The psychological organization of multiple personality disordered patients as revealed in psychological testing.

The reader familiar with the borderline literature may see a correspondence between certain of our MPD/DD Rorschach results and typical borderline characteristics. Most notably, both groups reason and view others in unusual but not psychotic ways and demonstrate certain difficulties in affect integration. On the other hand, the MPD/DD group exhibits many attributes that contradict predictions one would make from a borderline perspective, and which support Kluft's assertion that the majority of these patients have a more complex and structured personality system. Rather than holding oversimplif

Clinical presentations of multiple personality disorder.

It is hoped that this discussion of MPD will discourage stereotypic thinking about this condition and encourage the inclusion of MPD in virtually all differential diagnoses. The major thrust of what has been learned about the natural history of MPD is that most patients with this condition spend most of their lives not manifesting their MPD in classic manner. The typical presentation of MPD is the tip of a rather large iceberg. Covert and other nonclassic presentations are much more characteristic. An appreciation of this will help the clinician approach the diagnosis of MPD with a heightened

Multiple personality disorder. A psychoanalytic perspective.

This article links multiple personality disorder (MPD) and dissociation to related theories of defense, resistance, and development in treatment. Trauma, conflict, and deficiency all play contributing roles in the genesis of MPD; the blend of each ingredient may be decisive in each patient. Internal conflict and deficiency tend to be overlooked, whereas trauma is emphasized. Other psychoanalytic concepts, such as transitional phenomena, identity, aggression, repetition, and the seeking after quiescence, are applied to MPD. In the discussion of working through, emphasis is placed on the process

Iatrogenesis and malingering of multiple personality disorder in the forensic evaluation of homicide defendants.

In this article I have addressed the possibility of iatrogenesis and/or malingering of MPD in 19 individuals who were charged with homicide. Due to the fragmentary nature of most of these case reports, it is not possible to state definitively who had genuine MPD and who did not. There is a strong possibility that a substantial proportion were malingering, however, because of their rather atypical presentations. Data from other sources suggest that the use of hypnosis in individuals accused of homicide may iatrogenically produce MPD-like phenomena when coupled with the defendant's desire to esc

An office mental status examination for complex chronic dissociative symptoms and multiple personality disorder.

Chronic complex dissociative symptoms can be readily inquired about in the diagnostic interview leading to a clinical diagnosis of MPD in many cases. It is most useful to begin with inquiry about amnesia, autohypnotic, posttraumatic, pseudopsychotic, and passive-influence symptoms, and childhood abuse or traumatization. As this proceeds, overt dissociation is commonly noted including spontaneous trances, age-regression, blending or overlap of states, or frank switching. When this occurs, the interviewer can pursue more detailed information about the patient's experiences. Tracking these proces

Status dissociatus--a perspective on states of being.

During the course of routine clinical study, it has become apparent that the all-or-none concept of state determination (wakefulness, nonrapid eye movement sleep, rapid eye movement sleep) does not always exist, and that ambiguous, multiple, or rapid oscillation of state-determining variables appear in a wide variety of experimental and clinical situations. Six cases of extreme state dissociation are presented, with a review of the human and animal clinical and experimental literature. This multiple component concept of state determination must be kept in mind when pharmacologic or lesion stud

The comorbidity of multiple personality disorder and DSM-III-R axis II disorders.

Our ability to differentiate MPD from DSM-III-R Axis I disorders has become increasingly refined. Differentiation of MPD from the Axis II personality disorders is an area of more recent clinical investigation. MPD can be found comorbidity with many other psychiatric conditions. It is found in association with each of the DSM-III-R personality disorders. At the present time, however, we lack research data that define the prevalence of the comorbidity of MPD with the personality disorders. Objective study of this area is complicated by the paucity of instruments available to assess personality d

Propentofylline in adult-onset cognitive disorders: double-blind, placebo-controlled, clinical, psychometric and brain mapping studies.

In a double-blind, placebo-controlled parallel group trial, the therapeutic efficacy and central effects of propentofylline (HWA 285) - a xanthine derivative with neuroprotective, metabolic, hemorheologic and antithrombotic action - were studied in 190 elderly outpatients with mild to moderate chronic cognitive disturbances (mild dementia, DSM-III-R). They received after a 2-week run-in period (placebo) for 12 weeks either 3 x 300 mg propentofylline or 3 x 1 tablet placebo (given at least 1 h before meals). The verum group (n = 96, age 68 +/- 9) was comparable to the placebo group (n = 94, age

The concept of dissociation.

The concept of dissociation has been reviewed and redefined. It was also compared with other mental mechanisms. In the past, the process of dissociation and the content of the dissociation have often been lumped together although they are separate frames of reference. Moreover, very different psychic elements may be dissociated including memories, affects, and psychic structure. Dissociation is an important mental mechanism that plays a very significant role in people's adaptational efforts. In addition, dissociation is the underlying mechanism in a number of defense mechanisms. Repression, in

The diagnosis of multiple personality disorder based on subtle dissociative signs.

Patients with multiple personality disorder (MPD) often present with subtle dissociative signs and symptoms rather than with the clear-cut, overt dissociative signs and symptoms usually associated with MPD. Because most MPD patients usually conceal their condition, there are only limited periods in their lives when they show florid, overt symptoms that can be easily diagnosed. During these other times, MPD patients often show subtle dissociative signs in their affects, thoughts, memories, behaviors, object relations, and transferences. These indirect manifestations of MPD are evidence of the p

Accuracy of the MMPI in identifying multiple personality disorder.

A number of previous studies have delineated Minnesota Multiphasic Personality Inventory (MMPI) characteristics in patients with multiple personality. To test the accuracy of the MMPI in identifying such patients, the authors blindly rated 63 MMPIs as being either multiple personality or not. The over-all hit rate for the entire sample was 71.4%, with a 68% hit rate for correctly identified patients with multiple personality. These hit rates compare favorably with the hit rates in similar studies of other psychiatric disorders and further demonstrate the clinical usefulness of the MMPI in the

Hypnotizability and dissociation.

The author describes the multidimensionality of hypnosis itself and hence of hypnotizability. He also points to the lack of clarity regarding the concept of dissociation and the extent to which its roots lie in the clinical experience of hypnosis. The concept of dissociation increasingly preempts repression and other defense mechanisms in current nosological thinking. The author cautions against equating hypnotizability scores with dissociative capacity and advocates a clearer elaboration of the concept of dissociation. Meanwhile, restraint in the use of the term "dissociation" is recommended.

Dissociation and displacement: where goes the "ouch?".

Hypnosis is widely used to relieve pain. Current theory emphasizes its dissociative features. Multiple personality patients can eliminate pain in the primary personality by displacing it into underlying alters. The Hilgards demonstrated that normal hypnotized subjects can similarly dissociate pain into a covert cognitive structural system which they called the "hidden observer." The Watkins discovered that "hidden observers" appeared to be the same phenomenon as "ego states." "Ego-state theory" assumes that human personality develops through integration and differentiation. At one end of the c

Dissociative phenomena and the question of responsibility.

There are many controversies regarding the prevalence, causation, possible iatrogenicity, and treatment of multiple personality disorder. Those who view the disorder as much more prevalent than has previously been suspected believe it is caused by experiences of severe child abuse and have used rather unorthodox techniques to help the patient relate the experience of abuse to current problems of dissociation. Other clinicians believe the disorder is overdiagnosed and that it may be created or made worse by therapists who unwittingly reinforce symptoms of dissociation. Many of the controversies

Understanding and cooperation among alter and host personalities.

There is little occupational therapy literature that addresses the profession's efficacy in the treatment of persons with multiple personality disorder, yet the characteristics and complexity of this disorder render it one that can gain much from occupational therapy intervention. This paper examines the ways in which the occupational therapist can help in the treatment of patients with multiple personality disorder by introducing the alter personalities to each other, encouraging rapport among the personalities, and providing tasks that allow the alter personalities and the host to work toget

Intensity versus type of affect elicited by dissociative cognitions in moderating acute pain.

75 undergraduates were trained to use cognitions that elicited either high or low pleasure, high or low fear, or received an expectancy manipulation. Groups high in pleasure showed greater tolerance for pressure pain than other treatment groups but did not differ from expectancy; differences were not found on discomfort ratings. Results were not attributable to differential compliance with instructions, perceived effectiveness of cognitions used, or the number of cognitions used. The percentage of time during stimulation that assigned cognitions were used appeared to mediate tolerance. Expecta

Symptom and comorbidity patterns in World War II and Vietnam veterans with posttraumatic stress disorder.

Forty-four veterans with posttraumatic stress disorder (PTSD) from World War II and Vietnam were compared. The groups were comparable on many socioeconomic and combat measures and age at onset of PTSD. Vietnam veterans exhibited more severe PTSD symptoms, higher Hamilton depression scores, and higher scores on the hostility, psychoticism, and "additional symptom" Symptom Checklist-90 (SCL-90) scales. They also had more survivor guilt, impairment of work and interests, avoidance of reminders of trauma, detachment/estrangement from others, startle response, derealization, and suicidal tendencies

Factors that predict scores on the Dissociative Experiences Scale.

Bernstein and Putnam (1986) recently developed the Dissociative Experiences Scale (DES), a reliable and valid measure of dissociative experiences. The purpose of this study was to determine the relationship of the DES to other measures of psychopathology and cognitive functioning. Seventy-six female and 43 male university students were administered the DES, the Hopkins Symptom Checklist (HSCL-90), the Maudsley Obsessional-Compulsive Inventory (MOCI), the Tellegen Absorption Scale (TAS), and the Barnes-Vulcano Rationality Test (BVRT). A standard multiple regression was computed that used the DE

New uses of hypnosis in the treatment of posttraumatic stress disorder.

Hypnosis is associated with the treatment of posttraumatic stress disorder (PTSD) for two reasons: (1) the similarity between hypnotic phenomena and the symptoms of PTSD, and (2) the utility of hypnosis as a tool in treatment. Physical trauma produces a sudden discontinuity in cognitive and emotional experience that often persists after the trauma is over. This results in symptoms such as psychogenic amnesia, intrusive reliving of the event as if it were recurring, numbing of responsiveness, and hypersensitivity to stimuli. Two studies have shown that Vietnam veterans with PTSD have higher tha

[A clinical study of generalized amnesia].

Six cases of generalized amnesia were reported. Generalized amnesia caused by phenomena of genuinely psychogenic origin is a rare psychological disorder and spontaneous recovery from amnesia in a comparatively short period of time is one of the characteristics of this disorder. Three of the cases in this report developed amnesia which was prolonged in comparison with previously reported cases. A comparison between these six cases and previously reported cases of amnesia elucidated the general characteristics of this disorder, differential diagnosis from other disorders, the development of a ne
