Black EquaLity in OCD NeuroGenomics (BELONG): Study Protocol.

Obsessive-compulsive disorder (OCD) is a chronic, serious psychiatric disorder that affects 2-3% of the population and is associated with high personal and societal costs. Genetic factors are estimated to explain roughly half the risk of developing OCD, and genomic studies are just beginning to identify common and rare genetic variants mediating this risk. A major goal of genomic studies is to yield insights into the etiology of OCD and identify molecular targets for the development of novel therapeutics. However, the overwhelming majority of subjects in existing genetic studies are of Europea

OCD or Psychosis? Yes: A Case Study of a Diagnostic Conundrum and the Caregiver Role.

Obsessive-compulsive disorder (OCD) and schizophrenia spectrum disorders, while distinct phenomena, have considerable overlap in their presentations which can present a challenge in their approach to diagnosis and treatment. The changes in the DSM-5 bring these challenges to light, in allowing OCD to be specified as having absent insight or delusional beliefs. This can be especially distressing for caregivers, who play a major role in treatment, especially among youth. We present a case of a 16-year-old male presenting to care in his first episode of psychosis with overlapping obsessive-compul

Supporting people with hoarding behaviour: a health promotion perspective.

Hoarding disorder is a complex mental health condition that is increasingly recognised as both a health concern for the individual and a wider public health issue. Supporting individuals with hoarding disorder requires multi-agency intervention, involving collaboration between local authority environmental health teams, fire safety experts, mental health specialists and health and social care professionals, including community nurses. This article examines hoarding behaviour through a health promotion lens, with a focus on the role of the nurse. The author discusses potential underlying causes

DNA variants affecting chromatin structure are key to the genetic architecture of obsessive compulsive disorder.

BACKGROUND: Obsessive Compulsive Disorder (OCD) affects 2 to 3% of the population and is marked by intrusive thoughts and repetitive behaviours. OCD is increasingly recognized as a polygenic disorder involving gene-environment interactions, with genetic risk largely driven by common variants and a smaller contribution from rare pathogenic variation. Despite these insights, the functional roles of implicated genes remain unclear. Recent genomic studies suggest that chromatin dysregulation contributes to the pathogenesis of OCD. RESULTS: We analysed the recently published large-scale genome-wid

Association of parental prenatal mental health with offspring neurodevelopmental disorders: a systematic review and meta-analysis.

BACKGROUND: Parental prenatal mood and anxiety disorders (PMADs) are linked to child neurodevelopmental disorders (NDDs), but evaluations of the magnitude and mechanisms of this association are limited. This study estimates the strength of the association and whether it is impacted by genetic and environmental factors. METHODS: A systematic search of PubMed, CENTRAL, PsycINFO, OVID, and Google Scholar was performed for articles published from January 1988 to September 2025. Of 2,420 articles screened, 74 met the inclusion criteria. Meta-analyses were conducted on 21 studies, and 53 were inclu

Trade-off between speed and accuracy: associations between obsessive-compulsive symptoms and meticulousness in PANS.

INTRODUCTION: Despite increasing recognition of pediatric acute-onset neuropsychiatric syndrome (PANS), its neuropsychological underpinnings remain limited, particularly in relation to obsessive-compulsive disorder (OCD) symptoms. PANS manifests abruptly with severe OCD and/or eating restrictions alongside concurrent neuropsychiatric symptoms, causing distress and functional impairment. This study aimed to examine the relationship between neurocognitive performance and OCD symptom severity in children and adolescents with PANS. METHOD: Twelve children and adolescents with PANS were assessed o

A case study on using quantile regression in psychiatry research.

Commonly used linear regression focuses only on the effect on the mean value of the dependent variable and may not be useful in situations where relationships across the distribution are of interest. This study aimed to appraise the utility of Quantile Regression (QR), a technique that can model any quantile value of the dependent variable. The primary aim of this study is to provide an overview of the QR method and its practical applications in psychiatric research. We demonstrated this with an exploratory analysis of the data on neuropsychological test performance among 119 subjects with obs

Can adaptive hyperactivation result in a positive score on the Mood Disorder Questionnaire? Evidence from a case-control study over a community survey.

BACKGROUND: Bipolar Disorder (BD) remains challenging to identify, and the Mood Disorder Questionnaire (MDQ) may capture heterogeneous forms of hyperactivation, including adaptive patterns unrelated to psychopathology. OBJECTIVES: To determine whether MDQ-positive individuals include a subgroup with adaptive hyperactivation-characterized by high quality of life (QoL) and minimal psychiatric morbidity-and to examine whether MDQ positivity also identifies diagnoses beyond BD. METHODS: Using data from a large Italian community survey with DSM-IV clinical interviews and MDQ screening, we conduct

Thought suppression worsens mood and obsessions but improves them subsequently: First evidence of a contrast avoidance effect for the maintenance of thought suppression.

OBJECTIVE: The cognitive behavioral model for obsessive-compulsive disorder (OCD) suggests that suppression of unwanted thoughts (TS) is negatively reinforced by temporarily preventing obsessions and reducing distress. It further suggests that ceasing TS subsequently creates a rebound effect leading to increased obsessions/distress. However, the contrast avoidance model (CAM) suggests that TS concurrently increases and then sustains obsessions/negative affect, thus increasing the likelihood of negative and positive reinforcement (positive contrast) and avoiding a negative contrast later. We te

Managing Family Accommodation in OCD: A Sibling-focused Case Study.

Family accommodation (FA) is prevalent in obsessive-compulsive disorder (OCD). When patients refuse treatment, the impacts are more severe-with poorer clinical outcomes, greater FA, and more caregiver burden. In these cases, caregiver interventions are an essential clinical priority. However, clinical documentation of structured, culturally informed interventions for FA and caregiver burden for adults with OCD, in the absence of patient participation, remains limited. This case study describes an eight-session caregiver-focused intervention for a 26-year-old male experiencing burnout while acc

Negotiating Stigma: Comparing the Experiences of Self-Identified and Clinically Diagnosed Men with OCD in Russia.

This cross-sectional qualitative study investigates how Russian men with obsessive-compulsive disorder (OCD) experience and manage mental health stigma, focusing on differences between those with a clinical diagnosis and those who self-identify with the disorder. Through in-depth interviews with eleven participants, the research explores meaning-making, disclosure practices, coping strategies, and the impact of masculine norms on help-seeking. Findings reveal that self-identified men often leverage the OCD label for personal understanding and peer support, while clinically diagnosed men gain v

Flexible brain state engagement predicts cognitive control transdiagnostically.

Cognitive control supports adaptive responses in an ever-changing world. While alterations in cognitive control have been consistently observed in a range of psychiatric disorders, the neural mechanisms giving rise to this behavioral variation remain elusive. Here, we tested whether the ability to flexibly recruit recurring brain activation patterns (i.e., brain states) may serve as an intermediate phenotype supporting cognitive control in individuals with a spectrum of clinical symptoms. We leveraged machine learning and external validation to explore this question in three independent, trans

Personalized repetitive transcranial magnetic stimulation reduces frontal eeg complexity in patients with obsessive-compulsive disorder.

BACKGROUND: Repetitive transcranial magnetic stimulation (rTMS) shows therapeutic potential for obsessive-compulsive disorder (OCD). Brain entropy has recently emerged as a candidate biomarker in neuropsychiatry, yet its modulation by rTMS in OCD remains unclear. Given EEG's superior temporal resolution for capturing rapid fluctuations in neural complexity, it was used to evaluate the effects of fMRI-neuronavigated rTMS on frontal entropy and its potential as an objective treatment marker. METHODS: Resting-state EEG was recorded from 44 OCD patients and 24 healthy controls (HCs) to compute fr

Classifying Obsessive-Compulsive Disorder from Resting-State EEG Using Convolutional Neural Networks: A Pilot Study.

OBJECTIVE: Identifying obsessive-compulsive disorder (OCD) using brain data remains challenging. Resting-state electroencephalography (EEG) offers an affordable and noninvasive approach, but identifying predictive signals in EEG data has met with little success, even with the application of traditional machine learning methods. We explored whether convolutional neural networks (CNNs) applied to EEG time-frequency representations can distinguish individuals with OCD from healthy controls. METHOD: We collected resting-state EEG data from 20 unmedicated participants (10 with OCD, 10 healthy cont
