Comparing childhood, chronic, and recent adversity on internalizing symptoms and hypothalamic-pituitary-adrenal axis functioning.

OBJECTIVE: Examine how childhood, chronic, and recent adversity associate with internalizing symptoms and hypothalamic-pituitary-adrenal axis activity among women in a low-resourced context. METHOD: Data come from the Bachpan Study (2014-2022), a longitudinal cohort of mothers in rural Pakistan. Childhood adversity was assessed using retrospective reports of adverse childhood experiences. Chronic adult adversity was defined as adversities experienced across two or more prior study waves (baseline, 1 year, 2 years, and 3 years), and recent adversity was assessed at the 6-year wave. Outcomes, m

Symptoms in Persons With Either Active or Inactive Crohn's Disease Are Agnostic to Disease Phenotype: The Magic in Imagine Study.

BACKGROUND: We aimed to examine the relationship between disease symptoms and disease phenotype in a large Canadian cohort of persons with Crohn's disease (CD). METHODS: Adults (n=1515) with CD from 14 Canadian centers participated in the Mind And Gut Interactions Cohort (MAGIC) between 2018 and 2023. Disease activity was measured using the 24-item IBD Symptom Inventory-Short-Form (IBDSI-SF). We compared the symptoms commonly associated with active versus inactive disease, and explored symptoms patterns in relation to disease phenotype, based on the Montreal Classification. To assess psycholo

Do cervical medial branch blocks have a therapeutic role? a prospective cohort study.

BACKGROUND: Cervical medial branch blocks (CMBB) are used for the diagnosis of facet joint-related pain. There have been several reports suggesting that they can provide a benefit that significantly outlasts the expected duration of diagnostic blocks. We undertook this study to determine the frequency and extent of this effect in clinical practice. METHODS: 179 patients undergoing 830 individual block levels and who had previously reported 50% relief following a CMBB were recruited using a prospective cohort design and blocks were performed in an outpatient setting. Outcomes monitored at base

Blind Source Separation-Embedded Electroencephalogram Microstate Trajectory Modeling for Generalized Anxiety Disorder Identification.

Generalized anxiety disorder (GAD) diagnosis remains challenging due to lacking reliable biomarkers. Electroencephalography(EEG) microstate analysis shows promise in detecting GAD-related neural dynamics, but its clinical application is limited by insufficient spatial resolution and sensitivity. To address this challenge, we propose a novel framework integrating fast independent component analysis (FastICA) with microstate analysis to enhance spatial specificity in EEG signal decomposition, consequently, GAD can be more accurately identified.By isolating dominant independent components and proj

Noninvasive Suppression of Responses to Threat Using Focused Ultrasonic Waves.

The processing of threat- and fear-related responses is known to involve the amygdala and the bed nucleus of the stria terminalis. Malignant activity in these nuclei is associated with several important disorders, including generalized anxiety disorder and post-traumatic stress disorder. A noninvasive approach to modulate these deep brain circuits could provide novel treatment options for these disorders, but current noninvasive approaches cannot access these targets directly. This study modulates these nuclei using transcranial low-intensity focused ultrasound in nonhuman primates. The ultras

Smartwatch-Based Unobtrusive Continuous Anxiety Tracker for Evaluating Post-Stroke Patients' Quality of Life.

Post-stroke anxiety (PSA) affects 20-30% of stroke survivors and significantly impacts quality of life (QoL) and rehabilitation outcomes. Traditional emotion assessment relies on subjective self-reports, limiting to capture real-time fluctuations in emotional states. This study proposes a self-supervised learning (SSL) framework combined with a transformer-based emotion classifier to enable continuous anxiety tracking in patients with stroke using smartwatch-derived photoplethysmography (PPG). The SSL model was pretrained on the VitalDB dataset using R-peak-to-peak intervals (RRIs) extracted f
