Necrotizing soft tissue infections are a diverse group of severe infections of the skin, fascia, and musculature. These infections are characterized by the rapid and progressive destruction of soft tissue, as well as high incidence of morbidity and mortality. Prompt diagnosis, appropriate antimicrobial choice, and aggressive surgical interventions are essential in effecting a clinical cure and optimal outcomes for patients. Despite advances in medical care since necrotizing soft tissue infections were first described, the morbidity and mortality associated with them remains high. The etiology,

Streptococcal toxic shock syndrome (STSS) is a rapidly progressing and life-threatening illness caused by group A Streptococcus (GAS), typically associated with invasive infections such as necrotizing fasciitis and characterized by high mortality rates. Although GAS is an uncommon cause of community-acquired pneumonia (CAP), it can lead to severe illness in both healthy individuals and those with comorbidities. We report a case of STSS with pneumonia in a previously healthy 69-year-old female who presented with severe respiratory distress and shock without skin lesions. Despite the rapid progr

Streptococcal toxic shock syndrome (STSS) is a life-threatening condition characterized by a sudden onset and rapid progression to multiple organ failure. The pathogen involved is Group A streptococcus (GAS). We report two cases of STSS that developed following post-traumatic cellulitis in elderly individuals residing in an assisted living facility. The first case involved a 93-year-old male. He developed cellulitis in his right leg after a fall, which rapidly progressed to multiple organ failure. Despite antibiotic therapy, he succumbed to the infection within 30 hours of admission. Blood cul

BACKGROUND: Immortal time bias is a spurious or exaggerated protective association that commonly arises in naive analyses of observational data. It occurs when people receive the intervention because they survive, rather than survive because they received the intervention. Studies in conditions with substantial early mortality, such as acute severe infections, are particularly vulnerable.
METHODS: We developed IMMORTOOL, an R package accessible via a user-friendly web interface (https://petedodd.github.io/IMMORTOOL-live/). This tool will estimate the potential for immortal time bias using emp

BACKGROUND: Staphylococcus aureus clonal complex 30 (CC30) is a historically significant pathogen affecting both hospital and community settings. The notable pandemic clones, phage-type 80/81 (PT80/81) and the Southwest Pacific clone (SWP) have spread internationally, contributing to significant morbidity and mortality. Despite their importance, research on the evolution of sequence type (ST) 30 has been limited, often focusing on a small number of strains or specific regions.
METHODS: In this study, we analyzed over 500 ST30 genomes from diverse sources, including Brazilian strains sequenced

The present study aimed at examining the nasal and ear carriage of Staphylococcus aureus of Rwandan dogs and cats. Sixty-five S. aureus isolates were detected, all originating from the nostrils of dogs. Resistance to penicillin, penicillin/erythromycin/clindamycin, penicillin/tetracycline, and tetracycline solely was observed. The isolates were assigned to 23 different spa types, among them three novel (t21589, t21661, and t21662) variants, associated with eleven clonal complexes (CCs) (CC1, CC5, CC12, CC15, CC22, CC30, CC45, CC97, CC152, CC707, and CC834). Four isolates could not be assigned

UNLABELLED: The objective of this research was to investigate the genomic epidemiology of methicillin-resistant Staphylococcus aureus (MRSA) in a pediatric population in Shanghai, China. Whole-genome sequencing was conducted for 492 randomly selected MRSA isolates obtained from a pediatric hospital between 2013 and 2022. ST59 (37.4%), ST398 (22.4%), ST88 (5.7%), and ST22 (5.5%) were the predominant lineages among these children. While ST59 maintained a dominant annual proportion before 2017, the proportion of ST398 gradually increased from 2013 to 2016, with ST398 ultimately emerging as a prev

BACKGROUND: Streptococcal toxic shock syndrome (STSS) is a serious and potentially fatal complication of invasive Streptococcus pyogenes (Group A Streptococcus, GAS) infection, often stemming from severe soft tissue infections. While uncommon, tonsillitis can also lead to STSS, with lethality rates reported as high as 70%.
METHODS: We present three cases of patients diagnosed with tonsillitis who developed STSS. Point-of-care testing for GAS antigens was utilized to facilitate rapid diagnosis.
RESULTS: In all three cases, the characteristic clinical presentations, including scarlatiniform ra

Fever following decannulation from extracorporeal membrane oxygenation (ECMO) poses diagnostic challenges. While infectious causes and thrombosis are typically considered, rare etiologies may be overlooked. We report the case of a 47-year-old woman who developed a high, refractory fever following veno-arterial (va) extracorporeal membrane oxygenation decannulation. Extensive infectious and non-infectious workups failed to identify a definitive cause as cultures, imaging modalities, and inflammatory markers were inconclusive. On post-ECMO day 9 and 23 days after initial admission to the hospita

Bacterial superantigens are potent immune activators that trigger T cell proliferation and intensive release of cytokines, leading to toxic shock syndrome. Also, they impair host immune responses, increasing bacterial carriage and transmission. Several studies proposed that superantigens can induce regulatory T (Treg) cells, which may suppress immune responses against bacterial infection. However, the mechanism of Treg cell induction by superantigens is still elusive. We here demonstrated that streptococcal pyrogenic exotoxin A (SPEA) promoted human CD4+CD25+Foxp3+ T cell induction in a dose-

Necrotizing fasciitis (NF) is a rare and potentially fatal infection of the soft tissues characterized by widespread fascial necrosis and rapid evolution. Diagnosis is often difficult and relies on a high index of suspicion. The approach to this disease requires a timely diagnosis, early antibiotic therapy, aggressive surgical debridement, and reconstructive procedures. All patients treated by the Plastic Surgery department with the diagnosis of NF between 2006-2019 were identified. A retrospective study was conducted with the treatment of patients' demographic and clinical data. Sixty patient

Since the onset of the COVID-19 pandemic, the incidence of streptococcal toxic shock syndrome (STSS) has increased. Additionally, complications involving influenza virus infections in STSS cases have been reported more frequently than in the pre-pandemic period. We report the case of a 69-year-old man who presented to the ED with progressive dyspnea and fever, without any signs suggestive of soft tissue infection. Chest CT revealed right-sided pneumonia, and a rapid influenza test returned positive. On the second day of hospitalization, Streptococcus pyogenes (emm1) was isolated from both bloo

BACKGROUND: Group A streptococcal necrotizing soft tissue infection (GAS-NSTI) is a severe condition that can be complicated by streptococcal toxic shock syndrome. Management necessitates multidisciplinary care, including emergency medicine, surgery, plastic surgery, and infectious diseases. Early diagnosis and appropriate therapeutic intervention are essential for GAS-NSTI, as effective prevention strategies have not been established. However, it is often misdiagnosed as cellulitis, resulting in delayed treatment.
METHODS: In a single-center retrospective study, 25 patients with GAS-NSTI wer

Pyogenic spondylitis can be life-threatening; however, its diagnosis remains challenging because of the initial presentation of nonspecific symptoms. Given the vulnerability of the infected site, patients are highly at risk for severe complications, such as epidural abscesses or bacterial meningitis, which can considerably worsen the prognosis. Herein, we report a case of lumbar pyogenic spondylitis initially identified through methicillin-susceptible Staphylococcus aureus bacteremia, which subsequently progressed to an epidural abscess. The abscess rapidly ascended to the cervical region, cau

Streptococcus hemolyticus has recently regained public attention as "man-eating bacteria" and raised significant public health concerns due to the ever-increasing incidence of infections and severe toxic shock syndrome resulting from inadequate treatment. The growing challenge of antimicrobial resistance further emphasizes the urgent need for the development of novel wound dressings to prevent infections. This study demonstrated that ferrous gluconate (FeGlu) facilitated ferroptosis in S. hemolyticus accompanied with hallmarks of iron dependence, reactive oxygen species (ROS) burst, and lipid

Bovine mastitis is a continuous problem in the dairy industry. The infection has significant impact on health and welfare of animals and causes financial losses for the farmers and the dairy industry. A key bacterium associated with bovine mastitis is Staphylococcus aureus (S. aureus). It produces different exotoxins including more than 24 different staphylococcal enterotoxins and toxic shock syndrome toxin-1 (TSST-1). We have investigated the role of enterotoxin C (SEC) and TSST-1 in the infection process. Five different S. aureus sec or tst-1 deletion mutants, derived from three wild type st

Although non-aureus staphylococci (NAS), such as coagulase-negative staphylococci (CoNS), can substantially affect human and animal health, information on NAS species distribution in poultry slaughterhouses and their antimicrobial resistance (AMR) profiles is limited. In this study, we analyzed the prevalence of NAS species and AMR profiles of NAS isolates collected from poultry slaughterhouses, including chicken carcasses and facility environments. In total, 100 NAS isolates were collected from six poultry slaughterhouses in Korea. The AMR patterns of the NAS species and the major genetic ele

INTRODUCTION: Postoperative Staphylococcus and streptococcal toxic shock syndrome (TSS) are associated with significant morbidity and mortality rates. As a result, surgical awareness is required to recognize and treat TSS appropriately. We report a pediatric case of TSS after thoracentesis for a pneumothorax.
CASE PRESENTATION: A 14-year-old boy was diagnosed with a right pneumothorax and underwent thoracentesis with a trocar catheter. After 2 days, the patient developed a fever, headache, vomiting, and diarrhea. No obvious contamination of the drain puncture wound was observed. He was diagno
