. perfringens type A strain ATCC3624 and ii) the right hind thigh with

Non-clostridial gas gangrene involving the thyroid gland complicated by multiple infections in a diabetic patient: a case report.

The thyroid gland is usually resistant to infection due to its nature, including rich blood supply and lymphatic drainage. Gas gangrene involving the thyroid is extremely rare and may be related to preexisting thyroid disease or diabetes. Few related cases have been reported, among which non-clostridial gas gangrene (NCGG) is particularly uncommon. Here, we briefly describe the diagnosis and treatment of a rare NCGG case involving the right lobe of the thyroid gland. A 60-year-old female patient presented with neck pain accompanied by discomfort during eating, which acutely worsened with sudde

Traditional Bone Setting Leading to Compartment Syndrome of Arm: A Case Report.

INTRODUCTION: The traditional practice of bone setting is more prevalent in developing countries, including India. These traditional practices of fracture management lead to numerous limb-threatening complications like non-union, pressure necrosis, gas gangrene, nerve palsies, necrotizing fasciitis, and most importantly, compartment syndrome. Approximately 75% of acute compartment syndrome (ACS) occurs due to an underlying long bone fracture, most commonly fractures of the tibia, followed by forearm fractures. We report a case of rare occurrence of compartment syndrome of the arm, highlighting

Hip disarticulation and external hemipelvectomy for infectious indications: a systematic review.

BACKGROUND: Hip disarticulation and external hemipelvectomy are radical lower-extremity amputations typically reserved for oncologic, traumatic, or severe infectious indications. Outcome estimates for refractory infectious cases remain sparse, and prior reports mix case reports, small series, and administrative databases with inconsistent mortality windows and limited functional follow-up. METHODS: This systematic review was reported in accordance with PRISMA 2020 and prospectively registered in PROSPERO (CRD420261277037). PubMed, Embase, Web of Science, and Cochrane CENTRAL were searched fro

Managing High Exudate Wounds: Improvised Negative Pressure Wound Therapy in Clostridial Infection.

We report a rare and life-threatening case of pediatric gas gangrene caused by Clostridium septicum following an intramuscular injection. The child presented with severe pain and blisters over the left gluteal region. Rapid progression of surgical emphysema coupled with systemic toxicity necessitated immediate surgical debridement, intensive care, and broad-spectrum antibiotic therapy. The causative pathogen was C. septicum. Wound management was particularly challenging due to high exudate volumes. A cost-effective negative pressure wound therapy system was devised using sterilized, readily av

Negative Pressure Wound Therapy Versus Conventional Dressings After Debridement for Necrotizing Fasciitis or Gas Gangrene: Impact on Hospital Stay and Wound Closure Methods.

BACKGROUND: The efficacy of negative pressure wound therapy (NPWT) in managing severe soft tissue infections (SSTIs), including necrotizing fasciitis and gas gangrene, remains controversial. This study aimed to compare clinical outcomes of NPWT versus conventional dressings following surgical debridement in patients with these conditions. METHODS: We conducted a multicenter retrospective cohort study across 11 tertiary hospitals (2015-2024). We included adult patients with necrotizing fasciitis or gas gangrene of the lower extremity and trunk who underwent surgical debridement. Patients were

Gas gangrene caused by Clostridium chauvoei in suckling piglets.

Clostridium chauvoei is an anaerobic bacterium, widespread in the environment, and one of the causes of gas gangrene in animals. We describe an outbreak of gas gangrene in 1- to 3-day-old suckling piglets in a commercial farrowing farm caused by C. chauvoei. Twenty-five litters were affected, resulting in 30 deaths and a ~1% increase in preweaning mortality. Affected piglets had lethargy; fever; anorexia; and purple discoloration of the abdomen, head, and limbs. Without intervention, the disease progressed to death or required euthanasia within 24 hours. Gross and microscopic findings included

A Case of Naja atra Bite Complicated by Gas Gangrene Leading to Amputation: Pathogen and Management Lessons.

Gas gangrene after a Naja atra bite may be caused by Enterococcus faecalis from the snake's mouth, which is resistant to ceftriaxone. Emergency management must include early, aggressive surgical debridement and antibiotics effective against this organism (e.g., piperacillin-tazobactam) to prevent amputation.

Benign Subcutaneous Emphysema of the Lower Limb Following a Trivial Injury: A Case Report.

Subcutaneous emphysema involves air entrapment beneath the subcutaneous tissue and fascial planes. It can range from a benign condition to a life-threatening emergency. Benign subcutaneous emphysema is typically confined to the subcutaneous tissues, lacks systemic features, and is non-progressive. We report a case of benign subcutaneous emphysema following a trivial trauma to the left leg of a 16-year-old male. Examination revealed a localized puncture wound and palpable crepitus. Following debridement of the puncture wound, the patient was managed conservatively with a below-knee posterior sl

Clostridium perfringens gas gangrene in a canine splenic liposarcoma.

Histotoxic clostridial infections occur when clostridial spores germinate in anaerobic tissue environments and release a variety of extracellular toxins, causing necrosis, secondary inflammation, and often death. These infections are seen in humans and animals, especially ruminants and horses, but are rare in dogs. An 11-y-old, intact male Border Collie-cross dog was inappetent and lethargic after abdominal blunt-force trauma. Clinical examination and imaging revealed a splenic mass containing gas and an abdominal effusion, identified cytologically as a neutrophilic exudate. At splenectomy, an

Clinical Features of Clostridial Necrotizing Soft-tissue Infections: Analysis of a Prospective Scandinavian Cohort Study.

BACKGROUND: Necrotizing soft-tissue infections (NSTI) caused by Clostridium spp. are rare. High rates of amputation and mortality are reported, but knowledge on clinical characteristics and outcome is largely based on retrospective cohorts. The role of Clostridium spp. as part of polymicrobial NSTI is also unclear. METHOD: We identified all clostridial cases from the INFECT cohort, a prospective study including patients with confirmed NSTI admitted to 5 Scandinavian referral hospitals over a 4-year period. Selected parameters were compared to a control group of nonclostridial polymicrobial in

Extensive subcutaneous emphysema secondary to axillary penetrating wound in a mule: a case report and literature review.

Generalized subcutaneous emphysema (GSE) is an uncommon clinical presentation in equids, typically associated with penetrating trauma and rarely compromising systemic health. This case report documents a 4-year-old working mule presenting with diffuse subcutaneous emphysema secondary to an axillary penetrating wound in the right forelimb. Clinical examination revealed pronounced crepitus without concurrent ataxia, dyspnea, dehydration, or gastrointestinal dysfunction. Diagnostic investigations, including ultrasonography, radiography, complete blood count, paracentesis, and bacterial culture, y

Taxifolin impairs colonization of Clostridium perfringens in association with iron limitation-related physiological stress.

UNLABELLED: Flavonoids are plant-derived polyphenols with diverse biological activities; however, the mechanisms by which individual compounds inhibit intestinal pathogens remain unclear. In this study, we found that taxifolin (dihydroquercetin) showed pronounced inhibitory activity against Clostridium perfringens, a major gram-positive enteropathogen, whereas most other bacterial species tested were less affected. Notably, at concentrations below those causing complete growth inhibition, taxifolin significantly impaired mucin adhesion and biofilm formation, indicating suppression of colonizat

Fatal equine gas gangrene associated with Clostridium septicum in the Brazilian Amazon: clinicopathological and molecular characterization of a 20-case series.

Clostridial gas gangrene is an acute necrotizing infection of skeletal muscle characterized by rapid tissue destruction and severe systemic toxemia in horses; however, systematically documented case series from tropical settings remain scarce. Here, we describe a clinicopathological and molecularly confirmed series of 20 fatal equine gas gangrene cases diagnosed in the eastern Brazilian Amazon (Pará State) between 2018 and 2026. Horses presented with abrupt onset of severe lameness, rapidly progressive swelling of a limb or muscle group, and palpable subcutaneous crepitus, frequently following

Dynamic motion of bacterial surface pili based on structural analyses of covalently linked complexes formed by tip and shaft pili proteins from Clostridium perfringens.

The pathogenesis and infectivity of Gram-positive bacteria are mediated by many surface proteins covalently attached to the bacterial cell wall. Pili are types of surface appendages that play important roles in the initial adhesion of bacterial cells to host tissues and bacterial colonization. The Gram-positive bacterium Clostridium perfringens (C. perfringens), one of the pathogenic clostridial species causing gas gangrene and food poisoning, has sortase-mediated pili composed of shaft/major pilin A (CppA) and tip/minor pilin B (CppB). The pilus shaft is formed by covalent polymerization of C

Burden of non-communicable diseases by Socio-demographic Index: a national and subnational analysis in Iran.

The gold standard for central arch defects of the mandible is reconstruction with vascularized bone free flap. Free flap repair is always not an option due to lack of resources and in patients who are not healthy enough for lengthy surgery. In these situations, a standardized method of repair is by reconstruction plate and wrapping it with a regional flap like pectoralis major myocutaneous flap (PMMC). However, this method of repair has increased risk of plate exposure, plate fracture, partial or complete flap necrosis, and delayed decannulation, swallowing, adjuvant treatment. Hence, we descr
