Investigating the relationship between intolerance of uncertainty and threat extinction learning whilst controlling for anxiety-related symptoms.

Exposure-based therapies operate on principles of threat conditioning and extinction. Previous research has demonstrated that trait-level intolerance of uncertainty (IU) is specifically associated with worsened extinction learning whilst controlling for trait anxiety. Hence, IU may serve as a useful focus for researchers aiming to improve the effectiveness of exposure-based therapies as research suggests that IU can be modified clinically. Yet, little is known as to whether IU is associated with extinction learning whilst controlling for anxiety-related symptoms. A two-day Pavlovian conditioni

Detached mindfulness as a stand-alone intervention: a Systematic Review and meta-analysis.

BACKGROUND: Detached Mindfulness (DM) is a central, transdiagnostic technique within Metacognitive Therapy (MCT). It involves increasing meta-awareness of intrusions while decentring from and disengaging with them, and is used to reduce the Cognitive Attentional Syndrome and dysfunctional metacognitive beliefs-key components of the metacognitive model of psychological disorders. Although DM is typically delivered within a full MCT protocol, recent research has begun to evaluate DM as a stand-alone intervention. The current study aimed to systematically review and meta-analyse its effects. MET

Cyberchondria and Anxiety Sensitivity in Patients with Panic Disorder: A Case-Control Study.

Background and Objectives: Cyberchondria (CYB) has been associated with health anxiety and anxiety sensitivity (AS); however, its role in panic disorder (PD) remains unclear. This study aimed to compare CYB and AS levels between patients with PD and healthy controls and to examine their associations with PD severity. Materials and Methods: This cross-sectional case-control study included 71 patients with PD and 69 age- and sex-matched healthy controls. Participants completed the Cyberchondria Severity Scale (CSS), Anxiety Sensitivity Index-3 (ASI-3), and Beck Anxiety Inventory (BAI). PD severi

Hyperarousal transdiagnostically dissected: different dimensions characterize mood, anxiety, insomnia, posttraumatic stress and attention deficit hyperactivity disorders.

BACKGROUND: Hyperarousal is a common symptom key to the severity of insomnia-, depression-, anxiety-, posttraumatic stress- and attention deficit/hyperactivity disorders. Hyperarousal however remained a loosely defined construct assessed with different questionnaires in different disorders. Here we addressed the unresolved question whether hyperarousal may be one common transdiagnostic construct or rather has multiple, possibly disorder-specific, dimensions. METHODS: In this cohort study, participants were recruited through media and from the Netherlands Sleep Registry between Dec 2023 and Ju

Exploration of wearable sensor measures associated with panic attacks differs across mental health conditions.

Panic attacks (PAs) are acute anxiety episodes that are pervasive, with one in 10 individuals having experienced a PA in the past year. PAs impair daily functioning and are associated with an increase in emergency room visits and suicide attempts. Despite their impact, the unpredictable nature of PAs makes them challenging to manage. PAs are transdiagnostic, occurring in individuals across and without a mental health diagnosis. However, prior work has largely focused on PA indications within individuals with panic disorder. This study identifies PA risk factors from over 6 months of passive se

Measuring approach-avoidance behavior in patients with panic disorder using the human elevated plus maze in mixed reality.

BACKGROUND: Avoidance behavior is a hallmark of panic disorder (PD), yet experimental evidence remains limited due to challenges in assessing unconditioned avoidance without specific threats. The human Elevated Plus Maze (EPM), implemented in mixed reality, provides an ecologically valid paradigm to measure approach-avoidance behavior under controlled conditions. METHODS: Twenty PD patients and 20 healthy controls (HC) were tested on the human EPM. Approach-avoidance behavior was assessed alongside subjective anxiety, tension, and cardiovascular responses across multiple time points. Hormonal

Cushing's syndrome: a systematic review of psychiatric and cognitive symptoms in case studies.

OBJECTIVE: Cushing's syndrome, characterized by chronic hypercortisolism, is associated with various health risks, including psychiatric symptoms and cognitive impairments. This systematic review of case studies aimed to map and categorize these symptoms. MATERIALS AND METHODS: We hypothesized that a broader range of psychiatric and cognitive manifestations would be observed beyond anxiety, depression, and memory impairment. The review followed PRISMA guidelines and was preregistered in PROSPERO (CRD42024433186). We conducted searches in PsycINFO, Embase, PubMed, and Scopus, identifying 273 p

Revisiting the prevalence of bipolar disorder: implications of a broader spectrum model.

Bipolar affective disorder has traditionally been defined by episodic mania and depression, largely restricted to Bipolar I and II categories. However, growing evidence suggests this framework underrepresents the broader longitudinal spectrum and may misclassify a subset of cases as borderline personality disorder (BPD) or other comorbidities. A narrative review was conducted, integrating historical and conceptual analyses of diagnostic frameworks (DSM-III to DSM-5), epidemiological studies over four decades, and the author's extensive clinical experience with longitudinal follow-up of complex

Big ideas series: avoidance-to-embrace: the death discontinuity continuum between panic disorder and acute suicidal affective disturbance.

BACKGROUND: Acute Suicidal Affective Disturbance (ASAD) is a proposed clinical diagnosis characterized by sudden, intense escalations in suicidal intent, profound social and self-alienation, hopelessness, and heightened physiological arousal. Unlike traditional linear frameworks implying an "ideation-to-action" trajectory, ASAD captures temporally discrete, high-risk states occurring within hours or days. Although accumulating evidence supports the construct validity of ASAD, direct research remains constrained by ethical and practical challenges. OBJECTIVES: This paper proposes Panic Disorde

Orodispersible tablets in psychiatric and mood disorder management: clinical value, pharmacokinetics, and patient-centric formulation strategies.

OBJECTIVE: To analyze the extensive literature underlying the concept of using and formulating Orodispersible tablets (ODTs) for managing psychiatric and mood disorders. SIGNIFICANCE: Psychiatric and mood disorders represent a major global health burden across all age groups, significantly impairing quality of life and daily functioning. Emotional distress associated with these conditions often leads to poor medication adherence and difficulty swallowing, particularly in vulnerable populations. Thus, an increased emergency intervention is required during acute panic or agitation states. ODTs

Associations between self-reported interoception and resting-state EEG markers in panic disorder: heartbeat-evoked potentials and spectral power.

BACKGROUND: Panic disorder (PD) is associated with altered interoceptive experience, but it remains unclear how self-reported interoception relates to neurophysiological indices of bodily-signal processing. This study examined whether associations between self-reported interoception and resting-state electroencephalography (EEG) markers differ between participants with PD and healthy controls (HC). METHODS: This analysis included 19 PD participants and 21 HCs from a previously published resting-state EEG dataset. Self-reported interoception was assessed using the Body Perception Questionnaire

Quantitative and Morphometric Measures in the Human Hippocampal Tail Using Ex Vivo Imaging Validated With Histology.

The human hippocampal tail is the posterior-most portion of the hippocampus and has been implicated in brain disorders, such as depression, panic disorder, and epilepsy. The tail's clinical relevance and volumetric variability exhibit a need for improved characterization with high-resolution imaging and histology at baseline. MRI provides three-dimensional context without missing slices and histology validates with architecture; combining these methods provides an optimal approach. Twenty control brain hemispheres were evaluated, including 10 males and 10 females. We scanned the hemispheres us

Hair cortisol concentrations to picture the dysregulation of the hypothalamic-pituitary-adrenocortical axis in panic disorder.

Panic attacks are characterized by intense feelings of uncontrollable threat and induce experiences of intense stress. As one of the major physiological stress systems of the body, the hypothalamic-pituitary-adrenocortical axis (HPA) may be a key to our understanding of a biological basis of this anxiety disorder. While recent studies have found evidence for short-term HPA changes in panic disorder with flattened cortisol responses to stimulation, no data have been available on tonic, i.e., long-term HPA activity in these patients. The current study therefore investigated the cumulative cortis

Anxiety Disorder Types From a Metabolomics Perspective: A Mendelian Randomization Analysis Based on 1400 Plasma Metabolites.

BACKGROUND: Different anxiety types demonstrate overlapping clinical features while retaining distinct characteristics, often leading to diagnostic challenges. Existing diagnostic approaches predominantly rely on symptom-based criteria, which may result in ambiguity. The identification of biomarkers is essential for elucidating the metabolomic mechanisms underlying anxiety, thereby informing improved diagnostic and therapeutic strategies. METHODS: We conducted a Mendelian randomization (MR) study, utilizing data from genome-wide association studies (GWAS) on plasma metabolites and three anxie

University students' interest in intervention research participation is associated with gender, age and symptoms of depression, general and social anxiety, panic disorder and self-harm.

OBJECTIVE: Research in the mental health field has focused on exploration and implementation of digital interventions both in research and society. However, demographic and mental health factors that may influence engagement with research on such interventions have not been explored. This study aims to investigate the relationship between demographics, signs of mental disorders, and interest in participating in an intervention study. Using data collected in the Swedish arm of the World Health Organization’s World Mental Health International College Student (WHO-WMH-ICS) initiative (n = 9140),

Rare protein-disrupting variants in NPY5R, DLGAP1 and MAPK8IP3 segregate with OCD in two multiplex pedigrees.

Obsessive compulsive disorder (OCD) is significantly heritable, but only a fraction of the contributory genetic variation has been identified, and the molecular etiology involved remains obscure. Identifying rare contributory variants of large effect would be an important milestone in helping to elucidate the mechanisms involved. Analysis of densely affected pedigrees is a potentially useful strategy to bypass the sample size challenges of standard case-control approaches. Here we performed whole genome sequencing (WGS) of 25 individuals across two multiplex OCD pedigrees. We prioritised rare

Molecular recognition of axonal injury-associated candidate hub genes and predicted inhibitors across Alzheimer's disease, Parkinson's disease, and amyotrophic lateral sclerosis.

Limpopo is a very important area for pig production in terms of animal populations and contributions to transboundary animal disease spread. Emerging small-scale pig farmers (ESSPF) are being encouraged to establish operations and spread in South Africa; however, for these farmers to perform optimally, they need to understand the basics of animal agriculture and contribute to enhancing biosecurity and efficient production systems. In the present study, the limitations to efficient production amongst ESSPF were evaluated and some improvements were suggested. It was found that the ESSPF are domi

Neurological Complications in Children with Moyamoya Disease-Case Report and Literature Review.

Background: Moyamoya disease (MMD) is a rare, progressive cerebrovascular arteriopathy characterized by stenosis and occlusion of the distal internal carotid arteries with the development of compensatory collateral networks. In children, MMD is a major cause of ischemic stroke; however, neurological morbidity frequently extends beyond cerebrovascular events to include epilepsy, headache, cognitive impairment, and psychiatric manifestations. Data regarding the long-term evolution of these complications in Caucasian pediatric patients remains limited. Case Report: We present the longitudinal cas

Genome-wide association study of major anxiety disorders in 122,341 European-ancestry cases identifies 58 loci and highlights GABAergic signaling.

The major anxiety disorders (ANX; including generalized anxiety disorder, panic disorder and phobias) are highly prevalent, often onset early and cause substantial global disability. Although distinct in their clinical presentations, they probably represent differential expressions of a dysregulated threat-response system. Here, we present a genome-wide association meta-analysis comprising 122,341 European ancestry ANX cases and 729,881 controls. We identified 58 independent genome-wide significant risk variants and 66 genes with robust biological support. In an independent sample of 1,175,012
