Hair as a window to mood disorders: Discrimination of bipolar affective disorder using mapping spectroscopy.

This study explores the applicability of hair as a non-invasive matrix for the spectroscopic assessment of mood disorders. Hair specimens were collected from individuals diagnosed with: recurrent depressive disorder or depressive episodes, mixed anxiety and depressive disorders, and bipolar affective disorder, alongside a control group of healthy individuals. Using Fourier-transform infrared microspectroscopy in an attenuated total reflectance configuration (μ-ATR-FTIR), 30 spectra were recorded from the cortex of each 0.5 cm hair segment. The normalized and smoothed second derivative spectra

A Curcumin-Loaded Liposomal Thermoresponsive In Situ Nasal Gel for Brain Targeted Antidepressant Therapy.

Major depressive disorder (MDD) is a chronic and recurrent neuropsychiatric condition that imposes a profound burden on individuals and society. Despite its increasing prevalence, effective therapeutic options that provide rapid, sustained, and well-tolerated relief remain limited. Mounting evidence implicates neuroinflammation, particularly microglial activation, as a critical pathological driver of MDD. Reprogramming reactive microglia toward a neuroprotective phenotype has thus emerged as a promising strategy to ameliorate MDD-related symptoms. Herein, we developed a thermoresponsive in sit

Retinal electrophysiology in mood disorders: Toward clinically relevant biomarkers. A systematic review and meta-analysis.

Mood disorders, including major depressive disorder (MDD) and bipolar disorder (BD), are associated with substantial disability and frequent misdiagnosis, particularly in early stages and in the differential diagnosis between unipolar and bipolar depression. Current diagnosis remains largely based on clinical assessment, highlighting the need for objective biomarkers to improve early detection and diagnostic accuracy. The retina, sharing structural and neurochemical properties with the brain, is a promising candidate. To assess its function, Electroretinography (ERG) is widely used. This study

Predicting early psychiatric readmission among people with major depressive disorder: A machine learning analysis from the prospective multicentre DEEP READ study.

BACKGROUND: Major depressive disorder (MDD) is associated with high relapse rates, with around 20% of inpatients experiencing readmission within 90 days after discharge. Routinely collected clinical information may help identify individuals at higher risk of readmission. Machine learning (ML) models could complement more traditional statistical approaches by exploring complex relationships among multiple vulnerability factors. METHODS: The DEEP READ study is a prospective, multicentre cohort study conducted across 13 Italian provinces, enrolling adults aged 18-65 years with a DSM-5-TR MDD dia

Predictors of suicide attempt versus isolated suicidal ideation in major depressive disorder: Novel insights from LASSO-boruta screening and binary logistic regression.

OBJECTIVES: Phenotypic heterogeneity between suicidal ideation (SI) only and suicide attempt (SA) in major depressive disorder (MDD) suggests distinct suicide risk profiles, yet key clinical and symptomatic factors distinguishing SA from SI remain incompletely defined. METHODS: A total of 799 MDD patients were allocated into SI only (n = 407) and SA (n = 392) groups. Sociodemographic indicators, clinical characteristics and depressive symptoms were compared. Predictors were selected using combined Boruta and LASSO regression, followed by binary logistic regression to identify independent fact

An Open-Label, Dose-Escalation Trial of Psilocybin-Assisted Therapy for Bipolar II Depression.

Objective: Individuals with bipolar II disorder (BD-II) and depression face limited treatment options and are often excluded from psilocybin therapy trials due to concerns about precipitating mania or psychosis. The objective of this study was to evaluate the safety, tolerability, and preliminary efficacy of psilocybin therapy in individuals with BD-II and depression. Methods: We conducted an open-label, single-arm pilot trial from 2022-2025 in which 14 participants with BD-II depression, diagnosed using adapted criteria requiring ≥2 consecutive days of hypomanic symptoms, received 10 mg of ps

Connectivity- and frequency-individualized accelerated intermittent theta burst stimulation in treatment-resistant depression: A randomized controlled trial.

Accelerated intermittent theta burst stimulation (aiTBS) is an effective neuromodulatory intervention for treatment resistant depression (TRD). aiTBS delivers repetitive transcranial magnetic stimulation sessions over a compressed timeframe and can be individualized to specific locations using resting-state functional magnetic resonance imaging (rs-fMRI) to target pathological cortical networks or individualized in time using electroencephalography (EEG) to determine optimal participant-specific stimulation frequencies. In this randomized, placebo-controlled trial, we tested these forms of per

The VOICE-DEP study protocol: Multimodal analysis of voice and speech during medical interviews to support diagnosis and longitudinal monitoring of major depressive disorder.

BACKGROUND: Major depressive disorder is a severe, recurrent and disabling condition. Although diagnosis and clinical monitoring are based on medical interviews and validated rating scales, voice and speech analysis may provide complementary digital biomarkers reflecting depressive severity and clinical evolution. However, current evidence remains limited by methodological heterogeneity, predominantly cross-sectional designs, limited longitudinal data and underrepresentation of non-English-speaking clinical populations. OBJECTIVE: The aim of the VOICE-DEP study is to develop and formalize a s

Remission status in persistent depressive disorder following acute treatment with psychotherapy plus medication or medication alone: A 2-year naturalistic follow-up.

OBJECTIVE: Long-term benefits of acute-phase treatments for persistent depressive disorder (PDD) are unclear. Treatment guidelines disagree on the efficacy of cognitive behavioral analysis system of psychotherapy (CBASP). We examined whether acute CBASP protected against recurrence in PDD patients over two years of naturalistic follow-up. METHODS: An observational study evaluated major depressive episode (MDE) recurrences in patients completing the Research Evaluating the Value of Augmenting Medication with Psychotherapy (REVAMP) trial. Enrollees 18-75 years had PDD: current chronic MDE, recu

Youth Football and the Epidemic of Pediatric Brain Injury: Neurologic and Psychiatric Outcomes in a Cohort of 72,025 Concussions.

BACKGROUND AND OBJECTIVES: Sports-related traumatic brain injury (TBI) is among the most common causes of long-term neurologic and psychiatric morbidity in children and young adults. Football, in particular, has been implicated as a leading source of TBI, yet large-scale comparative studies across sports and developmental stages are limited. The objective of this study was to quantify the burden of football-related TBI in youth compared with other sports and recreational activities, and to evaluate long-term neurologic and psychiatric outcomes across developmental stages. METHODS: This retros

Distinguishing bipolar disorder from unipolar depression in general practice: A systematic review.

OBJECTIVE: Bipolar disorder affects approximately 1% of the population, although some studies report prevalence rates of up to 6%. Distinguishing it from unipolar depression remains challenging, with diagnostic delays often exceeding ten years. This systematic review aims to identify clinical and historical features of depressive episodes that support the earlier detection of bipolar disorder in general practice. METHODS: A systematic review was conducted in accordance with PRISMA guidelines. The PubMed database was searched using MeSH terms. Study quality was assessed using the Newcastle-Ott

Clinical and psychological phenotypes of type 1 diabetes and disordered eating derived from a case vignette series: T1DE phenotypes.

AIMS/HYPOTHESIS: Type 1 diabetes and disordered eating (T1DE) is a common, complex comorbidity of type 1 diabetes with high morbidity and mortality risk. T1DE currently lacks a clear definition, diagnostic differentiation into phenotypes and clinical severity grading, which results in exclusion of people with T1DE from accessing appropriate treatments to improve outcomes. METHODS: We developed anonymised T1DE case vignettes from the baseline assessments of the Safe management of people with Type 1 diabetes and EAting Disorders studY (STEADY) and the London-T1DE pilot service at King's College

Peripheral biochemical parameters for discrimination between bipolar disorder and major depressive disorder in female patients of reproductive age: a CRP-stratified exploratory study.

BACKGROUND: Bipolar disorder (BD) is often misdiagnosed as major depressive disorder (MDD) due to overlapping clinical syndromes. Although numerous studies have investigated biomarkers to differentiate these two disorders, no clinically applicable differential diagnostic panel has been established. Notably, sex-specific differences play a critical role in this context. Females are more prone to experience depressive episodes and exhibit more pronounced immune-inflammatory dysregulation, which further increases the risk of misdiagnosis. This study aimed to explore whether C-reactive protein (CR

Acceptability and safety of two sequential doses of psilocybin in bipolar II depression: protocol for an open-label single-arm feasibility study.

INTRODUCTION: Bipolar II disorder (BD-II) is characterised by hypomanic and depressive episodes with the latter tending to dominate the clinical course of the disorder, and they are often associated with suicidality. Despite available guideline-based recommendations for first-line pharmacological treatments, patient outcomes remain suboptimal, especially for those with chronic depression and suicidal ideation. The dearth of viable treatment options for this population is evident, which is where psychedelics propose an additional alternative path. Psilocybin has recently shown promise in treati

Using multi-trait polygenic scores to predict lithium responsiveness in patients with bipolar disorder.

BACKGROUND: The predictive power of polygenic scores (PGSs) for lithium treatment response in bipolar disorder (BD) remains limited. AIM: To enhance prediction of lithium responsiveness by developing a multi-trait PGS (mt-PGS) combining genetic information from multiple phenotypes implicated in lithium response and/or BD aetiology. METHODS: We analysed data collected from BD patients who had received lithium treatment for at least six months and participated in the International Consortium on Lithium Genetics (ConLi+Gen, N=2,367) study. Shared genetic architecture and pathways linking major depressive disorder and autoimmune thyroid disease: insights from common and rare variants.

Major depressive disorder (MDD) and autoimmune thyroid disease (AITD) frequently co-occur, yet the genetic factors underlying their comorbidity remain unclear. We performed a population-matched cohort study from the UK Biobank to evaluate the phenotypic association between MDD and AITD. Genetic correlation, causal relationships, and pleiotropic loci/genes shared between the diseases were assessed based on common variants using genome-wide association study (GWAS) summary statistics, complemented by individual-level validation through polygenic risk score analysis. We additionally performed an
