Faecal Microbiota Transplantation in Anorexia Nervosa: A Systematic Review of Methodologies, Outcomes, and Challenges With Recommendations for Future Studies.

OBJECTIVE: Anorexia nervosa (AN) is a severe psychiatric disorder displaying an altered gut microbiome. Faecal microbiome transplantation (FMT) has emerged as a powerful research tool and potential treatment option in AN due to the microbiome-gut-brain axis. Current studies are limited and reveal variable FMT protocols. This leads to heterogeneous outcomes and complicates drawing definitive conclusions from existing literature. This review aims to compile and assess the different protocols and develop recommendations on ideal donors, handling of faeces, recipients, duration/frequency of FMT, a

Anorexia nervosa: 150 years of critical theory.

BACKGROUND: Anorexia Nervosa (AN) was first classified 150 years ago, although descriptions of pathology consistent with the illness can be traced as far back as the Hellenistic era and the Middle Ages. Despite decades of clinical theorising and a wide range of treatment approaches, recovery rates remain very low. In light of the limited efficacy of current therapeutic paradigms, there have been growing calls to critically reassess the taxonomy and conceptualisation of AN. This narrative review paper traces the evolution of theoretical frameworks surrounding AN, from early historical accounts

A comprehensive analysis of global, regional, and national trends in anorexia nervosa burden (1990-2021) and projections to 2040: insights from the global burden of disease study 2021.

BACKGROUND: Anorexia nervosa is a severe psychiatric disorder with high morbidity and mortality. Traditionally linked to high-income regions, its recognition is growing in low- and middle-income countries. This study analyzes the global, regional, and national burden of anorexia nervosa from 1990 to 2021 using GBD 2021 data and offers projections to 2040. METHODS: Data on the prevalence, years lived with disability (YLDs), and age-standardized rates of anorexia nervosa were obtained from the GBD 2021 study, covering 204 countries and regions. The study used various statistical methods, includ

State-Level Analysis of Access to Intensive Eating Disorder Care for Medicaid Beneficiaries.

OBJECTIVE: Eating disorders are often underdiagnosed and untreated. Among Medicaid beneficiaries, standard outpatient care is often the only service received for eating disorders. More severe cases, however, may require more intensive services (e.g., intensive outpatient, partial hospitalization, residential, or inpatient care). Medicaid is the largest public health insurance payer, but whether Medicaid beneficiaries have access to these services and what barriers to access they face remain unclear. This study aimed to investigate the availability of intensive eating disorder services across t

Dicing, Smearing, Wasting: Maladaptive Mealtime Behaviors in Hospitalized Patients With an Eating Disorder.

BACKGROUND: Maladaptive mealtime behaviors (e.g., cutting food up into small pieces, eating foods in a particular order) are common among individuals with eating disorders (EDs) and are associated with higher meal anxiety, lower caloric intake, and longer duration of treatment (Gianini et al. 2015). Few studies have focused on maladaptive mealtime behaviors, which are often overlooked in ED treatment. METHODS: The current study used a retrospective chart review of inpatients with EDs (N = 142) admitted to a behavioral specialty unit to create a comprehensive list of maladaptive mealtime behav

Effects of teriparatide on hip structure and bone microarchitecture in women with anorexia nervosa: a placebo-controlled randomized trial.

UNLABELLED: Women with anorexia nervosa have low bone mineral density and increased fracture risk. We conducted a randomized, placebo-controlled trial investigating the effects of 6 months of teriparatide in anorexia nervosa. Teriparatide led to trends in improvements in bone structure. Longer-term studies are needed to evaluate the benefit of teriparatide in anorexia nervosa. PURPOSE: Women with anorexia nervosa have low BMD and increased fracture risk. Currently, there are no approved therapies for bone loss in anorexia nervosa. This study investigated the effects of teriparatide (TPT) on b

Anorexic Readiness Syndrome in Women Engaging in Body-Shaping Exercise.

Background: Eating disorders are increasingly diagnosed in young women, particularly during adolescence. The recently described Anorexic Readiness Syndrome (ARS) is more common than full-blown anorexia. It has been identified in female athletes engaging in disciplines focusing on the aesthetics of the body, in women involved in recreational exercise and in those who are not physically active but strive to achieve the "perfect" figure. The study aimed to assess the severity and prevalence of ARS in women regularly engaging in body-shaping physical activity. Methods: The study included 659 women

Physical and Psychological Effects of Nasogastric Tube (NGT) Use in Adolescents with Anorexia Nervosa: An Exploratory Study.

Background: Anorexia nervosa (AN) may require nasogastric tube (NGT) feeding when oral intake is insufficient. Evidence on the psychological impact and prognostic correlates of NGT use in adolescents affected with AN is limited. Methods: Fifty-seven adolescent inpatients (96.5% female; age range 12-18 years; and mean age 15.0 ± 1.51 years) affected with AN admitted in a child psychiatry ward and treated with NGT re-feeding in addition to oral nutrition were included in the study. A 21-item VAS questionnaire was administered at intake (T0), after NGT introduction (T1), after one week of NGT use

The Impact of Emotion Coaching on Changes in Caregiver Burden and Distress Tolerance During Family-Based Treatment.

OBJECTIVE: Caregivers often experience elevated stress while supporting a loved one with an eating disorder. This study was a secondary analysis exploring changes in caregiver burden (EDSIS), distress tolerance (DTS), and emotion regulation (ERQ) during family-based treatment (FBT) among caregivers high in expressed emotion (EE) participating in an emotion coaching or control intervention. METHOD: Forty-one adolescents with anorexia nervosa (AN) or atypical AN (M age = 14.88, SD = 1.63) and a caregiver scoring high in EE participated in FBT and were randomized to an emotion coaching or caregi

Hypothermia in patients with severe anorexia nervosa.

Hypothermia is frequently observed in anorexia nervosa (AN) but is often overlooked clinically. External heat application has been proposed as a supportive adjunct. We conducted a retrospective study to determine the frequency of hypothermia in a highly selected inpatient population with AN admitted to a specialized medical (somatic) unit because of low BMI and/or somatic complications, and to explore clinical correlates and implications. In this naturalistic clinical setting, we retrospectively reviewed routinely collected clinical and biochemical data from admissions. Hypothermia was defined

Taking care of the body to cure the mind: family-based therapies as attachment-based programs for adolescents with Anorexia Nervosa.

Anorexia Nervosa (AN) is a severe psychiatric disorder that typically emerges in adolescence and presents a high risk of medical complications and mortality. Early, family-centered interventions are critical in reducing both physical and psychological impairments associated with the illness. This article explores the clinical integration of Family-Based Treatment (FBT) with attachment-informed parenting interventions, particularly Circle of Security-Parenting (CoS-P). We argue that FBT can be conceptualized not only as a behavioral protocol for weight restoration but also as a structured frame

Adherence to dietary recommendations according to the General Dietary Behavior Inventory (GDBI) and its association with bioelectrical impedance analysis (BIA) parameters among young, healthy and normal weight women.

BACKGROUND: The General Dietary Behavior Inventory (GDBI) is a low effort instrument with only 16 items to assess the general dietary behavior based on the dietary recommendations of the World Health Organization and the German Nutrition Society. In an online survey with a convenience sample, higher total GDBI scores indicating healthier dietary behavior were associated with a lower body mass index (BMI). AIM: Since mean value of the self-reported BMI in that sample was in the overweight range, the aim of the current study was to examine the adherence to dietary recommendations using the GDBI

The Effectiveness of Psychological Treatment for Anorexia Nervosa in Adolescents: A Critical Review of Randomized Controlled Trials.

OBJECTIVE: Although international treatment guidelines for eating disorders recommend varying psychological approaches for adolescents with anorexia nervosa (AN), most existing reviews have combined adolescent and adult samples, leaving the overall evidence base for this population poorly defined. This systematic review is the first to synthesize randomized controlled trials (RCTs) of psychological treatments for adolescents with AN across outpatient, inpatient, and day-patient settings. METHOD: A systematic search of databases (e.g., PubMed, APA PsycINFO) identified 22 relevant studies throu

Causal effects of lipid-lowering drug targets on psychiatric disorders: A drug-target Mendelian randomization study.

ObjectivesThe pleiotropic effects of lipid-lowering therapies on mental health remain incompletely understood. This study aimed to investigate the causal impact of genetically proxied inhibition of three major lipid-lowering drug targets, 3-hydroxy-3-methylglutaryl-CoA reductase (HMGCR), Niemann-Pick C1-like protein 1 (NPC1L1), and proprotein convertase subtilisin/kexin type 9 (PCSK9), on a spectrum of psychiatric disorders using a drug-target Mendelian randomization approach.MethodsWe used genetic variants located within or near the HMGCR, NPC1L1, and PCSK9 gene regions that are associated wi

Structural brain alterations in anorexia nervosa: a global brain volume and anatomical likelihood estimation (ALE) meta-analysis combined with a functional decoding approach.

Substantial brain volume loss is well-documented during acute anorexia nervosa (AN); however, longitudinal outcomes are unclear. Our comprehensive meta-analysis investigated global and regional structural brain alterations in adult and adolescent individuals with AN by extracting reported brain volume scores and neuroimaging coordinates from the literature. Results showed significant global brain volume reductions in gray matter (GM), white matter (WM), and increases in cerebrospinal fluid (CSF) in acute AN (N = 1130 patients; N = 40 papers), gradually improving upon weight rehabilitation. The importance of learning from people with lived experiences of eating disorders.

Improving eating disorder (ED) care in a meaningful and equitable way requires clinicians and researchers to learn from people with lived experiences (PWLE) of EDs. Thus, epistemic injustice faced by individuals who are denied the right to share their experiential knowledge with professionals is a barrier to improving ED care. The present commentary builds on a two-part article series published last year in the Journal of Eating Disorders, where recommendations for improving ED care are offered from my lived experience and quality improvement (QI) perspective. In this complementary piece, I pr

Treatment of anorexia nervosa in Ireland: striking an ethical balance.

Anorexia nervosa (AN) is a complex psychiatric illness with severe and life-threatening medical sequelae, including death. Existing evidence-based treatments are linked to good prognosis and full recovery in many. For a small minority of critically ill patients, treatment decisions extend beyond voluntary engagement. Severe cases may involve involuntary hospitalisation, nasogastric feeding, physical restraint, and other coercive measures. While these interventions are sometimes necessary to prevent death, they raise profound ethical concerns. This article explores the ethical tensions in treat

Modeling metacognition and executive functions in the metacognitive wisconsin card sorting test using the neuropsychological digital-twin method.

Executive functions rely on goal-directed manipulation of representations, while metacognition reflects the evaluation and control of one’s own representations. Several studies have examined these processes separately, but none formalise the neuro-representational computations underlying their interaction during goal-directed behaviour. This gap prevents comprehensive frameworks and extended model-based neuropsychological investigations. Here we address these issues by introducing a neuropsychological digital-twin method - a translational modelling framework that integrates clinical and experi
