Living with an imagined odor: first case report of olfactory reference syndrome in East Africa treated using a multimodal approach.

BACKGROUND: Olfactory reference syndrome is a psychiatric disorder characterized by a persistent and distressing preoccupation with emitting a foul body odor despite the absence of objective evidence. It is classified under "other specified obsessive-compulsive and related disorders" in the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision. Although olfactory reference syndrome has been described in several regions globally, there are no published case reports from East Africa. CASE PRESENTATION: A 50-year-old Ethiopian man was referred from the otorhinolaryn

Demographic and clinical characteristics of participants in the Neuropsychiatric Genetics of African Populations-Psychosis (NeuroGAP-Psychosis) study: A descriptive analysis.

BACKGROUND: Approximately 1.3 million individuals in sub-Saharan Africa, as well as millions of others worldwide, live with psychotic disorders. However, there is limited mental health research based on these populations and even less of a focus on neuropsychiatric genetics research within these settings. METHODS: To address this gap, the Neuropsychiatric Genetics of African Populations-Psychosis (NeuroGAP-Psychosis), a case-control study, collected data identifying genetic and environmental risk factors associated with psychotic disorders in Ethiopia, Kenya, South Africa, and Uganda. This pa

[What do we know about compassion focused therapy for anxiety and depressive symptoms: A scoping review].

OBJECTIVE: Anxiety and depressive disorders are among the most common psychiatric conditions and have a high comorbidity rate. While cognitive behavioral therapy (CBT) remains the gold standard psychotherapeutic treatment, its protocols which focus on a specific disorder have limitations when faced with clinical complexity. Transdiagnostic approaches aim to overcome these constraints by targeting processes common to different mental disorders. Compassion-Focused Therapy (CFT) is part of this approach. It is particularly aimed at patients with overwhelming shame and marked self-criticism, often

Family-based treatment at home in adolescents with eating disorders and co-occurring mental health conditions: rationale and study design of a mixed methods trial.

BACKGROUND: The significant impact of eating disorders on adolescents necessitates the evaluation of current treatments. Family-based Treatment (FBT) is the standard treatment but has modest remission rates, highlighting the need for improvements. Assessing its effectiveness in adolescents with co-occurring mental health conditions is also crucial. In the Netherlands, there is a growing focus on home-based treatment. This study aims to enhance remission rates in FBT by adapting it for use in a home setting (FBT-H). OBJECTIVE: This mixed-method study combines single case studies with qualitati

Gray Matter Differences in Adolescent Psychiatric Inpatients: A Machine Learning Study of Bipolar Disorder and Other Psychopathologies.

BACKGROUND: Bipolar disorder (BD) is among the psychiatric disorders most prone to misdiagnosis, with both false positives and false negatives resulting in treatment delay. We employed a whole-brain machine learning approach focusing on gray matter volumes (GMVs) to contribute to defining objective biomarkers of BD and discriminating it from other forms of psychopathology, including subthreshold manic presentations without a BD Type I/II diagnosis. METHODS: Five support vector machine (SVM) models were used to detect differences in GMVs between inpatient adolescents aged 13-17 with BD-I/II (n

A critique of International Classification of Diseases 11th Revision criteria for the mood disorders.

In 2024, WHO published the Clinical Descriptions and Diagnostic Requirements for International Classification of Diseases 11th Revision Mental, Behavioural and Neurodevelopmental Disorders manual. Stated objectives were to determine valid and reliable diagnoses and defined so as to differentiate conditions both from other conditions and from normative states. This paper provides a critique of the criteria sets derived for both the depressive and the bipolar disorders. It is argued that failure to specify a minimum number of criteria for certain conditions compromises valid intrinsic and differ

Depression Remission and Response Rates and Anxiety Response as a Predictor of Depression Response in a Community Electroconvulsive Therapy Clinic.

Objective: To assess depression response and remission rates with electroconvulsive therapy (ECT) in a community clinic and to identify factors predicting success in treatment. Methods: Thirty-five patients were identified by a retrospective chart review with a diagnosis of major depressive disorder or depressive disorder not otherwise specified (according to the DSM-IV-TR) who were treated with an acute series of ECT at a community ECT clinic from March 1, 2014, to March 9, 2015. Logistic regression examined associations between clinical variables and depression response (defined as ≥50% redu

Developmental shifts in irritable behaviors from preschool to school age: Longitudinal network analysis of heterotypic and homotypic continuity.

BACKGROUND: Irritability comprises a set of behaviors that span normal:abnormal proneness to anger. When dysregulated and developmentally atypical, irritability indicates neurodevelopmental vulnerability for mental health problems. Yet, mental health risk indicators such as irritability likely present differently during specific developmental stages, especially across the crucial transition from preschool to early school age, when the presence of sustained elevated irritability predicts psychiatric disorders, increased impairment, and service use in school-age children. The goal of this study

Trauma-Informed Understanding of Depression Among Justice-Involved Youth.

The association between adverse childhood experiences (ACEs) and depression has been the focus of a number of prevalent studies in recent years-particularly among high-risk youth. Depression remains a significant mental health issue among justice-involved youth. There is a well-established correlation between depressed mood and conduct problems (e.g., conduct disorder and oppositional defiant disorder) during childhood and adolescence, which tends to become more prevalent during adolescence. Studies of justice-involved youth reveal high prevalence rates of depression and other mood disorders.

Towards improved specificity in mental health syndromes: projection-based clustering of depressive phenotypes.

BACKGROUND: Quantitative attempts to improve syndrome specificity typically produce large heterogenous subgroupings, impacting the validity of treatment and research targets. Assessing barriers to the valid application of existing methods and examining improvements from an interpretable projection-based clustering alternative may improve the precision and reproducibility of our research targets and classification systems. METHODS: This exploratory, cross-sectional, study recruited 2820 participants aged 12-to-25 years, from primary-healthcare services in Australia, between November 2018 and J

Successful treatment of five cases of catatonia treated with guanfacine without ECT: a case series from a psychiatric hospital in Japan.

BACKGROUND: Catatonia, including malignant catatonia, is a severe neuropsychiatric condition historically associated with schizophrenia or schizoaffective disorder. However, recent diagnostic frameworks, such as the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision (DSM-5-TR) and the International Classification of Diseases, 11th Revision (ICD-11), offer a broader understanding. These systems classify catatonia not as a standalone diagnosis but as a specifier that may occur alongside various psychiatric disorders, including schizophrenia spectrum disorders, mo

Systematic review and meta-analysis of the effectiveness of ECT in reducing suicidal ideation, self-harm, suicide, and mortality.

Suicide and self-harm in people with depression are major public health concerns; electroconvulsive therapy (ECT) is a treatment recommended in UK clinical guidelines for severe mood disorders. We aimed to investigate published literature on the effect of ECT on the incidence of suicide, self-harm, and the recorded presence of suicidal thoughts (suicide-related outcomes). We hypothesized that ECT would be associated with a reduced incidence of suicide-related outcomes and all-cause mortality. We reviewed systematically all eligible studies as specified in our protocol (PROSPERO 293393). We inc

Real-world analysis of pharmacological treatments to prevent relapse after electroconvulsive therapy for major depressive disorder: A nationwide cohort study.

During the first year following electroconvulsive therapy (ECT) for major depressive disorder (MDD) there is a large risk of relapse. Previous studies have shown favourable results for lithium after ECT for MDD. However, lithium is infrequently prescribed after ECT. While some evidence exists for other pharmacological strategies such as TCAs and TCA-lithium combinations, comparative data remain limited. The aim of this study was to explore pharmacological treatments after ECT for MDD and analyse their association with relapse following response to ECT for MDD. We hypothesized that lithium woul

The use of antidepressants for bipolar disorder - a controversy between science and clinical practice.

Depressive episodes are more challenging for patients and clinicians than other bipolar disorder episodes. The reasons for this include: 1. the prevalence of depressive episodes is higher than for other episodes; 2. functioning; 3. cognition is more impaired; 4. suicide is more prevalent; 5. and treatment is more complex. This paper aims to highlight the controversy between the poor evidence from science on the effects of antidepressants for bipolar depression versus the high use of antidepressants in clinical practice, and to specify the clinical role of antidepressants in bipolar disorder in

Suicidal Ideation in Adult Women: The Unique Roles of Binging, Purging, and Restricting.

OBJECTIVE: Suicidal ideation represents a serious experience common in individuals with eating disorders. To inform screening and clinical assessment and advance theoretical work, the current study compared clinical characteristics between adult women with and without current suicidal ideation and examined whether binging, fasting, and purging behaviors are uniquely associated with suicidal ideation above and beyond existing psychiatric diagnoses. METHOD: Data come from four studies conducted between 2000 and 2023 (N = 732) that evaluated current suicidal ideation in all participants. Partici
