Efficacy and safety of a mobile app-based, flexible-schedule digital therapeutic intervention versus psychoeducation for panic disorder: An open-label randomized controlled trial.

BACKGROUND: Digital therapeutics (DTx) have the potential to improve the accessibility and personalization of cognitive behavioral therapy (CBT) for panic disorder. However, trials comparing DTx with structured active comparators remain limited. OBJECTIVE: We evaluated the efficacy and safety of PanicDoc, a mobile app-based DTx for panic disorder, versus structured psychoeducation. METHODS: In this open-label randomized trial, 52 patients with panic disorder were randomly assigned to PanicDoc or psychoeducation, both delivered with therapist guidance (n = 26 per group). PanicDoc provided CBT

The role of venlafaxine in the modern treatment of anxiety disorders: a critical update and expert clinical perspectives.

Anxiety disorders are among the leading contributors to global disability and frequently co-occur with major depressive disorder (MDD), chronic pain, and various psychiatric or somatic comorbidities. Effective management of these conditions necessitates a personalized approach informed by current scientific evidence. Extended-release (XR) venlafaxine, a serotonin-noradrenaline reuptake inhibitor, has emerged as a first-line pharmacological option, endorsed by international guidelines for generalized anxiety disorder, panic disorder, and social anxiety disorder. Its dose-dependent dual mechanis

A randomised controlled feasibility study of brief cognitive therapy for the treatment of PANic Disorder in Adolescents (PANDA).

BACKGROUND: Panic disorder occurs in 1-3% of adolescents and without treatment often has a chronic course. Treatments need to be effective and efficient to be delivered in routine services. Given that a brief version of cognitive therapy has been shown to be effective in adults, an adapted version could be beneficial for adolescents with panic disorder. AIMS: We aimed to establish clinical and economic proof-of-concept for brief cognitive therapy for adolescent panic disorder and examine the feasibility of a future definitive trial compared with a brief generic form of CBT, using several well

From accuracy to expectation: A reframed view of heartbeat counting and panic disorder.

Enhanced heartbeat perception is believed to contribute to the development and maintenance of panic disorder. Studies have demonstrated better Heartbeat Counting Task (HCT) performance in individuals with panic disorder compared to controls. This has been proposed to reflect increased interoceptive accuracy, which is thought to increase the risk of individuals perceiving bodily sensations, which in turn may trigger panic attacks. Drawing on contemporary evidence that HCT performance may reflect heartbeat estimation, rather than perception, we argue that greater HCT performance in panic disorde

A meta-analysis of heart rate variability and psychopathology: An investigation guided by the hierarchical taxonomy of psychopathology framework.

Although low resting heart rate variability (HRV) is associated with various forms of psychopathology, patterns of specificity and nonspecificity in the apparent transdiagnostic nature of resting HRV have not been systematically investigated. Existing research is dominated by disorder-specific studies, limiting conclusions about whether HRV reductions reflect mechanisms unique to particular diagnoses or a broader transdiagnostic vulnerability. Using the Hierarchical Taxonomy of Psychopathology framework, this Stage 2 Registered Report synthesized evidence from 430 studies and 732 effect sizes

Impact of Pre-existing Psychiatric Conditions on Trauma Outcomes: A National Analysis.

BACKGROUND: Understanding the impact of psychiatric pre-existing conditions (PsyPECs) on trauma outcomes is crucial, as trauma is the leading cause of mortality for Americans aged 1-45. Although previous regional studies have shown higher mortality rates and longer hospital length of stay (H-LOS) for injured patients with PsyPECs, this has not been studied at the national level. The purpose of this study is to identify the PsyPECs associated with mechanism of injury and other outcomes in adult patients admitted for trauma in the United States' trauma centers. METHODS: Data were extracted from

Evaluation of the Results of a Multicomponent Emotional Health Intervention in a School Setting.

BACKGROUND: The global prevalence of anxiety and internalizing disorders in adolescence has increased epidemiologically over the past decade, compounded by the impact of the COVID-19 pandemic and the expansion of digital social networks. Despite the availability of empirically based interventions, their systematic implementation in Spanish educational settings-especially in socioeconomically vulnerable environments-remains insufficient. This study aimed to assess the emotional symptomatology profile in a sample of adolescents enrolled in the 2nd year of Compulsory Secondary Education (CSE) and

Review of the impact of short-term use of alprazolam on cognitive, psychomotor, and quality-of-life impairments associated with anxiety disorders.

Anxiety disorders are often associated with impairments in memory, attention, decision-making, and psychomotor function, leading to a reduced quality of life (QoL). Alprazolam, commonly prescribed for acute anxiety disorders, has generated interest regarding its potential impact on anxiety-associated cognitive and psychomotor impairment, especially since effects of short-term use have not been well studied. This review summarizes clinical evidence on the short-term (up to 8 weeks) effects of alprazolam on cognitive performance, psychomotor function, and QoL outcomes in individuals with anxiety

Pharmacotherapy, acupoint stimulation, and psychotherapy for perimenopausal women with anxiety, depression, and panic disorder: a systematic review and network meta-analysis of randomized controlled trials.

BACKGROUND: Perimenopausal women frequently experience physiological and psychological symptoms, including anxiety, depression, and panic disorders, mainly due to declining ovarian function and hormonal changes. Current options include pharmacotherapy, acupoint stimulation (AcuStim), and psychotherapy (psych), but their comparative efficacy and safety remain controversial. OBJECTIVE: This network meta-analysis (NMA) systematically compared pharmacotherapy, AcuStim, and psychotherapy for perimenopausal anxiety, depression, and panic disorder, assessing clinical efficacy, adverse events (AEs),

The Anxiety-AFib connection: a systematic review of mental health and arrhythmia interactions.

Background: Atrial fibrillation is the most common cardiac arrhythmia, which has effects that goes beyond the heart, often affecting mental health. Over the years, increasing evidence has highlighted a bidirectional connection between anxiety and atrial fibrillation. However, the connection between the two in clinical practice is vague. Objective: This systematic review aims to investigate the relationship between anxiety and atrial fibrillation, including how anxiety might contribute to the onset and relapse of atrial fibrillation, how it may impact the treatment outcomes and its effects on t

The association between combustible tobacco use, panic attacks, and insomnia symptoms among adolescents.

Previous research suggests that individuals who experience panic attacks are more likely to engage in combustible tobacco use and report higher levels of insomnia symptoms. However, no studies to date have examined this association among adolescents. Additionally, no work has examined if adolescent panic attacks may be indirectly associated with insomnia through nicotine dependence among adolescents who use combustible tobacco and experience panic attacks. The current study aims to address these gaps in the literature by examining the following among adolescents: (1) the level of insomnia symp

Mothers' home management practices during febrile seizures in children under six years of age in Jordan.

BACKGROUND: Febrile seizure is the most prevalent seizure disorder among young children, and it commonly occurs in the home setting, where mothers are often the first responders. First-aid management of febrile seizures is also important to avoid complications and help parents feel less anxious. Nevertheless, there is a paucity of evidence describing the home-management practices of mothers in Jordan. OBJECTIVE: The purpose of this study was to describe the home-management practices of mothers of children under six years of age with febrile seizures in Jordan and to determine factors associat

Prediction of Clinically Meaningful Improvement After Internet-Delivered Cognitive Behavioral Therapy for Depression and Anxiety Disorders: Machine Learning-Based Predictive Model Development and Temporal Validation Study.

BACKGROUND: Up to 50% of patients treated with internet-delivered cognitive behavioral therapy (ICBT) for depression and anxiety disorders do not experience clinically significant symptom reduction. Identifying these patients prior to the initiation of ICBT can support treatment planning. OBJECTIVE: The aim of this study was to enhance baseline prediction of clinically meaningful improvement in patients treated with ICBT for common psychiatric disorders in routine care, which could ultimately inform treatment planning at intake. METHODS: We developed multimodal predictive models integrating

Comparing short-form vs full-length mental health disorder symptom measures among public safety personnel.

Mental health disorders are prevalent among public safety personnel (PSP; e.g., correctional workers, firefighters, paramedics, police, public safety communicators) due to frequent exposures to occupational stressors. PSP report having minimal time for research participation and needing efficient screening tools validated for their unique needs. The current study evaluates the validity of short-form mental health disorder questionnaires as alternatives to the full-length versions. Using data from 5838 PSP, the psychometric properties of the 4-item PTSD Checklist for DSM-5, 2-item Patient Healt

Structural Characterization of 1,2-Bis(4-cyanophenyl)diazene Oxide and Its Cyclotrimerization to a Triazine-Linked Polymer.

Azoxybenzenes have been widely studied due to their diverse applications, but reports on their detailed structural characterization are rare. It is often the case that even when the structure is obtained, the azoxy group is disordered, which makes it difficult to determine its geometrical parameters correctly and, in some cases, even its chemical identity (due to oxygen disorder at both nitrogen sites). Here, the molecular structure of a nitrile-substituted azoxybenzene derivative, 1,2-bis-(4-cyanophenyl)-diazene oxide (1), was accurately determined by single-crystal X-ray diffraction (SCXRD)

Associations among anxiety sensitivity, intolerance of uncertainty, and experiential avoidance: Are these key processes underlying anxiety-related pathology adequately distinct from each other?

Anxiety sensitivity (AS), Intolerance of Uncertainty (IU), and Experiential Avoidance (EA) have been conceptualized as key transdiagnostic processes underlying anxiety pathology. However, little work has been done to examine whether AS, IU, and EA are adequately distinct from one another in relation to concerns about jangle fallacies - whether two or more measures with different labels may actually assess the same construct. The present study used data from 300 treatment-seeking individuals with a primary diagnosis of one of five anxiety-related disorders-panic disorder, social anxiety disorde

How observing others influences the subsequent acquisition of fear of bodily sensations.

Observational fear learning preceding a first-hand learning experience is assumed to potentially sensitize individuals, resulting in greater fear acquisition. In a laboratory setting, we therefore investigated how different demonstrator responses to temporary dyspnea influence consecutive direct fear acquisition. 74 participants were randomly assigned to three experimental groups. In the two observational learning groups, participants observed a demonstrator completing an interoceptive differential fear conditioning paradigm, during which one of two visual cues (conditioned stimuli, CS) was pa

Bidirectional temporal relationships between emotional state and eating across eating disorders: a network approach.

OBJECTIVE: Several models of eating disorders (EDs) suggest that emotions and eating influence each other in a vicious cycle, producing part of the observed symptoms. However, no research tested whether such cycles exist across EDs. We therefore explored networks of prospective relationships between negative and positive emotions and eating-related behaviors (hunger, food craving, calorie intake, and binges) across EDs and healthy controls (HCs). METHOD: These variables were assessed six times a day for eight days in women with restrictive (AN-R, N = 29) and binge-purge (AN-BP, N = 26) Anorex
