Thirteen of 14 patients with vaginismus have been successfully treated using systematic in vivo desensitization and other simple but individualized behavioral techniques. Resolution of symptoms was usually effected within 3 to 4 weekly sessions with important supplementary home assignments. All but 1 patient were seen without partners. This paper reviewed 36 studies which examined the effects of the group format in the treatment of sexual dysfunctions. Three cases of orgasmic inhibition by clomipramine are reported, one in a male and two in females. All were depressed patients with obsessive-compulsive features. Sexuality was investigated in 35 males and 25 females with chronic back pain. Prior to onset of pain orgasmic dysfunction was common in 60 per cent of the females with relatively lower level of sexual frustration. Markedly less sexual dysfunction characterized the males. Forty-eight couples who complained of female orgasmic dysfunction received one of two therapies: sex therapy and communication therapy. Thirty subjects seeking treatment for primary inorgasmia accompanied by sexual anxiety or aversion were assigned to one of three groups: systematic desensitization (SD), directed masturbation (DM), or waiting list control (WL). This paper describes a structured, time-limited method for treating couples in which both people are experiencing sexual dysfunctions. In our previous research, we found that group treatment was as effective as single-couple treatment for treating men experiencing premature ejaculation and women experiencing secondary orgasmic dysfunction. Changes in personality scores and self-perception of 92 couples through sex therapy are reported. The sample included 52 couples with female symptoms (orgasmic dysfunction and vaginismus) and 40 couples with male symptoms (erectile dysfunction and premature ejaculation), who all completed therapy. One-year follow-up showed significant changes indicating a general increase of emotional stability and a reduction of neurotic tendencies in both partners.

This article describes a group of 23 Irish women with vaginismus. This figure represents 42% of the total female referrals to the author, and is quite high when compared with other reports. The type of treatment before referral, and the characteristics of the patients, tend to confirm the author's diagnosis, which was made only after vaginal examination. Of 17 women ages 19 to 38 with primary orgasmic dysfunction, 7 achieved orgasm within a week after viewing an audiovisual sex education program, a statistically significant change (p less than .02). No other intervention (educational, behavioral, psychotherapeutic) occurred between the audiovisual session and later inquiry about subsequent sexual response. These results suggest that explicit sex education can help remedy sexual dysfunction in a significant number of women and that further investigation is needed.

In analyzing the responses of 100 predominantly white, well educated and happily married couples to a self-report questionnaire, this study examined the frequency of sexual problems experienced and the relations of those problems to sexual satisfaction. This study investigated the impact of group systematic desensitization (SD) on varied aspects of sexual functioning in primary and secondary nonorgasmic women.
