Psychoimmunoendocrine aspects of panic disorder.

Immunological, neuroendocrine and psychological parameters were examined in 14 psychophysically healthy subjects and in 17 panic disorder patients before and after a 30-day course of alprazolam therapy. T lymphocyte proliferation in response to the mitogen phytohemagglutinin, lymphocyte beta-endorphin (beta-EP) concentrations, plasma ACTH, cortisol and beta-EP levels were examined in basal conditions and after corticotropin-releasing hormone (CRH) stimulation. Cortisol inhibition by dexamethasone (DST) and basal growth hormone (GH) and prolactin levels were also examined. Depression, state or

[Possibilities in therapy of anxiety disorders in DSM-III-R].

Between 2% and 5% of the population are affected by anxiety disorders. Several studies have shown that imipramine possesses anti-panic and anti-phobic properties. Its response rate is at least 70%. In panic disorder with agoraphobia it is therefore the medication of first choice. Agoraphobia, simple and social phobias are highly responsive to behavioural therapy (desensitization, flooding, social skills training, cognitive restructuring). Drugs have not been found useful in controlled studies. In conjunction with the tricyclic antidepressant clomipramine, behavioural therapy is superior to oth

Pilot study of anxiety and depression in prepubertal children.

Early detection of anxiety and depression is important because of the relationship between these disorders, suicide and other problems (i.e. social, phobias, learning problems). This study screened prepubertal students in a school setting for symptoms of anxiety and depression using self-report measures (Revised Children's Manifest Anxiety Scale and the Children's Depression Inventory). Controls and children who scored high on measures of anxiety or depression or both were interviewed with their parents and were tested using the Diagnostic Interview for Children and Adolescents (computer versi

[Beta-blocking drugs and anxiety. A proven therapeutic value].

The therapeutic value of propranolol and other beta-adrenergic receptor blockers in anxiety disorders is reviewed. The potential side-effects of benzodiazepines, eg psychomotor impairment and dependance, suggest that the main indicators of these compounds are the acute anxiety states and the short term treatment of anxiety disorders requiring sedation. Thus, other psychotropic drugs may be of potential value in the treatment of these disorders. The results of the placebo-controlled studies reviewed here strongly suggest that propranolol is useful in the treatment of anxiety disorders, especial

Psychologic aspects of acne.

Psychologic factors may play a significant role in acne in at least three ways. First, as many patients readily report, emotional stress can exacerbate acne. Second, it is common for patients to develop psychiatric problems as a consequence of the conditions, such as those related to low self-esteem, social phobias, or depression. Finally, primary psychiatric illnesses such as obsessive-compulsive disorder and psychosis may be based on a complaint that is focused on acne. In this review we hope to facilitate an understanding of how emotional factors and acne connect, and aid the dermatologist

Levels of urinary free cortisol in social phobia.

Levels of urinary free cortisol were measured in 10 patients with social phobias and in 15 age- and sex-matched normal controls. No differences were found either in cortisol levels or in the ratio of free cortisol to creatinine. These nonsignificant differences between groups do not necessarily rule out the possibility that the hypothalamic-pituitary-adrenal axis may be altered in individuals with social phobia.

Social phobia: the Anxiety Disorders Associated of America helps raise the veil of ignorance.

Social phobias affect some 2.4 million American adults, and more than 5 million can expect to develop a social phobia during their lifetime. Despite their prevalence, social phobias have been virtually ignored until this past decade. The Anxiety Disorders Association of America (ADAA) was founded in 1980 to promote awareness among professionals and the public of anxiety disorders. This paper outlines the objectives of ADAA and its programs, including the Self-Help Group Network, helpful publications, and its partnership with psychiatrists and patients. The benefits of self-help programs to per

Treatment of social phobias.

Social phobia, despite a prevalence in the general population of 1.5% denoting a common disorder, has been relatively neglected from the viewpoints of psychopathology and of treatment. Two subtypes of social phobia have been differentiated: specific (characterized by anxiety in one situation, e.g., public speaking) and generalized (characterized by anxiety in several social situations). The syndrome is frequently complicated by alcohol abuse or dependence. Among the treatment targets are symptoms of anxiety, avoidance behavior, negative cognitions concerning the reactions of others, and, less

Panic and phobic disorders in patients with obsessive compulsive disorder.

Obsessive compulsive disorder shares numerous clinical features with other anxiety disorders. To study the relationship between OCD and other anxiety disorders, the authors administered the Structured Clinical Interview for DSM-III to 36 OCD patients. Thirty-nine percent (14) of patients reported a lifetime history of panic attacks, and 14% (5) met DSM-III-R criteria for panic disorder at the time of interview. Fourteen percent (5) met criteria for social phobias, and 19% (7) met criteria for simple phobias. Eighteen patients were treated with clomipramine in doses of at least 100 mg/day for 3

Psychiatric diagnosis in an outpatient population. A comparative study of PSE-Catego and DSM-III.

Two diagnostic systems are compared in a psychiatric outpatient population of 175 patients. The Present State Examination (PSE)-Catego system identified 121 patients (69%) as "cases," whereas DSM-III identified 152 patients (87%) as cases. The two systems converged in 115 patients, yielding a kappa coefficient of only .32. Approximately one third of the DSM-III cases that were not detected by the PSE-Catego system was due to the restricted coverage of the latter system; the remaining two thirds could be attributed to differences in threshold and time framing. Compared with DSM-III, the PSE-Cat

Social phobias: behavioural and pharmacotherapy.

Five to 10% of the general population suffers from symptoms of an anxiety disorder. Social phobias, while less common than panic disorder, agoraphobia or simple phobias, are just as debilitating. Patients present with somatic, behavioural, mood and cognitive disturbances, of which unrecognized social isolation, depression, loss of employment, and drug and alcohol abuse can be the result. A symptomatic approach can be implemented through the use of education, insight, support, behaviour therapy, cognitive therapy, and pharmacological intervention.

Anxiety disorders in Japan: a review of the Japanese literature on shinkeishitsu and taijinkyofusho.

As culture-bound syndromes, Japanese shinkeishitsu ("constitutional neurasthenia") and taijinkyofusho ("anthropophobia") have received considerable attention in the Japanese literature. While these disorders are viewed as diagnostically distinct from Western psychiatric categories, recent studies by the Japanese suggest some affinity with Western social phobias, depression, and schizophrenia. The paper reviews this literature and offers suggestions for further cross-cultural research.

The new and newer antianxiety agents.

Anxiolytic drugs are adjuncts to non-pharmacological treatments for anxiety. Alcohol and tobacco remain the major psychoactive agents that are used in our community. Benzodiazepine drugs are the agents of choice if an acute reduction in anxiety or intermittent therapy is needed and are helpful for long-term use in a few patients. For panic disorders, alprazolam is effective for short- and long-term treatment, although it needs a slow reduction in dosage and carries a risk of withdrawal reactions in about 30% of sufferers. Clonazepam also may help panic attacks and possibly other benzodiazepine

Four types of social phobia in a community sample.

Point prevalence rates and demographic characteristics associated with four specific forms of social phobia (public speaking/performing, writing in front of others, eating in restaurants, and use of public restrooms) were examined in a sample of adult residents of the greater St. Louis area. Diagnoses were determined by structured interview in accordance with DSM-III criteria. An unadjusted prevalence rate of 22.6% was found for all four social phobias combined. Application of DSM-III significant distress criteria resulted in a prevalence rate of 2.0%. Public speaking/performing phobias were b

The relationship between agoraphobia, social phobia and blood-injury phobia in phobic and anxious-depressed patients.

This paper reports the results of principal components and stepwise discriminant analyses of anxiety, depression and fear scores for 74 phobic and anxious-depressed psychiatric patients. Factor analysis indicated a coherent agoraphobia factor, with less coherent blood-injury and social phobia factors. Discriminant analysis showed a high degree of correct classification of diagnosed agoraphobic, blood-injury and social phobic patients particularly for agoraphobia. A frequency distribution of the phobia scores indicated an all or nothing quality to agoraphobic fears. The results indicate that ag

Prevalence of some symptoms in adolescence and maturity: social phobias, anxiety symptoms, episodic illusions and idea of reference.

In order to compare the prevalence of certain symptoms in adolescence with those at other ages, 6,034 apparently normal subjects aged 9-60 years were studied by questionnaire. Fear of blushing and fear of being looked at showed a maximum in the mid-teens and a subsequent gradual decrease both in males and females. They were more common in females at most ages. The anxiety symptoms also showed a peak in the mid-teens but there was a period of relative increase in the forties. Episodic illusions and the idea of being talked about started to decrease with age in adolescence.

Seventy-three agoraphobic and 31 social phobic women, all rated unsuitable for insight-oriented psychotherapy, were compared regarding family and personal history, intelligence, personality and factors pertaining to the disorder. The same patients, with 11 men included in the social phobic group, were also compared regarding response to four randomly assigned types of treatment given over a 3 month period, with a 9 months' follow-up. The following differences were revealed: 1) Social phobias were associated with a higher social class of the parental home, higher education, higher scores on ver

Alcohol dependence and phobic anxiety states. II. A retrospective study.

Twenty-four hospitalised problem drinkers who were currently suffering from a phobic anxiety state served as subjects for this study. Each was given a detailed retrospective interview concerned with the development over time of both their alcohol dependence and phobic problems. The test-retest and inter-rater reliabilities of the data collected were found to be high. In accordance with previous experimental work, periods of heavy drinking and dependence upon alcohol were associated with an exacerbation of agoraphobia and social phobias. Subsequent periods of abstinence were associated with sub

The treatment of social phobia. Real-life rehearsal with nonprofessional therapists.

Sixteen socially phobic outpatients underwent a 4-week course of social skills training that attempted to maximize rehearsal in real-life settings. The therapy was largely conducted by nonprofessional volunteers after a 2-hour training workshop. It consisted of multiple role-played practice in the clinic, followed immediately by rehearsal in a real-life setting in the company of a nonprofessional therapist and a fellow patient. Subsequently, patients were randomly paired to perform further rehearsals between sessions. Drug therapy was controlled by double blind assignment to propranolol or ine
