The antidepressive and anxiolytic efficacy of flupenthixol has been investigated in numerous controlled and open trials involving patients with endogenous, reactive as well as senile depressions. When administered at a mean daily single or multiple dose of 1-2 mg, flupenthixol proved to be a very effective and well-tolerated antidepressant. As opposed to some of the currently available antidepressants, flupenthixol has a rapid onset of action which is often displayed within the first 2-3 days following its application. Flupenthixol decanoate has also a pronounced antidepressive and anxiolytic

The relationships between patients speaking out and measured outcome were investigated over 18 months in an out-patient group. The parameters of therapeutic progress were derived from the Symptom Check List and individualized repertory grid rating scales. Each patient's verbal participation was compared to that of other group members and to his own participation over other measurement phases. Speaking out phases were associated with more chance of change and improvement, while silent phases were associated with less chance of change and an increased likelihood of negative change. Fifteen patients with bilateral hemisphere brain injury secondary to thromboembolic stroke or trauma were evaluated for depression, cognitive impairment, and functional physical impairment. Analysis of CT scans and standardized mood scales revealed that patients with left anterior brain injury were significantly more depressed than patients without such injury. Depression severity was directly and significantly correlated with left lesion proximity to the frontal pole. Left lesion age, lesion temporal sequence, right lesion location, cognitive impairment, and functional physical impairment did

The plasmatic levels of free, sulfoconjugated and total dihydroxyphenylethyleneglycol (DOPEG), the main deaminated metabolite of noradrenaline, have been measured in thirty DSM3 major depressive inpatients and in thirty healthy controls matched for sex and age. DOPEG levels have been measured by a radioenzymatic assay. Almost fifty per cent of depressed inpatients were D.S.T. non suppressors, thirteen patients were unipolar and thirteen bipolar. Plasmatic DOPEG levels were significantly lower in depressed patients as compared to healthy controls despite a wide interindividual range of DOPEG va

Self-reports of memory problems have been evaluated prospectively in depressed patients receiving bilateral ECT or unilateral ECT, and in depressed patients receiving treatments other than ECT. Depressed patients did not complain of poor memory at seven months after hospitalization. Compared to bilateral ECT, right unilateral ECT was associated with only mild memory complaints. At three years after treatment approximately one-half of the persons who had received bilateral ECT reported poor memory. These reports seemed to be influenced by three factors: (1) recurrence or persistence of conditio

Data on the changes in the EEGs, the tone of the vegetative system and peripheral vessels, the metabolism of biogenic amines (catecholamines and serotonin), and adrenal function in patients with depressions of both endogenous and exogenous genesis were analyzed. Considerable changes in the somatic status of patients with manic-depressive psychosis during its remission and, particularly, during its preparoxysmal period were shown to be associated with the activity of the vagoinsular and sympatheticoadrenal homeostatic mechanisms through a positive feed-back system between them and the phasic ve

The safety and efficacy of bupropion in the preventive care of depression was studied in a long-term open trial. Forty patients from an active general psychiatric practice which emphasizes the treatment of affective disorders have been followed for an average of 336 days (range, 44-791) and seen at least monthly for evaluation with the Hamilton Depression Scale, Zung Self-Rating Scales for Depression and Anxiety, Clinical Global Impression Scale and an adverse reaction report form. One third of the patients had received a diagnosis of bipolar disorder and 50% a diagnosis of recurrent major dep

Two cases of recurrent major depression, three cases of dysthymic disorder (depressive neurosis), and one of adjustment disorder with depressed mood beginning in childhood or adolescence have been identified in the 133 subjects of the New York Longitudinal Study. The prospective behavioral data from early infancy to early adult life in each of the six cases are summarized. Differences in etiology are emphasized and the implications for treatment indicated. There was no evidence for a separate clinical entity of depression for the childhood period. Review of the longitudinal data did not show a

Clinical outcome following discontinuation of long-term maintenance therapy with lithium and manic-depressive patients was studied. Although the majority showed recurrence of affective episodes, the relapses tended to be spread over a period of months. There was no evidence of immediate and inevitable "rebound" of psychotic episodes following lithium withdrawal. This finding is discussed in the light of results from other lithium discontinuation studies.

Thorough reexamination of 70 patients in a long-term psychiatric ward led to partial revision of the diagnoses. Classified on the basis of Leohards's differential diagnostics, the large proportion of schizophrenics has dropped by over a third from 78% to 47%. Half of the 22 psychosis originally considered to be schizophrenia proved to be curable phasic and cycloid psychoses which had previously been overlooked and the other half were mental disturbances caused by brain-organic damage. A third of the chronic mental illnesses were found to be rooted in somatic brain lesions.

A consecutive series of 94 patients recently admitted to hospital from a defined catchment area and diagnosed as having a depressive illness were examined using standardized techniques for eliciting mental state, etc. The overlap between psychotic and neurotic diagnoses was 8.5% on discriminant function analysis of mental state items. A sub-sample of 71 patients was given a comprehensive postal and case record follow-up 5 years later. When patients developing organic and other complicating illnesses were excluded, those suffering from psychotic depression showed a significantly poorer outcome,

Selected a brief, written case history of Major Depression, without melancholia, single episode from the DSM-III Case Book (Spitzer, Skodol, Gibbon, & Williams, 1981) and mailed to 369 male psychiatrists and male psychologists in the Midwest. One-half of the clinicians were sent the psychiatric case in its true form, a depressed male, and the other half were sent the case in an altered form, a depressed female. A 32% return resulted in 90 usable diagnoses. The results found no significant differences between psychiatrists and psychologists in diagnosis. The sex of the case did not affect signi

Clinical features of manic-depressive illness, such as diurnal variation in mood, early morning awakening, and cyclicity and seasonality of recurrences, have led to speculation that the circadian system may be involved in its pathophysiology. At least three types of circadian rhythm abnormalities have been described in such patients: blunting of circadian rhythms' amplitudes, advanced position (or even nonentrainment) of circadian rhythms' phases, and doubling of the length of the sleep-wake cycle from 24 to 48 h. Several types of experiments indicate that alterations in the timing of sleep an

As lithium has a wide range of biological effects, it is not surprising that the benefit from lithium treatment has been observed in several types of psychiatric disorders. Mood stabilization has been seen in episodic disorders; antiaggressive effect has been reported in mental retardation and other illnesses, and some endocrine and hematological effects have been utilized in internal medicine and neurology. To date, however, only the stabilizing effect on recurrent mood disorders appears to be reliably predictable. The prediction is based primarily on the diagnosis, quality of free interval a

The prevalence of an abnormal response to the dexamethasone suppression test (DST) was examined in 119 in-patients suffering from a major depressive disorder and in 79 normal controls. Only 11 per cent of controls showed an abnormal DST as against 70 per cent of depressed patients. The specificity of the DST was examined by testing patients with other psychiatric disorders. Abnormal responses were found in one-fifth of a sample of schizophrenics, over one-quarter of abstinent alcoholics, two-fifths of neurotics (including neurotic depressives) and almost half of senile dements. Abnormal DST wa

Somatization in neurotic disorders was noted as a significant complication in their classification. Besides being an aspect of other neurotic disorders somatization becomes primary and chronic neurotic problem running true to type for several years in some patients. Recent evidence indicated that the syndrome "Somatic Neurosis" occurs not only in Muslim women but in middled-aged women of other communities too. A group of 20 middle-aged women with somatic neurosis were compared with an equal number of age-matched Neurotic Depressive patients. The former had significantly lower scores on Hamilto

A review of the literature suggests that exercise may have antidepressant effects and, like other treatments for depression such as electroconvulsive therapy (ECT), antidepressant medication, and REM sleep deprivation, may enhance aminergic synaptic transmission in the central nervous system. In addition, the effects of exercise and other antidepressants on sleep are similar. Therefore, it is suggested that exercise is an antidepressant that enhances aminergic synaptic transmission in the central nervous system. Many more psychological and physiological studies must be performed in order to ve

The pharmacology, pharmacokinetics, clinical trials, side effects, and dosage of amoxapine are reviewed. Amoxapine is a tricyclic dibenzoxazepine antidepressant that is chemically similar to the antipsychotic agent loxapine. In animal tests, amoxapine and its metabolites block reuptake of the neurotransmitter norepinephrine, with little effect on serotonin. It is rapidly and virtually completely absorbed when administered orally; peak serum concentrations occur one to two hours after ingestion. Amoxapine is widely distributed throughout body tissues and is 90% bound to serum proteins. Aromatic

We have investigated some biochemical properties of platelet monoamine oxidase (MAO) isolated from chronic schizophrenic and agoraphobic patients, nonschizophrenic institutional controls, and healthy volunteers. The enzyme activity level in the healthy population was reasonably constant over at least a 6-week period. High correlations were found between MAO activity assessed for different substrates (p-tyramine, beta-phenylethylamine, and tryptamine). Some heterogeneity of the platelet MAO may exist, however, at least in some of the chronic schizophrenics, since the substrate specificities wer

At discharge, a significantly larger percentage of unipolar patients treated with electroconvulsive therapy (ECT) respond with marked improvement as compared with those receiving antidepressants or other treatment. No treatment appears to be more efficacious in the bipolar group. In studying the covariables related to the length of time between hospitalizations, we found that the type of treatment is not important for the unipolar patient, whereas a combination of ECT and antidepressants lengthens the time to rehospitalization of the bipolar patient. Previous hospitalization is an important pr
