Management on late diagnosed Fournier's gangrene in elderly patient and it's complication: A case reports.

INTRODUCTION AND IMPORTANCE: One of the risk factors linked to mortality in Fournier Gangrene (FG) is the elderly. When this risk is present and diagnosed too late, patient care may become difficult. This case report discusses the treatment of an older patient with late-diagnosed Fournier's gangrene and its consequences in this background. CASE PRESENTATION: An emergency department (ER) referral was made for a 65-year-old male. For one week, the patient's main complaint was a sporadic high fever that got worse along with sporadic scrotal soreness. An X-ray of the kidney, ureter, and bladder (

A Case of Gas Gangrene of the Right Foot: Clinical Presentation and Management.

Gas gangrene is a rare, life-threatening infection caused mainly by Clostridium perfringens and other Clostridium species, such as C. septicum, C. novyi, C. bifermentans, and C. histolyticum. Other microbial pathogens also reported to cause gas gangrene are Staphylococcus aureus and Klebsiella pneumoniae. It is fulminant and associated with high rates of morbidity and mortality, especially in patients with underlying comorbidities such as diabetes mellitus. Major underlying etiology is associated with a history of trauma or surgical wounds exposed to soil and fecal matter containing the causat

Silent Entry, Rapid Demise: Fulminant Clostridium septicum Necrotizing Fasciitis in an Immunosuppressed Host.

Clostridium septicum is a rare but highly virulent anaerobic pathogen associated with spontaneous gas gangrene and necrotizing soft tissue infections, particularly in immunocompromised individuals and patients with underlying malignancies. Its rapid progression and subtle early symptoms make timely diagnosis and treatment extremely challenging. We report the case of a 39-year-old woman with metastatic neuroendocrine breast carcinoma and diffuse bone marrow infiltration who developed fulminant necrotizing fasciitis due to C. septicum. Two weeks after initiating systemic therapy with a selective

Inhibition of Air-Exposure Stress-Induced Autolysis in Clostridium perfringens by Zn2.

Clostridium perfringens is a pathogenic anaerobe that causes gas gangrene and food poisoning. Although autolysin-mediated reorganization of the bacterial cell wall is crucial for cell division, excessive autolysin activity induced by stressors can lead to cell lysis. In C. perfringens, air exposure is a significant stressor that causes cell lysis, and Acp (N-acetylglucosaminidase) is known to be a major autolysin. To further facilitate C. perfringens research, a technology to prevent air-induced cell lysis must be developed. This study investigated the role of Acp in air-induced autolysis and

Angiography-Guided Limb Salvage Planning in a Surgical Candidate With Gas Gangrene of the Foot.

Gas gangrene is a fulminant soft-tissue infection that requires rapid intervention to prevent extensive tissue destruction and mortality. Diabetic patients with peripheral neuropathy often have delayed recognition of injuries and impaired wound healing, placing them at substantially higher risk of severe complications. This report presents a 60-year-old male patient with diabetes and peripheral neuropathy who developed gas gangrene of the right foot. Despite two initial debridements, concern remained for progressive infection necessitating limb amputation. An abdominal aortogram with run-off d

Liver-Microbiome Crosstalk Mediates the Protective Effects of Artemisinin in Clostridium perfringens Models.

Clostridium perfringens is a multi-host opportunistic pathogen whose plasmid-encoded toxins cause gas gangrene, necrotic enteritis and enterotoxemia, resulting in substantial economic losses in animal husbandry. In light of antibiotic bans and the need for alternatives, we employed reverse network pharmacology to screen and in vitro validate artemisinin (ART), then assessed its efficacy in murine and rabbit infection models challenged with C. perfringens type F. ART treatment did not significantly affect body weight change or intestinal histopathological damage. However, it significantly modul

Construction and evaluation of a chimeric vaccine against Clostridium perfringens type a of sika deer.

Clostridium perfringens α-toxins are lethal, neurotoxic factors that play a critical role in the pathogenesis of gas gangrene and hemorrhagic enteritis. In this study, the full-length α-toxin protein and two multi-epitope tandem proteins, CPA1 and CPA2, were recombinantly expressed using a prokaryotic expression system, and their immunogenic effects were evaluated in a mouse model. Compared with the full-length α-toxin, CPA1 and CPA2 induced comparable lymphocyte proliferation and differentiation, as well as sustained high antibody levels. Additionally, significantly elevated serum levels of c

Non-clostridial gas gangrene due to Anaerococcus tetradius following a cesarean section: A case report.

A 25-year-old healthy woman was admitted to our hospital with fever and abdominal pain. Abdominal contrast-enhanced computed tomography revealed a fluid collection with foamy gas formation extending from both rectus abdominis muscles to the subcutaneous tissue. Antimicrobial therapy with meropenem (3 g/day), clindamycin (1800 mg/day), and daptomycin (300 mg/day) was initiated as empiric therapy. Subcutaneous abscess culture revealed gram-positive cocci, which was identified as Anaerococcus tetradius. Histopathological examination of the necrotic tissue showed skeletal muscle necrosis. On the b

Clostridium perfringens alpha toxin drives pathological NETosis via immature neutrophil mobilization and functional reprogramming.

Clostridium perfringens alpha toxin (CPA), a zinc-dependent phospholipase C, is a key virulence factor in gas gangrene. While its membrane-disrupting cytotoxicity is well characterized, its capacity to modulate neutrophil function and promote pathological inflammation is poorly defined. Here, we show that CPA induces neutrophil extracellular trap (NETs) formation by mobilizing and functionally reprogramming immature neutrophils. In a murine model, CPA challenge caused dose-dependent mortality and multi-organ injury, driven by a dramatic expansion of a pro-NETotic immature neutrophil subset ide

Mechanisms of miRNAs (MicroRNAs) and Their Expression in Gastric Cancer.

Leptospirosis, a significant yet often overlooked zoonotic disease, is distributed worldwide, particularly in tropical and subtropical regions. The manifestation of its symptoms can be deceptive, often leading to misdiagnosis due to its resemblance to other infectious diseases. The development of rapid diagnostic tests and the identification of potential vaccine candidates for leptospirosis pose significant challenges. Surface-exposed antigens, found on the outer layer of Leptospira, likely contribute to the initial interactions between the host and the pathogen.Lipl32 is highly conserved and

The evaluations of the inhibition of orlistat on Clostridium perfringens sialidase (NanI) activity by in vitro and in silico approaches.

Clostridium perfringens (C. perfringens) is a bacterium that causes serious problems in humans and animals such as food poisoning, gas gangrene and infections. C. perfringens has three sialidases (NanH, NanI, NanJ) and inhibition of NanI constitutes an approach in the treatment of C. perfringens since NanI provides the carbohydrate source necessary for the growth of bacteria. In our study, the inhibition effect of some drugs belonging to different drug groups on NanI activity was investigated. Among these drugs, orlistat (0.21±0.05 μM) was determined to have a lower IC50 value than the positiv

The new insights of hyperbaric oxygen therapy: focus on inflammatory bowel disease.

Inflammatory bowel diseases (IBD), with an increasing incidence, pose a significant health burden. Although there have been significant advances in the treatment of IBD, more progress is still needed. Hyperbaric oxygen therapy (HBOT) has been shown to treat a host of conditions such as carbon monoxide poisoning, decompression sickness, and gas gangrene. In the last few years, there has been an increase in research into the use of HBOT as an adjunct to conventional treatment for IBD. Related research has shown that HBOT may exert its therapeutic effects by decreasing oxidative stress, inhibitin

Does Negative Pressure Wound Therapy Impact the Outcome for Patients With Necrotizing Soft Tissue Infection Infected With Anaerobic Bacteria?

Background: A notable improvement in the treatment of necrotizing soft tissue infections (NSTIs) is the development of negative pressure wound therapy (NPWT). Clinicians are still debating whether NPWT is as successful as conventional wet-to-dry dressings at removing bacteria. Recent research has revealed potential oxygen deprivation effects of NPWT in underlying wound tissues, although clinical trials regarding the effects of reduced oxygen on anaerobic bacterial soft tissue infections remain noticeably lacking. Hypothesis: We hypothesized that NPWT-treated patients with NSTIs who were solely

Negative Pressure Wound Therapy With and Without Instillation in Necrotizing Soft Tissue Infections.

Background: Necrotizing soft tissue infections (NSTIs) are rare but deadly infections that require early and often extensive surgical debridement. After debridement, patients frequently have substantial morbidity because of large, open wounds. Hypothesis: Negative pressure wound therapy with instillation (NPWTi) results in higher wound closure rates compared with traditional negative pressure wound therapy (NPWT) or wet to dry dressings (moist wound care dressing). Patients and Methods: A prospectively maintained Acute and Critical Care Surgery database spanning 2008-2018 was queried for patie
