Connectivity-Guided Individualized iTBS of the Posterior Parietal Cortex in Enduring Anorexia Nervosa: Network-Level Target Engagement and Multimodal Response in a Single Case.

AIMS: Anorexia nervosa (AN) is a severe, often enduring disorder with scarce biological treatments, largely unable to address its core implicit features: habit-like food restriction and body-experience disturbances. We report a single-case application of resting-state connectivity-guided intermittent theta-burst stimulation (iTBS) to the posterior parietal cortex (PPC), a hub for body representation, peripersonal space, and affordance-based action. METHODS: A 31-year-old woman with 12 years of treatment-resistant restricting AN, enrolled in an ongoing sham-controlled feasibility trial, underw

Persistent Sleep Disturbance Following Pandemic Lockdowns: Longitudinal Findings From the UK COVID-19 Symptom Study Biobank.

Sleep disturbance and fatigue were highly prevalent during the COVID-19 pandemic, in both infected and non-infected individuals, though mostly assessed cross-sectionally. To assess these symptoms after lockdown cessation, we conducted a prospective longitudinal study in a large UK cohort. Validated questionnaires assessing sleep disturbances, fatigue, depression, and anxiety were administered to COVID Symptom Study Biobank participants at two time periods-Autumn 2021 and Spring 2022 equinoxes-corresponding to lockdown and lifting of restrictions, respectively. After adjusting for differential

Role of Serotonin in the Neurobiology of Schizophrenia and Association With Negative Symptoms.

IMPORTANCE: The involvement of the serotonin system in the pathophysiology of schizophrenia has been proposed for over 60 years, but there has been no prior study to test if there is altered serotonin release in vivo in schizophrenia (to the authors' knowledge). OBJECTIVE: To investigate serotonin release in vivo in schizophrenia and its association with negative symptoms. It was hypothesized a priori that frontal cortex serotonin release capacity would be lower in schizophrenia compared with healthy controls and that this would be associated with more severe baseline negative symptoms. DESI

The unique contributions of adverse childhood experiences to increases in post-traumatic stress symptoms and problematic substance use after trauma exposure.

This study investigated the complex relationships between adverse childhood experiences (ACE), post-traumatic stress symptoms (PTSS), and substance use disorder (SUD) following a large-scale collective trauma in Israel. We utilized a longitudinal design with a quasi-representative sample of 1,343 Jewish Israeli adults, measuring PTSS and SUD both before (April 2022) and after (December 2023) the October 7th attack. Using regression analysis and structural equation modeling, we tested the sensitization and self-medication hypotheses, and the direct effect of ACE on SUD beyond PTSS. Results demo

Choosing and Managing Non-Abstinent Recovery: Experiences of Adults in Untreated Recovery From Alcohol Use Disorder.

INTRODUCTION: We sought to characterise non-abstinent recovery from alcohol use disorder (AUD), particularly to understand why some people choose non-abstinence instead of abstinence and to illuminate how they may continue drinking in a way consistent with recovery. METHODS: We conducted semi-structured interviews with 65 adults with resolved AUD and no history of treatment who were enrolled in a national United States study of recovery definitions and strategies. Using inductive and deductive coding, we identified and elaborated themes. RESULTS: Non-abstinent participants reported infrequen

"I too deserve good": A qualitative exploration of corrective relational experiences in psychodynamic therapy for depression.

Corrective relational experiences (CREs) occur specifically within the therapeutic relationship and are perceived by patients as distinct and meaningful. Despite their potential importance in facilitating therapeutic change, CREs received limited theoretical, clinical, and empirical attention. The present study aimed to identify different types of CREs and to examine whether they suggest distinct mechanisms of change. Posttreatment semistructured qualitative interviews with 57 patients diagnosed with major depressive disorder following short-term supportive-expressive psychodynamic psychothera

Repetitive Mild Traumatic Brain Injury Causes Neuronal Damage in the APP/PS1 Mouse Model of Alzheimer's Disease Without an Enduring Impact on Amyloid Pathology, Sleep, or Epileptiform Activity.

Traumatic brain injury (TBI) is a known risk factor for Alzheimer's disease and related neurodegenerative diseases. Sleep disturbances and epileptiform abnormalities can appear after TBI and may contribute to the development of neuropathology. In this study, we characterized sleep, epileptiform activity, and neuropathology after repetitive mild traumatic brain injury (rmTBI) in a mouse model of Alzheimer's disease. We used the Closed Head Impact Model of Engineered Rotational Acceleration to deliver rmTBI or sham (control) treatment to 6-month-old APP/PS1 mice (N = 19). One month post-injury,

Open-label randomised controlled trial of aripiprazole/sertraline combination in comparison with quetiapine for the clinical and cost-effectiveness of treatment of bipolar depression (the ASCEnD study): study protocol.

INTRODUCTION: Bipolar disorder affects around 2% of the population and is linked with reduced life expectancy and socioeconomic burden. Depressive episodes are difficult to treat and typically more prevalent, enduring and burdensome than manic episodes. The use of antidepressants alone has limited effect and is associated with significant clinical risk through polarity switch. Current National Institute for Health and Care Excellence guidelines recommend quetiapine, olanzapine (with or without fluoxetine) and lamotrigine; however, these medications have limited efficacy, tolerability and accep

A comparative investigation of transcranial pulsed and direct current stimulation on motor learning and brain oscillation patterns in older adults with mild cognitive impairment: a randomized controlled trial.

BACKGROUND: Age-related declines in motor performance are associated with reduced cortical excitability and impaired neuroplasticity. Noninvasive transcranial electrical stimulation (tES) can improve motor learning, but the comparative efficacy of anodal transcranial direct current stimulation (a‑tDCS) and anodal transcranial pulsed current stimulation (a‑tPCS) in older adults with mild cognitive impairment (MCI) is unclear. This study aimed to compare the effects of a-tDCS and a-tPCS on motor learning and cortical activity in older adults with MCI. METHODS: Thirty‑nine older adults (age 60-7

Gained social identification and sense of shared experience after flood exposure: Psychosocial pathways to post-traumatic symptoms and growth.

This research provides the first quasi-experimental and longitudinal examination of the Social Identity Model of Traumatic Identity Change among individuals affected by the September 2024 flood in Poland. Advancing the state of the art, we analyzed the emergence of identification with flood-affected people and extended the model by incorporating the sense of shared experience (SSE) within this group. Surveys were completed 3 months (N = 630) and 7 months after the flood (N = 315). The results indicated that the severity of flood exposure was linked with higher post-traumatic stress (PTS) and p

EEG Microstate Alterations in Anhedonia Subtypes of Major Depressive Disorder and Their Modulation by Intermittent Theta-Burst Stimulation.

BACKGROUND: Anhedonia is a core symptom of major depressive disorder (MDD) and is associated with functional impairment, treatment resistance, and relapse. Emerging evidence suggests that anhedonia severity reflects distinct neural alterations; however, current neuromodulation protocols have not yet accounted for this clinical heterogeneity. We investigated whether intermittent theta-burst stimulation (iTBS) differentially modulates electroencephalography (EEG) microstate dynamics across anhedonia subtypes and aimed to identify electrophysiological signatures for treatment stratification. Evolution and factors associated with pediatric post-traumatic stress disorder 1 year after mild traumatic brain injury: a prospective, longitudinal study.

Mild traumatic brain injury (mTBI) can lead to lasting adverse outcomes, including post-traumatic stress disorder (PTSD) or post-traumatic stress symptoms (PTSS). This study examined whether PTSD and PTSS can occur even after mTBI and tracked the evolution of PTSD in the long term. A total of 85 youth post-mTBI (median age: 10.00, 25-75th percentile: 8.50-2.62; 24% girls) and their mothers participated in this study. Assessments included PTSS/PTSD, postconcussion symptoms, loss of consciousness status, child's anxiety, and maternal mental health, both shortly after mTBI (T1) and 1 year later (

Understanding participant experiences of cognitive behavioural therapy and health education for sleep disturbance and fatigue after acquired brain injury: a qualitative study.

PURPOSE: A recent randomised controlled trial demonstrated the efficacy of a cognitive behavioural therapy intervention for sleep disturbance and fatigue (CBT-SF) after acquired brain injury (ABI), leading to more rapid improvement than an active health education (HE) control intervention. This study qualitatively examined participants' experiences of receiving the CBT-SF and HE interventions. MATERIALS AND METHODS: 20 individuals with ABI shared their experiences with the interventions through semi-structured interviews, with transcripts analysed using reflexive thematic analysis. RESULTS:

Immediate and long-term effects of transcranial direct current stimulation on pain relief in older adults with Alzheimer's disease and related dementias: A pilot study.

We simultaneously assessed the impact of transcranial direct current stimulation (tDCS) on mitigating both self-reported pain and pain behaviors to more objectively explore its effects in older adults with Alzheimer's disease and related dementias. The analysis investigated 40 participants randomly (1:1) subjected to active and sham tDCS for 20 min on 5 consecutive days. Multi-group latent transition analysis enabled the simultaneous evaluation of both pain domains in a single model and analysis of their changes as a function of intervention exposure by modeling the transition probabilities of

Association of psychosocial factors and biological pathways identified from rare-variant analysis with longitudinal trajectories of treatment response in major depressive disorder.

BACKGROUND: Antidepressant efficacy is influenced by a multitude of factors, yet predicting treatment outcomes remains challenging. This difficulty is partly due to the commonly employed dichotomous classifications of treatment response that rely on a single primary endpoint. METHODS: The study enrolled 972 patients diagnosed with depression, including both first-episode and recurrent cases. All patients received treatment with a single class of antidepressant medication over an eight-week period. Treatment response trajectories were identified through cluster analysis using normalized score

Analyzing Trends in Suicidal Thoughts Among Patients With Psychosis in India: Exploratory Secondary Analysis of Smartphone Ecological Momentary Assessment Data.

BACKGROUND: India has the world's largest number of suicides, but there is little research on the trends in suicidal thoughts, especially for individuals with psychosis. More research is necessary to develop preventive interventions. Smartphone-based ecological momentary assessments (EMAs) can assess dynamic symptoms, but most EMA studies are conducted in higher income settings and have shorter (≤1 month) follow-up periods. OBJECTIVE: This study aimed to examine the duration of onset to offset of suicidal ideation (SI) in tertiary hospital outpatients with psychosis in India. METHODS: This s

Persistent differences in autistic symptom severity after one year in individuals at clinical high risk of psychosis and with first-episode psychosis.

BACKGROUND: Autistic symptoms have been shown to influence functional outcomes and prognosis not only in schizophrenia but also in individuals at clinical high-risk of psychosis (CHR-P) and those experiencing their first-episode psychosis (FEP). However, it remains unclear whether these symptoms reflect enduring traits that persist over time or whether they are states that can improve with treatment in CHR-P and FEP. This study aimed to investigate the one-year trajectory of autistic symptoms in individuals at CHR-P and with FEP. METHODS: A total of 59 participants who completed a one-year fo
