Metabolic and pathophysiological characterization of constitutional thinness.

PURPOSE: Constitutional thinness (CT) is characterized by a persistently low body weight in the absence of eating disorders or overt disease. Distinguishing CT from anorexia nervosa (AN) is often challenging, and the metabolic and cardiovascular features of CT remain incompletely defined. The present study investigated nutritional, metabolic, and cardiovascular parameters in women with CT and compared them with those of women with AN and normal-weight healthy controls. METHODS: Data from 7 women with CT were compared with those from 6 women with AN and 6 normal-weight healthy women serving as

Individual Cognitive Remediation Therapy and Individual Emotion Skills Training for Anorexia Nervosa: A Case Series Study in a Paediatric Ward.

INTRODUCTION: Adolescents with anorexia nervosa (AN) often present cognitive rigidity, impaired decision-making, and difficulties processing emotions, all of which can impede engagement in treatment. The marked rise in paediatric hospitalisations for AN during the COVID-19 pandemic highlights the urgent need for brief, targeted interventions in inpatient settings. METHODS: This case series explored the feasibility and preliminary effects of combining Cognitive Remediation Therapy (CRT) and Cognitive Remediation and Emotion Skills Training (CREST) for eight female adolescents (12-16 years) adm

Individualized targeting of the posterior parietal cortex with intermittent theta burst stimulation in anorexia nervosa: a randomized, double-blind, sham-controlled protocol.

BACKGROUND: Anorexia nervosa (AN) is a severe psychiatric disorder frequently marked by poor treatment response and chronic progression toward rigid, restrictive behaviors. These features highlight the need for innovative strategies targeting core mechanisms and symptoms of the disorder. This feasibility trial evaluates individualized intermittent theta-burst stimulation (iTBS) of the posterior parietal cortex (PPC) as an adjunct to treatment-as-usual, with the goal of identifying patient-specific targets and paving the way for personalized neuromodulation in AN. METHODS: Thirty-four adults w

More than just awareness: Longitudinal evaluation of an eating disorders training program for school counselors.

BACKGROUND: Eating disorders (EDs) are complex psychiatric conditions that often remain undetected in school settings. School counselors (SCs), as frontline observers, may play an important role in early recognition and referral. This study evaluated the effectiveness of a training program designed to support SCs' knowledge, attitudes, and practices regarding EDs in Türkiye. METHODS: Data were obtained from forty-nine SCs from Erzincan, Türkiye, who had previously provided baseline data (T0) and participated in a longitudinal study with two additional assessment time points: post-training (T1

Involuntary treatment of anorexia nervosa in Ireland: Challenges and changes in the legal framework.

Anorexia Nervosa is an eating disorder typified by low body weight, restrictive eating behaviours, and body image distortion. It is associated with significant risk of medical complications, with one of the highest mortality rates of any mental illness. While the majority of patients receive treatment on a voluntary basis, a small proportion of severely ill patients refuse treatment and are treated involuntarily. The legal mechanisms used for involuntary treatment vary between jurisdictions, including mental health law, capacity-based law, guardianship, and use of inherent jurisdiction, the po

Establishing General and Context-Specific Eating Disorder Examination Questionnaire (EDE-Q) Cut-Off Scores for Anorexia Nervosa.

OBJECTIVE: Our aim was to evaluate the diagnostic accuracy of the Eating Disorder Examination Questionnaire (EDE-Q) scores and establish general and context-specific cut-off points for anorexia nervosa (AN) in Australian women. METHODS: The sample included 67 women with AN, 41 women with a history of AN and restored weight, 100 women with self-reported AN, 27 sisters of individuals with AN, 73 healthy controls, and 597 community controls. ROC analysis assessed the EDE-Q's accuracy and determined cut-off scores in different contexts. RESULTS: The EDE-Q showed varying discriminative ability, w

Latent Trajectories of Body Mass Index and Depressive Symptoms in Inpatients With Anorexia Nervosa: Predictors and Outcomes.

OBJECTIVE: Inpatient treatment aimed at weight restoration and psychiatric stabilization is often required for individuals with anorexia nervosa (AN). This study aimed to identify distinct trajectories of change in body mass index (BMI) and depressive symptoms during inpatient treatment, examine clinical predictors and outcomes, and test reciprocal associations between BMI and depressive symptom changes. METHOD: Weekly BMI and depressive symptom data were collected from 156 inpatients with AN (mean treatment duration = 11.6 weeks). Growth mixture modeling identified trajectory classes. Baseli

Medication use in severe anorexia nervosa: a Danish register-based study.

BACKGROUND: Severe anorexia nervosa (AN) represents a subgroup of individuals with AN with prolonged illness duration and poor prognosis. Previous research has reported increased medication use in AN, but prescription patterns in severe AN remain unexplored. OBJECTIVES: This study aimed to: (1) compare medication prescriptions between individuals with severe and less-severe AN; (2) explore the heterogeneity of prescription patterns among individuals with severe AN across different comorbidity profiles. METHODS: Using Danish registers, this cohort study included 7654 individuals diagnosed wit

"Invented medicine"? Alliance building in first encounters between clinicians and adolescents with anorexia nervosa: a qualitative study.

BACKGROUND: Caring for adolescents with anorexia nervosa (AN) involves gaining patients' and families' trust in a condition fraught with ambivalence. While essential for subsequent care and the development of a therapeutic alliance, the first clinical encounter is also known to be emotionally challenging both for frontline professionals and for families. There are few studies investigating the experience of clinicians working in specialized care settings. Our study aims to explore the challenges experienced by specialized professionals during the initial encounter with patients presenting with

GHSR agonism increases blood glucose but delays food intake in GHSR hyperresponsive rats.

Severe calorie restriction in mouse models has highlighted the crucial role of the ghrelin system in maintaining glycemia and promoting survival. We hypothesized that if ghrelin acts as a survival signal, enhancing the responsivity of the GH secretagogue receptor (GHSR) should favor GHSR protective responses. To test this, we used rats with genetically enhanced GHSR sensitivity (GhsrQ343X) and wild-type littermate controls and examined their acute responses to pharmacological challenges. Consistent with our hypothesis, GhsrQ343X rats, despite normal glucose and insulin tolerance, exhibited a s

Bridging inpatient and community treatment: feasibility, acceptability, and short-term outcomes of step care, a virtual CBT-E pathway for adults with severe eating disorders.

BACKGROUND: Transitions between inpatient and community care carry a high risk of disengagement or relapse for adults with severe eating disorders. Step Care was developed to address this gap by adapting intensive enhanced cognitive behavioural therapy (CBT-E) for remote, multidisciplinary delivery in patients' homes. The virtual service operates through three pathways, Starting Well, Staying Well, and Working Towards Recovery, and aims to improve engagement, reduce hospitalisation, and prevent relapse. METHOD: This paper describes the Step Care service innovation and reports a prospective se

Intact delay discounting but more optimal reward-based decisions in anorexia nervosa during an experiential task.

AIMS: Individuals with anorexia nervosa (AN) have an apparent increased capacity to delay gratification and forgo immediate food rewards in their long-term pursuit of thinness. Prior research probed this capacity using delay discounting tasks that assess how rapidly the subjective value of rewards decreases as a function of time until receipt. However, significant effects in AN were mostly subtle or absent using traditional tasks. Here, we tested whether altered delay discounting might be revealed in AN during an experiential gamified task. METHODS: Eighty acutely underweight females with AN

Beliefs of delusional intensity about body image or eating-related concerns in individuals with Feeding and Eating Disorders: a systematic review and meta-analysis.

OBJECTIVE: In Anorexia Nervosa (AN) distorted body image beliefs range from overvalued ideas to delusions, significantly influencing illness trajectory. However, these phenomena remain underexplored, particularly in other Feeding and Eating Disorders (FED). This systematic review and meta-analysis investigates the prevalence and clinical significance of delusional-like beliefs related to body image and eating concerns in FED. METHOD: Following PRISMA guidelines, PubMed, Web of Science, and the Cochrane Library were searched for quantitative studies (1960-January 2025) investigating delusional

Assessment of serum concentrations of the "satiety hormone" - peptide YY concentration in body weight disorders in girls with anorexia nervosa and obesity.

INTRODUCTION: Peptide YY (PYY), a key satiety hormone, exhibits altered serum concentrations in body weight disorders, although its role remains debated. Conflicting adult studies highlight the need to clarify PYY 1-36 dynamics in pediatric populations with anorexia nervosa (AN) and obesity (OB). MATERIAL AND METHODS: Fasting serum PYY 1-36 concentrations were analyzed in 199 girls: 58 with restrictive AN [median age 15.0 (interquartile range; IQR): 14.0-16.0 years], 52 with OB [14.3 (12.3-16.3) years], and 89 healthy controls (C) [16.5 (15.5-17.5) years]. Evoked emotions in anorexia nervosa: neural and behavioural correlates of social-emotional processing.

Previous work suggests people with anorexia nervosa (AN) display reduced facial expression of emotion. This may influence illness progression as blunted emotional reactions can negatively impact social relationships and increase isolation. The present study aimed to replicate and further build on previous findings by examining facial and brain responses to naturalistic, emotional films. In total, 141 women (71 AN/weight restored AN, 70 healthy comparison) completed two tasks in a fixed order: 1.) facial affect task and 2.) functional magnetic resonance imaging (fMRI) task. In both tasks, parti

The Relationship Between Personality Psychopathology Features and Readmission Rates Among Adolescents With Anorexia Nervosa Admitted to a Medical Inpatient Eating Disorder Unit.

Many features of personality psychopathology are shared among patients with Anorexia Nervosa (AN), yet the impact of these shared features is not fully understood, and the impact of having more personality psychopathology features during adolescent onset of AN, especially among adolescents admitted to an inpatient eating disorder unit, has not been explored. This study evaluated how personality psychopathology, utilizing a measure of borderline personality disorder (BPD) symptom severity, has a potential predictive value in understanding risk of readmission for an eating disorder over time.
