Recognizing and treating uncommon behavioral and emotional disorders in children and adolescents who have been severely maltreated: dissociative disorders.

Children who have experienced severe maltreatment may use dissociation as a strategy to cope with the ongoing trauma of their maltreatment. Although dissociation may help children cope with maltreatment, it can become maladaptive if it is used in other contexts. This article explores the symptoms of dissociate disorders and the ways that maladaptive dissociation can develop in maltreated children. Strategies for distinguishing dissociative disorders from other disorders in children are reviewed and treatment strategies for children and adolescents with maladaptive dissociation, and their famil

Double dissociation between picture naming and comprehension: an electrostimulation study.

We performed a cortico-subcortical language electrical mapping, using a picture naming task and a semantic test of association, during resection of a left posterior temporal glioma in an awake patient. Two discrete naming sites were identified in the posterior part of the left superior temporal gyrus, plus a third distinct site between the two previous areas, inducing specific comprehension disorders without anomia when stimulated. After resection, white matter stimulation elicited anomia without comprehension disorder. We suggest the existence of distinct naming and comprehension systems, whi

Acute stress disorder as a predictor of post-traumatic stress disorder in physical assault victims.

The authors' objective was to examine the ability of acute stress disorder (ASD) and other trauma-related factors in a group of physical assault victims in predicting post-traumatic stress disorder (PTSD) 6 months later. Subjects included 214 victims of violence who completed a questionnaire 1 to 2 weeks after the assault, with 128 participating in the follow-up. Measures included the Harvard Trauma Questionnaire, the Trauma Symptom Checklist, and the Crisis Support Scale. Twenty-two percent met the full PTSD diagnosis and 22% a subclinical PTSD diagnosis. Previous lifetime shock due to a trau

Symptom subtypes of obsessive-compulsive disorder and their relation to dissociation.

We examined relations between obsessive-compulsive disorder (OCD) symptoms and dissociation in three studies. Studies 1 and 2 established a strong level of convergence between our two OCD symptom measures. Specific types of symptoms showed a clear convergent/discriminant pattern, indicating that they can be meaningfully distinguished from one another. In both studies, dissociation correlated more strongly with checking and obsessive intrusions than with cleaning, ordering, and hoarding. Moreover, these associations remained substantial even after controlling for neuroticism (Study 1) and other

Relationship of purported measures of pathological and nonpathological dissociation to self-reported psychological distress and fantasy immersion.

In order to investigate both the psychometric structure of the Dissociative Experiences Survey (DES) and the discriminant validity of the DES-Taxon (Waller, Putnam, & Carlson, 1996) as a specific marker of pathological dissociation, 376 non-clinical community based respondents completed the DES and a battery of psychopathology and imaginative involvement self-report measures. The DES was scored for both the Taxon and the DES-Absorption subscale. A DES subscale purported to tap normative dissociative processes. The two DES subscales demonstrated substantial overlap, both with each other (r = .8

Club drugs: MDMA, gamma-hydroxybutyrate (GHB), Rohypnol, and ketamine.

Club drugs are substances commonly used at nightclubs, music festivals, raves, and dance parties to enhance social intimacy and sensory stimulation. The most widely used club drugs are 3,4-methylenedioxymethamphetamine (MDMA), also known as ecstasy; gamma-hydroxybutyrate (GHB); flunitrazepam (Rohypnol); and ketamine (Ketalar). These drugs are popular because of their low cost and convenient distribution as small pills, powders, or liquids. Club drugs usually are taken orally and may be taken in combination with each other, with alcohol, or with other drugs. Club drugs often are adulterated or

The online disinhibition effect.

While online, some people self-disclose or act out more frequently or intensely than they would in person. This article explores six factors that interact with each other in creating this online disinhibition effect: dissociative anonymity, invisibility, asynchronicity, solipsistic introjection, dissociative imagination, and minimization of authority. Personality variables also will influence the extent of this disinhibition. Rather than thinking of disinhibition as the revealing of an underlying "true self," we can conceptualize it as a shift to a constellation within self-structure, involvin

A validation study of the Adolescent Dissociative Experiences Scale.

OBJECTIVE: The purpose of this study was to further the validation process of the Adolescent Dissociative Experiences Scale (A-DES). In this study, a 6-item Likert response format with descriptors was used when responding to the A-DES rather than the 11-item response format used in the original A-DES. METHOD: The internal reliability and construct validity of the A-DES were examined by analyzing the relationships between A-DES scores, sexual abuse, and behavior. The sample consisted of 65 females ranging in age from 11 to 18 who were categorized into two groups. Thirty-four adolescents who we

[Psychogenic disorders: concepts, terminology and classification].

The neurological interest on functional or psychogenic disorders (mental or physical disturbances with no organic basis, generally unleashed by stressful situations) has been receiving increasingly more interest over the last few years. In this article we review concepts, terms and classifications of these disorders, very different over time and among different authors. Psychogenic disorders are divided into: a) dissociation (with memory, consciousness and self-identity impairment), and b) disturbances with somatizations, divided into somatoform (unconscious), factitious (voluntary search for

Is traumatic amnesia nothing but psychiatric folklore?

Some psychotherapists believe that certain experiences are so overwhelmingly traumatic that some victims become incapable of remembering their worst trauma except under special circumstances (e.g. therapy) many years later. Unfortunately, clinicians who endorse this concept of traumatic amnesia often misinterpret the very studies they adduce in support of it. More specifically, they misinterpret other, unrelated memory phenomena as evidence for traumatic amnesia, such as ordinary forgetfulness, psychogenic amnesia, organic amnesia, incomplete encoding of traumatic experiences, non-disclosure o

Gambling and suicidality in treatment-seeking pathological gamblers.

One hundred twenty-five adult participants recruited from gambling treatment centers were included in an examination of gambling-related suicidal ideation and attempt. In this sample, 48% (N = 60) had a history of gambling-related suicidal ideation, and an additional 12% (N = 15) reported at least one gambling-related suicide attempt. Measures of gambling experience, impulsiveness, and dissociation were evaluated across groups. Level of suicidality was associated with greater gambling severity, gambling escape, dissociation and attention seeking, impulsivity, and generalized dissociative exper

[Factitious disorder and factitious disorder by proxy].

Similar to the adult patient, a child or adolescent may actively feign or produce artificial symptoms (synonymous: Munchausen syndrome). The more frequent case is that the child suffers from being an object of symptom fabrication induced by a close person caring for the child, regularly the mother (Munchausen syndrome by proxy). This review focuses on psychopathological aspects of the clinically more relevant factitious disorder by proxy. Typical behaviour and personality characteristics are presented that can be taken as clinical warning signs. Doctor-mother-interaction is affectively challen

Self creation and the limitless void of dissociation: the 'as if' personality.

The concept of the 'as if' personality has been used variously in analytic literature without having formed part of a clinically based theoretical development over time. The author discusses the bases of her notion of the 'as if' personality, as observed across a number of patients and supervised patients in intensive, long term analytic treatment. In this composite clinical picture, a grouping of elements that form a particular kind of defence of the self is identified in certain patients with an exceptional capacity for creative engagement in the world, surpassing expectations given their ba

Does dissociation offer a useful explanation for psychopathology?

BACKGROUND: Dissociation is often conceptualised as an altered state of consciousness, a trance-like state in which normal barriers between conscious and unconscious memories, desires and beliefs break down and other amnestic barriers emerge. This review explores whether it is likely that there is a neurophysiology of pathological dissociative processes that will elucidate management. METHOD: A critical reading of current research, sourced through Medline and Psychinfo searches from 1990 to 2002, using subject headings: dissociative disorders, hypnosis and stress disorder (post-traumatic), as

Trauma-related dissociation: conceptual clarity lost and found.

OBJECTIVE: Imprecise conceptualizations of dissociation hinder understanding of trauma-related dissociation. An heuristic resolution for research and clinical practice is proposed. METHOD: Current conceptualizations of dissociation are critically examined. They are compared with a new theory that incorporates classical views on dissociation with other contemporary theories related to traumatization, viewing dissociation as a lack of integration among psychobiological systems that constitute personality, that is, as a structural dissociation of the personality. RESULTS: Most current views of

Establishing and running a trauma and dissociation unit: a contemporary experience.

OBJECTIVE: To evaluate the functioning of a trauma and dissociation unit that has run for the past 8 years in a private hospital, with particular regard to operating philosophy, operating parameters, challenges encountered, research and educational initiatives, and the applicability of the treatment model to other settings. CONCLUSIONS: Despite the challenges associated with significant difficulties in the corporate management of a private health-care system, it has been possible to operate an inpatient and day hospital programme tailored to the needs of patients in the dissociative spectrum,

[Hospitalization of non-stroke patients in a Stroke Unit].

BACKGROUND AND OBJECTIVE: Stroke care in Germany has substantially improved during the last decade. One column of modern stroke care is the institution of stroke unit which allows rapid diagnosis and treatment. The aspect of admission of nonstroke patients to a stroke unit is poorly evaluated. The aim of this study is to evaluate the number of patients who are admitted to a national stroke unit but do not suffer from stroke. Furthermore, we related the proportion of nonstroke referrals to the different referral modes. PATIENT AND METHODS: Observational study recording all suspected stroke ref

Childhood adversities as risk factors for alexithymia and other aspects of affect dysregulation in adulthood.

BACKGROUND: Affect regulation is assumed to be a biologically based function that can become disrupted by inadequate parenting and by traumatic experiences. We studied the relation between the perceived parental parenting style, and sexual and physical abuse, with alexithymia, dissociation, anxiety and depression. METHODS: In a cross-sectional study psychiatric outpatients were administered a structured interview on childhood physical and sexual abuse and they completed a number of questionnaires about the parenting styles of their parents, and about alexithymia, dissociation and mood patholo

Patterns of self-cutting: a preliminary study on differences in clinical implications between wrist- and arm-cutting using a Japanese juvenile detention center sample.

The present study was aimed to clarify the differences in clinical implications between wrist- and arm-cutting. Subjects were 201 delinquent adolescents (178 males and 23 females) who had been admitted to a detention center from February to March 2003. A self-reporting questionnaire and the Adolescent Dissociative Experience Scale (ADES) were given. Traumatic events and other self-injurious behavior were compared among four groups. In total, 33 (16.4%) subjects reported wrist- and/or arm-cutting. Of the females, 60.9% (n = 14) had experienced self cutting behaviors compared to 10.7% of males (

Understanding 'not': neuropsychological dissociations between hand and head markers of negation in BSL.

Simple negation in natural languages represents a complex interrelationship of syntax, prosody, semantics and pragmatics, and may be realised in various ways: lexically, morphologically and prosodically. In almost all spoken languages, the first two of these are the primary realisations of syntactic negation. In contrast, in many signed languages negation can occur without lexical or morphological marking. Thus, in British Sign Language (BSL), negation is obligatorily expressed using face-head actions alone (facial negation) with the option of articulating a manual form alongside the required
