Dose dependent effects of transcranial photobiomodulation on blood-oxygenation-level-dependent power in major depressive disorder.

BACKGROUND: Transcranial photobiomodulation (t-PBM) with near-infrared light stimulates mitochondria and may have antidepressant effects. We evaluated dose-dependent effects of t-PBM on the hemodynamic blood-oxygenation-level-dependent (BOLD) power in major depressive disorder (MDD). METHODS: 31 MDD subjects underwent randomly assigned t-PBM sessions, 1/week, in the MRI scanner, with 1) Sham; 2) High dose: pulse wave, irradiance ∼300 mW/cm2; 3) Medium dose: continuous wave, ∼300 mW/cm2; and 4) Low dose: continuous wave, ∼50 mW/cm2 t-PBM (808 nm) was delivered to the prefrontal cortex, bilater

Behavioral telehealth in low-resource primary care settings for anxiety and depression in youth: protocol for a randomized effectiveness-implementation study.

BACKGROUND: Anxiety and mood disorders in youth are prevalent and impairing, with high current and lifetime comorbidity. Untreated, these disorders lead to sustained functional impairment and increased risk for recurrent disorder and suicide. In addition, there are notable disparities in care, with both families experiencing increased social risk and youth of color significantly less likely to receive mental health services. In previous research, a primary-care-based transdiagnostic brief behavioral therapy (BBT) was developed and found to be effective for a broad population of youths with anx

Assessing Practice Patterns of Antidepressant Medication and Tamoxifen Use Among Patients With Hormone Receptor-Positive Breast Cancer.

BACKGROUND: Some commonly prescribed antidepressants inhibit CYP2D6, an enzyme that metabolizes tamoxifen into its active metabolite, endoxifen. This inhibition could theoretically decrease the efficacy of tamoxifen, which could hypothetically lead to higher breast cancer recurrence rates and mortality. There is inconsistent data reported on the potential drug-drug interaction between concomitant use of tamoxifen and CYP2D6-inhibiting antidepressants. AIM: We investigated prescribing patterns of concomitant tamoxifen and antidepressant medications. METHODS AND RESULTS: Our study population i

Best practices for using biological data in psychological research.

Advances in statistical techniques for analyzing biological data have driven the need for larger sample sizes, which are ultimately made possible through team science efforts. Findings from these efforts have led to a significant shift in approaches to biological data collection over the past decade, from traditional approaches relying on single measures or assays collected by individual investigators in small lab-based studies to high-dimensional methodologies embedded within large cohorts or multi-site studies. These recent large scale consortium efforts necessitate careful consideration acr

Deutetrabenazine and Modified Electroconvulsive Therapy for Tardive Dyskinesia With Recurrent Depression: A Case Report.

This study reports a case of tardive dyskinesia (TD) and depression in a patient with long-term use of antipsychotic and anticholinergic drugs, through the tapering and discontinuation of anticholinergic drugs and the concurrent use of deutetrabenazine combined with modified electroconvulsive therapy (MECT). Her TD symptoms have completely improved. The score on the Hamilton Depression Rating Scale (HAMD-17) has dropped from 20 points to eight points, and the depressive symptoms have improved by 60%. A 31-year-old woman with a mood disorder, treated with risperidone (dose range: 4 mL/day, dura

An evaluation of aripiprazole long-acting injectable monthly and bimonthly in the maintenance treatment of bipolar I disorder.

Bipolar disorder (BD) is a chronic and severe mental disorder with a high risk for recurrence, requiring long-term mood stabilization treatment approaches. The hazards of recurrence in BD include illness progression, decreasing treatment response, chronicity, comorbidity, suicidality, increased health service utilization, and greater direct and indirect economic costs. Treatment nonadherence is one of the most common modifiable deficiencies in the maintenance treatment of BD, which exacerbates the risk of recurrence and unfavorable course and outcome. Aripiprazole is the only dopamine partial

From molecules to minds: Integrative multi-omics in psychiatry.

Psychiatric disorders are biologically complex conditions arising from interactions across genomic, epigenomic, transcriptomic, proteomic, metabolomic, and metagenomic layers. Single-omics approaches rarely capture more than a fraction of the variance in complex conditions, underscoring the importance of integrative multi-omics frameworks. This mini-review summarizes key methodologies and their application in psychiatric research, with a focus on systems-level integration of genomic risk scores, transcriptomic networks, and neuroimaging data to advance biological understanding of disorders suc

Therapist-guided and self-guided internet-based behavioural activation versus treatment as usual for adolescents with depression: randomised controlled trial with economic evaluation.

BACKGROUND: Adolescent depression is a major public health concern with limited access to effective treatments. Behavioural activation (BA) is a suitable intervention for digital delivery, but definitive adolescent trials are scarce. OBJECTIVE: To evaluate whether therapist-guided and self-guided internet-based BA (I-BA) are more efficacious and cost-effective than treatment as usual (TAU) for adolescents with mild to moderate major depressive disorder (MDD). METHODS: Single-blinded, parallel-group randomised controlled trial with economic evaluation. A total of 219 adolescents (13-17 years)

Distinguishing different psychiatric disorders using DDx-PRS.

Despite great progress on case-control polygenic prediction, an unmet need remains for a method that genetically distinguishes clinically related disorders (e.g., schizophrenia (SCZ) versus bipolar disorder (BIP) versus major depressive disorder (MDD) versus controls). We introduce differential diagnosis-polygenic risk score (DDx-PRS), which jointly estimates the posterior probabilities of each diagnostic category (e.g., SCZ = 50%, BIP = 25%, MDD = 15%, control = 10%) by modeling variance-covariance structure across disorders, leveraging case-control polygenic risk scores and prior clinical pr

Speech-based relapse prediction in psychosis using explainable AI: Protocol for the international multicentre observational TRUSTING study.

INTRODUCTION: The course of psychotic disorders typically involves relapses. Early warning signs vary between individuals and are difficult to detect in clinical practice, especially in outpatient settings. Speech provides a quantitative clinical marker for detecting such early warning signs. The EU Horizon project TRUSTING (A TRUSTworthy speech-based AI monitoring system for the prediction of relapse in individuals with schizophrenia) aims to develop and evaluate a speech-based monitoring system for predicting imminent psychotic relapses. The study will examine the potential for prospective r

Pathway-specific regulation of the paraventricular thalamic nucleus in depressive-like behavior.

Bipolar disorder is one of major psychiatric disorders afflicting about 1% of the world population. While genetic factors have been increasingly recognized as contributing to the disorder, the underlying etiology of bipolar disorder, such as associated brain regions and cellular pathology, are yet to be elucidated. Our previous studies using a genetic bipolar disorder animal model indicated potential role of the paraventricular thalamic nucleus (PVT) in recurrent depressive-like episodes that exhibit phenotypic similarity to the depression phase of bipolar disorder. Notably, chronic activation

MELODY trial: study protocol for a 12-week randomised controlled trial of adjunctive melatonin, digital cognitive behavioural therapy for insomnia or pill placebo to improve depressive symptoms in young adults with mood disorders.

OBJECTIVES: Sleep and circadian rhythm disturbances are proposed to be pathophysiological mechanisms underlying some cases of depressive and bipolar (mood) disorders. An unresolved clinical question is whether sleep and circadian-based therapies are effective antidepressants for young adults (18-30 years) with a mood disorder. METHOD AND ANALYSIS: MELODY (Melatonin for Depression in Youth) is an investigator-initiated, single-centre, phase 3, randomised controlled trial with two blinded pharmacological arms (melatonin vs placebo) and one open-label digital intervention arm (digital cognitive

Recurrent Catatonia in the Setting of Urinary Tract Infection and Medical Comorbidity: A Case Report.

Catatonia is a neuropsychiatric syndrome characterized by disturbances in motor activity, affect, and autonomic function. Although often associated with primary mood or psychotic disorders, it can be precipitated or sustained by systemic infections such as urinary tract infection (UTI). We describe a 60-year-old man with recurrent catatonia and diagnoses of schizophrenia, bipolar disorder, and major neurocognitive disorder, whose latest episode of catatonia, described in this case report, was preceded by psychosocial stress and appeared to be exacerbated by polymicrobial UTI, bacteremia, and u

Mendelian Randomization Studies of Major Depressive Disorder, Schizophrenia, Bipolar Disorder, and Cancers: A PRISMA Review and Meta-Analysis.

INTRODUCTION: Previous literature has reported ambiguous results regarding the association of severe psychiatric disorders and an increasing risk of cancer disease. The aim of our analysis was to explore any associations between severe psychiatric disorder (SPD), like schizophrenia (SCZ), Bipolar Disorder (BD) and recurrent major depression (MDD), and cancer diseases by meta- analyzing Mendelian Randomization (MR) studies available in the current literature. METHODS: The present systematic review and meta-analysis was registered in the Prospero register (id. no. CRD420251072022). The database

Exploring serum biomarkers in paediatric psychiatry: the impact of NfL and GFAP.

Serum neurofilament light chain (sNfL) and glial fibrillary acidic protein (GFAP) are biomarkers of neuroaxonal and astrocytic damage but remain understudied in adolescent psychiatric populations. This study investigated sNfL and GFAP levels in 412 adolescents diagnosed with anorexia nervosa (AN) (n = 52), depression (n = 237), and other psychiatric disorders (n = 123). We assessed their diagnostic utility, correlation with disease severity, and longitudinal changes during AN treatment. Biomarkers were measured using Single Molecule Array technology, with Z-scores derived from reference datase

State-dependent associations between striatal dopamine transporter availability and large-scale functional networks in bipolar disorder: An exploratory multimodal imaging study.

BACKGROUND: Dopaminergic dysregulation has been implicated in the pathophysiology of bipolar disorder (BD), yet how state-dependent alterations in presynaptic dopaminergic markers relate to large-scale brain networks remains unclear. We aimed to examine longitudinal, within-subject associations between striatal dopamine transporter (DAT) availability and functional connectivity (FC) across mood states in BD. METHODS: Outpatients with BD underwent repeated neuroimaging during euthymic and depressive states. Striatal DAT availability was measured using ^99mTc-TRODAT-1 single-photon emission com

Therapeutic effects of TRPC4/5 antagonism in a rat model of bipolar disorder.

Bipolar disorder, a chronic neuropsychiatric condition, is characterized by alternating episodes of mania and depression. Although acute manic episodes have become increasingly manageable with the advent of third-generation neuroleptics, the primary therapeutic challenge remains the prevention of recurrences and the effective management of often refractory depressive phases. Emerging evidence implicates transient receptor potential (TRP) channels in mood regulation through their roles in calcium signaling, synaptic plasticity, and neuroinflammation. Among these, TRPC5 has gained attention as a

Higher semaglutide dose is associated with lower neuropsychiatric event incidence independent of weight loss.

GLP1 receptor agonists (GLP-1RAs) have transformed obesity treatment, but their impact on neuropsychiatric outcomes remains poorly understood. We conducted an observational study of 63,215 patients with preexisting neuropsychiatric conditions and evaluated 24 incident neuropsychiatric outcomes following treatment initiation. In propensity-matched analyses, semaglutide was associated with broadly lower neuropsychiatric event risk over two years compared with metformin, SGLT2 inhibitors, and DPP-4 inhibitors. Within the semaglutide-treated cohort, higher attained dose during the first two years
