Acute Generalized Exanthematous Pustulosis Mimicking Toxic Shock Syndrome in a Pediatric Patient.

Acute generalized exanthematous pustulosis (AGEP) is a rare, drug-induced hypersensitivity reaction that can mimic other serious febrile exanthems when pustule formation is delayed. We present a 16-year-old male who developed fever, diffuse erythema, and hypotension after amoxicillin exposure, initially raising concern for toxic shock syndrome. Classic pustules emerged several days into hospitalization, leading to a diagnosis of AGEP supported by clinical scoring. This case highlights the diagnostic challenges of AGEP in its early stages and underscores the importance of considering it in the

Advances and updates in antibiotic combination therapy.

INTRODUCTION: While antibiotic combination therapy remains a common clinical practice, its scientific foundation is fragile. The available evidence is fragmented, biased, and fails to capture the complexity of modern infectious disease scenarios. This perspective reinterprets the role of combination therapy through a critical lens, challenging current dogmas and proposing a pathogen-specific approach, focusing also on severe acute infections. AREAS COVERED: This Critical Perspective focused on selected studies from 2018 to 2025 identified through a focused PubMed search on the place in therap

Characterization of Staphylococcus aureus CC1 and CC1660 of Human and Equine Origin.

Background/Objectives: Staphylococcus aureus isolates from humans and horses of the equine-associated clonal complexes (CCs) CC1 and CC1660 were comparatively investigated for their genomic relationships. Methods: A total of 91 S. aureus isolates (64 human, 27 equine) were subjected to whole-genome sequencing (WGS), sequence analysis, and antimicrobial susceptibility testing. Results: WGS confirmed 75 CC1 and 16 CC1660 isolates, comprising nine sequence types (STs) in CC1 and four STs in CC1660. Ten spa types were present in CC1 and five in CC1660. In the arcC gene of three CC1 isolates, a 285

Fatal Toxic Shock Syndrome Resulting from Clostridium sordellii Bacteremia after Perforation of a Low Rectal Cancer.

BACKGROUND: Clostridium sordellii infections are rare and highly lethal. They are most associated with medical abortions and other genitourinary procedures. We report a fatal case of C. sordellii bacteremia from a perforated low rectal cancer with rectovaginal fistula. CASE PRESENTATION: A 60-year-old female presented with 10 days of abdominal pain, loose stools, and hematochezia. She developed rapidly progressive septic shock. Imaging identified a perforated rectal mass, pneumovagina, and a large pelvic abscess. She underwent fecal diversion and drainage of the pelvic abscess. Despite this i

Febrile Rash, Shock, and Hyperferritinaemia in A Young Woman: Navigating the Diagnostic Overlap of Hemophagocytic Lymphohistiocytosis, Toxic Shock, and Viral Exanthema.

BACKGROUND: Fever, rash, and shock in adults pose diagnostic and therapeutic challenges. Markedly elevated ferritin often raises suspicion for hemophagocytic lymphohistiocytosis (HLH), yet hyperferritinaemia is also encountered in sepsis, adult-onset Still's disease (AOSD), and viral infections. CASE REPORT: We describe the case of a 27-year-old woman who developed a high fever, diffuse erythematous rash, hypotension, and markedly elevated ferritin levels. The initial differential diagnoses included toxic shock syndrome, HLH, AOSD, and viral exanthema. Extensive evaluations for infectious, au

[Retrospective study of invasive group A streptococcal infections in paediatric patients during the recrudescence period alerted by the World Health Organization].

Following an increase in the incidence of invasive Group A Streptococcal infections (iGASi) in the paediatric department of our institution, concomitant with a World Health Organization alert at the end of December 2022, we conducted a retrospective study at the Citadelle hospital in Liège until the end of 2023. As described in the literature, the patients most affected are healthy children under the age of 5. On admission, the clinical picture was non specific. The diagnosis of iGASi was mostly made by identifying the germ on a pleural fluid sample or on a blood culture (predominance of purul

Streptococcal toxic shock syndrome with acute respiratory distress syndrome following adenotonsillectomy in a child: a case report.

This article reports a case of an 8-year-old child who developed Streptococcal Toxic Shock Syndrome (STSS) and Acute Respiratory Distress Syndrome (ARDS) following adenotonsillectomy, summarizing the diagnostic and treatment process. Preoperative blood tests, CRP levels, and chest x-rays of the child showed no abnormalities. However, within two hours after surgery, the patient exhibited high-grade fever, dyspnea, and altered mental status, rapidly progressing to shock and ARDS. Although shock was not diagnosed promptly in the early stage, leading to non-standardized fluid resuscitation therapy

Rhodomyrtus tomentosa-incorporated silicone inhibits planktonic growth and surface adhesion of Staphylococcus spp. and Candida albicans in simulated vaginal fluid.

Menstrual cups are increasingly used for menstrual hygiene. However, their use may alter the vaginal microenvironment and increase susceptibility to opportunistic infections caused by Staphylococcus spp. and Candida albicans. The current study aimed to develop an antimicrobial menstrual cup by incorporating silicone with Rhodomyrtus tomentosa (RT) leaf extract, a medicinal plant recognized for its antimicrobial properties. RT-loaded silicone was fabricated using an internal mixer and compression molding at 160 °C for 3 min. Surface roughness and wettability remained unchanged after incorporati

The Pathogenicity Mechanisms of Staphylococcus aureus.

Staphylococcus aureus is one of the most important bacterial pathogens affecting both humans and animals. This review discusses the most significant factors that contribute to the pathogenicity of these bacteria and the mechanisms that regulate their expression. We also focus on the factors that play a role in the pathogenesis of skin diseases. S. aureus possesses a wide array of virulence factors that allow it to bypass passive and active mechanisms of the host's immune system and effectively infect and spread within the infected organism. These include factors that facilitate colonization of

Multiple Cerebral Infarctions and Clinically Mild Encephalitis/Encephalopathy with a Reversible Splenial Lesion Associated with Streptococcal Toxic Shock Syndrome: A Case Report.

INTRODUCTION: Streptococcal toxic shock syndrome (STSS) is a rapidly progressing, fatal disease. Sepsis can be complicated by multiple cerebral infarctions, clinically mild encephalitis/encephalopathy with a reversible splenial lesion (MERS), and sepsis-related encephalopathy, resulting in a poor prognosis. To our knowledge, there have been no reported cases of sepsis complicated by both multiple cerebral infarctions and MERS. We report a case based on a literature review. CASE PRESENTATION: The patient was a 49-year-old man who experienced fever and a sore throat for 2 weeks prior to his vis

An Extremely Rare Case of Sequential Onset of Streptococcal Toxic Shock Syndrome and Kawasaki Disease Following Varicella Infection in a 2.5-Year-Old Girl.

Varicella is a common viral disease of childhood that is usually benign and self-limiting, but it can lead to rare complications, including streptococcal toxic shock syndrome (STSS) and Kawasaki disease (KD). The simultaneous or sequential occurrence of these 2 complications after varicella has been reported very rarely. A 2.5-year-old girl presented with fever, swelling, and neck erythema 5 days after varicella infection and a history of ibuprofen use. She was admitted with a diagnosis of cellulitis. Despite initial antibiotic therapy, the patient's clinical condition rapidly deteriorated, an

Paeniclostridium sordelli toxic shock syndrome after a septic abortion - A case report.

Paeniclostridium sordellii is a highly virulent, spore-forming, Gram-positive anaerobe responsible for life-threatening infections, including endocarditis, myositis, and toxic shock syndrome. Although less well known than other clostridial species, it poses a significant risk, particularly to postpartum women, intravenous drug users, and the immunocompromised. In recent times, clostridial sepsis with toxic shock syndrome is more commonly seen in patients who underwent medical abortion and IV drug users, with mortality as high as 70 % within a few days of symptom onset. Here, we present a case

Molecular Epidemiology Clinical Manifestations, Decolonization Strategies, and Treatment Options of Methicillin-Resistant Staphylococcus aureus Infection in Neonates.

Preterm and low-birth-weight neonates are particularly susceptible to methicillin-resistant Staphylococcus aureus (MRSA) colonization, whereas MRSA infection is associated with significant neonatal morbidity and mortality globally. The objective of our study was to examine the current body of knowledge about molecular traits, epidemiology, risk factors, clinical presentation, decolonization techniques, and available treatments for MRSA infection in neonates. MRSA strains that predominate in neonatal units, namely healthcare-associated (HA)-MRSA, differ from community-acquired (CA)-MRSA strains

The Therapeutic Efficacy of MS473, a Fully Human Single-Chain Variable Fragment Targeting Staphylococcus aureus Toxic Shock Syndrome Toxin-1, in a D-Galactosamine-Sensitized Mouse Model of Lethal Shock.

Toxic shock syndrome toxin-1 (TSST-1), produced by Staphylococcus aureus, is one of the most potent superantigens involved in causing life-threatening TSS and contributes to the onset of some autoimmune diseases. To this end, we have previously identified a fully human single-chain variable fragment antibody (scFv), MS473, exhibiting high binding affinity and specificity for TSST-1 and demonstrating in vitro neutralization activity. In the present study, the therapeutic activity of MS473 was assessed in a D-galactosamine-sensitized mouse model of lethal shock. D-galactosamine-sensitized mice w
