Multi-Dot/Icm effector mutants reveal distinct host responses in Legionella infected macrophages.

Legionella pneumophila is the causative agent of Legionnaires' disease, an accidental and severe form of pneumonia. The replication of L. pneumophila within alveolar macrophages requires the Dot/Icm type IV secretion system that translocates more than 330 bacterial effector proteins into the infected host cell. Many Dot/Icm effectors possess eukaryotic-like domains and manipulate host processes by functional mimicry. However, only a minority of effectors have been fully characterized. Redundancy among effector protein activities and host targets has complicated functional characterization of i

Bacterial effector modifies host messenger RNA to promote virulence.

Bacterial pathogens harbor specialized secretion systems that inject effector proteins into the host cell to establish infection and disease. While many bacterial effectors post-translationally modify proteins to influence host responses, the extent to which effectors modify host RNA is currently unknown. Here we performed RNA-interactome capture (RIC) to isolate effectors bound to host cellular messenger RNA (mRNA) during Legionella pneumophila infection. RIC identified an uncharacterized effector, FadA (Lpw16921), which localized to the host-cell nucleus and interacted with host mRNAs at ura

Impact of urine concentration and automatic interpretation on sensitivity and specificity of Legionella BinaxNOW® results.

PURPOSE: Urinary antigen tests (UATs) are widely used for rapid diagnosis of Legionnaires' disease (LD), primarily detecting Legionella pneumophila serogroup 1 lipopolysaccharide (LPS). Strategies, such as urine concentration, were shown to improve performance but mostly in retrospective studies. METHODS: We evaluated the impact of urine concentration on the diagnostic sensitivity and specificity of the BinaxNOW® assay. A set of 90 positive and 200 negative urine samples were prospectively collected. Another set of 48 positive urine samples were selected retrospectively to evaluate both urine

Encrypting the message: Bacterium turns host RNA into a target.

Pathogenic bacteria are remarkably inventive when it comes to manipulating their hosts. In this issue of Cell Host & Microbe, McCaffrey et al. identify a previously uncharacterized bacterial effector from Legionella pneumophila that modifies not host proteins but messenger ribonucleic acid (mRNA) to dampen the host inflammatory response.

Metagenomic next-generation sequencing reveals airway microbial dysbiosis and functional alterations in Legionella pneumophila pneumonia.

BACKGROUND: Legionella pneumonia (LP), primarily caused by Legionella pneumophila, is a clinically important subtype of community-acquired pneumonia (CAP) associated with substantial morbidity and mortality. However, the microbial characteristics and functional profiles of LP remain insufficiently understood. METHODS: In this study, metagenomic next-generation sequencing (mNGS) was used to compare airway microbial communities between patients with LP and those with non-Legionella CAP. RESULTS: A total of 60 patients were included, comprising 23 patients with LP patients and 37 patients with

A surge of Legionnaires' disease at a tertiary hospital in southern Taiwan, 2024: the potential diagnostic pitfall of urinary antigen testing.

We reported a surge of Legionnaire's disease at a tertiary hospital in Taiwan in 2024, partially attributed by falsely positive Legionella urinary antigen testing (UAT) results. Of 20 patients with positive UAT results and complete investigations, only seven (35%) had legionellosis confirmed by Legionella growth, nucleic acid detection, or seroconversion.

Ex vivo functional immune alterations in ICU patients with Legionnaires' Disease, a proof-of-concept study.

OBJECTIVES: Legionnaires' disease (LD), caused by Legionella pneumophila intracellular bacterium, leads to intensive care unit (ICU) admission in 20-40% of cases. While ICU-LD patients display lung injury or septic shock, their functional immune response remains poorly understood. This proof-of-concept study assessed ex vivo immune gene expression in ICU-LD and non-ICU-LD patients. METHODS: Whole blood from ICU-LD (n=6), non-ICU-LD (n=8), LD-unrelated septic shock (n=14), and healthy volunteers (n=9) was stimulated with lipopolysaccharide (LPS). Expression of 93 immune-related genes was quant

Genomic and phenotypic characterization of a novel recombinant Legionella pneumophila lineage associated with long-term persistence in a building water system and recurrent legionellosis outbreaks.

Understanding Legionella pneumophila microevolution within building water systems is crucial for effective long-term water safety plans. We investigated an exceptional 20-year contamination case within a closed, state-managed residential facility where routine engineering controls, including hyperchlorination and thermal shocks, systematically failed. Integrating genomic surveillance, transcriptomics, 3D biofilm characterisation and cell infection models, we show this persistence was driven by the in situ emergence of a novel recombinant strain (ST3043), which achieved over 80% dominance. Phyl

Pathogen genome enrichment sequencing to support a city-wide Legionnaires' disease outbreak investigation.

Pathogen genomics can provide high-confidence source attribution for epidemiological investigations of Legionnaires' disease (LD) outbreaks. However, the utility of culture-based genomics can be undermined by long lead times for Legionella pneumophila culture or a lack of isolates when dealing with non-culturable bacteria or low-biomass specimens. Here, during a large 2024 LD outbreak in Melbourne, Australia, we compared direct specimen, hybridization-based enrichment sequencing with traditional culture-based whole-genome sequencing of 18 clinical and 5 environmental L. pneumophila specimens.

Sequential Talaromyces marneffei and Legionella pneumophila infections leading to the diagnosis of anti-interferon-γ autoantibody-associated immunodeficiency: a case report.

BACKGROUND: Opportunistic infections caused by uncommon intracellular pathogens can serve as important clues to underlying immune dysfunction. CASE PRESENTATION: We report a 57-year-old male who presented with chronic cough, dyspnea, fever, and weight loss. Chest imaging revealed diffuse pulmonary infiltrates and mediastinal lymphadenopathy, mimicking tuberculosis or lymphoma. Despite empirical anti-tuberculosis and broad-spectrum antibiotic therapy, his condition deteriorated. Metagenomic next-generation sequencing (mNGS) and culture confirmed Talaromyces marneffei infection. During antifung

Burn injury is often a devastating event with long-term physical and psychosocial effects. Burn scars after deep dermal injury are cosmetically disfiguring and force the scarred person to deal with an alteration in body appearance. In addition, the traumatic nature of the burn accident and the painful treatment may induce psychopathological responses. Depression and post-traumatic stress disorder (PTSD), which are prevalent in 13-23% and 13-45% of cases, respectively, have been the most common areas of research in burn patients. Risk factors related to depression are pre-burn depression and fe

Among women around the globe, breast cancer is both the most common cancer and the leading cause of cancer-related death. Women in economically disadvantaged countries have a lower incidence of breast cancer, but poorer survival rates for the disease relative to women in affluent countries. Evidence suggests that breast cancer mortality can be reduced if resources are applied to the problem in a systematic way. The purpose of the Global Summit Consensus Conference was to begin a process to develop guidelines for improving breast health care in countries with limited resources-those with either

Narcolepsy is a disorder of impaired expression of wakefulness and rapid-eye-movement (REM) sleep. This manifests as excessive daytime sleepiness and expression of individual physiological correlates of REM sleep that include cataplexy and sleep paralysis (REM sleep atonia intruding into wakefulness), impaired maintenance of REM sleep atonia (e.g. REM sleep behaviour disorder [RBD]), and dream imagery intruding into wakefulness (e.g. hypnagogic and hypnopompic hallucinations). Excessive sleepiness typically begins in the second or third decade followed by expression of auxiliary symptoms. Only

Identifying people's energy information needs through Energy Citizen Personas.

UNLABELLED: Renal tuberculosis (TB) is an extrapulmonary form of TB, and is becoming more common and challenging to treat. This is because many anti-tubercular drugs have nephrotoxicity and have poor drug penetration into renal tissues. These problems highlight the urgent need for safer, kidney-targeted treatment regimes, particularly in patients with compromised kidney function. Thus, this study investigates lysozyme, which is a low-molecular-weight protein and has natural kidney-affinity, as a potential carrier for targeted renal drug delivery. Here, a comprehensive in silico approach has be

Cardiogenic shock following an acute coronary syndrome (ACS) continues to be associated with significant mortality despite modern reperfusion strategies and inotropic support. There is mounting evidence that an acute inflammatory response accompanies the well documented decrement in left ventricular systolic function associated with cardiogenic shock and that this response may affect outcomes. In the past 2 decades it has also become apparent that nitric oxide (NO), a heteroatomic free radical has numerous biologic activities, among them the maintenance of vascular tone. The production of NO i

Assessment of physicians' awareness and clinical practice regarding gingival enlargement caused by calcium channel blockers.

BACKGROUND: Gingival enlargement is a side effect of calcium channel blockers (CCBs), which are usually used to treat cardiovascular conditions. Physicians have an important role in identifying and managing the disease, but their level of awareness and ability to apply their knowledge in clinical practice remain unclear in Iraq. OBJECTIVE: This study aims to assess physicians' knowledge of calcium channel blocker (CCB)-induced gingival enlargement and determine whether they apply this knowledge in their clinical practice. METHODS: A cross-sectional survey was conducted among 331 physicians w

Occupational noise exposure and ischemic heart disease: a pooled analysis of six Nordic cohorts.

BACKGROUND AND STUDY AIMS: Is there a post-acute infection syndrome for Legionnaires' disease? Legionnaires' disease is a form of primarily community-acquired pneumonia caused by Legionella spp. bacteria. Legionnaires' disease and other forms of bacterial community-acquired pneumonia may lead to persistent health and wellbeing impairments. It remains unclear whether these are caused by the community-acquired pneumonia-causing pathogen or the pneumonia itself. We present the rationale and design of a matched cohort study to investigate the persistent health impacts of Legionnaires' disease and
