Momentary Temporal Associations Among Sleep Disturbances, Eating Disorder Behaviors, and Suicidal Ideation in Anorexia Nervosa Spectrum Disorders.

OBJECTIVE: Anorexia nervosa spectrum disorders (ANSD) are associated with high chronicity and mortality, with suicidality representing one of the most common and clinically significant comorbidities. Sleep disturbances are prevalent in individuals with ANSD and have independently been linked to both eating disorder (ED) symptom severity and suicidal ideation (SI), yet their temporal role in maintaining ANSD symptoms and SI remains poorly understood. This study examined sleep disturbances as a potential shared, transdiagnostic mechanism contributing to daily fluctuations in ANSD symptoms and SI

Diagnostic Criteria for Atypical Anorexia Nervosa: A Proposal.

OBJECTIVE: To review the history of the term atypical anorexia nervosa and the challenges surrounding its current description and to propose a new name and diagnostic criteria. METHOD: We review the use of the term "atypical" in the literature on eating disorders and in the Diagnostic and Statistical Manual of the American Psychiatric Association (DSM) and note several terms in the current description that are in need of clarification. We also describe how the ICD-11 has grappled with atypical anorexia nervosa. RESULTS: The first use of atypical anorexia nervosa was by Hilde Brȕch to describ

Children and adolescents with psychiatric disorders have high relative leptin levels upon adjustment for sex, BMI, and pubertal status.

The relationship between leptin levels and psychiatric disorders has been studied more extensively in adults than in children and adolescents. However, the results are conflicting. We investigated serum leptin levels in children and adolescents (11 to 18.9 years) with psychiatric disorders (n = 363). Absolute and relative (body-mass-index (BMI)-, sex- and pubertal-stage-adjusted z-scores using reference values of healthy children and adolescents) leptin levels of different patient groups according to diagnosis were compared. The association between leptin levels and depression (Beck Depression

Early Response to an Integrated Family and Adolescent-Focussed Treatment for Anorexia Nervosa and Atypical Anorexia Nervosa.

OBJECTIVE: Recovery rates for adolescent anorexia nervosa (AN) remain poor, requiring creativity within the field to explore alternative treatment options. One novel, evidence-informed approach integrates both family/parent and individual adolescent-focussed treatment components, through an individualised developmental lens. This study aimed to offer preliminary insights on early response to Adolescent and Parent Treatment (APT) in terms of 8-week weight trajectory. METHOD: A naturalistic, repeated measures design was employed. Twenty-four adolescents with AN or atypical AN receiving APT as s

Premorbid Weight Status Predicts Weight Gain During Treatment of Adolescents with Anorexia Nervosa and Atypical Anorexia Nervosa.

Weight gain is instrumental in recovery from anorexia nervosa (AN)/atypical AN, yet the effect of premorbid weight status on treatment-related weight gain is unclear. In 71 medically hospitalized adolescents with AN/atypical AN, relative weight gain, measured via change in BMI z-scores, was assessed over 12-months of outpatient treatment following hospitalization and compared for premorbid weight status. Patients with premorbid overweight/obesity had a longer illness duration (p = .03) and more weight loss (p < .001) than patients with premorbid normal weight, and less weight gain at 3-, 6-, 9

Eating disorder psychopathology relates to weight loss medication and supplement use 6 months later in women.

PURPOSE: This study examined whether eating disorder (ED) psychopathology related to weight loss medication and over-the-counter weight loss supplement use over time in women. It was hypothesized that ED psychopathology at baseline would relate to the following 6-months later: (1) use of any weight loss medications or supplements; and specific use of: (2) over-the-counter weight loss supplements, (3) prescription weight loss medications, (4) glucagon-like peptide 1 (GLP-1) agonist medications used for weight loss. METHODS: Women in the United States were recruited for a 6-month long online st

Eating disorders among adolescents and young adults in Kingston, Jamaica.

INTRODUCTION: Eating disorders are complex disorders characterized by a persistent disturbance of eating that impairs health and psychosocial functioning, with accompanying long-lasting effects on the individual's overall health and well-being. Presumed stereotypes may affect reporting and access to care, especially in the Caribbean, where ethnic groups are mostly non-White. This study reviewed the clinical presentation, management, and outcomes of eating disorders in adolescents and young adults in Kingston, Jamaica. It explored the factors that influence outcomes - hospital admission and rec

Assessing Clinical Severity and Prognosis in Adolescents With Anorexia Nervosa and Atypical Anorexia Nervosa Using the Albumin-Globulin Ratio.

OBJECTIVE: The albumin-globulin ratio (AGR) is a biochemical marker reflecting nutritional and inflammatory status, with significant prognostic value in chronic conditions. This study examined its association with clinical/biochemical markers in adolescents with anorexia nervosa (AN) and atypical AN (AAN). METHOD: This retrospective study analysed data from 294 adolescents (103 AN, 191 AAN) aged 12-18 years, presented at an adolescent medicine clinic between 2016 and 2023. AGR values were calculated and relationship between AGR and markers of both disease severity (rate of weight loss, vital

Moving Beyond BMI: The Need for Biomarkers of Physiological Recovery in Anorexia Nervosa and Atypical Anorexia Nervosa.

OBJECTIVE: In the treatment of individuals with atypical anorexia nervosa (atypical AN), it is unclear how to determine when sufficient renourishment has been achieved. Reliance on body mass index (BMI) to define acute treatment goals in AN has led to gaps in understanding of the physiology of renourishment. Biological markers that reflect restoration of physical health during acute nutritional rehabilitation are needed to move beyond weight-based metrics. METHOD: This Spotlight briefly describes the physiology of starvation in patients with AN and atypical AN and existing strategies for acut

Examining Comorbid Psychopathology Symptoms as Predictors of Family Based Treatment for Adolescents With Anorexia Nervosa and Atypical Anorexia Nervosa in a Real-World Setting.

BACKGROUND: For adolescents with Anorexia Nervosa (AN) or Atypical Anorexia Nervosa (AAN), Family Based Treatment (FBT) is an effective treatment. However, outcomes remain suboptimal, making investigations into predictors of outcome important. Most prior research into FBT has focussed on parental and family factors as predictors. OBJECTIVE: The current study aimed to identify predictors of FBT for adolescents with AN and AAN in a real-world setting, specifically focussed on comorbid patient psychopathology as a predictor of outcome. METHOD: A prospective cohort of 135 young people engaging i

Optimal predictors of weight, weight loss, and weight gain following hospitalization for anorexia nervosa and atypical anorexia nervosa in adolescents.

The methodology of characterizing weight and measuring weight change in adolescents with anorexia nervosa (AN) and atypical AN is variable, limiting the cross-comparability of research studies. We aimed to compare correlations between absolute weight, weight loss, and weight gain during and following inpatient treatment in a combined sample of 110 medically hospitalized adolescents with AN (n = 33) and atypical AN (n = 77) and the two diagnostic subgroups. We hypothesized stronger correlations in the combined sample versus diagnostic subgroups. Body weights (premorbid, admission, discharge, 3-

Transforming percentage of median body mass index (%mBMI) scores into corresponding BMI z-scores yields discrepancies for age and sex: implications for pediatric eating disorder researchers and clinicians.

Anorexia nervosa (AN) and atypical AN are distinguished by the presence or absence of a significantly low body weight. In children and adolescents, the population-based age- and sex-adjusted weight characteristics, BMI centile, BMI z-score (BMIz), and percentage of median BMI (%mBMI) are used to diagnose underweight. Whereas BMI centile and BMIz are interconvertible, %mBMI is not. We extend previous analyses to illustrate (a) age- and sex-dependent discrepancies between %mBMI and BMIz and (b) the non-linear relationship between BMIz and absolute body weight. Based on Centers for Disease Contro

Socioeconomic Deprivation in Adolescents Hospitalized with Anorexia Nervosa and Atypical Anorexia Nervosa.

Objectives: We sought to examine associations between area-level socioeconomic deprivation and: (1) hospitalization length; (2) severity; and (3) clinical outcomes in adolescents hospitalized for atypical anorexia nervosa or anorexia nervosa (AN). We hypothesized that more socioeconomic deprivation would be associated with longer length of hospitalization, greater illness severity, and poorer clinical outcomes. Methods: We conducted a study of individuals aged 11-18 years with AN who were admitted for medical instability. Tract-level socioeconomic deprivation was characterized by using a depri

Vasoconstriction as a translational target in eating disorders: Autonomic dysregulation, insular mechanisms, and a novel multimodal detection system.

Eating disorders, particularly anorexia nervosa and atypical anorexia nervosa, collectively anorexia nervosa spectrum disorder, remain among the most lethal psychiatric conditions, with high relapse rates and limited effective treatments for adults. Autonomic dysregulation and interoceptive dysfunction have emerged as promising mechanistic contributors to symptom maintenance, yet no existing framework integrates these processes into a coherent, physiologically grounded model. This theoretical manuscript introduces vasoconstriction, the sympathetically mediated narrowing of peripheral blood ves
