Effectiveness and Safety of Venlafaxine in Japanese Major Depressive Disorder Patients with or without Anxious Distress (EXPAND Study): A 12-Week Prospective Observational Study.

PURPOSE: This study evaluated outcomes of venlafaxine treatment in Japanese patients with major depressive disorder (MDD) in routine clinical practice. Since anxiety is a common and clinically relevant feature of MDD, descriptive analyses were conducted based on the presence or absence of Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) anxious distress. PATIENTS AND METHODS: This 12-week, multicenter, single-arm, observational study consecutively enrolled adult patients with MDD initiating venlafaxine in Japan. DSM-5 anxious distress was assessed at baseline. The

Cognition and future depression: associations with risk in those with and without a history of depression.

BACKGROUND: Cognitive impairments are common in depression and often persist beyond mood resolution. However, the relationship between cognitive performance, its neurological underpinnings, and future depression risk is unclear, limiting strategies for primary and secondary prevention. OBJECTIVE: Our objective was to determine whether cognition associates with subsequent depression, both relapse and first-episode occurrences. METHODS: 1862 UK Biobank participants with a history of International Classification of Diseases (ICD)-10-defined depression in remission (RD) (mean (SD) age: 52.7 (7.1

Mental health safety profile of CFTR modulators in pediatrics and adults: insights from the French pharmacovigilance database and from VigiBase.

RATIONALE: CFTR modulators (CFTRms) have transformed cystic fibrosis care, yet concern exists regarding their mental health safety profile. OBJECTIVE: To provide further information, we conducted an analysis of CFTRm-related mental health disorders using two spontaneous pharmacovigilance reporting databases in real-world settings. METHODS: We analyzed reports from the French Pharmacovigilance Database (FPD) and VigiBase, the WHO global individual case safety reports database. Reports recorded between 2012 and 2024 involving any CFTRm and psychiatric disorders were included. In VigiBase, we c

Nationwide Transdiagnostic Age and Sex Patterns in Mental Disorder Disease Development.

IMPORTANCE: Individuals with mental disorders often experience multiple disease episodes and diagnostic changes after initial psychiatric contact. However, studies that comprehensively investigate heterogeneity in disease development beyond pairs of comorbid diseases and mortality from a transdiagnostic perspective have been lacking. OBJECTIVE: To comprehensively study sex and age patterns of disease development for 7 major groups of mental disorders. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included data from linkage of national Danish registers of all adult individuals living i

An intersectional analysis of the socio-structural factors associated with anxiety and mood disorder diagnoses in young people in Catalonia: a primary care cohort study (2008-2022).

Mental health inequities among young people are often examined through single social dimensions, potentially obscuring heterogeneity across intersecting social positions. This study examined how anxiety and mood disorder diagnoses varied according to intersections of sex, socioeconomic status (SES), and nationality in Catalonia. Population-based cohort included 2,086,641 individuals aged 10-24 years (2008-2022) using anonymized primary care records. Sex-stratified regression models examined associations between SES-nationality intersections and incident anxiety and mood disorder diagnoses. Amo

Transient Mood Elevation Without a Manic Switch: Goldenseal-Augmented Oral Glutamatergic Regimen in Bipolar Depression With Comorbid Obsessive-Compulsive Disorder (OCD).

Treating bipolar depression that comes with obsessive, looping thoughts can be tricky because standard antidepressants often help only a little and can sometimes push the mood too high. In this report, we followed a 32-year-old man whose low mood and health-related ruminations lingered despite mood stabilisers (valproate, low-dose aripiprazole, pregabalin, and Deanxit). Hoping to copy some of ketamine's brain-circuit effects, he tried an over-the-counter mix of dextromethorphan, piracetam, and L-glutamine; the result was modest at best. Ten days after he added 600 mg of the herbal CYP2D6 inhib

Association of betel quid chewing with specific major psychiatric disorders in Taiwan Biobank-NHIRD linked data.

AIMS: Betel quid chewing (BQC) is a common psychoactive habit in Taiwan and other Asian regions. Evidence regarding its association with specific psychiatric disorders remains limited. This study examined associations between BQC assessed at Taiwan Biobank enrolment and major psychiatric disorders recorded in linked health-insurance claims data. METHODS: We analysed 51,522 male participants from the Taiwan Biobank successfully linked to the National Health Insurance Research Database, including 7,973 ever-users (BQC) and 43,549 never-users of betel quid (non-BQC). Psychiatric disorders includ

Functional electrical stimulation for major depressive disorder: Protocol for a pilot randomized controlled trial.

BACKGROUND: Conventional treatments for major depressive disorder (MDD) fail to benefit one-third of patients, underscoring the need for therapies with novel mechanisms of action. Bilateral functional electrical stimulation (FES) of the facial muscles uses electrical currents to activate the muscles involved in facial expression and may offer a novel "mind-body" intervention for MDD. Based on preliminary work demonstrating the feasibility of FES for MDD, this proposed study will assess the efficacy, safety, and tolerability of FES in MDD. METHODS: This single-site, double-blind, randomized, s

White matter abnormalities in bipolar II and unipolar depression - Evidence from fixel-based analysis.

BACKGROUND: Challenges in correctly identifying bipolar disorder II (BD-II) during depressive states have led to poor clinical outcomes. BD-II-specific diffusion-weighted imaging studies are lacking. This study addresses current knowledge gaps by comparing white matter (WM) integrity in BD-II and unipolar depression (UD) using fixel-based analysis (FBA). METHOD: Primary analyses compared fibre density (FD) among 33 individuals with BD-II, 50 with UD, and 51 healthy controls (HC) in 11 a priori tracts. Sensitivity analyses among unmedicated participants examined whether medication status affec

Targeting distinct facets of anhedonia via transcutaneous auricular vagus nerve stimulation: Effects on heart rate variability and reward-based behavior.

Anhedonia, a core feature of major depressive disorder, is associated with disrupted reward processing and may represent a mechanistically relevant target for neuromodulation. This study examined the effects of transcutaneous auricular vagus nerve stimulation (taVNS) on reward-related behavior and autonomic regulation in individuals with higher (n = 34) and lower (n = 34) depressive symptomatology. In a randomized, within-subject crossover design, participants received active taVNS and sham stimulation on two separate days. During each session, incentive motivation and reward learning were ass

Illness Severity and Longitudinal Gray Matter Volumes Among People With Major Depressive Disorder.

IMPORTANCE: Although cross-sectional research in major depressive disorder (MDD) is extensive, long-term longitudinal neuroimaging studies are rare, despite being crucial to uncovering how brain changes evolve with illness progression. OBJECTIVES: To disentangle brain structural trajectories within the fronto-limbic circuitry in MDD and to distinguish progressive decline from dynamic state-like alterations. DESIGN, SETTING, AND PARTICIPANTS: This single-center case-control study (Münster Neuroimaging Cohort) conducted repeated magnetic resonance imaging (MRI) assessments over up to 10 years

Greater depression severity predicts improvements in physical exhaustion following aerobic exercise in youth with bipolar disorder.

OBJECTIVE: Despite the potential benefits of exercise on mood and physical health in bipolar disorder (BD), exercise rates in youth with BD remain low. Given that exercise-induced feelings predict future exercise participation, the present study examined the association of baseline depression and mania severity with exercise-induced feelings in youth with BD. METHOD: Fifty-two youth with BD (17.3 ± 1.3 years) completed semi-structured diagnostic and mood symptom rating scales. The Exercise-Induced Feeling Inventory (EFI) was completed before and after a single session of aerobic exercise. Mul

Ketamine Dose and Duration Effects in Internalizing Disorders.

The dose-response profile of racemic ketamine in internalizing disorders is poorly characterized. This was a secondary analysis of outcome responses in patients with treatment-resistant depression, post-traumatic stress disorder, generalized and/or social anxiety, or obsessive-compulsive disorder. For each disorder, they received single intramuscular doses of racemic ketamine (0.5 and 1.0 mg/kg) and a psychoactive control, using a three-way within-subject double-blind active-controlled cross-over design. Intramuscular racemic ketamine doses of 0.5 and 1.0 mg/kg produced robust effects on diagn

Temporal dynamics of affect, screen time, and step count in individuals at risk for mood disorders.

BACKGROUND: Identifying how affect and daily behaviors influence each other is central to understanding mood disorder development. This study investigated how negative affect (NA), positive affect (PA), physical activity, and smartphone use influence one another in daily life, and whether familial risk or mood disorder history shapes these relationships. METHODS: In a 14-day experience sampling protocol, we measured NA and PA five times a day along with passive sensing of step count and screen-on time. We examined the effect of familial high risk and history of mood disorders on within-person

Imagery Rescripting for Pathological Affective Dependence in Intimate Partner Violence (ImRs-PAD): An In-Person Proof-of-Concept Study.

This study evaluated the feasibility and preliminary effects of in-person Imagery Rescripting for Pathological Affective Dependence (ImRs-PAD), a trauma-focused and mechanism-based intervention for survivors of intimate partner violence (IPV). ImRs-PAD targets trauma-related relational dynamics-specifically Pathological Affective Dependence (PAD)-together with the related relational processes that may contribute to difficulties disengaging from abusive relationships. PAD is conceptualized as a relational constraint rooted in trauma, rather than individual pathology or responsibility. This stud

Association of Lithium Exposure With Alzheimer's Disease: A Propensity Score-Matched Cohort Study.

BACKGROUND: Lithium has been widely studied for its potential neuroprotective effects through mechanisms implicated in Alzheimer's disease (AD), yet evidence from large real-world populations remains inconsistent. Clarifying the association between lithium exposure and AD risk in routine clinical practice is important given lithium's widespread use in mood disorders. OBJECTIVE: This study aims to examine the association between lithium exposure and the incidence and time to diagnosis of AD among adults with bipolar disorder or recurrent major depressive disorder. METHODS: We conducted a retr

Habenular structural-functional dysconnectivity in bipolar disorder: evidence from multimodal imaging and transcriptomic integration.

BACKGROUND: Bipolar disorder (BD) is a highly heritable condition characterized by recurrent shifts between manic and depressive states. Here we investigated the potential involvement of the habenula because it plays a central role in negative affect and behavioral regulation. METHODS: We investigated bilateral habenular volume and seed-based resting-state functional connectivity in a discovery cohort (78 BD, 102 controls) and an independent replication cohort (72 BD, 85 controls). Associations among habenular features, clinical symptoms, and molecular correlates were examined by integrating
