[Schizophrenia diagnostic announcement in a French psychiatric unit].

Announcement of schizophrenia diagnostic to the patients is a topical issue in France. The evolution in clinical practices, a better efficiency in therapeutic procedures and the fundamental right of the patient to obtain information have initialised the discussion of its interest. Spontaneous claim for information from the patient is rarely observed although awareness troubles might be reported at the instauration of the mental disorder or during its evolution. Methodological studies concerning the diagnostic announcement are limited. Except the Bayle studies recently published, only a few pub

From dissociation to trauma? Individual differences in dissociation as predictor of 'trauma' perception.

In clinical literature, dissociative complaints are generally considered to be the result of traumatic experiences. However, it has been argued that dissociative complaints, in turn, may indulge over-reporting of traumatic experiences. Hence, correlations between dissociation and self-reported trauma may not only reflect the causal path from trauma to dissociation, but possibly also an increased trauma-item endorsement due to dissociation. To test the idea that dissociation fosters a positive response bias on trauma self-reports, participants completed a dissociation scale, and subsequently ra

Dissociative disorders.

The dissociative disorders, including "psychogenic" or "functional" amnesia, fugue, dissociative identity disorder (DID, also known as multiple personality disorder), and depersonalization disorder, were once classified, along with conversion disorder, as forms of hysteria. The 1970s witnessed an "epidemic" of dissociative disorder, particularly DID, which may have reflected enthusiasm for the diagnosis more than its actual prevalence. Traditionally, the dissociative disorders have been attributed to trauma and other psychological stress, but the existing evidence favoring this hypothesis is p

Fantasy proneness, but not self-reported trauma is related to DRM performance of women reporting recovered memories of childhood sexual abuse.

Extending a strategy previously used by , we administered a neutral and a trauma-related version of the Deese-Roediger-McDermott paradigm to a sample of women reporting recovered (n=23) or repressed memories (n=16) of childhood sexual abuse (CSA), women reporting having always remembered their abuse (n=55), and women reporting no history of abuse (n=20). We found that individuals reporting recovered memories of CSA are more prone than other participants to falsely recalling and recognizing neutral words that were never presented. Moreover, our study is the first to show that this finding even

Is dissociation a multidimensional construct? Data from the Multiscale Dissociation Inventory.

The dimensionality of dissociation was examined in a combined sample of 1,326 general population, clinical, and university participants who completed the Multiscale Dissociation Inventory (MDI). Principal components analysis identified five moderately intercorrelated factors (mean r = .39): Disengagement, Identity Dissociation, Emotional Constriction, Memory Disturbance, and Depersonalization/Derealization. Differential relationships were found between individual MDI factors and demographics, trauma history, clinical status, posttraumatic stress, and scores on other dissociation measures. Surp

A proposed trial of dialectical behavior therapy and trauma model therapy.

Dissociative identity disorder and borderline personality disorder resemble each other in trauma histories and comorbidity. Each disorder is frequently comorbid with the other. Treatment outcome data for Dialectical Behavior Therapy of borderline personality disorder and Trauma Model Therapy of dissociative identity disorder are reviewed. The author proposes a psychotherapy treatment study in which there are three subject groups and two treatment conditions. The subject groups are borderline personality disorder without dissociative identity disorder; dissociative identity disorder without bor

Discriminating among diagnostic categories using the Dissociative Disorders Interview Schedule.

The Dissociative Disorders Interview Schedule was administered to 1,308 subjects in eight diagnostic categories, including 296 with dissociative identity disorder. The study tested three hypotheses: (1) the Mahalanobis distance between dissociative identity disorder and each of seven other diagnostic categories would be large, (2) the closest diagnostic category to dissociative identity disorder would be dissociative disorder not otherwise specified, and (3) nondissociative diagnostic categories would be closer to each other than any one to dissociative identity disorder. All three hypotheses

Psychogenic nonepileptic seizures.

Psychogenic nonepileptic seizures are episodes of movement, sensation, or behaviors that are similar to epileptic seizures but do not have a neurologic origin; rather, they are somatic manifestations of psychologic distress. Patients with psychogenic nonepileptic seizures frequently are misdiagnosed and treated for epilepsy. Video-electroencephalography monitoring is preferred for diagnosis. From 5 to 10 percent of outpatient epilepsy patients and 20 to 40 percent of inpatient epilepsy patients have psychogenic nonepileptic seizures. These patients inevitably have comorbid psychiatric illnesse

Relationships between trauma and psychosis: an exploration of cognitive and dissociative factors.

OBJECTIVE: There is a growing awareness of the relationship between trauma and psychosis. Studies have found associations between traumatic life experiences and the development of psychosis. METHOD: This study examined the prevalence of trauma exposure and post-traumatic stress disorder (PTSD) in 32 people with psychotic diagnoses and investigated associations between specific traumas and psychotic symptoms. The role of dissociation and cognitive factors was also explored. RESULTS: The prevalence of lifetime trauma was high (94% of the sample reported at least one traumatic event). The preva

Self-burning versus self-cutting: patterns and implications of self-mutilation; a preliminary study of differences between self-cutting and self-burning in a Japanese juvenile detention center.

The purpose of the present paper was to examine the differences in clinical features between self-cutters and self-burners, to clarify clinical implications of self-mutilating behaviors other than self-cutting. Subjects were 201 delinquent adolescents consecutively entering a Japanese juvenile detention center from February 2003 to March 2003. The subjects were assessed using a self-reporting questionnaire to evaluate self-mutilation, traumatic events, and problematic behaviors. Beck Depression Inventory-2 (BDI-2) and Adolescent Dissociative Experience Scale (A-DES) were also tested. Subjects

Reality monitoring and motor memory in checking-prone individuals.

Studies concerning reality monitoring and motor memory abilities in checkers have provided mixed results. The aim of this study was to re-examine this question by asking 75 undergraduate students to perform, watch the experimenter perform, imagine themselves performing, imagine the experimenter performing, or verbally repeat different daily actions. Two groups were created (checking-prone and nonchecking-prone subjects) based on participants' checking subscores on the revised version of the Obsessive-Compulsive Inventory. First, results suggested that checking-prone subjects have a poorer moto

The validity of DSM-IV dissociative disorders categories in south-west Uganda.

There is little systematic research on the cross-cultural validity of the dissociative disorders, especially in non-western countries. This study evaluates the fit of the DSM-IV classification and concepts of these disorders with local concepts, experiences and local presentations in south-west Uganda. We conducted focus group discussions with medical students, traditional healers, religious leaders, counselors, community members and other health workers (n=48). They were supplemented by key informant interviews with religious people, traditional healers and leaders (n=11). The responses were

[The relationship between OPD features of personality structure and symptom-related and interpersonal outcome of inpatient psychotherapy].

OBJECTIVE: To investigate the prognostic relevance of the axis "structure" of operationalized psychodynamic diagnostics (OPD) with respect to the symptom-related and interpersonal outcome of inpatient psychotherapy by applying group comparisons as well as individual criteria. METHODS: The structural level of 102 psychiatric inpatients was related to the general and dissociative psychopathology (SCL-90; FDS-20) as well as interpersonal problems (IIP) at the beginning and at the end of inpatient treatment. RESULTS: There was a tendency toward an association between the structural level and psy

Ritual homicide during dissociative trance disorder.

A case report is described of a man who developed a single period of recurrent episodes of dissociative trance disorder (DTD) during a family stress. The cultural environment was permeated by thoughts and beliefs in magic. During one trance state, the man performed an exorcism on the 6-month-old baby of his brother-in-law, and the baby died from cerebral hemorrhage. In the 5-year follow-up, no other episodes of DTD developed, and the man is serving an 18-year sentence.

The epidemiology of depersonalisation and derealisation. A systematic review.

BACKGROUND: Symptoms of depersonalisation (DP) and derealisation (DR) are increasingly recognised in both clinical and non-clinical settings, but their importance and underlying pathophysiology is only now being addressed. METHODS: This paper is a systematic review of the current state of knowledge about the prevalence of depersonalisation and derealisation using computerised databases and citation searches. All potential studies were examined and numerical data included. Three categories of study are reviewed: questionnaire and interview surveys of selected student and non-clinical samples;

[Psychiatric disease in patients with psychogenic non-epileptic seizures referred from an epilepsy unit in a general hospital].

INTRODUCTION: We evaluate psychiatric disease associated with psychogenic non-epileptic seizures (PNES) and study the role played by previous traumatic experiences, dissociative episodes and personality disorders. METHOD: After diagnosing PNES in our epilepsy unit, we obtained a sample of 46 patients and carried out a structured psychiatric interview (SCID) following DSM-III-R criteria. We looked for previous dissociative episodes and prepared a questionnaire for traumatic experiences and basic clinical data. RESULTS: The most frequent psychiatric disorders were depression, anxiety and somat

[Depersonalization: from disorder to the symptom].

In contrast with the growing interest in dissociative disorders over the last few years, depersonalization continues to be very scarcely approached. There is no agreement among clinicians regarding the concept of depersonalization, and little is known about its etiology, epidemiology and treatment. This paper has two main aims: first, review the literature on this pathology focusing on nosological, historical, psychophysiological and treatment aspects, and second, explore the incidence of the depersonalization symptom in other psychiatric conditions, in particular in panic disorder. The Medlin

Dissociative disorder presenting as catatonia.

Three cases of dissociative disorder presenting with catatonia are described. Catatonia is generally believed to be associated with schizophrenia. However, many other conditions are also known to cause catatonia. A brief review of literature is provided. All the cases improved rapidly with a few ECTs. This report aims to highlight the presentation of dissociative disorders with catatonia. It also seeks to bring to notice the need to avoid lumping all non-organic catatonics under the rubric of schizophrenia so as to ensure proper treatment.
