Direct and Indirect Associations Between War-Related Nightmares, Anxiety-Depression Symptoms, and Somatization in a Sample of Displaced Lebanese Adults During the 2024-2025 Lebanese War: A Cross-Sectional Study.

BACKGROUND AND AIMS: Somatization, the physical manifestation of emotional and psychological distress, is a prevalent issue in populations exposed to war and trauma. War-related nightmares are common among displaced individuals and are linked to depression, anxiety, and somatization. Yet, the indirect role of depression-anxiety in the association between war-related nightmares and somatization remains underexplored, particularly in the Lebanese context during the 2024-2025 war. This study aimed to examine whether war-related nightmares were associated with somatization both directly and indire

Perceived effectiveness, tolerability, and discontinuation of lemborexant in real-world use: Evidence from online patient reports.

BACKGROUND: Lemborexant is a dual orexin receptor antagonist (DORA) approved for the clinical management of insomnia. Clinical trials indicate that lemborexant improves sleep onset latency and sleep maintenance, with mild somnolence as the most common side effect. However, there is limited real-world evidence characterizing patient experiences with lemborexant, including perceived benefits, side effects, and overall treatment satisfaction, and how these factors influence treatment adherence and discontinuation. METHODS: We conducted a mixed-methods infoveillance study analyzing publicly avail

Rates and correlates of nightmares and insomnia in active duty military service members pre- and postdeployment.

OBJECTIVE: This study aimed to determine the rates, chronicity, and correlates of nightmares and insomnia, alone and in combination, in U.S. Army personnel at predeployment and postdeployment. METHOD: Service members were assessed prior to (N = 3,952) and following (N = 1,762) a combat deployment between 2010 and 2016. Participants were categorized predeployment into nightmare and insomnia groups using the PTSD Checklist-Military Version distressing dreams item (Item 2 score ≥ 3) and the Insomnia Severity Index (total score ≥ 15). Postdeployment, participants were further classified into 16 g

Stress, sex, and symptomics: Individual posttraumatic stress symptoms and sexual behaviors in adolescents.

Posttraumatic stress disorder (PTSD) is linked with sexual risk, but less is known about which aspects of posttraumatic psychopathology may be most relevant in these associations. In trauma-exposed adolescents from a national sample (N=384, M age =16.4, 72.4% female), we applied a novel "symptomics" approach-which focuses on the role of individual symptoms of psychopathology-to examine associations between PTSD symptoms and two sexual behaviors linked with potential risk: age at first sex and number of recent partners. In relative importance analyses, individual PTSD symptoms explained more va

The Development and Evaluation of a Web-Based Training Program for Cognitive Behavioral Therapy for Nightmares: CBTNightmares.org.

OBJECTIVE: Chronic nightmares are associated with a range of mental and physical health problems, including posttraumatic stress disorder, depression, alcohol use, and suicide risk. Cognitive behavioral therapy for nightmares (CBT-N) is a recommended treatment, but is underutilized because of the lack of providers trained in CBT-N. We developed and evaluated a web-based provider training for CBT-N: CBTNightmares.org. MATERIALS AND METHODS: Experts developed the content and a prototype of CBTNightmares.org. Testing occurred in 3 phases. First, licensed behavioral health providers and trainees

Development and co-creation of the Baby-related Anxiety and Behavior Inventory (BABI): An experience sampling measure.

BACKGROUND: Many women experience anxiety in the postpartum period, often with specific worries related to their newborn. Most existing anxiety questionnaires do not take into account the specific nature of postpartum- and baby-related anxiety symptoms and often omit anxiety-behaviors. Here, we share the process of constructing the Baby-related Anxiety and Behavior Inventory (BABI), a questionnaire catered to the Experience Sampling Method. METHODS: The BABI questionnaire was developed in four different phases: 1) construction of a draft questionnaire, 2) focus groups with mothers (N = 14, 5

Vivid Dreams and Nightmares as an Adverse Effect of Beta-Blockers in the Treatment of Episodic Migraine.

Beta-adrenergic blockers are effective in migraine prevention but can induce neuropsychiatric side effects, including vivid dreams and nightmares. Their lipophilicity allows penetration of the central nervous system, where β1-adrenergic blockade may disrupt REM sleep, alter noradrenergic activity, and suppress melatonin secretion, contributing to emotionally intense dreams. While reported in cardiovascular patients, this adverse effect remains underrecognized in migraine therapy. Adult patients were identified from those seen in the outpatient department of the Clinical Hospital of University

The association between serum lipid metabolism and nightmares: insights from Mendelian randomization analysis of two samples.

Background: nightmares are a common sleep disorder characterized by distressing dreams, leading to sleep disruption and psychological distress. Emerging evidence suggests a potential link between lipid metabolism and nightmare occurrence, but the causal relationship is poorly understood. Objective: to investigate the causal relationship between blood lipid metabolism characteristics and the risk of nightmares using Mendelian randomization (MR) analysis. Methods: a two-sample Mendelian randomization (MR) analysis was conducted using data from genome-wide association studies (GWAS). The analysis

Considerations for Initiation and Maintenance of Foslevodopa/Foscarbidopa for Advanced Parkinson's Disease.

BACKGROUND: As Parkinson's disease (PD) progresses, motor fluctuations become increasingly difficult to manage with oral medications. Foslevodopa/foscarbidopa (LDp/CDp), delivered as a continuous 24-h/day subcutaneous infusion, offers continuous levodopa delivery and stable plasma levodopa levels that reduce motor fluctuations. LDp/CDp has been widely utilized since becoming commercially available. OBJECTIVES: To provide practical guidance for clinicians on the real-world use of LDp/CDp, including information on patient selection, treatment initiation, dose adjustments, and long-term manageme

A pilot feasibility randomized clinical trial of a culturally adapted accelerated group treatment for posttraumatic stress, insomnia, and nightmares in firefighters.

OBJECTIVE: Firefighters experience frequent traumatic events and occupational stressors, increasing risk for posttraumatic stress disorder (PTSD), insomnia, and nightmares, which are frequently comorbid and interrelated. However, firefighter schedules and mental health stigma limit engagement in weekly therapy. The present study was a randomized pilot feasibility trial of an integrated, accelerated, culturally adapted treatment for firefighters with clinically significant PTSD, insomnia, and nightmare symptoms. METHOD: Participants (N = 49) were randomized to immediate or delayed treatment. T

Treating war- and conflict-related nightmares in children and youth: outcomes of a school-based intervention.

INTRODUCTION: Recurrent nightmares often severely impair the quality of life, school functioning, and daily functioning of trauma-exposed children and adolescents. However, research to date is limited for treatments focused on reducing the impact of posttraumatic nightmares among youth in conflict zones. This study aimed to investigate the outcome of the Better Learning Program 3 (BLP) as it was implemented in over 100 schools in Gaza (2012-2017). METHODS: Treatment outcome was investigated in an open trial among war- and conflict-exposed students (6-17 years of age) in Gaza (N = 1093). All p

Challenging Case: "Digital Addiction" in a Neurodivergent Adolescent.

Adam is a 15-year-old boy who was born prematurely, with prenatal substance exposure, and was diagnosed in early childhood with combined-type attention-deficit hyperactivity disorder and oppositional defiant disorder. He was not found to meet criteria for fetal alcohol spectrum disorder. Despite treatment with stimulant medications and other adjunctive medications, Adam experienced ongoing difficulties with impulse control, sleep, and aggression. Adam was introduced to digital devices at an early age, resulting in unfiltered, poorly supervised, and prolonged screen exposure.Over time, Adam's d

A narrative review on the neurobiology of lucid dreaming: mechanisms and therapeutic potential.

Источник пометил публикацию как отозванную

BACKGROUND: Lucid dreaming (LD), which involves being aware of and controlling one's dreams, is of particular interest in the study of consciousness and has potential therapeutic applications. In this narrative review, we highlight the neurobiological aspects of LD, specifically its mechanisms associated with the activation of prefrontal and parietal areas, increased gamma oscillations, and the influence of cholinergic and dopaminergic signaling, primarily during rapid eye movement (REM) sleep. OBJECTIVE: To evaluate human data on brain mechanisms and clinical effects, synthesizing the neurob

Enhancing identification and diagnosis of nightmare disorder in active duty patients at a sleep disorders center: A quality improvement initiative.

STUDY OBJECTIVES: Nightmare Disorder (NDO) is an underdiagnosed sleep disorder causing significant impairment, particularly in active duty U.S. military personnel who experience unique stressors. Despite 59.28% of referred service members reporting nightmares and 19.76% screening positive for probable NDO, baseline diagnosis rate was only 0.60%. Our quality improvement project aimed to improve identification and diagnosis of NDO among active duty service members at our sleep disorders center by increasing the confirmed diagnosis rate from 0.60% to at least 5% within 12 months. METHODS: A qual

Physiological markers of Trauma-related nightmares among military personnel suffering from PTSD: A multicenter home-recording study.

AIM: Despite being one of the most disabling symptoms of post-traumatic stress disorder (PTSD)-disrupting sleep continuity, reinforcing hyperarousal, and worsening psychiatric comorbidity-the physiological signature of Trauma-related nightmares (TRNs) under naturalistic sleep conditions remains poorly characterized. METHODS: We used home-based, multi-sensor devices from the SOMMEPT cohort to assess whether TRNs display distinct autonomic dynamics before and after awakening. TRN awakenings were self-marked with the wristband button and validated through electroencephalography (EEG) inspection
