An epidemic of collective conversion and dissociation disorder in an indigenous group of Colombia: its relation to cultural change.

We describe a collective episode of psychogenic illness in an indigenous group (Embera) of Colombia, geographically isolated from its native homeland and surrounded by non-indigenous settlers. The condition, which affected three young adult men and six adolescent women, was attributed by them to a spell (maleficio). It was designated as ataques de locura (madness attacks) according to their traditional medical system; and as a conversive disorder with dissociative features by psychiatrists. Different therapeutic approaches, including antipsychotic medication, religious healers and traditional

Complement to "ratio of male and female characters in a dream series".

This note relates recent research on dream content to other findings on imaginal experiences which suggest that state or situational factors frequently define the content of experiences related to altered states of consciousness. As with hallucinatory, dissociative, and autohypnotic phenomena, dream content reflects a continuity between waking and imaginal experience.

The detection and measurement of depersonalization disorder.

Depersonalization disorder comprises one of the four major dissociative disorders and yet remains poorly studied. There are no reports describing the application of dissociation scales to this population. Our goal was to investigate the applicability of four such scales to depersonalization disorder and to establish screening criteria for the disorder. Two general dissociation scales and two depersonalization scales were administered to 50 subjects with DSM-III-R depersonalization disorder and 20 healthy control subjects. The depersonalization disorder group scored significantly higher than th

Pseudoseizures.

Pseudoseizures represent the opposite end of the spectrum from seizures that mimic psychiatric disorders: they are paroxysmal changes in behavior that resemble epileptic seizures, but which are without organic cause and expected EEG changes. Accurately distinguishing pseudoseizures from epilepsy and other illnesses is difficult because of the breadth and overlap of symptoms seen in each condition and because of the frequent co-occurrence of pseudoseizures and epilepsy. This article summarizes the various neurological and psychiatric clues that help the clinician identify this form of psychiatr

Treatment of compulsive behaviour in eating disorders with intermittent ketamine infusions.

We have previously shown that eating disorders are a compulsive behaviour disease, characterized by frequent recall of anorexic thoughts. Evidence suggests that memory is a neocortical neuronal network, excitation of which involves the hippocampus, with recall occurring by re-excitement of the same specific network. Excitement of the hippocampus by glutamate-NMDA receptors, leading to long-term potentiation (LTP), can be blocked by ketamine. Continuous block of LTP prevents new memory formation but does not affect previous memories. Opioid antagonists prevent loss of consciousness with ketamin

Effectiveness of treatment for dissociative identity disorder.

In a recent study by Ellason and Ross, patients with Dissociative Identity Disorder reported a decrease in symptoms on the Millon Clinical Multiaxial Inventory-II over a 2-yr. follow-up period. Patients judged to have achieved integration of their personalities rated themselves as more substantially improved on the Millon-II than did patients judged not to have achieved integration. Ellason and Ross suggested that this improvement reflected the influence of treatment; however, for several reasons, their findings are open to alternative interpretations. First, in the absence of proper control c

Dissociative identity disorder: an Australian series.

OBJECTIVE: Series of patients fulfilling diagnostic criteria for Dissociative Identity Disorder (DID), otherwise known as multiple personality disorder, have particularly been reported on in North America and increasingly in other countries. The present study investigated the trauma and past treatment histories, symptom profiles and dissociative phenomenology of 62 patients fulfilling diagnostic criteria for DID seen in Brisbane (Queensland, Australia). METHOD: From 1992, systematic assessments, including the Dissociative Experience Scale and the Dissociative Disorders Interview Schedule, wer

Patients' experience of being critically ill or severely injured and cared for in an intensive care unit in relation to the ICU syndrome. Part I.

The intensive care unit (ICU) syndrome is regarded principally as a complex interaction of several contributory factors, all of which can be seen as partly responsible for the development of the syndrome. The purpose of this study was to describe and give a deeper insight into patients' experiences and memory recall, both during and after their stay in the ICU. Nineteen patients who had been respirator treated (ventilated), and had stayed at least 36 hours in the ICU, were interviewed about one week after discharge, and again 4-8 weeks after their discharge from the ICU. Patients' experiences

Neuropsychological dissociation between recognition familiarity and perceptual priming in visual long-term memory.

The present study examined whether the same brain region mediates visual-perceptual repetition priming and a familiarity component of visual recognition memory. In two experiments, familiarity-based recognition was measured in an individual (M.S.) with impaired visual repetition priming due to a lesion of right occipital cortex. In both experiments, M.S. demonstrated intact recognition familiarity despite his visual nondeclarative memory impairment. These results converge with other behavioral results to indicate that recognition familiarity does not depend on the same memory system that media

An attachment theory approach to treatment of the difficult patient.

This article reviews the role of attachment in difficult-to-treat patients. It is suggested that difficulty often arises in the treatment of these patients because of their inadequate understanding of mental functioning in themselves and in others. The capacity to mentalize is seen as a function of early attachment relationships. Vulnerability introduced by insecure attachment is frequently compounded by a history of intense trauma, leading these patients to defensively inhibit their capacity to think about mental states in their abusers, which then generalizes to other attachment relationship

Relationship between dissociation, childhood sexual abuse, childhood physical abuse, and mental illness in a general population sample.

OBJECTIVE: The aim of this study was to examine the relationship between childhood sexual abuse, childhood physical abuse, current psychiatric illness, and measures of dissociation in an adult population. METHOD: The authors used a randomly selected sample of 1,028 individuals. Each subject completed a semistructured face-to-face interview that included measures of childhood sexual abuse, childhood physical abuse, DSM-III-R psychiatric diagnoses, and selected items from the Dissociative Experiences Scale. RESULTS: Many individuals experienced occasional dissociative symptoms, and 6.3% of the

Multiple personality disorder in Japan.

The aim of this study was to determine whether the features of multiple personality disorder (MPD) in Japan are similar to those in North America, although a wide disparity exists in the prevalence of MPD between the two areas. In order to describe the features of MPD in Japan, we obtained clinical data from MPD case reports, including two of our own cases, published in Japanese academic journals and compared it with the data from other countries. The cases in Japan differed significantly from those in North America in the mean number of personalities and prevalence of sexual and/or physical a

Degree of somatoform and psychological dissociation in dissociative disorder is correlated with reported trauma.

In this study, the prevalence and severity of traumatic experiences as reported by patients with dissociative disorders and with other DSM-IV psychiatric diagnoses were compared. Furthermore, the predictive value of emotional, physical, and sexual trauma with respect to somatoform and psychological dissociation was analyzed. In contrast with comparison patients, dissociative disorder patients reported severe and multifaceted traumatization. Physical and sexual trauma predicted somatoform dissociation, sexual trauma predicted psychological dissociation as well. According to the memories of the

Questionable validity of 'dissociative amnesia' in trauma victims. Evidence from prospective studies.

BACKGROUND: We reviewed evidence from prospective studies to test whether individuals can develop amnesia for traumatic experiences, a process variously termed 'repression', 'dissociative amnesia' or 'psychogenic amnesia'. METHOD: Using specified criteria, we selected and analysed studies which prospectively assessed memory in victims of documented traumatic experiences. RESULTS: In studies in which people were asked directly about a past traumatic experience, they consistently reported memories. Non-reporting occurred only in studies where subjects were not asked directly about the experien

An individual patient comparison of response to a memory training program--psychogenic V organic amnesia: brief report.

Psychogenic amnesia ("dissociative amnesia" in DSM-IV) has received relatively little attention in the literature. We compared neuropsychological and behavioral findings between a patient with psychogenic amnesia and one with organic amnesia from their early stages in a rehabilitation program. A 52-yr-old man developed anterograde and retrograde amnesia and short-term memory disturbance 1 mo after an operation for esophageal carcinoma, with no loss of personal identity and information. No apparent organic causes were identified, but because the patient and his wife rejected psychiatric treatme

Comparison of dissociative identity disorder with other diagnostic groups using a structured interview in Turkey.

Twenty patients with dissociative identity disorder (DID), 20 with schizophrenic disorder, 20 with panic disorder, and 20 with complex partial epilepsy were evaluated with the Dissociative Disorders Interview Schedule (DDIS) and the Dissociative Experiences Scale (DES). Subjects with dissociative identity disorder were more frequently diagnosed as having somatization disorder, past or concurrent major depressive episode, borderline personality disorder, depersonalization disorder, and dissociative amnesia than other groups. They reported Schneiderian symptoms and extrasensory perceptions more

Dissociative symptomatology in children and adolescents as displayed on psychological testing.

The purpose of this study was to investigate psychological testing features of children and adolescents with dissociative disorder diagnoses to provide diagnostic information that might facilitate early intervention. The psychological testing protocols of 30 children diagnosed with dissociative disorders were compared with the testing protocols of 30 consecutive admissions to the Sheppard Pratt Hospital who did not receive a dissociative identity disorder (DID; formerly termed multiple personality disorder) or dissociative disorder not otherwise specified (DDNOS) diagnosis. Hypnotizability, the dissociative experiences scale, HGSHS: A amnesia, and automatic writing: is there an association?

The present study examined whether participants (N = 112) selected on the basis of high and low dissociative ability (Dissociative Experiences Scale [DES]; Bernstein & Putnam, 1986), high and low/simulating hypnotizability (Harvard Group Scale of Hypnotic Susceptibility, Form A [HGSHS:A]; Shor & Orne, 1962), and past performance on the HGSHS:A amnesia item differentially passed an automatic writing suggestion administered during a follow-up experiment. Results from a loglinear analysis supported a single main effect for hypnotizability. Low hypnotizable, simulating participants were more than

Symptoms of neuroses: profile patterns and factor structure of clinic attenders with non-psychotic functional psychiatric disorders.

Consecutively referred out-patients (n = 102) with non-psychotic functional psychiatric disorders were examined. Nine syndrome scores (panic attack, phobic anxiety, generalized anxiety, obsessive-compulsive, dissociative, conversion, somatoform, depersonalization, and depressive) showed patterns corresponding to ICD-10 diagnoses. However, some also showed moderate correlations between each other. Factor analysis revealed that the nine syndrome scores could be accounted for by three factors-anxiety and somatic, depression and obsessive-compulsive, and hysterical. These findings cast doubt on th
