The Overlooked Burden of Atypical Anorexia Nervosa: Commentary on Melville et al. (2025).

Obesity and eating disorders (EDs) have historically been viewed as distinct conditions; however, emerging evidence suggests a significant overlap, particularly among individuals seeking obesity treatment. While binge-eating disorder (BED) is commonly identified in this population, restrictive EDs such as atypical anorexia nervosa (atypical AN) can go largely undetected. This paper comments on findings from Melville et al.'s systematic review of 85 studies assessing ED prevalence in adults with high Body Mass Index (BMI) seeking obesity treatment. We highlight the striking absence of atypical

Effect of cognitive bias modification training on body image dissatisfaction in adolescents with anorexia nervosa or depression-a pilot feasibility randomized controlled crossover study.

INTRODUCTION: Body image disturbance presents transdiagnostically with an impact on the development and maintenance of psychiatric disorders. Addressing body image disturbance, a cognitive bias modification training (CBM) was developed using a two-alternative forced choice task (2-AFC) to alter patients' individual perceptual boundary between what they classify as a fat versus a thin body. This pilot study aimed to evaluate, for the first time, the paradigm's feasibility and efficacy in adolescents in a clinical context. METHODS: This pilot study included adolescent inpatients aged 13-17 year

A Weight-Inclusive Approach to Restrictive Eating Disorders: De-Centering Weight.

OBJECTIVE: To examine the case for de-centering weight in the diagnosis of both anorexia nervosa (AN) and atypical anorexia nervosa (AAN). METHOD: We summarized research examining the weight-based similarities and differences between AN and AAN as well as how weight is measured and discussed in research on AAN. RESULTS: We suggest that weight-based differences in the diagnosis of AN and AAN may unintentionally perpetuate weight stigma experienced by individuals with AAN throughout the diagnostic and treatment processes. We extend the work of previous researchers by considering how AAN and AN

Demographic and clinical characteristics of adolescent patients with eating disorders before and during the Covid-19 pandemic.

PURPOSE: It is known that the frequency of eating disorders (EDs) increased during the Covid-19 pandemic. This study evaluates the presentation of demographic/clinical characteristics of patients with EDs during the pandemic, and compares them to those that presented during 2014-2015. METHODS: Our division conducted a study on EDs in adolescent patients presenting from May 2014 to April 2015. That study examined presenting demographic/clinical variables. This current study looked at those same variables for patients presenting from September 2020 to May 2021, during the pandemic. The variable

Maximal Weight Loss Is Strongly Correlated With Maximal BMI in Adolescents With Anorexia Nervosa and Atypical Anorexia Nervosa.

INTRODUCTION: In hospitalised patients with both anorexia nervosa (AN) and atypical AN, premorbid body mass index (BMI; kg/m2), estimated from premorbid weight (i.e., weight at the onset of weight loss preceding eating disorder development) and admission height, strongly predicts admission weight loss (i.e., premorbid weight minus admission weight). We hypothesised that maximal weight loss (i.e., maximal historical weight minus minimal weight since eating disorder onset) is also correlated with maximal BMI. METHODS: In 520 female adolescents with AN (n = 503) and atypical AN (n = 17), relatio

Clinician Practices in Determining Expected Body Weights Across Eating Disorders: A Mixed Methods Study.

OBJECTIVE: Eating disorder (ED) treatment often involves establishing expected body weights (EBWs). While individualised approaches are commonly used, whether and how EBWs are set across EDs, clinician perspectives on different approaches, and clinician training in this practice remain poorly understood. This study addressed these gaps, examining how clinicians navigate determination of EBWs. METHOD: A convergent mixed methods design was employed. Semi-structured qualitative interviews were conducted with 24 multidisciplinary ED clinicians to explore their experiences determining EBWs. Interv

Associations in intentions to use anabolic-androgenic steroids among non-consuming boys and men with probable eating disorders and muscle dysmorphia.

BACKGROUND: Anabolic-androgenic steroids (AAS) are used to achieve a muscular and lean body to align with current ideals for boys and men. Identifying associations with intentions to use AAS, a precursor to actual use, is critical for prevention efforts. This study aimed to examine whether boys and men with a probable eating disorder or probable muscle dysmorphia have stronger intentions to use AAS, and whether these intentions differ between these conditions. METHODS: Data from a sample of 1,515 participants from the Study of Boys and Men who had never used AAS were analyzed. To address the

Comparison of disease severity in adolescents with anorexia nervosa and atypical anorexia nervosa.

PURPOSE: Although atypical anorexia nervosa (AAN) is as common as anorexia nervosa (AN) it is often underrecognized due to normal or above-normal weight at presentation. We aimed to compare disease characteristics between adolescents with AN and AAN using a comprehensive panel of anthropometric, vital-sign, biochemical, and clinical variables. METHODS: A retrospective chart review was performed for adolescents aged 12-18 years diagnosed with AN and AAN according to DSM-5 criteria. Anthropometric, vital-sign, and laboratory measurements, and hospitalization data at presentation were recorded.

Screening of adolescent Atypical Anorexia Nervosa in primary care: is the current use of the eating disorders examination a barrier to accessing treatment?

BACKGROUND: General practitioners (GPs) are often the first point of contact for adolescents with eating disorders (EDs) in Australia and play a central role in initiating access to the Medicare-funded Eating Disorders Treatment and Management Plan (EDP). Symptom non-endorsement can hinder GPs' ability to recognise, diagnose, and enable access to EDPs at primary care, potentially contributing to the low proportion of Australians with EDs receiving treatment or support. This may be particularly relevant for individuals with Atypical Anorexia Nervosa (AAN), who must show elevated self-report sym

Food for thought: re-emergence of tardive dyskinesia amid nutritional rehabilitation of atypical anorexia nervosa; a case report.

BACKGROUND: Atypical anorexia nervosa is an eating disorder that involves significant restriction of food intake leading to clinically significant weight loss despite remaining at a normal or above normal weight for age or height. Part of rehabilitation involves increasing calorie intake for nutritional rehabilitation and weight gain to an appropriate percentage of ideal body weight, determined based on patient weight history. This report highlights the importance of considering how increased food intake in a previously malnourished patient can affect the metabolism of medications they are tak

Semaglutide-associated worsening of atypical anorexia nervosa in an adolescent girl: case report.

Anorexia nervosa is characterised by the fear of gaining weight and a disrupted relationship with food but atypical if the body mass index is within normal limits. Semaglutide is a glucagon-like peptide-1 receptor agonist, indicated in patients with diabetes type 2 and obesity. An adolescent girl was diagnosed with atypical anorexia nervosa. She had a fear of gaining weight, she increased her exercise and decreased her caloric intake. The adolescent was admitted to the paediatric ward with bradycardia and pericardial effusion. During her hospital stay she admitted to having used semaglutide. S

Slow-Channel Congenital Myasthenic Syndrome due to a CHRNA1 Variant Masquerading as Atypical Anorexia Nervosa: A Case Report.

Congenital myasthenic syndrome (CMS) is a group of inherited disorders characterized by skeletal muscle weakness resulting from dysfunction of the neuromuscular junction. Here, we report a case of a 42-year-old woman with slow-channel congenital myasthenic syndrome (SCCMS) whose phenotype included low body weight, which mimicked anorexia nervosa. The patient experienced progressive weight loss from 45kg (BMI 18.3) to 34kg (BMI 13.8) during her 20s despite not engaging in dieting, purging, or excessive exercising, nor did she express a desire to lose weight. The patient had asymmetrical fatigab

Associations between eating disorders and difficulties with emotion regulation in a sample of adolescent boys and young adult men.

This study aimed to examine the association between eating disorders and emotion regulation difficulties in a sample of adolescent boys and young adult men in Canada and the United States (2024; N = 925). Multiple linear regression analyses were used to explore whether boys and men with any probable eating disorder (i.e. anorexia nervosa/atypical anorexia nervosa, bulimia nervosa, binge-eating disorder) had higher scores on the Difficulties in Emotion Regulation-18 (DERS-18) measure, while adjusting for relevant sociodemographic confounders. Participants with any probable eating disorder, comp

Choledocholithiasis leading to diagnoses of severe malnutrition and atypical anorexia nervosa in an adolescent.

An adolescent female presented with acute cholecystitis associated with obstructive choledocholithiasis. As she was being prepared to undergo urgent endoscopic retrograde cholangiopancreatography and cholecystectomy, it was noted that she was profoundly bradycardic with resting heart rate as low as 33 beats per minute. Though her body mass index was within normal limits at presentation, her bradycardia prompted review of her growth curve which showed almost 100 pounds of weight loss in the previous year. History revealed severe disordered eating behaviours and led to a diagnosis of atypical an

Atypical (Other or Unspecified) Feeding or Eating Disorders: Diagnosis, Clinical Implications, and Treatment.

Atypical eating disorders (EDs) appear to be comparable to classical eating-disorder variants with regard to eating-symptom severity, illness burden, concurrent mental-health problems, medical complications, etiology, and treatment needs. It remains to be determined to what extent atypical EDs are truly distinct from classical variants, or simply represent early or residual phases of these disorders. Available evidence suggests that, except for variants for which symptom-specific treatment elements are indicated, conventional eating-disorder treatments are also well-suited for the treatment of

Comparison of clinical presentation and treatment response among youth with atypical anorexia nervosa and anorexia nervosa.

OBJECTIVE: Atypical anorexia nervosa (AAN) is an eating disorder that shares all symptoms with anorexia nervosa (AN) except "significantly low weight." Here, we aim to further understand the potential similarities and differences between AN and AAN in a clinical sample of patients receiving family-based treatment (FBT) for AN or AAN. The objectives of this study were to 1) compare clinical presentations among adolescent and young adult patients with AN and AAN; and (2) evaluate and compare treatment response to FBT among these patients. METHOD: Clinical data from 1,438 patients (M = 15.3 year

Examining the placement of atypical anorexia nervosa in the eating disorder diagnostic hierarchy relative to bulimia nervosa and binge-eating disorder.

OBJECTIVE: Some individuals meet the criteria for atypical anorexia nervosa and another eating disorder simultaneously. The current study evaluated whether allowing a diagnosis of atypical anorexia nervosa to supersede a diagnosis of bulimia nervosa (BN) or binge-eating disorder (BED) provided additional information on psychological functioning. METHODS: Archival data from 650 university students (87.7% female, 69.4% white) who met Eating Disorder Diagnostic Survey for DSM-5 eating disorder criteria and completed questionnaires assessing quality of life, eating disorder-related impairment, an

Eating disorder hospitalizations among children and youth in Canada from 2010 to 2022: a population-based surveillance study using administrative data.

BACKGROUND: Eating disorders (EDs) are severe mental illnesses associated with significant morbidity and mortality. EDs are more prevalent among females and adolescents. Limited research has investigated Canadian trends of ED hospitalizations prior to the COVID-19 pandemic, however during the pandemic, rates of ED hospitalizations have increased. This study examined rates of ED hospitalizations among children and youth in Canada from 2010 to 2022, by sex, age, province/territory, length of stay, discharge disposition and ED diagnosis. METHODS: Cases of ED hospitalizations among children and y
