Pathological gambling treated with cognitive behavior therapy: a case report.

A novel approach to the cognitive behavioral treatment of pathological gambling is described. The approach is based on the hypothesis that biased information may form the basis of the rules governing gambling behavior. The treatment focused on modifying assumptions and beliefs underlying the gambling behavior of a 47-year-old male with a 26-year gambling history. Based on a 6-month follow-up, the intervention successfully reduced the frequency of the client's gambling behavior from 10 times per month in the 3 years prior to treatment to three total times in the 6-month follow-up period.

Gambling: the psychology of a non-drug compulsion.

In 1968 an editorial in the British Medical Journal rejected the view that compulsive or pathological gambling should be included under the rubric of illness. Exactly 20 years later an invited paper to the same journal noted that the problem was an addictive behaviour or dependency disorder and listed the American Psychiatric Association's diagnostic criteria. The following review critically evaluates some of the key conceptual and research developments during the intervening period.

Hot-headed or impulsive?

A sample of men in prison was interviewed and rated for the presence of six behaviours commonly thought to be impulsive: alcohol abuse (CAGE positive), sedative dependence, other drug abuse, pathological gambling, repeated aggression and self-harm. They were also asked to complete the EPQ and 15 questionnaires. Out of 61 men on a 'special protection, wing, 51 agreed to take part. We found that the score on the Impulsiveness scale of the 15 correlated with the number of disorders present in individuals, even when corrected for age. Pathological gambling and self-harm did not correlate with the

A review of two measures of pathological gambling in the United States.

This paper addresses a debate that has emerged in the field of pathological gambling research. This debate concerns measurement of the prevalence of pathological gambling in the general population. Two instruments have been used to measure prevalence in the United States, the South Oaks Gambling Screen and the Cumulative Clinical Signs Method. These instruments are described and several problems with the statistical properties of the Cumulative Clinical Signs Method are discussed. The authors conclude with a call for continued research in the area of prevalence studies of pathological gambling

Demographic, personality, cognitive and behavioral correlates of off-course betting involvement.

A large (n=381) exploratory survey of off-course bettors was conducted as a step towards rectifying deficits in current gambling literature. Information collected by interviews and questionnaires from off-course betting agency customers showed the level of betting involvement to be largely unrelated to demographic and cognitive variables and sensation seeking, but related to a variety of betting behaviors. A factor analysis produced three independent factors accounting for 30% of the variance. Loadings on these suggested an independence of items involved in the process of betting and those rel

Boredom proneness in pathological gambling.

To test the hypothesis that pathological gamblers seek stimulation as a means of reducing aversive under-aroused states of boredom and/or depression, the Beck Depression Inventory, Zuckerman's Sensation Seeking Scale and a Boredom Proneness Scale were administered to 48 diagnosed pathological gamblers and a control group of 40 family physician patients. Analyses of variance showed pathological gamblers obtained significantly higher boredom proneness and depression scores than those of controls. That the Boredom Proneness Scale failed to correlate with the Zuckerman Boredom Susceptibility subsc

Smart money.

"Rick" was an ambitious entrepreneur beginning the first of several businesses at age 16. He amassed a fortune by age 35. Between the ages of 42 and 50, his gambling rampage liquidated his assets, he lost his business and license to practice in his field, and he ended up in debt of over $1 million. Today at age 53 he is starting a new career in counseling and working with compulsive gamblers.

State lotteries and compulsive gambling.

The legalization of gambling, including state lotteries, has resulted in a dramatic increase of compulsive gambling. Not surprisingly, a new profile of compulsive gambler has emerged which differs significantly from the previous profile of white, middle aged, middle class male. This paper discusses public policy issues, lottery advertising, and legislative/lottery support given to address the problems of compulsive gambling.

Characteristics of pathological gamblers identified among patients on a psychiatric admissions service.

A total of 105 patients admitted to a psychiatric admissions service for adults were screened for pathological gambling using the South Oaks Gambling Screen, a valid, reliable instrument for identification of this disorder. Seven of the 105 patients were identified as pathological gamblers, and ten were children of problem gamblers, rates that are higher than those for the general population. Pathological gamblers identified in this study had a broad range of primary diagnoses, including schizoaffective disorder, paranoid schizophrenia, major depression, and organic personality disorder. A hig

Anxiety and/or depression in the pathogenesis of addictive gambling.

The Spielberger State-Trait Anxiety Inventory and the Beck Depression Inventory were administered to 75 pathological gamblers seeking behavioral treatment, to investigate the hypothesis that anxiety and depression were important determinants in the maintenance of gambling behavior. Further, subgroups of "pure" poker-machine and "pure" horse-race gamblers were isolated and compared to determine whether differences existed between groups of individuals who selected different forms of gambling. Although the desire to win money was given as the main reason for participation, need for money played

Prevalence estimates of pathological gambling in New Jersey and Maryland.

This survey of the gambling behavior of 1,750 adults in New Jersey and Maryland confirmed findings in New York State. The substantial differences between problem and pathological gamblers in the general population and those entering treatment programs have serious implications for prevention, outreach, and treatment efforts.

Gamblers' self-perceptions of the determinants of impaired control.

Impaired control occupies a focal position in the medical model perspective of pathological gambling and is central to the concept of addictive processes generally. Nonetheless there have been few studies on self-control in gambling behaviour. A questionnaire was developed to measure subjectively assessed self-control in poker machine players. This Self-Control Scale comprised a framework adopted from Cameron (1985) distinguishing between (i) the likelihood of starting a session and continuing a session, and (ii) internal and external determinants. The Scale and a descriptive questionnaire on

Personality factors and pathological gambling.

Pathological gambling is a disorder in which personality factors are thought to be important. Therefore, we compared 19 male pathological gamblers with 18 male normal controls for their scores on 3 personality questionnaires. Pathological gamblers were found to have significantly higher psychoticism and neuroticism scores on the Eysenck Personality Questionnaire than controls. Gamblers also had significantly higher total hostility scores on the Hostility and Direction of Hostility Questionnaire. The possible implications of these findings are discussed.

Pathological gambling. A psychobiological study.

We investigated psychobiological substrates of pathological gambling by measuring levels of norepinephrine, monoamine metabolites, and peptides in cerebrospinal fluid, plasma, and urine. Pathological gamblers had a significantly higher centrally produced fraction of cerebrospinal fluid levels of 3-methoxy-4-hydroxyphenylglycol as well as significantly greater urinary outputs of norepinephrine than controls. These results suggest that pathological gamblers may have a functional disturbance of the noradrenergic system. This system has been postulated to underlie sensation-seeking behaviors, aspe

Refining prevalence estimates of pathological gambling.

Pathological gambling was officially defined as a psychiatric illness by APA in 1980. The authors report on the results of a state-wide study in New York that indicated significant differences between problem gamblers in the general population and pathological gamblers entering publicly funded treatment programs. These results, based on telephone interviews using standardized assessment instruments with 1,000 persons, raise serious clinical and programmatic issues regarding the development and organization of appropriate services for pathological gamblers. They also point to the fact that larg

Pathological gambling: estimating prevalence and group characteristics.

A problem which lies in the path of implementing policies designed to address the adverse effects of gambling is the lack of data on the size and composition of the compulsive gambler group. There is a pressing need, therefore, to develop a data base on gambling participation and its consequences. The present study sought to contribute to this goal by identifying the rate of pathological gambling among adults residing in a nine-county area of southeastern Pennsylvania and southern New Jersey. The study was designed as a scientific community survey employing behavioral criteria as the basis for

Pathological gamblers are neither impulsive nor sensation-seekers.

Ten pathological gamblers, ten alcoholics, ten heroin addicts and twenty five non-patients were compared using Zuckerman's Sensation Seeking Scale (SSS) and Barratt's Impulsivity Scale (BIS). The pathological gamblers did not differ from the non-patient group on either measure. Drug addicts scored significantly higher and alcoholics significantly lower than the pathological gamblers and the non-patient group on the SSS, while drug addicts scored significantly higher than the other groups on the BIS. The difficulties in defining impulsivity are discussed. It is suggested that the classification
