Factors Associated With Severe Mental Illnesses Newly Diagnosed in Perinatal Psychiatric Care: Findings From a Large Clinical Cohort.

BACKGROUND: The perinatal period represents a phase of increased psychiatric vulnerability, yet the factors linked to severe mental illnesses (SMIs) diagnosed during pregnancy or postpartum remain unclear. This study sought to identify associated and predictive factors of depression, bipolar disorder, and schizophrenia-related disorders diagnosed during pregnancy vs postpartum, with or without psychiatric comorbidity, in women seeking psychiatric care. METHODS: Using a retrospective cohort design, data from 927 women who accessed specialized perinatal psychiatric services in Quebec City (Cana

Unmasking Potential Impacts of Parent and Coparent ADHD on Mental Health in the First Year Postpartum.

OBJECTIVE: Parental mental health plays a critical role in the wellbeing of the family, yet few studies have examined how parental ADHD affects postpartum depression and anxiety. This study investigates the hypothesis that parent (including coparent) ADHD symptoms are associated with postpartum depressive and anxiety symptoms. METHODS: 117 mother-father dyads (half with parental ADHD) were recruited when their infants were 6 to 10 months old. Participants completed questionnaires on ADHD, depression, anxiety, and social support. Clinician-administered semi-structured interviews assessed ADHD

Focal impaired awareness seizures in the context of postpartum psychosis and double maternal filicide.

Post partum psychosis is a rare but severe psychiatric emergency, and its comorbidity with a new onset focal seizure disorder can pose a profound diagnostic and therapeutic challenge. We present a tragic traumatic case where a 30-year-old multiparous woman, 12 weeks postpartum, with delusions of control, persecution, and an EEG-confirmed focal impaired awareness seizure (complex partial seizure), killed her two children. She was referred to our facility by the police 3 days after the tragic event in a confused and disoriented state. She had no previous history of psychiatric or neurological di

Genetic architecture of postpartum psychosis: from common to rare genetic variation.

Postpartum psychosis is a severe psychiatric condition marked by the abrupt onset of psychosis, mania, or psychotic depression following childbirth. Despite evidence for a strong genetic basis, the roles of common and rare genetic variation remain poorly understood. Leveraging data from Swedish national registers and genomic data from the All of Us Research Program, we estimated family-based heritability at 55% and whole-genome sequencing-based heritability at 46%. Rare coding variant analysis identified HMGCR as a gene in which rare damaging variants confer risk for postpartum psychosis (FDR

Postpartum Mental Health Problems in the Mothers of Very, Moderately and Late Preterm Infants During and After COVID-19 Pandemic: A Cross-Sectional Comparison Study and Possible Risk Factors.

BACKGROUND: Preterm birth (PTB) affects ~11% of women worldwide. The COVID-19 pandemic put an additional burden on this already challenging situation. AIMS: We aimed to analyse (1) the impact of the COVID-19 pandemic on the severity of specific depressive, anhedonia, anxiety and self-harm thoughts (SHTs) symptoms in mothers of preterm babies, (2) differences in the severity of these symptoms among mothers who gave birth very preterm, moderately and late preterm and (3) potential socio-demographic, psychological, birth-related and COVID-19-related factors associated with an increase in the mot

The tokophobia severity scale: a psychometric multicountry study with childbearing-age women.

PURPOSE: Tokophobia refers to a clinically significant fear of childbirth that affects women across the reproductive lifespan. Given its prevalence and association with adverse mental health, obstetric, and reproductive decision-making outcomes, valid and reliable measures are essential to identify women at risk of tokophobia. METHODS: Tokophobia Severity Scale (TSS) psychometric properties were analyzed in nulliparous non-pregnant, pregnant, and women in the postpartum using data from cross-sectional and longitudinal studies conducted in four countries: Australia, Lithuania, Poland, and Port

Anxiety and depression after pregnancy-related cerebral venous sinus thrombosis: associations with functional disability at discharge.

BACKGROUND: Cerebral venous sinus thrombosis (CVST) during pregnancy and the puerperium is a rare but potentially life-threatening condition. While neurological and functional outcomes have been increasingly studied, the burden of anxiety and depression after CVST in pregnant and postpartum women remains poorly characterized. METHODS: We conducted a retrospective cohort study of pregnant and postpartum women diagnosed with CVST at a tertiary referral center. Anxiety and depression were assessed at 3 months after CVST using the Hamilton Anxiety Rating Scale (HAMA) and the 24-item Hamilton Depr

Multifetal gestation and long-term maternal health.

Multifetal gestations account for 2% to 3% of births in high-resource settings and impose substantially greater cardiovascular and metabolic stress than singleton pregnancies. Women carrying twins experience approximately 15% higher cardiac output, greater plasma volume expansion, and elevated placental angiogenic factors, translating into approximately twofold higher rates of hypertensive disorders of pregnancy and variable rates of gestational diabetes mellitus. The early postpartum period represents heightened cardiovascular and mental health vulnerability, with increased rates of cardiopul

Risk Factors Associated with Maternal Postpartum Hospital Readmission: A Systematic Review.

Background: Maternal postpartum hospital readmissions represent profound implications for maternal health outcomes and potential gaps in quality of maternal care. Objective: This study aims to synthesise evidence on risk factors for maternal postpartum hospital readmissions within 42 days of discharge following birth hospitalisation. Methods: An electronic database search utilised CINAHL, EMBASE (Ovid), and MEDLINE for relevant studies published from 1 January 2010 to 30 June 2024. Maternal and perinatal factors associated with childbirth-related post-traumatic stress symptoms: a cross-sectional study.

OBJECTIVE: The aim of this study was to examine how a set of selected interacting maternal and perinatal clinical characteristics are associated with symptoms of post-traumatic stress after birth. METHODS: We conducted a cross-sectional online survey among women between 3 and 18 months after birth. Participants completed self-report measures of post-traumatic stress symptoms and questions regarding clinical variables related to pregnancy, birth and the postnatal period. Statistical models were used to estimate the association between clinical factors and symptom severity. RESULTS: The strong

Validation of the Postpartum Sleep Quality Scale and assessment of postpartum sleep quality in Vietnam during the COVID-19 pandemic.

OBJECTIVE: This study aimed to evaluate the validity and reliability of the Postpartum Sleep Quality Scale (PSQS) for assessing sleep quality among postpartum women in Vietnam during the COVID-19 pandemic. The major hypothesis is that the PSQS will provide a reliable and valid measure of postpartum sleep disturbances and their correlation with mental health outcomes, particularly postpartum depression (PPD). DESIGN: A cross-sectional, observational study was conducted from May to December 2023. SETTING: The study was carried out at a leading maternal healthcare centre in Vietnam, located in

Self‑harm ideation in perinatal women: Prevalence across gestational and postpartum windows and general risk and protective factors.

INTRODUCTION: The perinatal period is marked by heightened vulnerability to mental health difficulties, including the onset of self-harm ideation (SHI). We aimed to (a) estimate the prevalence of self-harm ideation among perinatal women, (b) explore sociodemographic, clinical, and psychosocial correlates of SHI. METHODS: This is a cross-sectional study involving pregnant (n = 5,087) and postpartum women (n = 6,550). SHI was measured using item 10 of the Edinburgh Postnatal Depression Scale (EPDS), depressive symptoms using the remaining 9 EPDS items (EPDS-9), and anxiety symptoms using the Ge

Unpacking postpartum depression in rural India: an integrated analysis of risk factors at 12 months and child development outcomes at 18 months of age - findings from the SPRING study.

BACKGROUND: Postpartum depression (PPD) negatively affects maternal well-being and early child development (ECD). While research often focuses on the early postpartum depression, symptoms can emerge or persist later in the first year, potentially affecting mothers and children differently at this stage. Yet, the 12-month postpartum period remains understudied globally, particularly in low-resourced settings where contextual stressors are pronounced. This study addresses the urgent need to understand later-stage postpartum depression and how it influences child development at 18 months, a criti

Antidepressant continuation in pregnancy and postpartum depression risk across racial and ethnic groups.

BACKGROUND: Postpartum depression affects 1 in 7 women in the United States and antidepressant medications during pregnancy are recommended by obstetric and psychiatric professional societies for select patients with a history of depressive disorders. However, prenatal antidepressant medication use remains controversial, and current evidence does not allow conclusions regarding prenatal antidepressant medication effectiveness in preventing postpartum depression. OBJECTIVE: To determine postpartum depression risk in women stopping versus continuing antidepressant medications during pregnancy a

Prospective transitions in hemoglobin A1c following gestational diabetes using multistate Markov models.

We characterized the state-to-state transitions in postpartum hemoglobin A1c levels after gestational diabetes, including remaining in a state of normoglycemia or transitions between prediabetes or diabetes states of varying severity. We used data from the APPLE Cohort, a postpartum population-based cohort of individuals with gestational diabetes between 2009 and 2011, and linked A1c data with up to 9 years of follow-up (n = 34 171). We examined maternal sociodemographic and perinatal characteristics as predictors of transitions in A1c progression using Markov multistate models. In the first y
