Dissociative state and competence.

This report presents the results of forensic evaluation of the civil competence of a case of alleged dissociative identity disorder (DID) and discusses whether such dissociative states substantially jeopardize civil competence. A 40-year-old woman claimed that she had had many personalities since her college days. From the age of 37 to 40, she shopped excessively, which left her with millions of dollars of debt. She ascribed her shopping to a certain identity state, over which she had no control. (In this article, we use the term identity state to replace personality as an objective descriptio

Cortisol, dehydroepiandrosterone, and estradiol measured over 24 hours in women with childhood sexual abuse-related posttraumatic stress disorder.

Preclinical studies have shown long-term alterations in several hormonal systems including cortisol, dehydroepiandrosterone (DHEA) and DHEA-Sulfate, and estradiol. The purpose of this study was to assess cortisol, DHEA, and estradiol over a 24-hour period in women with early childhood sexual abuse and posttraumatic stress disorder (PTSD); with early abuse and without PTSD; and women without early abuse or PTSD. Forty-three women with early childhood sexual abuse and PTSD, early abuse without PTSD, and without abuse or PTSD, underwent a comprehensive assessment of hormones in plasma at multiple

Introduction to the special issue on personality assessment in medical settings.

This special issue of the Journal of Personality Assessment brings together 3 review articles and 5 research studies on personality assessment in medical settings that should help clinicians become more familiar with the current status of this field. In 1 review article, Sirri, Fabbri, Fava, and Sonino (2007/this issue) summarize evidence of a new approach to the assessment of psychological syndromes in somatizing patients based on the Diagnostic Criteria for Psychosomatic Research (Fava et al., 1995). The other reviews focus on 2 personality constructs widely used in investigations with medic

"Virginia Woolf's The waves: a lyrical 'sense of continuity' in a sea of dissociation".

The Waves, written in 1931 by Virginia Woolf, is an astounding depiction of an internal landscape richly full of dissociative phenomena. Although applauded for its genius and considered by many to be her highest literary achievement, critics complained that the language of the characters seemed artificial and a departure from the usual interior monologue appearing in other works that utilized stream of consciousness writing at the time. Woolf believed this work to be misunderstood as she often felt herself. This paper illustrates how The Waves might metaphorically connote the internal experien

Acute posttraumatic stress symptoms and depression after exposure to the 2005 Saskatchewan Centennial Air Show disaster: prevalence and predictors.

OBJECTIVES: The purpose of this study was to determine the prevalence of acute distress-that is, clinically significant posttraumatic stress symptoms (PTSS) and depression-and to identify predictors of each in a sample of people who witnessed a fatal aircraft collision at the 2005 Saskatchewan Centennial Air Show. DESIGN: Air Show attendees (N = 157) were recruited by advertisements in the local media and completed an Internet-administered battery of questionnaires. RESULTS: Based on previously established cut-offs, 22 percent respondents had clinically significant PTSS and 24 percent had cl

Internet-based treatment for PTSD reduces distress and facilitates the development of a strong therapeutic alliance: a randomized controlled clinical trial.

BACKGROUND: The present study was designed to evaluate the efficacy of an internet-based therapy (Interapy) for Posttraumatic Stress Disorder (PTSD) in a German speaking population. Also, the quality of the online therapeutic relationship, its development and its relevance as potential moderator of the treatment effects was investigated. METHOD: Ninety-six patients with posttraumatic stress reactions were allocated at random to ten sessions of Internet-based cognitive behavioural therapy (CBT) conducted over a 5-week period or a waiting list control group. Severity of PTSD was the primary out

[Pseudo-dementia conversion and post-traumatic stress disorder].

BACKGROUND: Post-traumatic stress disorder (PTSD) is often associated with other psychiatric syndromes. However, studies exploring conversion and PTSD comorbidity are scarce. CASE-REPORT: This paper reports the case of a 45 year-old patient without medical or psychiatric history. In 2003, he suddenly started suffering from amnesia and symptoms of delirium: he was at his office with a cup of coffee but he did not remember why. Aphasia, trembling, behavioural disorders appeared over the next hours and days. Numerous neurological examinations and laboratory tests (including cerebral imagery) wer

Multiplicity of selves and others: cognitive analytic therapy.

In cognitive analytic therapy (CAT), multiplicity is understood in terms of a range of self-other patterns (reciprocal role relationships) originating in childhood. These alternate in determining experience and action according to the situation (contextual multiplicity). They may be restricted by adverse childhood experiences (diminished multiplicity), and severe deprivation or abuse may result in a structural dissociation of self-processes ( pathological multiplicity). Therapeutic interventions, to be effective, must be based on an understanding of the structure of the individual patient's se

Childhood maltreatment, complex trauma symptoms, and unresolved attachment in an at-risk sample of adolescent mothers.

Associations between unresolved attachment, abuse history, and a wide range of trauma-related symptomatology were examined in an at-risk sample (N = 62). Fifty percent reported severe childhood physical and/or sexual abuse. An independent trauma interview elicited more reports of childhood sexual abuse than the Adult Attachment Interview (AAI); conversely, the AAI elicited more reports of physical abuse. Childhood physical abuse, sexual abuse, and general maltreatment were associated with unresolved status. Furthermore, sexual abuse history and general maltreatment predicted unresolved loss, s

Dispelling confusion about traumatic dissociative amnesia.

How survivors of trauma remember--or forget--their most terrifying experiences lies at the core of one of the most bitter controversies in psychiatry and psychology: the debate regarding repressed memories of childhood sexual abuse. Most experts hold that traumatic events--those experienced as overwhelmingly terrifying and often life-threatening--are remembered very well; however, traumatic dissociative amnesia theorists disagree. Although acknowledging that traumatic events are usually memorable, these theorists nevertheless claim that a sizable minority of survivors are incapable of remember

Amnesia and crime.

Amnesia for serious offenses has important legal implications, particularly regarding its relevance in the contexts of competency to stand trial and criminal responsibility. Forensic psychiatrists and other mental health professionals are often required to provide expert testimony regarding amnesia in defendants. However, the diagnosis of amnesia presents a challenge, as claims of memory impairment may stem from organic disease, dissociative amnesia, amnesia due to a psychotic episode, or malingered amnesia. We review the theoretical, clinical, and legal perspectives on amnesia in relation to

Neuropsychological evidence for a strategic control of multiple routes in imitation.

Previous studies have suggested that imitators can reproduce known gestures shown by a model using a semantic, indirect route, and novel gestures using a sublexical, direct route. In the present study we aimed at testing the validity of such a dual-route model of action imitation. Patients with either left-brain damage (LBD) or right-brain damage (RBD) were tested on an action imitation task. Actions were either meaningful (n = 20) or meaningless (n = 20), and were presented in an intermingled list and, on a different day, in separate lists. We predicted that, in the mixed condition, patients

Case series: mental health needs and perspectives of rural children reared by parents who abuse methamphetamine.

OBJECTIVE: This case-based, mixed-methods study was undertaken to understand the perspectives and mental health needs of rural children exposed to parental methamphetamine abuse. METHOD: Participants were 23 children involved with a state child protective agency because of parental methamphetamine abuse. A semistructured interview provided information on children's perspectives of their families. Information on children's mental health needs was obtained from the Child Behavior Checklist and Trauma Symptom Checklist. Case records and caseworker reports provided information on children's famil

Forward inference using functional neuroimaging: dissociations versus associations.

Many people are excited by functional neuroimaging as a new tool for cognitive science; many others are sceptical. In this opinion article, I describe a 'forward inference' that one can make from patterns of brain activity to distinguish between cognitive theories. I give an example of forward inferences in research on recognition memory, and outline some statistical criteria for a 'qualitative difference' in brain activity. Forward inferences resemble the dissociation logic long-used in behavioural studies of healthy and brain-damaged people, although I argue that dissociations in neuroimagin

Perceived safety in disaster workers following 9/11.

The perception of being safe, perceived safety, is an important component of health and the ability to work after exposure to traumatic events of all kinds. The relationship of perceived safety to posttraumatic stress disorder and depression has rarely been examined. This study examined symptoms of posttraumatic stress disorder, depression, and perceived safety in disaster workers 2 weeks after the 9/11 terrorist attacks. Perceived safety was lower in those with greater exposure (e.g., those who felt they were in physical danger, worked with dead bodies, or witnessed someone being killed or se

Human brain evolution and the "Neuroevolutionary Time-depth Principle:" Implications for the Reclassification of fear-circuitry-related traits in DSM-V and for studying resilience to warzone-related posttraumatic stress disorder.

The DSM-III, DSM-IV, DSM-IV-TR and ICD-10 have judiciously minimized discussion of etiologies to distance clinical psychiatry from Freudian psychoanalysis. With this goal mostly achieved, discussion of etiological factors should be reintroduced into the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-V). A research agenda for the DSM-V advocated the "development of a pathophysiologically based classification system". The author critically reviews the neuroevolutionary literature on stress-induced and fear circuitry disorders and related amygdala-driven, species-atypic
