Individual psychotherapy for the sexually abused child.

This paper describes one Center's experience using individual psychotherapy to help the sexually abused child. The approach described rests upon a five-part conceptualization of the traumatic experience: trauma, threat to ontogeny, neglect and emotional unavailability by the caregiver, child's feeling of exploitation, and the child's adaptation. The process of treatment is divided into three phases: starting, middle and closing. Some common issues which occur are guilt, loss and anger, as well as alterations in the child's sexual feelings. The psychotherapeutic approach to these and other issu

Sexual criminality and hypnotizability.

Thirty-three sexual offenders, 18 of whom had been convicted of rape, nine of pedophilia, and six of incest, were studied. Two thirds of the subjects had histories of "spontaneous self-hypnotic" experiences (dissociations); seven of these were DSM-III multiples and six were probable multiples. This group had very high hypnotizability scores. The other one third without histories of "spontaneous self-hypnosis" had normal scores. It was concluded that spontaneous self-hypnosis contributed to the perpetration of the crimes in many of these cases, although other factors also directed the antisocia

Neuropsychiatric dysfunction in primary Sjögren's syndrome.

Neuropsychiatric complications developed in 40 patients with primary Sjögren's syndrome, none of whom met American Rheumatologic Association criteria for systemic lupus erythematosus. Twenty-five patients had psychiatric abnormalities, the commonest of which were affective disturbances. Of 30 patients tested, 23 had an abnormal pattern in the Minnesota Multiphasic Personality Inventory, the commonest pattern being a "conversion V." In general, patients presented with hysteroid dysphoric features. Of 16 patients undergoing cognitive function testing, 7 showed mild memory impairment with attenti

Family violence: principles of intervention and prevention.

The dispersal of clinical and research information on family violence among many subspecialties has obscured the multidimensional nature of the problem and the need for clinicians in all settings to develop a coherent approach to family violence. Through an interpretive review, the author discusses specific mental health contacts that necessitate the exploration of family violence. Failure to do so may lead to underdiagnosis of dissociative disorders and overdiagnosis of depressive disorders in victims. The author also discusses three types of interventions--verbalization, violence-prevention

Incipient multiple personality in children. Four cases.

Despite extensive study of child abuse and increasing interest in multiple personality, there has been no way of identifying abused children who are potential multiples, and only rarely has multiple personality been diagnosed before adulthood. This article describes four cases where dual identity was claimed by the child and/or trance states were evident in response to questions or stimuli related to traumatic events. In addition, the three children who were treated according to a program developed to integrate multiple personalities in adults showed unification of consciousness and immediate

Towards a theory of multiple personality and other dissociative phenomena.

That dissociation and multiple personality occur has been well documented. In this article the guest editor asks the following question: Are they created? To arrive at an answer he brings together various concepts under the rubric of state-dependent learning and presents ideas about how the states come together psychologically, physiologically, neurophysiologically, and physically.

Spontaneous self-hypnosis in multiple personality disorder.

The authors posits that the central mechanisms in cases of multiple personality appears to be spontaneous self-hypnosis. As the syndrome is a product of hypnosis and as excellent hypnotic subjects are potentially able to induce a variety of psychiatric symptoms, these patients report many symptoms referable to other major syndromes. This accounts for the many diagnoses attributable to them. The study of multiple personalities offers insights into the capabilities of hypnosis, the genesis of an unconscious, and the mechanism of repression.

Flashbacks and memory phenomena. A comment on "Flashback phenomena--clinical and diagnostic dilemmas".

Alarcon, Dickinson, and Dohn (J. Nerv. Ment. Dis., 170: 217-223, 1982) recently reviewed the phenomenon of memory flashback following use of hallucinogenic drugs. They point out that while there are a considerable number of explanations concerning the flashback mechanism, little is known about the real causes. This paper examines flashback following drug ingestion in the light of other memory phenomena concerned with "cued retrieval" effects. Such phenomena may include dream recall, delayed post-traumatic stress, mood influence on memory, and drug effects on memory. Rather than view flashbacks

Fantasized companions and suicidal depressions: two case reports.

Two acutely suicidal adult patients with fantasized companions integral to the formation of suicide intent are presented. The phenomenon of the fantasized companion is reviewed, differentiated from true hallucinatory or psychotic phenomena, and related to other fantasy and dissociative states, such as daydreaming and multiple personalities. In this regard, the concept of hysterical psychosis is discussed. The functions served by the fantasized companions and their involvement in the successful treatment of the patients are described.

[Hysteria: problems of definition and evolution of the symptomatology].

What is the evolution of hysteria, now that the spectacular crises of previous times tend to occur less frequently? Are hysterical manifestations disappearing or do they take other forms? This question raises semantic and phenomenological problems concerning hysteria. A review of the literature shows the complexity and precariousness of some definitions, the question raised by associated organic disorders and the compounding influences of other nosological entities, such as depression and psychosomatic disorders. Clinical histories from the records of a psychiatric hospital for the period 1910

Multiple personality disorder as perpetrator of child abuse.

An eighteen-month-old Alaskan girl suffered transient coma with multiple superficial contusions when injured by her multiple personality disturbed caretaker. The relative disinterest in multiple personality disorder as a psychiatric diagnosis among American trained practitioners is reflected in the great difficulty encountered by child protection workers while seeking psychotherapy for this disturbed family. The public health nurse, pediatrician, and social workers of the child protection team were unsuccessful in attempting to secure psychiatric therapy for this child batterer. Multiple facto

The hidden observer as an experimental creation.

Laurence, Perry, and Kihlstrom appear to have misunderstood the theoretical positions advanced both by Hilgard and by Spanos and Hewitt. All of their criticisms of Spanos and Hewitt's "hidden observer" experiment are shown to be either misleading or invalid. The available data provide no support for a dissociation hypothesis of the hidden observer phenomenon. On the other hand, these data indicate strongly that this phenomenon is an experimental creation.

Multiple personality as a borderline state.

A case of multiple personality is presented. The primary personality met DSM-III criteria for borderline personality disorder, and displayed several "lower level" borderline defenses as well. The secondary personalities showed other borderline characteristics. Seen as a whole, the patient manifested a more complete spectrum of borderline features than did any of the personalities viewed individually. It is postulated that some patients with multiple personality may be a subset of borderlines, in whom the splitting of self and object representations is so severe that the disparate representatio

Multiple personality and primary affective disorder.

This report describes a young woman who met research criteria for both primary depression and multiple personality. Abnormal dexamethasone suppression test results supported the former diagnosis and returned to normal during electroconvulsive therapy in anticipation of depressive symptom resolution. This suggests that multiple personality may occur as an epiphenomenon of affective disorder or of other illnesses.

The multiple personality epidemic. Additional cases and inferences regarding diagnosis, etiology, dynamics, and treatment.

The past decade has witnessed a marked increase in the reported incidence of multiple personality patients. This review examines 16 papers published between 1970 and early 1981 that reported 29 cases of multiple personality other than the 50 cases reported earlier by George Greaves. These 29 case reports confirm that psychic and physical abuse, sexual trauma, and restrictive environments during childhood and consequent conflicts, especially regarding anger and sex, are likely to have etiological significance for this disorder. All but one of these patients were females, which might reflect sex

Multiple personality in India: comparison with hysterical possession state.

This article reports probably the first case of multiple personality from India and compares and contrasts it with the hysterical possession syndrome. Attention is drawn to the apparent rarity of multiple personality in contrast to the great prevalence of the possession syndrome in India (and other underdeveloped societies), while the reverse applies to Western Europe and North America. It is postulated that the disparity of frequency between the two manifestations of personal-identity disturbance derives from certain basic cultural differences. It is argued that polytheism and belief in reinc

Dissociative states with abnormal temporal lobe EEG. Multiple personality and the illusion of possession.

Twelve patients with clinical and EEG manifestations reminiscent of temporal lobe epilepsy reported symptoms of dissociative states. In seven of these patients, the clinical picture was consistent with multiple personality, whereas the other five had the illusion of supernatural possession. These cases suggest that in selected instances dissociative states may constitute complex behavioral manifestations of chronic limbic epilepsy.

Dermatitis artefacta induced in a patient by one of her multiple personalities.

A young woman suffered for 5 weeks with an inexplicable, severe dermatitis limited to her left arm. During hospitalization it was found that she had multiple personalities, one of whom had been applying the leaves of the poison ivy plant each night to her left arm, using 'his' gloved right hand. The factitial nature of the dermatitis was further documented by the fact that when the 'responsible personality' was cut off from supplies of poison ivy in the hospital, 'he' produced factitious haematomas by trauma to her left hand. Although this appears to be the first report of dermatitis artefacta

Multiple personalities. A report of 14 cases with implications for Schizophrenia and hysteria.

Evidence exists to support the concept of pathology or diseases of hypnosis. Multiple personalities is such a syndrome, as are many or perhaps all cases of hysteria (Briquet's syndrome). The crux of multiple personalities is the subject's unrecognized abuse of self-hypnosis, by which she creates personalities, beginning at age 4 to 6 years. The process of self-hypnosis allows the delegation of an experience or a function to an alter ago, henceforth relegated to unconsciousness by the amnesia of hypnosis. Most of these patients qualify for the diagnosis of hysteria (Briquet's syndrome), and man
