[Significance of and considerations relating to the diagnosis of dissociation].

Dissociation involves a variety of clinical concepts, and some disorders are also similar to but not the same as dissociation. Dissociative disorder is considered to be a pathological phenomenon based on a psychological mechanism. Accurate diagnosis is very important to ensure that patients receive adequate treatment, although such consideration is not unique to dissociative disorder. Accordingly, there is a need to carefully distinguish dissociation from other disorders. In this article, we outline a number of dissociative and dissociative-like states from the symptoms of amnesia, depersonali

Acute stress disorder revisited.

Acute stress disorder (ASD) was introduced into the Diagnostic and Statistical Manual (DSM) taxonomy in 1994 to address the lack of a specific diagnosis for acute pathological reactions to trauma and the role that dissociative phenomena play both in the short- and long-term reactions to trauma. In this review, we discuss the history and goals of the diagnosis and compare it with the diagnoses of acute stress reaction, combat stress reaction, and posttraumatic stress disorder (PTSD). We also evaluate the research on the validity and limitations of ASD as a diagnosis, the relationship between pe

[Toward the new classification of mental disorders--Polish psychiatrists' opinions about project of ICD-11].

AIM: The World Psychiatric Association (WPA) and the World Health Organization (WHO) initiated a series of national surveys regarding the revision of the chapter on Mental and Behavioural Disorders of the ICD-10. They are part of the global and systematic studies providing input into the new classification of mental disorders (ICD-11). METHOD: In Poland, the survey after translation and implementing was performed electronically in collaboration with the Polish Psychiatric Association (17 Jan - 22 Feb 2011). 206 psychiatrists presented their experiences and ideas regarding international and ot

Self-reported potentially traumatic life events and symptoms of post-traumatic stress and dissociation.

OBJECTIVE: To investigate single potentially traumatic events and cumulative effects of these events based on the reported symptoms of post-traumatic stress and dissociation. An additional goal was to evaluate the psychometric properties of Life Incidence of Traumatic Events-Student scale (LITE-S). METHODS: 400 adolescents from the normative population answered the questionnaire Life Incidence of Traumatic Experiences (LITE-S) together with Trauma Symptom Checklist for Children (TSCC), Dissociation-Questionnaire-Sweden (Dis-Q-Sweden) and Adolescent-Dissociative Experience Scale (A-DES). [Pierre Janet's views on the etiology, pathogenesis and therapy of dissociative and conversion disorders ("hysterias")].

Pierre Janet (1859-1947) is one of those more or less forgotten authors whose approach differed significantly from Freud's psychoanalysis. In the first part of this paper, Janet's conception of hysteria is discussed and his place in French psychiatry described. Different aspects of Janet's diathesis-stress-approach are presented (particularly his important pathogenic concept of fixed ideas) which refer not only to a conception of hysteria, but also to traumatic (stress) disorders and other psychological disturbances. The second part of the paper details the varieties of Janetian therapeutic tr

Adolescent dissociation and alexithymia are distinctive but overlapping phenomena.

Findings on the association between dissociation and alexithymia in adult populations are inconsistent. Dissociation has also been related to other types of psychopathology. Few studies have been published on these connections among adolescents. We examined the association between Adolescent Dissociative Experiences Scale (A-DES) scores and those for the Toronto Alexithymia Scale (TAS), the 21-item Beck Depression Inventory (BDI-21) and the Youth Self-Report (YSR), as well as subscales of the TAS and the YSR, in a sample of Finnish adolescents aged 13 to 18 years (n=4019). Out-of-body experiences and physical body activity and posture: responses from a survey conducted in Scotland.

Although there have been studies of some factors related to the features of out-of-body experiences (OBEs), the effect of physical body posture and activity has not been systematically explored. Over the years research has suggested that OBEs are more frequent in states of low physical activity and when the body is supine, in agreement with other findings related to alterations in consciousness. Thus, we predicted that OBEs would be associated with lying down and little or no physical activity, and that these factors would show a higher number of OBE features than OBEs in which the person was

Auditory pseudohallucinations in United Kingdom war veterans and civilians with posttraumatic stress disorder.

OBJECTIVE: Hearing voices is a little-known feature of posttraumatic stress disorder (PTSD), mainly reported in US war veterans; it may be better conceived of as a dissociative than as a psychotic phenomenon. We investigated this feature in a pair of studies: Study 1 tested whether hearing voices was also reported by United Kingdom war veterans and whether it was associated with other dissociative reactions. Study 2 tested whether reports of hearing voices would generalize to a civilian sample, evaluated whether it was specific to PTSD or could be explained by trauma exposure alone, and invest

Development and psychometric characteristics of the Dissociation Tension Scale.

The newly developed Dissoziations-Spannungs-Skala (Dissociation Tension Scale; DSS) is a self-rating instrument for the assessment of psychological and somatoform dissociative features (ranging from normal up to pathological) as well as aversive inner tension occurring within the past 7 days. The DSS contains 21 items assessing dissociative symptoms and 1 additional item assessing aversive inner tension. Ratings are made on a time-oriented scale ranging from 0% (never) to 100% (constantly). We measured the psychometric qualities of the DSS in a total of 294 patients and healthy controls. Inter

Association of trauma-related disorders and dissociation with four idioms of distress among Latino psychiatric outpatients.

Past research on idioms of distress among U.S. Latinos has revealed that ataque de nervios and altered perceptions, such as hearing and seeing things when alone, are independent markers of higher morbidity and mental health utilization despite having no one-to-one relationships with any single psychiatric diagnosis. It has been proposed that the idioms exert this effect because they are signs of distressing dissociative capacity associated with traumatic exposure. This study examines the relationships in an ethnically diverse Latino psychiatric outpatient sample (N = 230) among interpersonal t

[Disruptive nocturnal behavior in elderly subjects: could it be a parasomnia?].

Parasomnias are sleep-related abnormal behaviors. They are frequent and overlooked causes of nocturnal disruptive behavior in the elderly, especially when patients are cognitively impaired. Confusion and violence can result in sleep disruption, injuries for the patients or their bed partners, caregivers distress, and they can be a motive for institutionalization. Parasomnias include the NonREM sleep disorders of arousal (sleepwalking, sleep terrors, confusional arousals and sleep-related eating disorder), the REM sleep behavior disorder (RBD) and more rarely the parasomnia overlap syndrome, wh

Symboldrama, a psychotherapeutic method for adolescents with dissociative and PTSD symptoms: a pilot study.

A total of 15 clinically referred adolescents who had been sexually or physically abused participated in this pilot study of the use of symboldrama psychotherapy. Symboldrama is a psychotherapeutic method that uses imagery as the major psychotherapeutic tool. All adolescents reported to be suffering from a high level of dissociative symptoms and other symptoms such as anxiety, depression, posttraumatic stress, and anger after their traumas. The objective of the study was to test the hypothesis that symboldrama psychotherapy in addition to psycho-education of the non-offending parent would sign

[Memory and trauma impact: a development approach].

The purpose of this article is to review the main neuroanatomic structures and the neurotransmission process involved at memory. Memory is dynamic based on the reunion of aspects of neuronal activation, in a process based on the individual experiences. It will be described the importance of limbic regions and their interaction during processing, codification and consolidation of a memory - each one related with neuronal rebuild. Further the authors describe the memory types known (explicit and implicit memories); the characteristics and their progress during normal development. It will be addr

[The etiological models of posttraumatic stress disorder].

The author summarizes the social-cognitive, behavioral, information processing and psychobiological models of PTSD. All the theories assume that PTSD symptoms are mainly caused by the unique characteristics of trauma memories and the difficulty of integrating them into the other autobiographic memories. Thus integrating the trauma into the other autobiographic memories plays a crucial role in the therapy of trauma.

Using mirror visual feedback and virtual reality to treat fibromyalgia.

Fibromyalgia is a condition characterized by long term body-wide pain and tender points in joints, muscles and soft tissues. Other symptoms include chronic fatigue, morning stiffness, and depression. It is well known that these symptoms are exacerbated under periods of high stress. When pain becomes severe enough, the mind can enter what is known as a dissociative state, characterized by depersonalization - the feeling of detachment from one's physical body and the illusion of watching one's physical body from outside. In evolutionary terms, dissociative states are thought to be an adaptive me

Diagnosing somatisation disorder (P75) in routine general practice using the International Classification of Primary Care.

OBJECTIVE: (i) To analyze general practitioners' diagnosis of somatisation disorder (P75) using the International Classification of Primary Care (ICPC)-2-E in routine general practice. (ii) To validate the distinctiveness of the ICD-10 to ICPC-2 conversion rule which maps ICD-10 dissociative/conversion disorder (F44) as well as half of the somatoform categories (F45.0-2) to P75 and codes the other half of these disorders (F45.3-9), including autonomic organ dysfunctions and pain syndromes, as symptom diagnoses plus a psychosocial code in a multiaxial manner. METHODS: Cross-sectional analysis

Pain and emotional processing in psychological trauma.

Extreme psychological and physical traumas cause dramatic symptom patterns which are insufficiently described by the psychiatric diagnostic criteria of post traumatic stress disorders (PTSD). Additionally, due to the neurobiological proximity and similarity of processing mechanisms of physical and psychological pain stimulation and extremely negative emotions, the patients often suffer from persistent pains even after the somatic healing process is completed. Epidemiological studies confirm the joint occurrence of pain and PTSD. The close relationship and the etiological and behavioral similar

Apparitional experiences of new faces and dissociation of self-identity during mirror gazing.

Apparitional experiences during mirror gazing were studied. Under low levels of illumination, individuals gazed at their own reflected faces in a mirror for a duration of 10 min. Face illumination was relatively uniform, via a nonvisible light source. After about a minute of mirror gazing, individuals reported perceiving strange faces--archetypical and often unknown, human or animal, living or departed parents with traits changed, and fantastical monstrous beings--instead of their own faces in the mirror. During these apparitions, the observers reported feeling that a strange person was watchi
