Redefining Ketamine Pharmacology for Antidepressant Action: Synergistic NMDA and Opioid Receptor Interactions?

Ketamine is a racemic compound and medication comprised of (S)-ketamine and (R)-ketamine enantiomers and its metabolites. It has been used for decades as a dissociative anesthetic, analgesic, and recreational drug. More recently, ketamine, its enantiomers, and its metabolites have been used or are being investigated for the treatment of refractory depression, as well as for comorbid disorders such as anxiety, obsessive-compulsive, and opioid use disorders. Despite its complex pharmacology, ketamine is referred to as an N-methyl-d-aspartate (NMDA) receptor antagonist. In this review, the author

Dissociative psychosis or dissociative schizophrenia? Comparison of two phenomena.

BACKGROUND: In recent years, researchers have reported crucial advances in the understanding of "Dissociative psychosis" and "Dissociative schizophrenia". While clinical studies in this area have been sustained for well, it remains to be established for some aspects that a clear and valid relationship exists between dissociation, childhood traumatic experiences, and schizophrenia or psychotic spectrum disorders. METHODS: To test such hypotheses, we divided the patients into two groups; the first group consisted of patients with psychotic disorders not otherwise specified (PNOS), and the secon

Investigating risk factors of dissociative and complex posttraumatic stress disorder across diagnostic systems and potential implications: Latent class analyses.

OBJECTIVE: Posttraumatic stress disorder (PTSD) and more complex posttraumatic symptomatology (i.e., dissociative PTSD [D-PTSD] and complex PTSD [CPTSD]) are differently described in the Diagnostic and Statistical Manual of Mental Disorders (5th ed.; DSM-5) and the International Classification of Diseases (11th ed.; ICD-11). Although the choice of system may affect diagnostic prevalence rates and treatment outcome, less is known about the more complex symptoms and their associated risk factors. METHOD: To investigate both D-PSTD and CPTSD in Northern Irish military veterans (n = 436) using la

Brain white matter structural connectivity of trauma and trauma-related dissociation disorders and symptoms.

BACKGROUND: Experiencing repeated childhood traumatisation impacts brain structure and function in individuals with dissociative identity disorder (DID) and post-traumatic stress disorder (PTSD). Quantitative grey matter neuroimaging research has shown aberrant volumes in traumatised individuals, however studies examining white matter are sparse, particularly for DID. The present study aims to examine white matter alterations of people with trauma-related disorders. METHODS: Sixty-five female participants were included in this study: 33 diagnosed with a trauma-related disorder, namely 17 with

Dissociation and substance abuse among people with PTSD: Results from the National Survey for Stress and Health in Japan.

Patients with posttraumatic stress disorder (PTSD) usually have other psychiatric comorbidities. This study analyzed data from a national survey in Japan (N = 1005 trauma-exposed adults) and examined co-occurring dissociation and substance abuse in patients with probable PTSD. Participants completed standardized screening measures of PTSD, dissociative symptoms, and substance abuse at baseline (T1), and then reported their levels of substance abuse again after 3 months (T2). Of participants who screened for PTSD at T1 (n = 639), 36.1 % reported dissociative symptoms, and 61.8 % reported substa

A Report of Three Cases With Dissociative Disorder: Some Positive Response to Methylphenidate Extended Release Used for Their Concurrent Attention Deficit Hyperactivity Disorder.

Although no psychotropic medication is specifically effective for dissociative symptoms, certain abusive substances can markedly alter these symptoms. Regarding psychotropic medications, a positive response was observed in certain patients with dissociative symptoms who also have attention deficit hyperactivity disorder (ADHD), for which methylphenidate extended release (MER) was administered. A is a female in her twenties and has dissociative identity disorder (DID) and ADHD. Primary personality A1 is usually on duty but always on guard, as her other personality, A2, often attempts to disturb

Precipitating Factors for Functional Cognitive Disorder.

BACKGROUND: The underlying mechanisms of functional cognitive disorder (FCD) are still poorly understood. This hinders diagnostic identification and establishment of personalised and effective treatments. We aimed to describe the precipitating factors for new-onset FCD and explore differences between the acute and gradual onset FCD groups. METHODS: Retrospective analysis of a consecutive sample of FCD patients seen in three dedicated clinics in the United Kingdom between September 2023 and March 2024. Only patients with at least one-year symptom duration were included. We extracted mode of on

Effect of intravenous esketamine on postoperative sleep disturbance, anxiety, and depression in elderly patients undergoing laparoscopic abdominal surgery: a randomized controlled trial.

BACKGROUND: The population of elderly individuals undergoing surgical procedures is increasing, necessitating effective postoperative management strategies. Postoperative sleep disturbance, anxiety, and depression are significant contributors to overall recovery in this demographic, especially following laparoscopic abdominal surgery. METHODS: This study included 200 records of elderly patients undergoing laparoscopic abdominal surgery. Patients were divided into an esketamine group, receiving intravenous esketamine, and a control group, receiving normal saline. Parameters such as surgery and

Dissociative experiences in individuals with subclinical psychosis and a history of developmental trauma: a qualitative study.

Background: Among individuals with psychotic experiences, those with a history of developmental trauma face greater symptom severity and worse clinical outcomes compared to those without. Dissociation constitutes a prominent, characteristic of this group's clinical presentation, whose nuances and associated characteristics remain however understudied in psychosis research. We aimed to address this gap by conducting a qualitative study to investigate the phenomenology, context, and impact of dissociative experiences in individuals with subclinical psychosis and a history of developmental trauma

Employment and seizure frequency as important predictors of health-related quality of life in people with functional/dissociative seizures.

BACKGROUND: There are inconsistent results on health-related quality of life (HRQoL) in patients with functional/dissociative seizures (FDS), particularly in relation to the predictors. The aim of the present study was to investigate variables predicting HRQoL in a sample of patients with FDS who had been hospitalized for an average of 5.6 years. METHODS: People with a confirmed diagnosis of FDS who had already received inpatient treatment years ago were contacted to participate to the survey by completing a self-report questionnaire. They were asked to rate their current HRQoL and provide in

Is Internet Gaming Disorder Associated with Trauma and Stress? Testing the Trauma Model of Addiction Among Young Adults.

Internet gaming disorder (IGD) is a growing public health concern, with longitudinal studies linking it to a range of negative outcomes, including psychological distress and poor academic achievements. While the trauma model has been applied to other addictive behaviors, little is known about the relationship between childhood trauma, trauma-related symptoms, and IGD symptoms. This preliminary study investigated whether childhood betrayal trauma would have a stronger relationship with IGD symptoms than non-betrayal trauma and whether IGD symptoms would be associated with interpersonal stress a

Maladaptive daydreaming should be included as a dissociative disorder in psychiatric manuals: position paper.

Maladaptive daydreaming is a distinct syndrome in which the main symptom is excessive vivid fantasising that causes clinically significant distress and functional impairment in academic, vocational and social domains. Unlike normal daydreaming, maladaptive daydreaming is persistent, compulsive and detrimental to one's life. It involves detachment from reality in favour of intense emotional engagement with alternative realities and often includes specific features such as psychomotor stereotypies (e.g. pacing in circles, jumping or shaking one's hands), mouthing dialogues, facial gestures or en

Neuropsychological Insights Into Child Survival: Possible Protective Dissociation During Prolonged Entrapment After a Turkish Earthquake.

BACKGROUND Earthquakes and other catastrophic events can lead to conditions of being trapped under the rubble, which can cause minor-major health consequences. One of the most challenging issues in entrapment is the need to rescue people as fast as possible. A related issue for rescue forces is how long to continue searching survivors. Indeed, sometimes victims survive longer than expected. The cases described below led us to postulate that certain adverse psychological effects of being trapped under the rubble may be beneficial for survival in such extreme situations. CASE REPORT In this arti

How-To Study Dissociative Symptoms in a Broad Range of Mental Disorders: A Methodological Primer.

Dissociative symptoms constitute a transdiagnostic phenomenon not only characterizing dissociative disorders but also occurring across a broad range of other mental disorders such as posttraumatic stress disorder or borderline personality disorder. In the latter disorders, dissociative symptoms such as depersonalization, derealization, or gaps in awareness significantly burden patients' wellbeing and functioning. Many efforts have been undertaken to better understand these debilitating symptoms. However, empirical findings have not yet converged in many areas (e.g., considering neurobiological

The Relationship Between Functional Seizures, Dissociation, and Gender Dysphoria: A Case Report and Review.

Psychogenic non-epileptic seizures (PNES), also known as functional seizures, clinically mimic seizure-like activity without the presence of brain wave abnormalities on electroencephalogram (EEG). PNES, among other functional neurological disorders, demonstrates an increased prevalence among transgender and non-binary individuals, particularly those with ongoing gender dysphoria. It is established that PNES episodes can be triggered by states of severe distress and that mirror-gazing may cause distress in individuals with gender dysphoria. Herein, we review the available research on the pathop

The Role of Disgust Emotion in Eating Disorders and Its Relationship with Dissociative Symptoms.

Background: Disgust is a central emotion in eating disorders, influencing both their development and maintenance. High sensitivity to disgust has been associated with restrictive and purgative behaviors, as well as with greater severity of eating disorder symptoms. Despite its importance, the different aspects of disgust are rarely examined in depth, limiting the understanding of its role in these disorders. Since the emotion of disgust is more closely linked to the body and food than other emotions, the aim of this paper is to understand its possible role in eating disorders and obesity. Meth

Advances in the study of NMDA receptors in depression pathogenesis and the antidepressant efficacy of their antagonists.

N-methyl-D-aspartate receptors (NMDA receptors) play a crucial role as ionotropic glutamate receptors in regulating neuroplasticity, learning, memory, and a range of psychiatric disorders. Studies indicate that dysfunction of NMDA receptors is a key pathological mechanism in depression, where abnormal activation can result in neuronal excitotoxicity, excessive extracellular calcium ion accumulation, and disrupted neuroplasticity. As a non-competitive NMDA receptor antagonist, ketamine quickly relieves depressive symptoms by decreasing the activity of extracellular NMDA receptors and activating

Depersonalisation-derealisation as a transdiagnostic treatment target: a scoping review of the evidence in anxiety, depression, and psychosis.

INTRODUCTION: Depersonalisation and derealisation (DPDR) describe dissociative experiences involving distressing feelings of disconnection from oneself or one's surroundings. Such experiences are common transdiagnostically across the range of mental health presentations, with evidence to suggest they may even play an active role in the development and maintenance of other mental health concerns. If substantiated, DPDR could present a plausible novel transdiagnostic treatment target. The objective of this scoping review was to therefore to synthesise the evidence-base regarding DPDR as a transd

Dissociative symptoms in school-aged adopted children who experienced maternal separation and disruptive caregiving in infancy.

Amounting findings on maternal separation and early disturbed caregiving suggest that this type of early experience negatively influences socioemotional development and may be associated with behavioral and mental health problems in later life. Concerning previously published studies, we have assessed if maternal separation and disrupted caregiving before adoption in infancy could be related to heightened levels of dissociative symptoms and behavioral problems in middle childhood. We involved 30 children (sample S1) who had experienced maternal separation after birth and short-term institution
