Addressing Sexual Health in Primary Care.

Sexual health concerns are prevalent in primary care, and treatment includes both medication and nonpharmacologic options. Careful assessment, including both a general and tailored sexual health history, is important. Four of the most common sexual health concerns diagnosed and treated in primary care include erectile dysfunction, premature ejaculation, sexual pain, and low desire.

Genital-prioritized attentional bias in lifelong premature ejaculation: behavioral evidence from eye-tracking and machine learning.

Lifelong premature ejaculation (LPE) involves altered responses to sexual cues. Neuroimaging has identified attention-related neural abnormalities in LPE, but behavioral evidence for attentional bias remains limited. Using eye tracking, we compared visual-attention patterns in 35 heterosexual men with LPE (andrology outpatient clinic, Henan Provincial People's Hospital) and 38 heterosexual healthy controls (HCs) (online advertisements on official platforms of Henan Provincial People's Hospital and Xidian University), from Nov 2023 to Jun 2024. Participants freely viewed three categories of sex

Understanding health care practitioners' criteria regarding the decision to treat men with premature ejaculation.

BACKGROUND: The extent to which health care practitioners (HCP) follow clinical practice guidelines (CPG) regarding diagnostic criteria can affect treatment outcomes, yet little is known about the factors HCPs take into consideration regarding their decision to treat men with symptoms of premature ejaculation (PE), a field where professional definitions vary considerably. AIM: To describe HCP priorities regarding the use of PE diagnostic criteria in their decision-to-treat and, further, to assess whether HCP characteristics regarding specialization, professional identity, and experience impac

Evaluating the Role of Subcoronal Botulinum Toxin Injection in the Management of Premature Ejaculation.

BACKGROUND: Premature ejaculation (PE) is the most common male sexual dysfunction, which often causes significant psychological problems. Although botulinum toxin A (Botox) has shown promise for delaying ejaculation through chemodenervation, the optimal anatomical target remains uncertain. This study evaluated the efficacy and safety of Botox injection into the penile prepuce and subcoronal region as a novel approach for lifelong PE. METHODS: A prospective case series was conducted in 7 adult men (mean age 54.7 ± 6.5 years) diagnosed with lifelong PE (intravaginal ejaculatory latency time <1

Premature ejaculation: A comprehensive urological review of current evidence-based management strategies.

Premature ejaculation (PE), acquired or lifelong, is a ubiquitous male sexual dysfunction with profound psychosexual morbidity. Accurate diagnosis requires integration of symptom timing, perceived ejaculatory control, partner perspective, and comorbidities, notably erectile dysfunction, alongside validated patient-reported outcomes; IELT, while a useful objective metric for severity grading, is insufficient as a standalone diagnostic criterion. Initial therapy is on-demand topical anesthetics and dapoxetine, with enhanced durability when combined with psychosexual therapy. Off-label SSRIs prod

DIA-Based Plasma Proteomic Profiling Identifies Novel Biomarkers for Diagnosis of Premature Ejaculation and Their Association With Dapoxetine Treatment Outcomes.

BACKGROUND: Premature ejaculation (PE) is a highly prevalent male sexual dysfunction with undefined etiopathogenesis and a lack of validated objective biomarkers. Lifelong premature ejaculation (LPE) represents a more severe and persistent form of the disorder. Noninvasive plasma protein biomarkers for LPE diagnosis and their association with dapoxetine treatment outcomes remain insufficiently investigated. OBJECTIVES: This study aims to identify and validate novel plasma protein biomarkers for LPE via data-independent acquisition (DIA) proteomics, construct a diagnosis model and clinical nom

The effect of glandular frenulectomy on refractory lifelong premature ejaculation: a pilot study.

PURPOSE: Premature ejaculation (PE) is one of the most common male sexual disorders and is influenced by both psychological and biological factors. Despite available treatments, including psychosexual therapy and pharmacological interventions, no definitive cure exists. In addition, various interventional approaches have been investigated for lifelong PE, such as glandular filler injections, partial dorsal nerve neurectomy, and frenulectomy. PATIENTS AND METHODS: This pilot study evaluated the effect of glandular frenulectomy in 25 married men with lifelong PE who had not responded to convent

Men with Various Subtypes of Premature Ejaculation Differ in the Perceived Cause of Their Problem.

How men perceive the cause for their premature ejaculation (PE) impacts their understanding of their problem, likelihood of seeking treatment, and acceptance of and adherence to a treatment protocol. This study aimed to understand men's perceived reasons (attributions) for their PE, to explore differences across PE subtypes having different developmental progressions, and to identify covariates that might help explain subtype differences. 446 online survey respondents who indicated "ejaculating before desired" and met inclusion criteria were verified as having "definite" PE using an abridged v
