Shedding light on the efficacy of psychodynamic treatment targeting depression for patients with comorbid narcissistic personality disorder.

OBJECTIVE: Major depressive disorder (MDD) frequently co-occurs with narcissistic personality disorder (NPD). Despite the clinical relevance of this comorbidity, little is known about the efficacy of short-term depression-focused treatments for individuals with comorbid NPD. This study examines whether individuals with comorbid MDD and NPD benefit from treatment and whether NPD diagnostic status changes during therapy. METHOD: One hundred patients in a randomized controlled trial (RCT) were assessed for NPD (n = 14) using the Structured Interview for DSM-IV. Participants were randomly assigne

Depressive symptom severity and treatment outcome in posttraumatic stress disorder following childhood trauma: A secondary analysis of a randomized clinical trial.

Depressive symptoms are frequently comorbid with posttraumatic stress disorder (PTSD), yet it remains unclear whether depressive symptom severity before the start of treatment affects the results of trauma-focused psychotherapy. We hypothesized that baseline depressive symptom severity might reduce overall PTSD symptom improvement and could differentially affect treatment response to imagery rescripting (ImRs) versus eye movement desensitization and reprocessing (EMDR). This secondary analysis of a randomized controlled trial included 150 adults with childhood trauma-related PTSD. Participants

Relevance of the negative valence system, the cognitive system, and their interactions for understanding aggression in mental disorders.

BACKGROUND: The Research Domain Criteria (RDoC) domains of the negative valence system (NVS) and the cognitive system (CS) offer a framework for current research on aggression. In particular, the projects of the Transregio "Neuropsychobiology of Aggression" (Transregional Collaborative Research Center 379, TRR 379) are grouped according to these domains. OBJECTIVE OF THE STUDY: The present review article describes how these domains are conceptualized and what role they play in the investigation and understanding of aggressive behavior in mental disorders and its biological correlates within t

'Playing the Manic Game' revisited.

In this editorial, we argue that revisiting 'Playing the Manic Game', a paper whose clinical observations remain recognisable more than 50 years later, may help clarify a problem clinicians still face: when disruptive behaviour occurs during acute mania, how much reflects the episode itself and how much reflects a more enduring personality pattern? That distinction matters for treatment, team responses, families and stigma.

Self-Criticism as a Mechanism of Change in Compassion-Based Interventions: A Meta-Analytic Structural Equation Modelling Approach and Systematic Review.

Compassion-based interventions (CBIs) are effective transdiagnostic treatments for a wide range of psychopathological symptoms. While their efficacy is well established, their mechanisms remain insufficiently understood. Self-criticism (SC) has been proposed as a promising mechanism of change (MoC), given its central role in psychopathology and its theoretical link with CBIs. This study synthesises evidence from randomised controlled trials to examine whether reductions in SC mediate the effects of CBIs on psychopathological symptoms. Systematic searches in PubMed, Web of Science, Scopus and P

Experiences of mothers with borderline personality disorder following an intervention to support mother-infant relationships: "more tools in my toolbox".

Motherhood is inherently demanding, and some of these demands may be amplified for women with borderline personality disorder (BPD). The exacerbation of BPD symptoms from parenting-related stressors can have adverse impacts on mother-infant relationships and infant development. This qualitative study explores Australian mothers' views of the impacts of mother-infant dialectical therapy plus (MI-DBT+). MI-DBT+ is an innovative therapeutic intervention which integrates mother-infant dialectical behavior Therapy (MI-DBT), a known effective intervention for the mental health of mothers with BPD, w

Some Aspects for Understanding Psychodynamic Processes This article discusses psychodynamics as a central concept in psychoanalysis and psychodynamic psychotherapy. Psychodynamics refers to the interplay of unconscious conflicts, affects, relationships, and early experiences that shape the development of thinking, feeling, and behaviour, while also capturing the complexity of the psyche, which cannot be readily understood in a straightforward manner. The article emphasizes that psychic development invariably takes place within relationships with others and that internal and external influences

Struggling for resonance: Mediopassive interaction in a long-term treatment of severe personality disorder.

Although clinicians often report significant emotional challenges in psychotherapy for personality disorders, we still know relatively little about how these difficulties are enacted and reorganized in moment-to-moment patient-therapist interaction. We examine whether interactional resonance and mediopassive agency can help illuminate relational difficulties and change in a good-outcome long-term psychodynamic psychotherapy involving a patient with severe interpersonal pathology, chronic depressive symptoms, and somatic complaints. The patient met the diagnostic criteria for borderline and avo

Brain structural covariance networks in co-occurring PTSD and alcohol use disorder: An ENIGMA PTSD and ENIGMA addiction working group collaboration.

High rates of comorbid posttraumatic stress disorder (PTSD) and alcohol use disorder (AUD) may stem from a shared neurobiological basis; however, research exploring this overlap remains limited. We examined whole-brain structural covariance (SC) across PTSD, AUD, comorbid PTSD & AUD, trauma-exposed controls, and trauma-naïve controls to identify shared and unique SC profiles. Structural brain scans were acquired from individuals across 14 sites (N = 1565), and cortical thickness estimates were extracted to compute SC. Group differences in SC between pairs of cortical regions were examined usin

Shared Genetic Architecture Between Common Epilepsies and Subcortical Brain Volumes Is Associated With Cognition and Mental Health.

AIMS: Epilepsy is a common neurological disorder, often presenting with psychiatric comorbidities and cognitive impairment. Neuroimaging studies have suggested associations between epilepsy and subcortical volumetric abnormalities. The genetic basis between epilepsy and subcortical volume abnormalities is, however, poorly understood, particularly if they share a common genetic architecture. METHODS: To investigate polygenic overlap, we employed the bivariate causal mixture model (MiXeR) to analyze genome-wide association data from various epilepsy subtypes (genetic generalized epilepsy [GGE]:

Childhood emotional abuse and adult psychopathology: A multilevel meta-analytic review.

Childhood abuse is a known risk factor for adult psychopathology. The associations between childhood physical abuse (PA) and sexual abuse (SA) and adult psychopathology are more widely documented than those involving childhood emotional abuse (EA). We conducted a multilevel meta-analysis to examine how adult psychopathology relates to childhood EA in comparison to childhood PA and SA. Six electronic databases were searched to identify studies reporting associations between childhood EA, PA, and SA and adult psychopathology symptoms, and 414 effect sizes from 104 studies were analyzed. Incidence of mental disorders and its predictors among air force personnel exposed to counter-insurgency operations in a West African country: a 6-month follow-up study.

BACKGROUND: Despite robust reports in the literature concerning the effect of exposure to combat on mental health, there is a paucity of studies in the Nigerian Air Force (NAF) using a longitudinal design. This study was designed to determine the incidence of mental disorders among a cohort of NAF personnel exposed to combat and highlight its association with psychosocio-religious variables. METHODS: It was a follow-up study using a total population sampling in which all the 290 deployed personnel participated. Data was collected at two intervals: Baseline and 6 months after with sociodemogra

Alexithymia and personality pathology in relation to dimensions of general psychopathology.

OBJECTIVES: To investigate whether alexithymia is uniquely associated with major dimensions of psychopathology beyond maladaptive personality trait dimensions. METHOD: We conducted a cross-sectional study using archival data collected from a diagnostically heterogeneous sample of psychiatric outpatients (N = 154). Patients in this study sample had completed the 20-item Toronto Alexithymia Scale (TAS-20), the Personality Inventory for DSM-5 (PID-5), and the Restructured Clinical (RC) scales of the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF). The Effect of Weight Stigma on Adolescent Health: Mediating and Moderating Roles of Social Support and Personality Traits.

Weight stigma is a common psychosocial stressor among adolescents and an important determinant of mental and physical well-being. However, the mechanisms through which it affects health outcomes remain insufficiently understood. This study explored the mediating and moderating roles of personality traits and perceived social support in the relationship between weight stigma and health outcomes in adolescents. A cross-sectional self-report study was conducted with 1,011 high school students (59.64% female; Mage = 16.50 years, SDage = 1.10), using validated measures of weight stigma, Big Five pe

Hierarchical neural variability reveals adaptive and maladaptive mechanisms of non-suicidal self-injury.

Non-suicidal self-injury (NSSI) is a prevalent and clinically urgent condition in adolescence, marked by impairments in cognitive flexibility-a capacity thought to be supported by neural variability. However, most research to date has focused on static brain features, leaving the role of variability largely unexplored. Importantly, variability-based metrics have been emerging as promising biomarkers in psychiatry, offering superior test-retest and intersession reliability compared with traditional static metrics. Leveraging resting-state fMRI data from 160 psychiatric patients with NSSI, prima

Assessing treatment adherence in psychiatry: a comparison of clinical judgment and therapeutic drug monitoring in acute care.

Treatment adherence is the cornerstone of the management of psychiatric disorders in the prevention of acute symptom exacerbation, relapse, and hospitalization. Patients are usually treated with psychotropic polypharmacy, including complex regimens, which may contribute to nonadherence. In this real-world acute setting study, we used direct [therapeutic drug monitoring (TDM)] and indirect (emergency room and acute ward clinical judgments) measures to assess adherence in 145 patients with severe mental illnesses. Logistic regressions were used to predict adherence and agreement between adherenc

Revitalizing personality disorder research.

Not long ago, personality disorder (PD) research stood at the forefront of a paradigm shift toward dimensional models of psychopathology. But a decade after PD research promised to reshape diagnosis and treatment, the field finds itself stalled-methodologically stagnant and conceptually insular. This special issue aims to redirect that course. It features nine contributions organized around five themes: context, theory building, personality functioning, assessment and diagnosis, and treatment. Each article illustrates what becomes possible when PD research embraces contemporary tools-from comp

The interplay of personality functioning and affect-event dynamics in predicting future impairment and depression: A large mobile mental health ambulatory assessment study.

Since the Diagnostic and Statistical Manual of Mental Disorders, fifth edition, and the 11th revision of the International Classification of Diseases introduced dimensional assessment of personality functioning (PF), PF impairments have been linked to transdiagnostic mental-health risk factors. Using ecological momentary assessment data from 16,038 mental health app users, we tested whether baseline PF impairment, affect-event dynamics, and depression predict future PF impairment and depression severity, hypothesizing a central role of PF. In the first month, users completed 69.3 mood (range 3

Decomposing Spatial Effects of State-Level Health Outcomes: A Methodological Demonstration and Re-Analysis.

While spatial autoregressive (SAR) models are increasingly used in population-level psychological studies, researchers often overlook the crucial step of parsing effects into direct, indirect and total impacts, a standard practice in spatial econometrics. In this paper, we demonstrate the necessity of this practice by re-analyzing Gruda et al.'s (2024) U.S. Dark-Triad and health dataset with heteroskedasticity-robust SAR models and full impact decomposition, revealing significant changes. The previously observed direct protective effect of state-level narcissism on hypertension mortality disap
