Molecular genetic influences on sustained attention and executive processes and their links with psychopathology in the AFFECT study.

Both sustained attention (SA) and executive functioning (EF) are critical for adaptive behavior and compromised across a range of psychiatric disorders, yet their etiological relationships remain underexplored. Here, we report the first multivariate characterization of molecular genetic influences on SA (vigilance, lapse recovery cost, lapse propensity) and EF (processing speed, response selection, working memory). In collaboration with 23andMe, we report previously unpublished genome-wide association studies of these phenotypes in a single cohort of more than 20,000 individuals enriched for d

Obsessional jealousy: Provisional research criteria and report of 34 cases.

BACKGROUND: Obsessional jealousy is described in DSM-5 under Other Specified Obsessive-Compulsive and Related Disorders, yet operationalized criteria and clinical characterization are lacking. This study aimed to develop and field-test provisional research criteria for obsessional jealousy and describe the clinical features of individuals meeting these criteria. METHODS: Provisional research criteria were developed based on DSM-5 descriptions and applied in a self-referred sample of individuals reporting problematic jealousy (n = 58). Participants underwent structured clinical assessment, inc

The effects of precision intervention based on user profile on the improvement of postpartum parenting behavior for rural‑to‑urban floating women: a study protocol for a randomized controlled trial.

BACKGROUND: Rural-to-urban floating women often face great challenges in postpartum parenting, which negatively affects the well-being of these women and their infants. User profiles allow tailored support, making precision interventions possible. Consequently, a randomized controlled trial (RCT) protocol was designed to evaluate the effects of user profile-based precision intervention on improving postpartum parenting behaviors for rural-to-urban floating women. METHODS: This single-blind, three-arm multicenter RCT will enroll 360 mothers. The control group (n = 120) will receive standard po

Comparing family history indicators and polygenic scores in depressive disorder.

BACKGROUND: Genetic liability of depressive disorder can be captured by psychopathology in relatives (family history). Various methods summarize family history in a single score, differing in included information as well as the underlying model. We systematically compared the performance of family history indicators, including promising new indicators based on the liability threshold model, in predicting depressive disorder. METHODS: We calculated selected family history indicators for depression (dichotomous, proportion, novel genetically informed method - Pearson-Aitkens Family Genetic Risk

Reducing Caregiver Burden in First-Episode Psychosis: Effects of a Psychoeducational Pathway Led by an Advanced Practice Nurse-A Multicentre Stepped-Wedge Randomized Trial.

Family caregivers play a pivotal role in the recovery of people experiencing first-episode psychosis (FEP). However, they may suffer from high levels of psychological distress and lack access to structured support. Although international recommendations highlight the need for psychoeducational interventions for caregivers, their implementation in France remains limited. This study aims to evaluate the impact of a structured, multicomponent support pathway, coordinated by advanced practice nurses (APNs), for family caregivers of people with FEP. This multicentre, stepped-wedge (cluster with seq

Impact of Glucagon-Like Peptide-1 Receptor Agonists on Mental Health: A Systematic Review of Psychiatric Safety and Antidepressant Potential.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are increasingly used for type 2 diabetes mellitus (T2DM) and obesity, but concerns about their mental health effects have been raised. Early safety reports linked them to suicidal thoughts and worsening depression; however, evidence remained mixed and unclear. A systematic search was conducted across three databases for studies published between 2021 and May 2026. Out of 617 records identified, 11 studies were finally included after quality checks using appropriate appraisal tools based on study design. This review was not prospectively reg

A data-driven methodological framework for representative recruitment in psychiatric research: insights from the DOCUMENT study.

BACKGROUND: Slow participant recruitment is one of the predominant determinants of failure or delay across clinical research. Even when recruitment targets are met, study populations may be unrepresentative due to sampling biases introduced by recruitment pathways. However, the effectiveness and demographic consequences of recruitment strategies are frequently underreported, undermining the generalisability of clinical findings and contributing to research waste. This study provides a data-driven, quantitative evaluation of multimodal recruitment strategies in psychiatric research, leveraging

Dysregulated adult hippocampal neurogenesis in major depressive disorder.

Major depressive disorder (MDD) is associated with reduced hippocampal volume, altered connectivity and negative memory bias, suggesting disrupted hippocampal plasticity. Dysregulated adult hippocampal neurogenesis is a potential contributor, but its relevance in humans and role in MDD remain unclear. Here we investigated the molecular basis of hippocampal dysfunction in nonmedicated individuals with MDD by integrating analyses of neurogenic trajectories, cell-type- and subfield-specific gene expression, chromatin accessibility and protein expression. We identify a neurogenic lineage in the ad

Phenome-wide screening of the putative causal determinants of bipolar affective disorder using genetic data.

Bipolar affective disorder (BPAD) is a chronic psychiatric condition with high heritability (60-85%) and significant phenotypic variability, complicating its etiological understanding. Genome-wide association studies (GWAS) have revealed genetic overlaps with diverse traits, suggesting pleiotropy. This study aimed to identify shared genetic factors and infer causal relationships using large-scale GWAS data.BPAD GWAS summary statistics were obtained from the UK Biobank via the Complex Trait Genetics Virtual Lab (CTG-VL) platform. Genetic correlations with 1,504 traits were estimated using linka

Alterations in neurovascular coupling are present in adolescent patients with major depressive disorder: An integrated resting-state fMRI and arterial spin labeling study.

BACKGROUND: Alterations in neuronal activity and cerebral hemodynamics have been reported in adolescent patients with major depressive disorder (MDD), possibly resulting in neurovascular decoupling; however, no neuroimaging evidence has confirmed this disruption. The aim of this study was to investigate the possible presence of neurovascular decoupling and its clinical implications in adolescent MDD patients via resting-state functional magnetic resonance imaging (fMRI) and arterial spin labeling (ASL) imaging. METHODS: In this prospective single-center study, we recruited adolescent patients

UK Medical Cannabis Registry: A two-year case series of clinical outcomes in depression.

BACKGROUND: Whilst preclinical evidence details the effects of cannabinoids on mood regulation, there is a paucity of clinical evidence on the use of cannabis-based medicinal products (CBMPs) in individuals with depression. This study aims to evaluate longitudinal changes in patient-reported outcome measures (PROMs) and the incidence of adverse events over 24 months in patients treated with CBMPs for depression. METHODS: Patient data from the UK Medical Cannabis Registry were used for analysis. PROMs, including the Patient Health Questionnaire-9 (PHQ-9), Generalised Anxiety Disorder-7 (GAD-7)

A novel, experimental design to assess rapid antidepressant action: Results from a phase 1b randomized trial of NV-5138.

BACKGROUND: Assessment of rapid-acting antidepressants (RAADs) requires rating scales sensitive to symptomatic changes over short time frames (hours to days). An innovative trial design was employed in an exploratory, first-in-human, phase 1b trial assessing rapid-acting effects of NV-5138 (mefluleucine), an intracellular enhancer of mechanistic target of rapamycin 1 synaptic signaling, for treatment-resistant depression. METHODS: This randomized, double-blind, placebo-controlled, multisite inpatient study used centrally rated Montgomery-Åsberg Depression Rating Scale (MADRS) assessments to c

Clinical and Cost-Effectiveness of Blended Cognitive Behavioral Therapy or Psychodynamic Therapy Versus Face-to-Face Psychotherapy for Depression (BLENDED Study): Protocol for a Pragmatic, Multicenter, Assessor-Blinded Randomized Controlled Noninferiority Trial.

BACKGROUND: Depression is a highly prevalent disorder. Yet, there is still a considerable treatment gap because of capacity issues across clinical services, which create barriers to access to effective psychological therapies. In addition, many individuals with depression do not seek treatment, and waiting lists for psychotherapy are typically very long. Blended psychotherapy, which combines online components and in-person sessions, may help bridge the treatment gap as a cost-effective intervention that complements other types of treatment for depression, as it may reduce therapist time and po

Blunted arginine vasopressin secretion in individuals experiencing a major depressive episode with comorbid post-traumatic stress disorder: Results from an exploratory study using copeptin as a surrogate marker.

Arginine vasopressin (AVP) modulates stress responsivity and social-affective behaviors, but its role in mood and trauma-related disorders remains poorly defined due to challenges in peripheral measurement. This study used copeptin, a stable, reliable, and well-validated surrogate marker of AVP secretion, to assess vasopressinergic function in a transdiagnostic sample of individuals experiencing a major depressive episode (MDE) with and without post-traumatic stress disorder (PTSD), as well as healthy volunteers (HVs). Baseline levels of copeptin, corticotropin-releasing hormone (CRH), adrenoc
