[State-sponsored doping in GDR elite sport: health consequences, reappraisal, and questions of legal compensation].

Against the backdrop of the ideological rivalry between communism and capitalism, sport in the GDR was exploited for political ends. This was evident, among other things, in a state-directed, controlled, hierarchically organized and, in parts, totalitarian competitive sports system. A key component was a conspiratorial, state-controlled program of compulsory doping, which was systematically implemented from the 1970s onwards and affected an estimated 15,000 underage athletes. Performance-enhancing substances, primarily anabolic-androgenic steroids, were administered covertly, without the knowl

Expectancy updating predicts anxiety symptom reduction from exposure therapy: Predictive analyses from a randomized clinical trial.

Our objective was to assess predictors of exposure therapy outcome for social anxiety disorder and panic disorder. Specifically, we investigate whether expectancy updating or fear change during exposures are predictive of long-term anxiety reduction. We conducted a randomized controlled trial of 84 participants evaluating two forms of 9-session exposure therapy. One form of exposure therapy was based on the inhibitory retrieval model (i.e., expectancy updating), and the other was based on habituation principles (i.e., fear reduction). Our treatment outcome measures of anxiety symptomatology in

A Network Analysis of Panic Disorder, Agoraphobia, and Generalized Anxiety Disorder in 463 Patients From a Psychiatric Hospital.

INTRODUCTION: Panic disorder, agoraphobia, and generalized anxiety disorder (GAD) frequently co-occur and share overlapping symptoms, yet it remains unclear whether they reflect distinct or interconnected symptom systems. This study examined the network structure of these disorders using clinician-administered diagnostic data. METHODS: A total of 463 adults completed the Mini International Neuropsychiatric Interview (M.I.N.I.) conducted by trained clinicians. Eighteen items from the panic disorder, agoraphobia, and GAD modules were retained after a multistep selection procedure ensuring clini

Videoconferencing Cognitive Behavioral Therapy for Panic Disorder in an Emergency Room in Mexico: A Multiple Baseline Design.

The aim of this study was to evaluate the effectiveness of a brief cognitive behavioral therapy intervention conducted via videoconference for adults with panic disorder and agoraphobia in the emergency room (ER) of a Mexican hospital. We examined the impact of the intervention on quality of life, emergency room utilization, and participants' satisfaction with the intervention and the use of telepsychology. A multiple-baseline single-case experimental design was employed. Sixteen adults diagnosed with panic disorder were assigned to one of four cohorts and received seven individual therapy ses

Aberrant emotional conformity among individuals with social anxiety.

Social anxiety (SA) is characterized by heightened fear of negative evaluation and excessive motivation for social belonging, yet the neurocognitive mechanisms underlying aberrant emotional conformity-defined as adjusting one's emotional responses to align with others-remain unclear. This study combined behavioral tasks and event-related potentials (ERPs) to investigate how social anxiety modulates emotional conformity during social scene evaluations. Forty-four participants (22 high SA, 22 low SA) provided subjective affective ratings of neutral and negative social scenes based on imagining t

Beliefs about relationships between symptoms and experiences in daily life: Evidence from experience sampling and self-report in individuals with or without major depression or social phobia.

In clinical research and practice, individuals are frequently asked to evaluate how their experiences are related (e.g., "Does feeling down affect your interactions with others?"). These estimations are often used to inform diagnostic and therapeutic decisions. To avoid faulty clinical inferences, it is therefore important to understand whether such self-reported relationships correspond to associations observed in real-time data, and whether any discrepancies are particularly pronounced in specific clinical populations. In this study, 118 individuals with major depressive disorder, 47 with so

Music-based interventions for chronic pain management.

This is a protocol for a Cochrane Review (intervention). The objectives are as follows: Primary objective To evaluate the benefits and harms of music-based interventions (MBIs) for chronic pain management in adults with pain lasting more than 12 weeks, comparing MBIs with the following comparators: active care, placebo, or no treatment/waitlist control. Specifically, we aim to assess the effects of MBIs on critical outcomes, such as pain intensity, pain-related distress, and pain interference with daily functioning. Secondary objectives To evaluate the effects of MBIs on important outcomes, in

Overcoming dental phobia in a pregnant woman for nonsurgical management of pregnancy epulis.

Dental phobia (DP) is defined as a specific phobia, which is the fear or anxiety circumscribed to a particular situation or object in the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision. When an individual with a specific phobia encounters the phobic object or situation, they could feel startled or experience fear or anxiety. DP often results from a distressing dental experience, a lack of understanding by the dentist, the noises from the dental clinic and other factors. DP leads to avoidance of dental clinic visits, ultimately resulting in worsening dental

Understanding Blood-Injection-Injury Phobia: Do Not Miss the Point.

Needle and blood-injection-injury phobia is commonly encountered in the perioperative setting. It can significantly disrupt operating room throughput, increase healthcare costs, cause delays in care, and lead to patient disengagement with the healthcare system. These phobias, which involve intense fear and anxiety about needles and blood, can increase procedural risks, requiring interventions such as anxiolysis, sedation, and alternative behavioral health interventions to manage patient distress. Blood-injection-injury phobia is notably prevalent in females, with most individuals developing it

Modifying avoidance in social anxiety: The effects of a cognitive-behavioural instruction on extinction learning.

Although cognitive-behavioural therapy (CBT) with exposure is the first-line treatment for Social Anxiety Disorder (SAD), its efficacy is lower for SAD than for other anxiety disorders. One possible reason is that CBT may not effectively target the key maintaining features of SAD: persistent avoidance and heightened threat expectancy. Experimental research shows that individuals with elevated social anxiety exhibit generalised avoidance and persistent threat beliefs in conditioning paradigms, yet it remains unclear whether brief cognitive-behavioural (CB) strategies can modify these fear respo

Distinct Longitudinal Contributions of Social Anxiety and Self-Consciousness to Interpersonal Dysfunction.

We examined whether social anxiety (SA) and self-consciousness (SC) predicted unique variance in functioning among young adults with internalizing symptoms. We hypothesized that each would independently predict poorer interpersonal functioning and quality of life cross-sectionally and over 12 months. Participants (N = 152; ages: 18-30) included individuals with elevated depression and anxiety symptoms as well as healthy controls. At baseline, participants completed measures of SA, SC, and assessments of quality of life, social/leisure satisfaction, and four domains of interpersonal problems, w

Beyond Fear: Disgust, Anger, and the Affective Core of Interpersonal Phobias.

Interpersonal phobias-used here as an analytic label for enduring aversive orientations toward persons or groups, such as xenophobia, homophobia, transphobia, racism, and ageism-are often framed primarily as fear- or anxiety-driven. That view captures vigilance and avoidance, but it does not fully explain why such aversions are also expressed in the language of contamination, revulsion, exclusion, social cleansing, and sometimes violence. I argue that interpersonal phobias are better understood through a distinction among fear, anger, and disgust than through a fear-centered account alone. Fea

Predictors of Physical Activity Early After Anterior Cruciate Ligament Reconstruction.

CONTEXT: Individuals often demonstrate reduced physical activity (PA) between 6 months and 5 years after anterior cruciate ligament reconstruction (ACLR), but little is known about PA levels during the first 6 months postoperatively. OBJECTIVE: To identify factors that predict PA in this early period. DESIGN: Prospective cohort study. SETTING: Participants were recruited from an academic health science center and a private sports medicine clinic and assessed in a clinical and research setting. PATIENTS OR OTHER PARTICIPANTS: Sixty-three participants aged 13 to 35 years who underwent ACLR (

Mapping relative proximity within an internalizing symptoms network.

Understanding how symptom domains relate to one another (e.g., whether generalized anxiety is more similar to depression than to panic) is central to psychiatric classification. We demonstrate a procedure for quantifying relative proximity, defined as whether one symptom domain is statistically closer to another compared to other domains. As a motivating application, we examine the structure of internalizing symptoms characterized by fear, distress, avoidance, and heightened arousal. Using a network approach, we analyzed data from six questionnaires assessing depression, generalized anxiety di

Feasibility of assessing implicit motor imagery after dry needling in bilateral patellofemoral pain with kinesiophobia.

BACKGROUND: Patellofemoral pain (PFP) has been associated with altered central processing and implicit motor imagery, as assessed using left-right judgment tasks. However, the feasibility of assessing implicit motor imagery performance in individuals with bilateral PFP and high kinesiophobia, particularly with a dry needling intervention, has not been established. METHODS: This feasibility pilot evaluated the practicality and safety of administering an implicit motor imagery assessment before and after dry needling. Ten participants with chronic bilateral PFP and elevated kinesiophobia comple

Effectiveness of telerehabilitation in managing chronic low back pain: a pragmatic randomized controlled non-inferiority trial.

BACKGROUND: For patients with chronic low back pain (CLBP), telerehabilitation (TR) has been suggested as an alternative to conventional rehabilitation. However, evidence from pragmatic trials comparing TR with routine clinical rehabilitation is still limited. OBJECTIVES: To investigate whether TR is non-inferior to conventional in-person rehabilitation for pain intensity in individuals with CLBP. Secondary objectives included disability, pain catastrophizing, quality of life, and kinesiophobia. METHODS: A pragmatic non-inferiority randomized controlled trial was conducted with 50 adults wit

Emotional body odors alter autonomic nervous activity during mindfulness training in social anxiety.

Chemosignals found in human body odors (BOs) convey information about a person's emotional state and elicit observable responses in others. While mindfulness typically enhances parasympathetic tone and reduces anxiety, little is known about how BOs modulate these effects. This study investigated how emotional and non-emotional human BOs influence autonomic nervous system activity - heart rate variability (HRV) and electrodermal activity (EDA) - during mindfulness practice in individuals with social anxiety. The role of baseline HRV as a predictor of intervention outcome was explored. Ninety-ei
