We report a rare and life-threatening case of pediatric gas gangrene caused by Clostridium septicum following an intramuscular injection. The child presented with severe pain and blisters over the left gluteal region. Rapid progression of surgical emphysema coupled with systemic toxicity necessitated immediate surgical debridement, intensive care, and broad-spectrum antibiotic therapy. The causative pathogen was C. septicum. Wound management was particularly challenging due to high exudate volumes. A cost-effective negative pressure wound therapy system was devised using sterilized, readily av

BACKGROUND: The efficacy of negative pressure wound therapy (NPWT) in managing severe soft tissue infections (SSTIs), including necrotizing fasciitis and gas gangrene, remains controversial. This study aimed to compare clinical outcomes of NPWT versus conventional dressings following surgical debridement in patients with these conditions.
METHODS: We conducted a multicenter retrospective cohort study across 11 tertiary hospitals (2015-2024). We included adult patients with necrotizing fasciitis or gas gangrene of the lower extremity and trunk who underwent surgical debridement. Patients were

Clostridium chauvoei is an anaerobic bacterium, widespread in the environment, and one of the causes of gas gangrene in animals. We describe an outbreak of gas gangrene in 1- to 3-day-old suckling piglets in a commercial farrowing farm caused by C. chauvoei. Twenty-five litters were affected, resulting in 30 deaths and a ~1% increase in preweaning mortality. Affected piglets had lethargy; fever; anorexia; and purple discoloration of the abdomen, head, and limbs. Without intervention, the disease progressed to death or required euthanasia within 24 hours. Gross and microscopic findings included

Gas gangrene after a Naja atra bite may be caused by Enterococcus faecalis from the snake's mouth, which is resistant to ceftriaxone. Emergency management must include early, aggressive surgical debridement and antibiotics effective against this organism (e.g., piperacillin-tazobactam) to prevent amputation.

Subcutaneous emphysema involves air entrapment beneath the subcutaneous tissue and fascial planes. It can range from a benign condition to a life-threatening emergency. Benign subcutaneous emphysema is typically confined to the subcutaneous tissues, lacks systemic features, and is non-progressive. We report a case of benign subcutaneous emphysema following a trivial trauma to the left leg of a 16-year-old male. Examination revealed a localized puncture wound and palpable crepitus. Following debridement of the puncture wound, the patient was managed conservatively with a below-knee posterior sl

Histotoxic clostridial infections occur when clostridial spores germinate in anaerobic tissue environments and release a variety of extracellular toxins, causing necrosis, secondary inflammation, and often death. These infections are seen in humans and animals, especially ruminants and horses, but are rare in dogs. An 11-y-old, intact male Border Collie-cross dog was inappetent and lethargic after abdominal blunt-force trauma. Clinical examination and imaging revealed a splenic mass containing gas and an abdominal effusion, identified cytologically as a neutrophilic exudate. At splenectomy, an

BACKGROUND: Necrotizing soft-tissue infections (NSTI) caused by Clostridium spp. are rare. High rates of amputation and mortality are reported, but knowledge on clinical characteristics and outcome is largely based on retrospective cohorts. The role of Clostridium spp. as part of polymicrobial NSTI is also unclear.
METHOD: We identified all clostridial cases from the INFECT cohort, a prospective study including patients with confirmed NSTI admitted to 5 Scandinavian referral hospitals over a 4-year period. Selected parameters were compared to a control group of nonclostridial polymicrobial in

Generalized subcutaneous emphysema (GSE) is an uncommon clinical presentation in equids, typically associated with penetrating trauma and rarely compromising systemic health. This case report documents a 4-year-old working mule presenting with diffuse subcutaneous emphysema secondary to an axillary penetrating wound in the right forelimb. Clinical examination revealed pronounced crepitus without concurrent ataxia, dyspnea, dehydration, or gastrointestinal dysfunction. Diagnostic investigations, including ultrasonography, radiography, complete blood count, paracentesis, and bacterial culture, y

UNLABELLED: Flavonoids are plant-derived polyphenols with diverse biological activities; however, the mechanisms by which individual compounds inhibit intestinal pathogens remain unclear. In this study, we found that taxifolin (dihydroquercetin) showed pronounced inhibitory activity against Clostridium perfringens, a major gram-positive enteropathogen, whereas most other bacterial species tested were less affected. Notably, at concentrations below those causing complete growth inhibition, taxifolin significantly impaired mucin adhesion and biofilm formation, indicating suppression of colonizat

Clostridial gas gangrene is an acute necrotizing infection of skeletal muscle characterized by rapid tissue destruction and severe systemic toxemia in horses; however, systematically documented case series from tropical settings remain scarce. Here, we describe a clinicopathological and molecularly confirmed series of 20 fatal equine gas gangrene cases diagnosed in the eastern Brazilian Amazon (Pará State) between 2018 and 2026. Horses presented with abrupt onset of severe lameness, rapidly progressive swelling of a limb or muscle group, and palpable subcutaneous crepitus, frequently following

The pathogenesis and infectivity of Gram-positive bacteria are mediated by many surface proteins covalently attached to the bacterial cell wall. Pili are types of surface appendages that play important roles in the initial adhesion of bacterial cells to host tissues and bacterial colonization. The Gram-positive bacterium Clostridium perfringens (C. perfringens), one of the pathogenic clostridial species causing gas gangrene and food poisoning, has sortase-mediated pili composed of shaft/major pilin A (CppA) and tip/minor pilin B (CppB). The pilus shaft is formed by covalent polymerization of C

Background: Mechanically ventilated intensive care patients receiving hyperbaric oxygen therapy (HBOT) are uncommon and resource-intensive. We assessed temporal changes over 11 years in diagnostic composition, HBOT treatment activity, organ-support requirements, and in-hospital mortality at an academic centre. Methods: This single-centre retrospective cohort included mechanically ventilated ICU patients receiving HBOT during the period 2013-2023. Diagnoses were grouped as carbon monoxide (CO) poisoning, clostridial myonecrosis/gas gangrene, non-clostridial necrotising soft-tissue infections (N

Clostridium perfringens, a Gram-positive anaerobic clostridium, is a major zoonotic pathogen widely distributed in the soil, water sources, and food. It poses a severe threat to public health security and the livestock industry, since it is capable of causing various highly lethal diseases such as gas gangrene and necrotic enteritis, resulting in significant economic losses. This review systematically summarizes the biological characteristics, pathogenic mechanisms, epidemiological features, detection methods, and prevention and control strategies of this bacterium, aiming to provide a theoret

BACKGROUND: Understanding synapse-specific effects of neuroinflammation can provide mechanistic and therapeutically relevant insights across the spectrum of neurological diseases.
METHODS: We applied neuron-specific proteomic biotinylation in vivo, differential centrifugation of brain for crude synaptosome enrichment (P2 fraction) and mass spectrometry (MS) analysis of biotinylated proteins to derive native-state proteomes of Camk2a-positive neurons and their corresponding P2 synaptic compartments. Next, in an in vivo model of systemic lipopolysaccharide (LPS) dosing, we examined the effects

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

INTRODUCTION: Medial Hoffa fractures are rare intra-articular injuries of the femoral condyle, and their association with posterior cruciate ligament (PCL) avulsion fractures presents a highly uncommon and complex clinical scenario. This case report describes the successful surgical management of such a combination using a posterior open approach.
CASE REPORT: A 48-year-old male sustained a medial Hoffa fracture with a PCL avulsion following a road traffic accident. Surgical intervention involved open reduction and internal fixation (ORIF) through a posterior approach, allowing direct access

Hepatic gas gangrene (HGG) is a rare but life-threatening condition typically caused by anaerobic bacteria such as Clostridium perfringens, though Gram-negative bacteria like Escherichia coli and Klebsiella species have also been implicated. Traditionally diagnosed via computed tomography (CT), point-of-care ultrasound (POCUS) has emerged as a valuable tool in critical care settings for its non-invasive, bedside utility. We report the case of a 51-year-old female with choledochal syndrome secondary to cholangiocarcinoma who developed HGG following left extended hepatectomy and biliary reconstr

Clostridium spp. are anaerobic, Gram-positive, spore-forming bacteria comprising more than 150 species, some of which are important pathogens of humans and animals. Members of this genus have been isolated from a number of environments, but are rarely found in the atmosphere. In the present study, we exami-ned culturable airborne Clostridium spp. and clarified their pathogenicity. We obtained 19 culturable Clostridium isolates from size-fractionated samples collected at a suburban site in Toyama, central Japan. Culturable Clostridium spp. were detected in particles larger than 1.1 μm, and th
