Postpartum anemia and maternal wellbeing: A cohort analysis of the WOMAN-2 trial.

OBJECTIVE: We examined the association between postpartum anemia and maternal wellbeing in women who gave birth with anemia. METHODS: We conducted a cohort analysis using secondary data from the WOMAN-2 trial. Between August 2019 and September 2023, women with moderate or severe anemia who were giving birth vaginally were recruited from hospitals in Nigeria, Pakistan, Tanzania, and Zambia. Our exposure was postpartum anemia (hemoglobin concentration <70, 70-99, or ≥100 g/L: severe, mild/moderate, and no postpartum anemia, respectively). Our primary outcome was physical capacity (6-min walk te

Unpacking postpartum depression in rural India: an integrated analysis of risk factors at 12 months and child development outcomes at 18 months of age - findings from the SPRING study.

BACKGROUND: Postpartum depression (PPD) negatively affects maternal well-being and early child development (ECD). While research often focuses on the early postpartum depression, symptoms can emerge or persist later in the first year, potentially affecting mothers and children differently at this stage. Yet, the 12-month postpartum period remains understudied globally, particularly in low-resourced settings where contextual stressors are pronounced. This study addresses the urgent need to understand later-stage postpartum depression and how it influences child development at 18 months, a criti

Associations between exposure to intimate partner violence (IPV) and infant developmental delay: moderating role of women's empowerment at six weeks postpartum.

BACKGROUND: Intimate Partner Violence (IPV) is a significant threat to women’s wellbeing and child development, particularly in lower- and middle-income countries (LMICs). Despite its prevalence, research in LMICs on the effects of IPV on child development remains limited. This study investigates the association between IPV and different domains of child development as well as the moderating role of women’s empowerment. METHODS: Participants included 397 women with at least mild anxiety from a randomised trial of a perinatal anxiety intervention at Holy Family Hospital in Rawalpindi, Pakistan.

Prevalence, associated factors and outcomes of maternity blues among postpartum mothers: a multi-facility prospective cohort in rural Southwestern Uganda.

BACKGROUND: Maternity blues (MBs) are mild and self-limiting depressive symptoms that occur within the first weeks postpartum. However, failure to identify and manage affected mothers can lead to maternal emotional and cognitive impairment, disrupt infant care, and increase the risk of postpartum depression (PPD), infanticide, or suicide. The average prevalence of maternity blues in sub-Saharan Africa (SSA) is 39%, with 10–15% of affected mothers progressing to PPD. Therefore, providing timely support and management to mothers experiencing maternity blues is crucial to prevent this progression

Intergenerational support, conflict, and postpartum depression trajectories among Chinese mothers: Family-based care and multilevel predictors.

BACKGROUND: Postpartum depression (PPD) remains a major public health concern. In China, the postpartum period is embedded within a family-based caregiving system characterized by intensive intergenerational involvement. Within this context, intergenerational support (IS) and intergenerational conflict (IC) often coexist, forming a distinct stress-regulation environment. Guided by the stress-buffering model of social support, this study conceptualizes the Chinese postpartum family as a dynamic system in which IS and IC jointly shape maternal mental health over time. METHODS: A four-wave longi

Antidepressant continuation in pregnancy and postpartum depression risk across racial and ethnic groups.

BACKGROUND: Postpartum depression affects 1 in 7 women in the United States and antidepressant medications during pregnancy are recommended by obstetric and psychiatric professional societies for select patients with a history of depressive disorders. However, prenatal antidepressant medication use remains controversial, and current evidence does not allow conclusions regarding prenatal antidepressant medication effectiveness in preventing postpartum depression. OBJECTIVE: To determine postpartum depression risk in women stopping versus continuing antidepressant medications during pregnancy a

Psychological support and postpartum depression in twin gestations: a prospective analysis using the Edinburgh Scale.

OBJECTIVE: To assess the risk of postpartum depression (PPD) in women with twin pregnancies using the Edinburgh Postnatal Depression Scale (EPDS), and to explore associations between maternal and neonatal complications and depressive symptoms during the postpartum period. METHODS: This prospective, mixed-methods study was conducted at the Multiple Pregnancy Outpatient Clinic. Fourteen women with twin pregnancies received psychodynamic psychological prenatal care throughout the pregnancy-puerperal cycle. Postpartum depressive symptoms were screened using the EPDS, with scores ≥13 indicating pr

The feasibility and acceptability of implementing simplified cognitive behavioral therapy approaches to support postpartum mental health and address associated social and behavioral barriers to postpartum family planning in Amhara, Ethiopia: a qualitative study.

BACKGROUND: Poor mental health can negatively impact health outcomes across diverse health areas, including in the first year postpartum. Yet, the intersection of postpartum mental health and postpartum family planning (FP) is understudied. Cognitive Behavioral Therapy (CBT) is an evidence-based practice that has proven helpful for improving mental health and supporting positive behavior change across health areas, including in low-resource settings. Drawing on existing CBT tools, we created and piloted an intervention called Mothers Time, designed to be delivered in three sessions by a commun

Why do mood disturbances often occur in early postpartum? An integrative neurobiological and evolutionary perspective.

One of the most perplexing issues in clinical psychology is why the early postpartum period, supposed to be one of the happiest moments of a woman's life, is often associated with a significantly higher incidence of mental health problems. This manuscript advocates the idea that pervasive mood disturbances during the early postpartum period can be understood by exploring their proximate neurobiological mechanisms and ultimate adaptive functions. At the biological level, alterations in mood states are thought to proximately result from pregnancy- and motherhood-induced neurobiological changes,

Current concepts in postpartum anemia management.

PURPOSE OF REVIEW: Postpartum anemia (PPA) is common in women after childbirth and affects about 50-80% of all women worldwide. Iron deficiency (ID) is the main cause for anemia and constitutes a potentially preventable condition with great impact on the mother's physical and mental condition after delivery. In most cases, PPA is associated with antenatal ID and peripartum blood losses. Numerous published studies confirmed the positive effect of PPA diagnosis and treatment. RECENT FINDINGS: Iron deficiency as well as iron deficiency anemia (IDA) are common in the postpartum period and represe

Maternity Blues: A Narrative Review.

Puerperium is a period of great vulnerability for the woman, associated with intense physical and emotional changes. Maternity blues (MB), also known as baby blues, postnatal blues, or post-partum blues, include low mood and mild, transient, self-limited depressive symptoms, which can be developed in the first days after delivery. However, the correct identification of this condition is difficult because a shared definition and well-established diagnostic tools are not still available. A great heterogenicity has been reported worldwide regarding MB prevalence. Studies described an overall prev

Trajectories of sleep quality and depressive symptoms in women from pregnancy to 3 months postpartum: a prospective cohort study.

Sleep quality and depression during pregnancy often affect women's adaptation to motherhood and are linked with adverse maternal and neonatal outcomes. Using a prospective cohort study comprising 190 pregnant women in central Taiwan, we investigated the trajectories of sleep quality and depressive symptoms and their associated predictors in perinatal women from pregnancy to postpartum. Sleep and depressive symptoms were assessed using the Pittsburgh Sleep Quality Index and the Edinburgh Postnatal Depression Scale, respectively, from mid-pregnancy to 3 months postpartum. We used group-based tra

Real-world treatment modalities, health care resource utilization, and costs among commercially insured patients with newly diagnosed major depressive disorder in the United States.

BACKGROUND: In the United States, major depressive disorder (MDD) is one of the most prevalent mental health disorders. Treatment guidelines for MDD recommend pharmacologic and nonpharmacologic therapies tailored to the patient's disease severity, level of function, and comorbid health conditions. While previous studies examined real-world pharmacologic treatment patterns and costs among patients with MDD, few have examined the use of nonpharmacologic treatments and their association with health care resource utilization (HCRU) and cost. OBJECTIVE: To describe prevalence and associations betwe

Fostering Resilience Among Mothers Early (FRAME): using growth mixture modeling to identify resources that mitigate perinatal depression.

The primary objective of this study was to delineate classes of individuals based on depression trajectories from the antenatal period through 54-month postpartum and internal and external resources that are associated with low depression risk. Participants came from the Growing Up in New Zealand (GUiNZ) study (n = 5664), which is a pregnancy cohort study and is nationally representative of the ethnic and socioeconomic diversity of contemporary New Zealand births. Growth curve mixture modeling was used to identify distinct subgroups based on depression scores from the antenatal period through
