What do general adult psychiatry patients think we should call borderline personality disorder? A cross-sectional study to find the most acceptable diagnostic term.

AIMS AND METHOD: To determine the most acceptable term for borderline personality disorder (BPD). We conducted a cross-sectional study of patients who know what it feels like to be diagnosed with a mental disorder. The main outcome measures were the proportion of participants offended and confused by alternative terms for BPD. RESULTS: Seventy-two people participated in the study. Being diagnosed with a condition was more offensive than being diagnosed with a disorder (χ2 = 41.18, d.f. = 1, P < 0.01). Fluxithymia offended the fewest participants (13%), but was the most confusing term (31%). E

Accumulated lifetime violence load and interpersonal problems among suicidal women with emotionally unstable personality disorder.

BACKGROUND: Patients with emotionally unstable personality disorder (EUPD) report increased use and exposure to interpersonal violence and experience significant interpersonal difficulties. The relationship between the type of self-reported interpersonal problems and interpersonal violence among individuals with EUPD remains unclear. This study investigates the association between interpersonal problems and the cumulative lifetime violence burden among suicidal women with EUPD. METHODS: The study included 103 women diagnosed with EUPD who had attempted suicide at least twice. The inventory of

Prevalence and impact of comorbid PTSD, c-PTSD and EUPD on symptom severity in functional neurological disorder: protocol for a systematic review and meta-analysis.

INTRODUCTION: Previous trauma and adverse life experiences have been hypothesised to be aetiological factors for functional neurological disorder (FND) leading to the hypothesis of a 'trauma-subtype' of FND. Individuals who have experienced prior abuse are more likely to develop FND than healthy controls. Post-traumatic stress disorder (PTSD) and personality disorders have been described to be comorbid with FND at varying prevalence rates. Complex PTSD (c-PTSD) and emotionally unstable personality disorder (EUPD) have clinical similarities with PTSD and trauma is a common aetiological factor i

Managing Medically Severe Self-Injury in Borderline Personality Disorder and Autism: A Case Report of Self-Induced Toe Gangrene.

Medically severe self-injury (MSSI) in individuals with emotionally unstable personality disorder (EUPD) and co-occurring autism spectrum disorder (ASD) can be life- or limb-threatening, requiring urgent multidisciplinary intervention. We report a 31-year-old woman who developed self-induced toe gangrene following prolonged ligature application. Despite systemic stability, conservative management was pursued due to preserved vascular integrity, while psychiatric risk necessitated detention under the Mental Health Act. This case underscores the complex clinical, ethical, and legal challenges in

Transdiagnostic patient experiences of dialectical behavioural therapy: a systematic review and metasynthesis.

BACKGROUND: Dialectical Behaviour Therapy (DBT) combines cognitive-behavioural techniques and mindfulness practices to more skilfully regulate intense emotions and navigate interpersonal issues. While traditional DBT (skills group, individual therapy and crisis support) is well-studied in clinical populations, particularly for emotion regulation in conditions like borderline personality disorder or emotionally unstable personality disorder, recent research has explored alternative formats, such as skills-only groups. Although quantitative studies report positive outcomes (e.g., reduced self-in

A 'borderline' doctor: can you study medicine with a diagnosis of personality disorder?

Little has been written regarding the experience of training in medicine with a diagnosis of a personality disorder. The stigma of personality disorders, evidenced even within psychiatry, potentially marginalises affected students and resident doctors. This article provides a first-hand account of the lead author's (E.M.) lived experience of being a medical student with a diagnosis of emotionally unstable personality disorder (EUPD). Challenges that have been faced include a lack of understanding, limited literature about medical students and doctors with personality disorders, and derogatory

Blurring Fact and Fiction: A Complex Case of Recurrent Emergency Presentations, Polypharmacy, and Psychiatric Conflict.

A patient in their early 20s with a complex psychiatric and medical history presented with severe electrolyte imbalance, dehydration, and progressive functional decline. After initial stabilisation, the patient developed recurrent episodes of hypotension, hypoglycaemia, and seizure-like activity despite normal neuroimaging and an established diagnosis of non-epileptic attack disorder. Repeated nursing safety checks revealed concealed medications, including antihypertensives, anticoagulants, semaglutide, and central nervous system depressants, all temporally associated with clinical deteriorati

Mislabeling Chronic Depression: A Case Report of Persistent Depressive Disorder Misdiagnosed for 12 Years.

Persistent depressive disorder (PDD) is a chronic mood condition defined by a depressed mood lasting at least two years. However, it is frequently misdiagnosed as episodic depression, such as major depressive disorder (MDD) or recurrent depressive disorder (RDD), or attributed to personality-based pathology such as emotionally unstable personality disorder (EUPD). We present the case of a 48-year-old woman with a lifelong low mood and passive death wishes, who, for 12 years, received multiple shifting diagnoses and underwent extensive antidepressant and augmentation trials, as well as a full c

Challenges and opportunities in the diagnosis and treatment of early-onset psychosis: a case series from the youth affective disorders clinic in Stockholm, Sweden.

Early-onset psychosis is linked to adverse long-term outcomes, recurrent disease course, and prolonged periods of untreated illness; thus highlighting the urgency of improving early identification and intervention. This paper discusses three cases where initial emphasis on psychosocial treatments led to diagnostic and therapeutic delays: (1) a 15-year-old misdiagnosed with emotionally unstable personality disorder and autism, who improved on bipolar medication and antipsychotics; (2) another 15-year-old misdiagnosed with autism, who stabilized on lithium and antipsychotics, subsequently allowi

Frontal theta oscillations during emotion regulation in people with borderline personality disorder.

BACKGROUND: Borderline personality disorder (BPD) is a severe psychiatric disorder conceptualised as a disorder of emotion regulation. Emotion regulation has been linked to a frontolimbic network comprising the dorsolateral prefrontal cortex and the amygdala, which apparently synchronises its activity via oscillatory coupling in the theta frequency range. AIMS: To analyse whether there are distinct differences in theta oscillatory coupling in frontal brain regions between individuals with BPD and matched controls during emotion regulation by cognitive reappraisal. METHOD: Electroencephalogra

Characteristics and outcomes of people in suicidal crisis at two emergency departments: a service evaluation.

Emergency departments (EDs) provide critical opportunities for nurses to support suicide prevention. This article details a service evaluation that was undertaken to explore the characteristics and outcomes of people in suicidal crisis at two EDs in the East of England during June 2023. Data routinely collected by the ED mental health liaison team were combined with a retrospective case note review of the local NHS mental health trust's electronic patient records. Attendees had a mean age of 35 years and seven months, and were often diagnosed with depression or emotionally unstable personality

Assessment of Suicide Risk in Patients with Depressive Episodes Due to Affective Disorders and Borderline Personality Disorder: A Pilot Comparative Study.

This study assessed suicidal risk in patients suffering from non-psychotic depressive disorders within various clinical and nosological forms (F31-F34 mood disorders and F60.31-emotionally unstable personality disorder). Clinical and psychological features were presented, as well as predictors of suicidal risk in patients of these groups. We performed a comparative analysis of the anxiety and depression level, the level of mental pain, fear of death and the severity of anti-suicidal motives in patients with affective disorders and borderline personality disorder (BPD). Based on the results, 10

A two-year longitudinal evaluation of the impact of the COVID-19 pandemic on pre-existing mental health service attenders.

BACKGROUND: To examine if the COVID-19 pandemic was associated with a differential effect longitudinally in relation to its psychological and functional impact on patients with bipolar disorder and Emotionally Unstable Personality Disorder (EUPD). METHODS: Semi-structured interviews were conducted with 29 individuals attending the Galway-Roscommon Mental Health Services with an ICD-10 diagnosis of either bipolar disorder (n = 18) or EUPD (n = 11). The impact of the COVID-19 pandemic was assessed in relation to anxiety and mood symptoms, social and occupational functioning, and quality of life

Repeated intentional foreign body ingestion, what can be done?

Repeated intentional foreign body ingestion (RIFBI) in patients with Emotionally Unstable Personality Disorder (EUPD) is a common clinical presentation to the emergency department. The relationship between repeated foreign body ingestion and a co-existent personality disorder diagnosis is complex, making it challenging to manage. Our institution implemented a novel interdisciplinary model of care for RIFBI as a way of improving health outcomes in this cohort of patients. Our observations following the model of care are presented herein. We encourage other health networks to adopt this model of

Cariprazine in the management of emotionally unstable personality disorder in female patients: a case series.

BACKGROUND: Emotionally unstable personality disorder (EUPD) is debilitating psychiatric disorder, particularly common in female and forensic populations. However, appropriate pharmacological treatment to effectively manage symptoms of EUPD remains an unmet clinical need. Dopamine receptor partial agonists (DRPAs), such as aripiprazole, have a favourable tolerability profile and have demonstrated some benefits in targeting symptoms of emotional dysregulation, although, evidence regarding the effects of novel D2/D3 DRPA cariprazine in EUPD patients has been limited. OBJECTIVES: To evaluate the

Symptoms of the suicide crisis syndrome and therapist emotional responses: associations to self-harm and death by suicide within 18-months post-discharge among patients at an acute psychiatric department.

BACKGROUND AND AIMS: This study explored the associations between symptoms of the Suicide Crisis Syndrome (SCS) at admission and self-harm and death by suicide post-discharge. The association between clinicians' emotional responses toward inpatients at admission and post-discharge self-harm and suicide death was also explored. METHODS: Within the first 24-h of admission, patients completed a self-report measure of symptoms of SCS, and clinicians reported their emotional responses toward the patients. Follow-up data were obtained from the Norwegian Patient Registry and the Norwegian Cause of D

The experiences of autistic adults who were previously diagnosed with borderline or emotionally unstable personality disorder: A phenomenological study.

Autistic people face many barriers to receiving an autism diagnosis. Often, they may be misdiagnosed with borderline personality disorder instead. For our study, we interviewed 10 autistic adults who had previously been diagnosed with borderline personality disorder. This helped us to better understand their experiences. They explained how borderline personality disorder is quite stigmatised and may suggest that people are to blame for their differences in behaviour. They found the treatments they had to try for borderline personality disorder to be harmful. For example, these treatments promo

Differential diagnosis of autism, attachment disorders, complex post-traumatic stress disorder and emotionally unstable personality disorder: A Delphi study.

Individuals diagnosed with autism, attachment disorders, emotionally unstable personality disorder (EUPD) or complex post-traumatic stress disorder (CPTSD) can present with similar features. This renders differential and accurate diagnosis of these conditions difficult, leading to diagnostic overshadowing and misdiagnosis. The purpose of this study was to explore professionals' perspectives on the differential diagnosis of autism, attachment disorders and CPTSD in young people; and of autism, CPTSD and EUPD in adults. A co-produced three-round Delphi study gathered information through a series

Interoceptive Mechanisms and Emotional Processing.

Interoception, the sensing of internal bodily signals, is intricately linked with the experience of emotions. Various theoretical models of emotion incorporate aspects of interoception as a fundamental component alongside higher-order processes such as the appraisal of internal signals guided by external context. Interoception can be delineated into different dimensions, which include the nature of afferent signals, the accuracy with which they can be sensed, their neural processing, and the higher-order interpretation of these signals. This review methodically evaluates these interoceptive di

Amid magic and menace: psychiatrists' attitudes to psilocybin therapy.

OBJECTIVES: Understanding variations in knowledge and attitudes of psychiatrists to psilocybin therapy is important for the collective discourse about the potential impact on clinical practice and public health in Ireland. METHODS: A 28-item questionnaire was designed based on previous studies and distributed to psychiatrists in Ireland via online mailing lists and at in-person academic events. RESULTS: 151 psychiatrists completed the questionnaire (73.3% were under 40 years of age, 76.0% were trainees, and 49.0% were female). In the total sample, 81.5% agreed that psilocybin therapy shows p
