A unified general cognitive factor explains impairment across schizophrenia, bipolar disorder, OCD and substance use disorder.

BACKGROUND: Emerging evidence suggests that cognitive impairment cuts across traditional psychiatric diagnoses and may reflect a shared underlying cognitive liability. We examined whether a general cognitive factor (gFc) accounts for transdiagnostic deficits across schizophrenia (SZ), bipolar disorder (BD), obsessive-compulsive disorder (OCD) and substance use disorder (SUD). METHODS: A total of 797 individuals [544 affected and 253 healthy controls (HC)] completed a standardized cognitive battery. Bifactor confirmatory factor analysis was used to derive general cognitive factor (gFc) and dom

Psychological Process-Based Profiles in the Binge-Eating Spectrum: Clinical Implications Beyond DSM-5 Subtypes.

BACKGROUND AND OBJECTIVES: Binge-spectrum eating disorders (BSEDs) are clinically impactful, yet the psychological mechanisms underlying their persistence and heterogeneity remain insufficiently understood. Clarifying these mechanisms is essential for refining diagnostic models and improving treatment. This study focused on young to early-middle adults, a developmental period with elevated risk for the onset and aggravation of BSED, to identify empirical subgroups based on eating- and psychological-related variables. METHODS: A two-step cluster analysis was performed in 187 patients (men and

Altered prefrontal suppression of striatal activity during intrusive images in obsessive-compulsive disorder.

BACKGROUND: Current models of obsessive-compulsive disorder (OCD) emphasize dysfunction in frontostriatal circuits that support flexible behavioral control, leading to impulsive and compulsive actions. However, how these disruptions contribute to obsessions and intrusive thoughts remains poorly understood. Clarifying these mechanisms is essential for understanding cognitive control deficits in OCD. METHOD: We examined intrusive memories elicited by emotional and neutral scenes in individuals with OCD (N = 24) and healthy controls (N = 21). Participants learned scene-cue associations and were

Assessing the Impact of Metacognitive Therapy on OCD Severity and Beliefs: A Three-Wave Multisite RCT With Panel Mediation Analysis.

Obsessive-compulsive disorder (OCD) is a chronic and disabling condition, and although exposure and response prevention (ERP) is the first-line psychological treatment, many patients decline it, drop out, or retain residual symptoms. According to Wells' metacognitive model, OCD is maintained less by intrusive thoughts themselves than by metacognitive beliefs about the significance, danger, and control of thoughts, which metacognitive therapy (MCT) targets directly. However, individual MCT has been evaluated almost exclusively in high-income Western settings, and whether metacognitive belief ch

Obsessional jealousy: Provisional research criteria and report of 34 cases.

BACKGROUND: Obsessional jealousy is described in DSM-5 under Other Specified Obsessive-Compulsive and Related Disorders, yet operationalized criteria and clinical characterization are lacking. This study aimed to develop and field-test provisional research criteria for obsessional jealousy and describe the clinical features of individuals meeting these criteria. METHODS: Provisional research criteria were developed based on DSM-5 descriptions and applied in a self-referred sample of individuals reporting problematic jealousy (n = 58). Participants underwent structured clinical assessment, inc

Reliability and validity of the death anxiety scale in patients with mental illnesses in a general hospital.

BACKGROUND: Death anxiety is a transdiagnostic construct of mental illness. The aim of this study was to explore the reliability and validity of the death anxiety scale (DAS) in the context of mental illnesses in a general hospital. METHODS: Patients with generalized anxiety disorder, panic disorder, major depressive disorder, obsessive-compulsive disorder or somatic symptom disorder were included in the study. Internal consistency was explored using Cronbach's α. Construct validity was assessed by exploratory factor analysis (EFA), exploratory graph analysis (EGA) and confirmatory factor ana

[Excoriation (Skin-Picking) Disorder: A Comprehensive Review].

Excoriation disorder (ED), recently recognized as a distinct diagnosis in the DSM-5, is characterized by recurrent compulsive skin picking behaviors that lead to skin lesions and significant emotional, functional, and social impairment. This comprehensive review addresses its pathophysiology, clinical assessment, comorbidities, and treatment, highlighting the importance of a multidisciplinary approach. Advances in neurobiology and pharmacotherapy are discussed, including the use of Selective Serotonin Reuptake Inhibitors, N-acetylcysteine, and naltrexone, as well as the effectiveness of psycho

The Mediating Role of Maladaptive Coping in the Association Between Neuroticism and Family Accommodation in Obsessive-Compulsive Disorder.

OBJECTIVE: Family accommodation, in which relatives facilitate obsessive-compulsive symptoms, is highly prevalent in obsessive-compulsive disorder (OCD) and associated with poor treatment outcomes. Although neuroticism and maladaptive coping have been linked to OCD severity, the mechanisms by which personality traits relate to family accommodation remain poorly understood. This study examined associations between neuroticism, coping strategies, symptom severity, and family accommodation in OCD, and tested whether maladaptive coping mediates the neuroticism-family accommodation association. Intensive cognitive behavioral therapy for obsessive-compulsive disorder: a meta-analysis of randomized controlled trials.

BACKGROUND: Intensive cognitive behavioral therapy (CBT) delivers more or longer sessions over a shorter time than standard CBT. This review aimed to examine the effects of intensive CBT in comparison to non-active conditions and other active treatments for obsessive-compulsive disorder (OCD). METHOD: We analyzed standardized mean difference (SMD) at post-treatment and follow-up for OCD symptoms, depression, anxiety, and family accommodation (FA). Moderator analyses and meta-regression were conducted to investigate factors associated with OCD symptom reduction. RESULTS: Fourteen randomized c

Dynamic functional abnormalities of cerebellar networks in adolescents with obsessive-compulsive disorder.

BACKGROUND: Obsessive-compulsive disorder (OCD) commonly begins during adolescence, yet its neurobiological basis remains unclear. Beyond the classical cortico-striatal framework, the cerebellum has been implicated in cognitive, affective, and motor regulation, relevant to OCD. However, it is unknown whether adolescent OCD involves abnormal dynamic coordination of large-scale cerebellar networks. METHODS: Thirty-eight adolescents with OCD and 29 matched controls underwent dynamic functional network connectivity (dFNC) analysis of cerebellar networks using a sliding-window approach and k-means

Risk of schizophrenia in patients previously diagnosed with obsessive-compulsive disorder: a systematic review.

OBJECTIVES: This systematic review aimed to synthesize longitudinal evidence on the risk of developing schizophrenia (SCZ) among individuals previously diagnosed with obsessive-compulsive disorder (OCD). METHODS: A systematic search of longitudinal studies examining the temporal relationship between OCD and SCZ was conducted in PubMed, PsycINFO, Embase, and Scopus in September 2023 and subsequently updated in January 2026. We included original longitudinal observational studies reporting the incidence of schizophrenia in patients with a prior OCD diagnosis compared to controls. Cross-sectiona

Radiosurgical capsulotomy for treatment-resistant obsessive-compulsive disorder through a network neuroscience lens.

Severe, treatment-resistant obsessive-compulsive disorder (OCD) remains disabling despite optimized pharmacotherapy and exposure and response prevention (ERP). Radiosurgical capsulotomy is an irreversible ablative intervention reserved for highly selected patients. This narrative review examines evidence from structural MRI, diffusion imaging, functional connectivity, electrophysiology, and radiobiology. The search was supplemented by a reproducible PubMed search across eight prespecified modules (856 records before and 739 after deduplication), with an audited 102-record core SRS/OCD set. Str

Pleasure and compulsion in gaming: distinct roles of hedonic and compulsive game use in gaming involvement.

BACKGROUND AND AIMS: Stage-related models of problematic gaming suggest that the relative prominence of reward-oriented and compulsivity-related processes may vary across levels of gaming involvement. However, few studies have jointly examined hedonic and compulsive game use across different levels of gaming involvement while accounting for relevant trait-level vulnerabilities. This study investigated their distinct associations with gaming involvement. METHODS: A total of 2259 gamers were included in the cross-sectional analyses, of whom 725 completed a one-year follow-up. Associations of nonattachment with infant care-related obsessive-compulsive behaviours, depression, anxiety, and stress in postpartum women.

BACKGROUND: Postpartum women frequently experience depression, anxiety, stress, and infant care-related obsessive-compulsive behaviours, adversely affecting maternal psychological adjustment and well-being. Nonattachment is a potentially protective psychological construct associated with lower psychological distress and greater well-being, however; evidence on its relationship with postpartum psychological symptoms is limited. AIM: This study aimed to examine the associations between nonattachment and infant care-related obsessive-compulsive behaviours, depression, anxiety, and stress among p

Assessing the effect of subthalamic stimulation on metaperception in obsessive-compulsive disorder.

Most human studies on perceptual decision-making are correlational, and few addressed the causal role of the neural markers they identify. Here, we took the opportunity offered by deep brain stimulation to study the causal role of subthalamo-cortical networks on perceptual decision-making and perceptual monitoring. We asked participants with obsessive-compulsive disorder (OCD) treated with subthalamic stimulation to detect visual stimuli and provide confidence reports while we recorded scalp electroencephalography. Following a preregistered plan, we compared behavioural and neural results betw

Anxiety sensitivity as a transdiagnostic risk factor and mechanism of change in the treatment of youth anxiety disorders and OCD: Findings from the Unified Protocols for Transdiagnostic Treatment of Emotional Disorders in Children and Adolescents.

Anxiety sensitivity, or fear of anxiety-related symptoms, is a "core dysfunction" associated with the development and maintenance of youth anxiety disorders and obsessive-compulsive disorder (OCD). Accordingly, anxiety sensitivity is a common target of transdiagnostic cognitive behavioral therapy (CBT) interventions, like the Unified Protocols for Transdiagnostic Treatment of Emotional Disorders in Children and Adolescents (UP-C/A). However, no studies have empirically evaluated anxiety sensitivity as a mechanism of symptom change within UP-C/A treatment. This study examined anxiety sensitivit
