Phase 1 randomized trial of HS-10353, a novel GABA(A) positive allosteric modulator for treatment of major depressive disorder.

BACKGROUND: HS-10353 is a novel positive allosteric modulator of γ-aminobutyric acid A receptors that is under development for the treatment of major depressive disorder (MDD). METHODS: This is a double-blind, randomized, placebo-controlled, phase 1 trial comprising single ascending dose (SAD) and multiple ascending dose (MAD) parts. In SAD, 8 healthy volunteers of each cohort were randomized (6:2) to receive HS-10353 (2-55 mg) or placebo in a fasted state. In MAD, 12 MDD patients of each cohort were randomized (9:3) to receive HS-10353 (15-65 mg/day given for up to 7 days) or placebo. The pr

One brain, one mind: A joint EPA-EAN leadership perspective on brain health.

Neurology and psychiatry have operated as separate disciplines for over a century, yet this division reflects historical and institutional developments rather than the underlying biology of the brain. Contemporary neuroscience shows that brain and mental health disorders share genetic susceptibilities, inflammatory and metabolic pathways, environmental and social risk factors, and clinical features that cross diagnostic boundaries. Cognitive, emotional, sensory, and motor symptoms regularly appear across both neurological and psychiatric populations, and conditions such as seizures, psychosis,

Evidence for overlapping genetic architecture between lifestyle factors and severe mental disorders with different patterns across diagnoses.

BACKGROUND: Severe mental disorders (SMDs) are associated with unhealthy lifestyle, contributing to increased risk of comorbid cardiovascular disease. Genetic factors influence both SMDs and lifestyle behaviours, but their genetic relationships remain unclear. Here, we aimed to unravel the shared genetic architecture of SMDs and lifestyle factors. Additionally, we assessed if genetic propensity to SMDs predicts body mass index (BMI) and lipids through lifestyle factors. METHODS: We analysed genome-wide data on major depression (MD) (N = 480,359), schizophrenia (SCZ) (N = 130,644), bipolar dis

Co-existence of clinical high-risk for psychosis and bipolar disorder: a multi-dimensional psychopathological analysis.

BACKGROUND: While the clinical high-risk for psychosis (CHR-P) has been extensively studied and the clinical high-risk for bipolar disorder (CHR-BD) has gained increasing attention, overlapping symptoms-such as mood instability and subclinical psychotic features-complicate diagnostic boundaries. We aimed to investigate the psychopathological characteristics of high-risk individuals to address the feasibility of early-stage diagnostic differentiation. We hypothesized that CHR-P and CHR-BD could co-occur and that symptom structure would differ depending on overlap status. METHODS: We recruited

Genome-wide meta-analysis of quantitatively measured generalized anxiety symptoms in individuals of European ancestry.

Anxiety is heritable and exists on a continuum, with symptoms ranging from adaptive threat response to clinical disorder. Here we performed a genome-wide association meta-analysis of generalized anxiety symptom severity in 693,869 individuals of European ancestry from 14 cohorts. We identified 80 independent genome-wide significant variants within 74 loci, 39 of which were newly associated with anxiety. SNP-based heritability was 5.9% (posterior s.d. = 0.15%). Polygenic scores were significantly associated with anxiety symptom severity and disorder in European, African and South Asian ancestry

Integrative genomic characterization of the FADS1-2-3 mood disorder risk locus: implications for the role of polyunsaturated fatty acids.

BACKGROUND: Polyunsaturated fatty acids (PUFA) have been implicated in mood disorder pathophysiology, with Mendelian randomisation studies suggesting a causal role. However, much of this evidence centres on the highly pleiotropic FADS1-2-3 gene cluster, raising concerns about horizontal pleiotropy and uncertainty regarding the true effector gene(s). To assess the validity of these concerns, we performed integrative genetic characterization of the FADS1-2-3 mood disorder risk locus. METHODS: We analysed summary statistics from the largest (up to n = 4 million) genome-wide association studies (

Using functional MRI neurofeedback to modulate self-blame in major depressive disorder: A pilot study.

BACKGROUND: Growing evidence implicates self-blame-related neural networks in the pathophysiology of major depressive disorder (MDD). fMRI neurofeedback is an emerging technology with the potential to deliver interventions targeting the neural substrates of self-blame, but its feasibility and potential mechanism in current MDD remain unresolved. METHODS: The current pilot study employed a single-session, exploratory neurofeedback trial harnessing the subgenual cingulate cortex (SCC) BOLD activity as the sole training target. Two active interventions were compared. In Intervention A (n = 10),

Digital Attention Bias in Cancer Survivors Intervention for Adolescent and Young Adult Cancer Survivors: Protocol for a Pilot Randomized Controlled Trial.

BACKGROUND: Adolescent and young adult cancer survivors face a high burden of psychological late effects, with cancer-related anxiety being a prevalent mental health concern. Despite the significant need for care, more than half of adolescent and young adult cancer survivors, who require psychosocial services, remain untreated. Digital health interventions offer a promising solution to bridge this care gap. Attention bias modification (ABM) is an evidence-based digital intervention for anxiety disorders. This intervention targets automatic and unconscious negative attention biases and retrains

Genomic relationship between polyendocrine metabolic ovarian syndrome and bipolar disorder.

Women with bipolar disorder (BIP) have a higher risk of developing polyendocrine metabolic ovarian syndrome (PMOS). Shared genetic architecture may underlie this comorbidity. Valproate, a mood-stabilizer commonly used to treat BIP, increases the risk of PMOS. Still, the mechanism underlying PMOS in BIP remains unknown. Here, we aimed to identify genetic variants shared between BIP and PMOS, as well as their interaction with valproate. We used the results of large-scale genome-wide association studies of BIP (41,510 cases and 354,340 controls), and PMOS (3609 cases and 229,788 controls). Using

Investigating the effect of increased dopamine signaling on cerebral blood flow in Major Depressive Disorder: a double-blind randomized placebo-controlled study.

Anhedonia, a core symptom of major depressive disorder (MDD), is linked to dysfunction in reward-related brain regions and impaired dopamine (DA) signaling. This study used arterial spin labeling (ASL) to investigate the effects of a single low dose of amisulpride, a selective DA antagonist, on cerebral blood flow (CBF) in MDD patients and healthy volunteers (HVs). In a double-blind, placebo-controlled, randomized, parallel-group design, participants received either 100 mg amisulpride or placebo. N = 111 participants (MDD: n = 57; HV: n = 54) were included in the final analysis and group diffe

Evaluating the inter-relationships among depression, anxiety and somatic symptoms in haemodialysis patients: a cross-sectional structural equation modelling study.

BACKGROUND: Somatic symptoms frequently occur in haemodialysis patients, whereas depression and anxiety are also common but are often under-recognised issues. This study aims to examine the associations among depression, anxiety and somatic symptoms in haemodialysis patients. These symptoms may be linked together under a general distress construct in haemodialysis patients. METHODS: We surveyed 1085 dialysis patients between 1 March 2018 and 28 February 2023 at 17 dialysis facilities in Northeast Ohio with the 9-item Patient Health Questionnaire (PHQ-9), 7-item Generalized Anxiety Disorder an

Transcutaneous vagus nerve stimulation enhances reward-effort efficiency in severe major depressive disorder.

Major depressive disorder (MDD) is a highly prevalent psychiatric disorder with changes in motivation to work for rewards being a core symptom. Transcutaneous vagus nerve stimulation (tVNS) has emerged as a promising therapy but its effects on the core features of MDD, such as changes in motivation, remained relatively unexplored. In this randomised, single-blind, cross-over, controlled trial, we used a grip strength effort task to investigate how tVNS impacted choices to exert different levels of physical effort for varying monetary rewards in MDD patients (n = 53) and a non-depressed control

Stellate Ganglion Block for Post-traumatic Stress Disorder: A Comprehensive Review of Evidence, Technique Considerations and Symptom Outcomes in Military and Non-Military Patients.

PURPOSE OF REVIEW: Post-traumatic stress disorder (PTSD) is a psychiatric disorder which is defined by four symptom clusters of intrusive re-experiencing, avoidance, negative mood/cognitive changes, and hyperarousal. This pathway is mediated by a dysregulation in the amygdala-hippocampus-medial prefrontal cortex circuit resulting in an abnormal threat processing and other symptoms. Stellate ganglion blocks (SGB) are largely supported in a review of the current literature. The objective of this review is to synthesize available evidence and extrapolate findings to identify patient and symptom p

Real-world experience with daridorexant for insomnia in adults with intellectual disabilities: a retrospective single-centre case series.

Chronic insomnia is common in adults with intellectual disabilities, yet evidence to guide pharmacological management is limited. We report a real-world case series of five adults with intellectual disabilities and multimorbidity treated with daridorexant, a dual orexin receptor antagonist approved in Europe for insomnia. Daridorexant was associated with marked improvements in sleep quality and quantity, despite complex comorbid sleep, physical, mental and neurodevelopmental disorders. Subject to the limitations of a small, uncontrolled case series, these gains were associated with other repor
