Integrative GWAS identifies novel loci and genetic links between psychiatric and metabolic factors in anorexia nervosa.

Anorexia nervosa (AN) is a complex psychiatric disorder with both psychiatric and metabolic underpinnings. This study aims to explore the genetic architecture of AN and the interplay between its psychiatric and metabolic components. Through a meta-analysis of AN GWAS data from European and Finnish populations, we identified a novel genome-wide significant locus near the SOX5 gene. Genetic correlation and Mendelian randomization analyses revealed shared and potentially causal relationships between AN and multiple psychiatric and metabolic traits. Local genetic correlation analysis identified 18

The association between childhood traumatic events and weight restoration in adult anorexia nervosa inpatient treatment: a longitudinal study.

PURPOSE: To examine the association between childhood traumatic events and weight restoration during specialized inpatient treatment for anorexia nervosa (AN). METHODS: Within this naturalistic prospective exploratory single-center study, patients with AN were recruited between 2014 and 2019 from a specialized eating disorder inpatient unit. Childhood traumatic events were assessed using the Childhood Trauma Questionnaire Short Form (CTQ-SF) and the Adverse Childhood Experiences Questionnaire (ACE). Posttraumatic stress disorder (PTSD) was diagnosed via a clinical interview. A mixed linear mo

Longer duration of amenorrhea is associated with lower delay discounting and less hedonic eating in girls and young women with low-weight eating disorders.

Oligo-amenorrhea is common in low-weight eating disorders (LWEDs) and reflects cumulative suppression of the hypothalamic-pituitary-gonadal axis, leading to chronic or cumulative hypoestrogenism. Estrogen modulates mesolimbic dopaminergic signaling involved in reward behavior. Investigating amenorrhea-reward mechanisms in LWEDs is clinically relevant, as reduced reward responsiveness and associated altered decision-making may sustain disordered eating. We examined whether lifetime duration of amenorrhea is associated with reward-based decision-making and hedonic eating in females with LWEDs. W

Illness severity, treatment motivation and personality as predictors for the treatment outcome in adolescents and young adults with anorexia nervosa.

BACKGROUND: Anorexia nervosa (AN) is a severe psychiatric disorder with high chronicity, particularly affecting adolescents. Existing treatments for adolescents do not always lead to full recovery. Recently, the developmentally adapted Maudsley Model of Anorexia Nervosa Treatment for Adolescents (MANTRa) has emerged as a promising alternative, warranting investigation into its outcome predictors. METHODS: This secondary analysis is based on data from a multi-center cohort study conducted in several Austrian clinical settings. The study included 92 participants aged 13-21 years diagnosed with

Therapeutic approaches for endocrine dysregulation in anorexia nervosa in adolescents.

Anorexia nervosa (AN) is a complex eating disorder characterized by self-induced weight loss, distorted body image, and profound endocrine dysregulation. In adolescents, the illness leads to growth failure, pubertal delay, amenorrhea, and compromised bone mineral accrual. This mini review synthesizes current evidence regarding the pathophysiology and management of endocrine disturbances in adolescents with AN, with a particular focus on therapeutic strategies for growth failure, pubertal arrest, and reduced bone mineral density. Endocrine abnormalities in AN involve adaptive suppression of the

Ketamine Responsiveness of Mice Exhibiting Anorexia-Like Behavior Correlates With Synaptic AMPA Receptor Distribution.

Anorexia nervosa is a deadly eating disorder marked by extreme food restriction, compulsive exercise, and severe weight loss. In a prior study using the activity-based anorexia (ABA) animal model of anorexia nervosa, 30 mg/kg, but not 3 mg/kg, of ketamine reduced relapse vulnerability, measured as increased food intake, increased body weight (BW), and reduced excessive wheel running, though there were individual differences in responsiveness to treatment. This study investigated whether ketamine evoked sustained changes to AMPA receptors (AMPARs) at excitatory synapses of the prefrontal cortex

Neural circuits regulating activity-based anorexia.

Evolutionarily conserved neural circuits evolved that mediate switching between feeding and foraging for food, depending on environmental conditions such as food scarcity and internal state [1-3]. Activity-based anorexia is a phenomenon observed ubiquitously in normal mammals that emerges under conditions of time-restricted food availability and continuous access to running wheels. Under these experimental conditions, rodents progressively lose body weight and develop paradoxical hypophagia and compulsive wheel running, which can prove fatal if left unchecked. On the other hand, rodents surviv

Clinical and psychopathological follow-up of patients with anorexia nervosa with or without NSSI comorbidity admitted to an inpatient service.

PURPOSE: Approximately 27% of patients with an eating disorder (ED) present with comorbid non-suicidal self-injurious behaviors (NSSI). EDs comorbid with NSSI display worse eating psychopathology and overall psychological distress. This follow-up study aims to compare the clinical and psychopathological characteristics at the time of hospital admission and the long-term outcome of patients with and without comorbid NSSI. METHODS: 73 adolescents affected with an ED were included in the study and divided into two groups depending on the comorbidity with NSSI. Clinical and anamnestic data, perso

Updated Dietetic Prescribing Guidelines When Naso-Gastric Tube Feeding Under Physical Restraint Is Required: A Modified Nominal Group Technique Study.

INTRODUCTION: Naso-gastric tube (NGT) feeding under physical restraint is a lifesaving clinical intervention that is sometimes needed in mental health patients, particularly those with restrictive eating disorders. Initial guidance was published in 2020 and subsequently reviewed and updated to support dietitians in this specialist area. METHODS: Using purposeful recruitment, we brought together specialist mental health dietitians from England, Wales, Scotland and Northern Ireland who work in inpatient child and adolescent mental health services or adult inpatient mental health services. Association between anticipated follow-up duration and treatment outcomes in anorexia nervosa: Insights from a Japanese medical prison.

AIM: Recovery rates in eating disorders generally improve with longer follow-up periods. However, the effect of a longer follow-up period itself on prognosis remains unclear due to high dropout rates. This study examined whether treatment outcomes in anorexia nervosa are influenced by the anticipated follow-up duration, using a unique cohort from a Japanese medical prison, where follow-up periods are predetermined by sentence length. METHODS: Data from 149 patients diagnosed with anorexia nervosa were analyzed. Logistic and Cox regression analyses were performed to identify predictors of favo

Participation in the New Maudsley Model Workshops is Associated With Reductions in Caregiver Burden in Eating Disorders.

OBJECTIVE: Caregivers of individuals with eating disorders (ED) frequently experience psychological distress, expressed emotion, and accommodation behaviours, contributing to caregiver burden. The New Maudsley Model (NMM) is a structured intervention designed to improve caregivers' emotional regulation, communication, and coping strategies. This study examined changes in caregiver burden associated with participation in NMM workshops and explored the temporal relationship between psychological distress and ED symptom impact. METHOD: A total of 447 caregivers participated, with 189 completing

Investigating the causal role of sleep disorders in anorexia nervosa and obsessive-compulsive disorder: a bidirectional Mendelian randomization study.

BACKGROUND: Whether common sleep phenotypes causally influence anorexia nervosa (AN) or obsessive-compulsive disorder (OCD) is unclear; we used Mendelian randomization (MR) to test effects of daytime sleepiness (DS), insomnia, and sleep apnea (SA) on AN and OCD, and the reverse direction. METHODS: We conducted bidirectional two-sample MR using genome-wide association studies (GWAS) of DS (N = 452,071), insomnia (N = 453,379), SA (N = 523,366), AN (N = 72,517), and OCD (N = 33,943), predominantly of European ancestry. Bridging the gap between neuroscience and clinical practice: a review of the development of Temperament Based Therapy with Support (TBT-S).

Temperament Based Therapy with Support (TBT-S) is an emerging, personalized treatment approach for eating disorders (EDs) that integrates patients' temperamental traits and neurobiological research into therapeutic strategies to reduce symptoms. While research on the efficacy of TBT-S is still in its infancy, the development of TBT-S as a treatment for EDs spans over two decades. The approach is unique in that it was formulated from neurobiological research, and utilizes treatment tools which were created collaboratively with individuals with lived experience. Hence, the aim of this review of

Stimulant prescription for ADHD in people with a past history of anorexia nervosa.

BackgroundADHD is a common neurodevelopmental disorder. Stimulants are the gold standard treatment resulting in measurable improvements in associated functional impairment. However, people with a history of comorbid anorexia nervosa (AN), an illness characterised by restricted intake and malnutrition, find it challenging to access stimulant treatment. In our clinical experience, amidst a scarce safety-profile for this population, stimulants are ceased or withheld due to concerns about appetite suppression, weight loss, and cardiac risks. We responded to the paucity of literature by reviewing t

In-person interview versus online self-assessment of the eating disorder examination: an empirical validation of data equivalence.

PURPOSE: Online self-assessments in mental health are becoming increasingly common, and the COVID-19 pandemic has further accelerated the need for reliable digital tools. However, the degree of equivalence between online self-assessments and in-person interviews in the context of eating disorders remains under-researched. This study investigated the equivalence of adolescent patient responses on the Eating Disorder Examination (EDE) collected across these two modalities. METHODS: The EDE was administered in a cross-over test-retest design, both as an online self-assessment questionnaire and a

Virtual Reality Body Exposure and Attentional Bias Modification in the Treatment of Adolescents With Anorexia Nervosa.

Anorexia nervosa (AN) is a severe psychiatric disorder characterized by intense fear of gaining weight and persistent body image disturbance. Virtual reality-based mirror exposure therapy (VR-MET) may reduce body-related fear through embodied exposure, while attentional bias modification training (ABMT) may enhance exposure learning by promoting balanced attentional allocation. This controlled clinical study tested the hypothesis that adjunctive VR-MET would improve clinical outcomes beyond treatment as usual (TAU) alone, and that preceding VR-MET with ABMT would produce additional benefits in

Experiencing Extreme Hunger in Anorexia Nervosa Recovery: A Qualitative Analysis of Reddit Narratives.

OBJECTIVE: Recovery from anorexia nervosa involves psychological and social adjustments that extend beyond weight restoration. Online forums increasingly serve as spaces where recovery experiences are openly shared, including accounts of "extreme hunger" during refeeding-a phenomenon that has not been reported in the clinical literature. This study explores how extreme hunger is experienced in original posts shared within an online forum dedicated to anorexia nervosa recovery. METHOD: Using the Reddit API, 987 analyzable posts from r/AnorexiaRecovery published between July 25, 2023, and Janua

Disentangling food addiction-related symptom profiles in anorexia nervosa: a latent class analysis and clinical implications.

BACKGROUND: The construct of Food Addiction (FA) has generated growing interest in the context of Anorexia Nervosa (AN), especially for its potential clinical implications. While AN is traditionally associated with restrictive eating patterns, recent findings suggest that FA symptoms may also be present in this population, complicating the clinical picture. This study aimed to explore FA symptomatology in individuals with AN using a person-centered approach and to identify psychological variables associated with different FA profiles. METHODS: A sample of 202 individuals with a diagnosis of A
