Clinical and Microbiological Characteristics of Invasive Group A Streptococcus Infection: Four Case Series of Re-Emerging Pathogens.

INTRODUCTION: Group A Streptococcus (GAS), particularly the M1UK lineage, has re-emerged as a major global public health concern following the COVID-19 pandemic, with a rise in invasive GAS (iGAS) and streptococcal toxic shock syndrome (STSS). Although STSS is under national surveillance in Japan, comprehensive molecular monitoring of iGAS infections remains limited, and the clinical characteristics of M1UK-associated iGAS have not been fully elucidated. METHODS: We retrospectively reviewed four consecutive iGAS cases requiring intensive care between March and May 2024. Detailed clinical, mic

From Mandate to Maintenance: Quantifying the association between menstrual product availability and school attendance.

OBJECTIVE: Period poverty affects one-third of menstruating girls in high-income countries and is associated with toxic shock syndrome, psychosocial distress, and educational absenteeism. We conducted the first evaluation of implementation fidelity for a school-based menstrual equity policy and its association with student-reported health and attendance outcomes in a rural Ontario school board. METHODS: A cross-sectional analysis of a subsample of the Time 1 2025 Health and Peer Relations Study was conducted. Girls in Grades 5-12 who had reached menarche (N = 863) reported on dispenser stocki

Toxic Shock Syndrome Originating From A Tattoo: A Case Report.

BACKGROUND: Tattooing is increasingly common and generally considered safe but can rarely result in infectious complications ranging from localized skin infection to severe systemic illness. Toxic shock syndrome (TSS) is a rare, yet life-threatening condition caused by toxin-producing Staphylococcus aureus or Streptococcus pyogenes. Tattoo-associated TSS is uncommon, and awareness among emergency physicians may be limited, potentially delaying diagnosis and treatment. CASE REPORT: We describe a case of TSS in a previously healthy 21-year-old man following extensive tattooing of the upper arm.

Selective Expansion of TCR Vβ2+ Tregs and Impaired Lymphocyte TSST-1 Responsiveness in Bullous Pemphigoid Patients Colonised by Staphylococcus aureus.

Bullous pemphigoid (BP) is an autoimmune blistering disease of the elderly who are frequently colonised by Staphylococcus aureus, particularly strains producing the superantigen toxic shock syndrome toxin-1 (TSST-1). Given the ability of TSST-1 to engage the T cell receptor (TCR) Vβ2 chain, we investigated whether superantigen (SAg) exposure in BP is associated with alterations in peripheral T cell populations. CD4+ T cells were isolated from peripheral blood mononuclear cells (PBMCs) from BP patients and age-matched healthy controls. T cell responsiveness to TSST-1, staphylococcus enterotoxin

Fluorescent aptasensor targeting the fibrinogen-binding domain of clumping factor A for rapid detection of Staphylococcus aureus.

Staphylococcus aureus (S. aureus) is a versatile pathogen responsible for a wide range of illnesses from minor skin infections to critical conditions like pneumonia, sepsis, and toxic shock syndrome in humans. Clumping factor A (ClfA), a surface protein of S. aureus, plays a crucial role in bloodstream infections by attaching to fibrinogen, facilitating bacterial adherence and clumping. This study describes the selection of DNA aptamers, specific to ClfA using the technique of Ni-NTA affinity systematic evolution of ligands by exponential enrichment (SELEX). A DNA library of 76-mer single-stra

Necrotizing Pneumonia Caused by PVL-Negative Methicillin-Susceptible Staphylococcus aureus Complicated by Refractory Pneumothorax: A Case Report.

Methicillin-susceptible Staphylococcus aureus (MSSA) necrotizing pneumonia can progress to severe disease, particularly when Panton-Valentine leukocidin (PVL) is involved. Here we report the case of a previously healthy 56-year-old man who developed severe necrotizing pneumonia due to MSSA despite the absence of major toxin genes including PVL. The patient reported a 10-day history of low-grade fever and was admitted for bilateral pneumonia with sepsis. Despite antibiotic therapy, his condition rapidly progressed, and chest computed tomography revealed extensive ground-glass opacities and mult

Staphylococcal Toxic Shock Syndrome: The Critical Role of Early Recognition and Prompt Intervention.

PRESENTATION: We describe a 16-year-old girl presenting with four days of nausea, vomiting, malaise, and macular rash affecting the vulva and perineum during menstruation, following tampon use. She was febrile, hypotensive, tachycardic, and confused, with multi-organ dysfunction. DIAGNOSIS: Laboratory findings revealed leukocytosis, thrombocytopenia, renal impairment, markedly elevated CRP, and procalcitonin. Perineal swabs grew methicillin-sensitive Staphylococcus aureus resistant to clindamycin. Blood cultures were negative. The isolate was later confirmed to carry the tst gene encoding tox

Candida albicans promotes TSST-1 production by Staphylococcus aureus through glucose depletion and relief of CcpA-mediated repression.

Menstrual toxic shock syndrome (mTSS) is a rare yet life-threatening disease that is caused when the opportunistic pathogenic bacterium Staphylococcus aureus releases the toxic shock syndrome toxin 1 (TSST-1) superantigen, which triggers systemic inflammation. The main risk factor for mTSS is prolonged use of intravaginal products, which facilitates S. aureus growth and TSST-1 production in menstrual blood. However, some studies suggest that the vaginal microbiota may also play a role in mTSS occurrence. A previous study reported that the presence of S. aureus in menstrual fluids was correlate

Unusual Thromboembolic Sequelae of MRSA Bacteremia in A Pediatric Patient: A Case Report.

Methicillin-resistant Staphylococcus aureus infection can cause serious illness in children. It can progress to life-threatening bloodstream issues or bone infections, including: deep vein thrombosis, septic pulmonary embolism, toxic shock syndrome, and osteomyelitis. Treatment depends on control of infection with adequate antibiotics and early anticoagulation. We present a case of a 11-year old male child who presented with left arm and right leg pain and swelling that was associated with fever. However, there was no history of trauma. Blood culture was taken and showed Methicillin-resistant

Purpura Fulminans in a Patient With Cirrhosis and Methicillin-Sensitive Staphylococcus aureus (MSSA) Bacteremia: Diagnostic Challenges in Coagulopathy.

Disseminated intravascular coagulation (DIC) is a critical complication of systemic illness, and its diagnosis can be especially challenging in patients with cirrhosis, where baseline liver-related coagulopathy may mask the development of superimposed consumptive coagulopathy. In the setting of sepsis and liver dysfunction, DIC may progress to purpura fulminans (PF), a rare, life-threatening condition characterized by dermal necrosis and microvascular thrombosis. We present the case of a 54-year-old man with alcoholic cirrhosis and untreated hepatitis C who developed DIC and PF following methi

[Community-associated methicillin-resistant Staphylococcus aureus sepsis presenting with encephalopathy-like symptoms in a previously healthy young adult].

A 32-year-old previously healthy man developed abnormal behavior two days after the onset of fever. He had experienced epistaxis during consecutive work days prior to the onset. While clinical findings suggested encephalitis, blood cultures yielded methicillin-resistant Staphylococcus aureus (MRSA). No obvious local infectious foci were identified in the skin or soft tissues; however, brain MRI revealed multiple embolic-like lesions. The MRSA strain produced markedly elevated toxic shock syndrome toxin-1 (TSST-1), suggesting a toxic shock syndrome-like state. Although initial empirical therapy

Skin biopsy PCR enables rapid diagnosis of tattoo-associated staphylococcal toxic shock syndrome.

BACKGROUND: Staphylococcal toxic shock syndrome (TSS) is a rare but potentially fatal toxin-mediated disease. Early diagnosis is challenging because blood cultures are frequently negative and clinical manifestations may initially be nonspecific. CASE PRESENTATION: A 29-year-old man presented with fever, diarrhoea, hypotension, and purpuric lesions localized to a calf tattoo performed three days earlier. Laboratory investigations revealed systemic inflammation, acute kidney injury, thrombocytopenia, and hepatic dysfunction. Despite negative blood cultures and cerebrospinal fluid analyses, the

Fatal Panton-Valentine leukocidin-positive MSSA toxic shock syndrome: a call for rapid virulence detection and optimized antimicrobial therapy.

BACKGROUND: Toxic shock syndrome (TSS) is a rare but potentially life-threatening toxin-mediated infection. Cases associated with Panton-Valentine leukocidin (PVL)-producing Staphylococcus aureus are particularly uncommon in clinical settings and remain insufficiently documented in the literature. OBJECTIVES: This report aims to strengthen clinical understanding of TSS caused by methicillin-sensitive Staphylococcus aureus (MSSA) harboring PVL, assess optimal antimicrobial strategies for treating MSSA infections, minimize the risk of therapeutic failure, and offer insights that may inform time

Non-menstrual toxic shock syndrome - cases reported to the National Reference Laboratory for Staphylococci, Centre for Epidemiology and Microbiology, National Institute of Public Health, 1983-2025.

OBJECTIVE: To identify the virulence factors responsible for toxic shock syndrome in causative strains submitted by field laboratories throughout the Czech Republic. METHODS: To detect the production of TSST-1 and staphylococcal enterotoxins, the micro-slide gel diffusion test was initially used and later followed by reverse passive latex agglutination. Currently, the polymerase chain reaction is the preferred tool for the detection of toxin genes as well as genes for other toxins of S. aureus strains: exfoliatins A, B, and D, Panton-Valentine leukocidin, and the mecA and mecC genes responsib

Overwhelming post-splenectomy infection caused by Streptococcus agalactiae, sequence type 10 with capsular serotype Ib encapsulating ST10/1b occurring 30 years after splenectomy: a case report.

Overwhelming post-splenectomy infection (OPSI) is a rapidly progressive and potentially fatal condition that affects individuals without a spleen. Encapsulated bacteria are among the most common pathogenic microorganisms of OPSI. Streptococcus agalactiae (group B streptococci; GBS) is an encapsulated Gram-positive pathogen. Although it causes various invasive infections in adults, including bacteremia, septic shock, and toxic shock syndrome, it has rarely been associated with OPSI. We describe a case of GBS-associated OPSI in a 72-year-old man who developed septic shock 30 years after undergoi
