Fatal Clostridium septicum gas gangrene complicating ECMO: case report and review of literature.

Clostridium septicum gas gangrene is a severe and deadly infection caused by an anaerobic, spore-forming, Gram-positive bacillus. As previously described, two entities are observed: traumatic and spontaneous (or non-traumatic) forms. In this report, we aim to describe the case of a fulminant and ultimately fatal C. septicum myonecrosis occurring in a patient who was first admitted for refractory cardiac arrest and placed on veino-arterial extracorporeal membrane oxygenation (ECMO). Building upon prior studies that have documented cases of spontaneous gas gangrene caused by C. septicum, we prov

The presence of differentiated C2C12 muscle cells enhances toxin production and growth by Clostridium perfringens type A strain ATCC3624.

Clostridium perfringens type A causes gas gangrene, which involves muscle infection. Both alpha toxin (PLC), encoded by the plc gene, and perfringolysin O (PFO), encoded by the pfoA gene, are important when type A strains cause gas gangrene in a mouse model. This study used the differentiated C2C12 muscle cell line to test the hypothesis that one or both of those toxins contributes to gas gangrene pathogenesis by releasing growth nutrients from muscle cells. RT-qPCR analyses showed that the presence of differentiated C2C12 cells induces C. perfringens type A strain ATCC3624 to upregulate plc a

Advanced Calciphylaxis in a Patient With End-Stage Renal Disease: A Case Report Highlighting Diagnostic and Therapeutic Challenges in Late-Stage Presentation.

Calciphylaxis, also known as calcific uremic arteriolopathy, is a rapidly progressive, rare, and severe condition characterized by vascular calcification and skin necrosis. The pathophysiology involves cutaneous arteriolar calcification followed by subsequent tissue ischemia and infarction, which eventually causes extremely painful skin lesions. The condition is associated with substantial morbidity due to severe pain, non-healing wounds, increased susceptibility to infections, and frequent hospitalizations. Calciphylaxis is a highly fatal condition with one-year mortality rates greater than 5

Clostridial gas gangrene involving the brain, gallbladder, heart, and soft tissue: A case report and literature review.

Clostridial gas gangrene (CGG) is among the most rapidly spreading infections in humans, with mortality rates approaching 100 % if not treated promptly. Most cases follow traumatic inoculation, although spontaneous infections occur in a minority of patients with immunodeficiency. Spontaneous CGG is primarily caused by Clostridium septicum, whereas traumatic infection is associated with Clostridium perfringens. Patients with CGG present abruptly with rapidly progressive symptoms, underscoring the importance of early recognition, prompt surgical intervention, and appropriate antimicrobial therap

Review of advances in molecular structure and biological function of alpha toxin of Clostridium perfringens.

Alpha toxin has become the subject of research in recent years. The objective of this article was to review and summarize recent research on the molecular structure and biological function of the alpha toxin of Clostridium perfringens. This includes the work of our research team, as well as that of other researchers. Clostridium perfringens is an anaerobic, spore-forming, Gram-positive bacillus. It can cause various intestinal diseases, such as gas gangrene, food poisoning, non-foodborne diarrhea, and enteritis. Clostridium perfringens can be classified into 5 toxinotypes A, B, C, D, and E, ba

Gas gangrene with Clostridium septicum in a neutropenic patient.

Gas gangrene is a rare presentation of a necrotizing fasciitis, caused by Clostridium perfringens, C. septicum and other clostridial species. With its rapid progression it is a potentially life-threatening infection, that poses as a challenge in the clinical management requiring an interdisciplinary approach. Here we present a 62-year-old woman, who developed neutropenic fever while undergoing chemotherapy for triple negative breast cancer. She presented with a high fever, reporting little pain in her left thigh accompanied by redness and induration locally. Subsequently the patient developed p

[Fatal course of a fulminant gas gangrene of the right hemithorax].

The full clinical picture of a gas gangrene infection is an absolute rarity. The mechanism of development can be either traumatic or spontaneous (e.g., hematogenous seeding in occult colon carcinoma). In particular, the rare pathogen Clostridium septicum appears to be associated with spontaneously occurring gas gangrene. Diabetes mellitus is a significant risk factor. The mortality rate of the disease is around 50%, even with maximum therapeutic efforts, and the course of the disease is fulminant in the majority of cases. Initial symptoms are unspecific and make early diagnosis difficult. Trea

An autopsy case of gas gangrene, massive intravascular hemolysis, and cytokine storm due to Clostridium perfringens type A infection.

Clostridium perfringens bacteremia is a rare but rapidly fatal condition, especially in patients exhibiting massive intravascular hemolysis (MIH), gas gangrene, and septic shock. Herein, we present an autopsy case of C. perfringens septicemia exhibiting MIH, gas gangrene, and cytokine storm. The patient was an 84-year-old female with a history of biliary reconstruction surgery for congenital biliary dilatation. She developed MIH, elevated inflammatory mediator levels, thrombocytopenia, and coagulopathy. She went into shock within 1 h of the presentation and died within a few hours. Rapid progr

Large Clostridial Toxins: A Brief Review and Insights into Antigen Design for Veterinary Vaccine Development.

The group of large clostridial toxins (LCTs) includes toxins A (TcdA) and B (TcdB) from Clostridioides difficile, hemorrhagic and lethal toxins from Paeniclostridium sordellii, alpha toxin from Clostridium novyi (TcnA), and cytotoxin from Clostridium perfringens. These toxins are associated with severe pathologies in livestock, including gas gangrene (P. sordellii and C. novyi), infectious necrotic hepatitis (C. novyi), avian necrotic enteritis (C. perfringens), and enterocolitis (C. difficile). Immunoprophylaxis is crucial for controlling these diseases, but traditional vaccines face producti

Hypervirulent carbapenem-susceptible klebsiella pneumoniae ST412/K57 with strong biofilm formation: association with gas gangrene and sepsis.

Hypervirulent Klebsiella pneumoniae (hvKp) poses a serious public health threat. Gas gangrene caused by hvKp is rarely reported and potentially results in a poor prognosis. This study describes the case of a hospitalised patient with gas gangrene and sepsis caused by hvKP. Carbapenem-susceptible hypervirulent Klebsiella pneumoniae (CS-hvKp) strains KPLSN and KPLSX were isolated from the knee joint pus and blood specimens of the patient for further investigations. Whole genome sequencing revealed that KPLSN and KPLSX were highly homologous (single nucleotide polymorphisms [SNPs]<10) and belonge

A reverse vaccinomics approach for the designing of novel immunogenic multi-epitope vaccine construct against gas gangrene and related colorectal cancer for Clostridium septicum DSM 7534.

PURPOSE: Costridium septicum, the causative agent of clostridial myonecrosis or gas gangrene, has been linked with colorectal malignancies, particularly tumors of the ascending colon. Due to the pathogen's resistance and the limited understanding of its genomic structure, effective therapeutic strategies are needed. This study focuses on developing a vaccine to address this issue. METHOD: In this study, we employed both subtractive proteomics and reverse vaccinology to identify potential therapeutics against C. septicum. Through subtractive proteomics, we pinpointed antigenic membrane protein

Case Report: Medial Clavicle Physeal Injury Mimicking a Posterior Clavicular Dislocation in a Young Adult.

INTRODUCTION: Gas gangrene is a rare, often lethal infection of soft tissue that is commonly associated with the Clostridial species due to penetrating injuries. Case reports of spontaneous atraumatic gas gangrene due to the Clostridium septicum species are exceedingly rare. Atraumatic C. septicum is notably related to bowel pathologies, immunodeficiencies, and individuals with vascular insufficiency. The rapid progression of Clostridium myonecrosis contributes to its high mortality rates. CASE REPORT: The authors report a case of a 71-year-old right-hand-dominant female who developed fulmina

Surgical Management of Concurrent Septic Arthritis and Forearm Necrotizing Fasciitis in an Immunocompromised Patient With a 20-Year History of Chronic Lymphocytic Leukemia: A Case Report.

Shoulder septic arthritis is a severe infection of the shoulder joint, commonly caused by bacteria such as Staphylococcus aureus. It leads to inflammation, severe pain, swelling, and reduced mobility in the affected shoulder. The condition is typically diagnosed through clinical evaluation, blood tests, imaging studies, and joint aspiration. Prompt treatment, usually involving antibiotics and sometimes surgical intervention, is crucial to prevent joint destruction and other serious complications. Necrotizing fasciitis of the upper extremity is a rapidly progressing, life-threatening bacterial

Gas gangrene due to Clostridium haemolyticum in a cat with aortic thromboembolism treated using clopidogrel and rivaroxaban.

A 12-year-old Bengal cat with unclassified cardiomyopathy presented for signs consistent with aortic thromboembolism (ATE). It was treated with clopidogrel and rivaroxaban as antithrombotic therapy, combined with narcotic analgesia and frusemide. After initially making a clinical improvement, the cat's condition suddenly deteriorated, with a presumptive diagnosis of clostridial myonecrosis. We posit that ischaemia due to ATE allowed dormant clostridial spores in the left hindlimb to germinate and multiply vegetatively, resulting in the elaboration of potent Gram-positive exotoxins, resulting i

Fulminant non-traumatic Clostridium perfringens necrotizing fasciitis and myonecrosis in a child with acute myeloid leukemia.

Necrotizing fasciitis is a potentially life-threatening infection that can lead to rapid muscular and fascial necrosis, often resulting in sepsis. In addition to the rapid disease progression, diagnosing this disease in children can be challenging as they cannot accurately communicate their symptoms. Spontaneous necrotizing fasciitis secondary to Clostridial infection has rarely been described in the literature but occurs in neutropenic patients with significant morbidity and mortality from myonecrosis and gas gangrene. This case report describes fulminant nontraumatic necrotizing fasciitis in

Successful Salvage Therapy of Late-onset Arterial Disorders due to Recurrent Vasospasms Following Free Flap Transfer under the IL-6 and TNF-α Signaling-downregulated Environment: A Case Report.

A 77-year-old woman who had been taking iguratimod and sarilumab for rheumatoid arthritis for 3 months had gas gangrene. After hospitalization, she underwent two debridement surgeries, one drainage procedure, and a free latissimus dorsi musculocutaneous flap transfer for the resulting tissue defect on the oral cavity through the temple. Following the free flap surgery, she experienced flap ischemia, possibly caused by the vasospasms of the intraflap and recipient arteries on postoperative days 5 and 6. These immunomodulating drugs might cause vasospasms by downregulating the interleukin-6 and/

A rare case of necrotizing fasciitis of the leg in an intravenous drug user caused by Prevotella denticola.

BACKGROUND: Intravenous drug users (IDUs) have a high risk of developing skin and soft tissue infections such as erysipelas, abscesses, and less frequently necrotizing fasciitis (NF) or gas gangrene. Rarely, the cause of the infection is microorganisms residing in the oral cavity and can lead to life-threatening infections. METHODS: We describe the case of a 43-year-old man intravenous drug user (IDU) who was admitted for intense leg pain following an injection of cocaine at that site. RESULTS: A clinical and radiological diagnosis of NF was made, so the patient was started on empirical anti

Possible manufacture of test allergens in public pharmacies for the diagnosis of type I allergies: Legal aspects.

The availability of high-quality skin test allergens is a prerequisite for the reliable diagnosis of occupational type I allergies. Due to the withdrawal of existing marketing authorizations (MAs) by pharmaceutical companies and the lack of new MAs for commercial test allergens, there is an increasing diagnostic gap in Germany and other EU member states, which makes it necessary to investigate alternative ways of providing in vivo diagnostics. The German Medicinal Products Act (Arzneimittelgesetz = AMG) allows for the possibility of preparing medicinal products in pharmacies without the need f
