Guanidine-quaternary ammonium polymer-derived carbon dots differentially modulate bacterial respiration and osteoblast mitochondria to resolve infection and foster osteointegration.

Implant-associated infections (IAIs) represent one of the most challenging complications following orthopedic implantation. Current antibacterial strategies struggle to achieve complete pathogen eradication while maintaining sustained osteogenic activity at the implant-bone interface. Leveraging the spatial divergence in energy metabolism between prokaryotic and eukaryotic cells, we designed and synthesized guanidine-quaternary ammonium polymer-derived carbon dots (GQCDs) with confirmed structure. Under non-infected conditions, GQCDs enhance mitochondrial metabolism to promote bone regeneratio

Theoretical and Experimental Investigations of 2,2-Dimethyl-1,3-dioxolane-4-methanamine-imprinted Polymers as AI-2-Inspired Surrog-ate-Recognition Materials for the Modulation of Quorum-Sensing-Regulated Phenotypes.

Quorum sensing (QS) is a bacterial communication mechanism that regulates collective behaviors such as virulence, motility, and biofilm formation. Consequently, materials capable of interacting with molecules associated with QS signaling pathways are of increasing interest as potential tools for studying and modulating bacterial communication processes. In this study, molecularly imprinted polymers (MIPs) were designed and synthesized using (R/S)-2,2-dimethyl-1,3-dioxolane-4-methanamine, an autoinducer-2 (AI-2)-inspired surrogate template to investigate molecular recognition of structures rele

Reducing Antibiotic Drug Use Effects on Clostridioides difficile Colonization in Nursing Home Residents1.

Antibiotic drug-mediated changes in the human gut microbiota affect Clostridioides difficile colonization. We created a compartmental model of C. difficile colonization in nursing home residents to improve understanding of how the gut microbiota mediates the transmission process. When analyzing C. difficile colonization as a function of the daily percentage of the nursing home resident population on antibiotic drugs, the model displayed increases in C. difficile colonization as antibiotic drug use increased. Reducing the daily percentage of the nursing home resident population receiving antibi

Molecular epidemiology and resistance mechanisms of carbapenem-resistant Gram-negative bacilli in a referral hospital in Lima, Peru.

BACKGROUND: Carbapenem-resistant Gram-negative bacilli (CR-GNB) represent a significant public health threat, as they are associated with high mortality in healthcare settings. Carbapenemase production is the predominant resistance mechanism, and horizontal gene transfer drives the rapid dissemination of antimicrobial resistance (AMR). OBJECTIVES: This study aimed to characterize the phenotypic and genotypic AMR profiles of CR-GNB isolated from patients at Arzobispo Loayza National Hospital (Lima, Peru) between October 2021 and January 2022. METHODS: We conducted a cross-sectional study and

Creatinine Clearance, Ceftazidime-Avibactam Exposure, and Clinical Outcomes in a Real-World TDM Cohort: A Retrospective Observational Study.

PURPOSE: Ceftazidime-avibactam (CAZ-AVI) is a key option for carbapenem-resistant Enterobacterales (CRE) infections, but whether creatinine clearance (CrCL) independently predicts outcomes or reflects disease severity is unclear. We investigated CrCL, drug exposure, and outcomes in patients with severe Gram-negative infections receiving CAZ-AVI with therapeutic drug monitoring (TDM). PATIENTS AND METHODS: This retrospective study included 224 patients receiving CAZ-AVI TDM at a tertiary hospital in China (May 2024-May 2025). Three cohorts were defined: trough-concentration (n=224) for factors

Updated UK good practice recommendations for outpatient parenteral antimicrobial therapy (OPAT): ensuring quality of care and antimicrobial stewardship in an evolving healthcare landscape.

Outpatient parenteral antimicrobial therapy (OPAT) offers safe, effective and patient-centred care for patients with infections requiring IV or complex oral antimicrobial therapy. The UK OPAT Good Practice Recommendations (GPRs) have recently been updated through a systematic literature review, expert consensus and consultation. In this article we outline the key changes in the updated GPRs. There are five new recommendations on (i) self-administration, (ii) OPAT in hard-to-reach groups, (iii) patient education, (iv) developing monitoring protocols for patients on complex oral antimicrobials a

Exploring the antibiofilm potential of marine Streptomyces sp. VITNABS17 against biofilm forming Klebsiella pneumoniae: an in vitro and in silico approach.

Biofilm-associated bacterial infections pose a significant challenge in clinical settings due to their persistence and reduced susceptibility to current antimicrobials, underscoring the need for novel antibiofilm agents. This study aimed to evaluate the antibacterial and antibiofilm activities of cell-free extracts from marine Streptomyces sp. using integrated in vitro and in silico approaches. Initial antibacterial screening by agar well diffusion assay identified VITNABS17 as the most active isolate, exhibiting a broad antibacterial spectrum against the tested bacterial pathogens. Evaluation

Post-Competitive Interface Reprogramming for Amplified Multiplex Bacterial Fingerprinting in Biofluids.

Low-burden bacterial infections demand rapid and reliable pathogen fingerprinting, yet cross-reactive sensor arrays remain limited by weak and overlapping optical signatures. Here we introduce postcompetitive interface reprogramming (PCIR), a sensor-array strategy that extracts latent bacterial-surface information by reprogramming preformed reporter-bacterium interfaces rather than merely expanding readout dimensionality. Boronic-acid-modified perylene diimide (PDI) reporters first bind bacterial surfaces, generating weak aggregation-caused quenching signals. Subsequent addition of D-chitosan

Expanding the Utility of Colistin Adjuvants To Encompass Next-Generation Polymyxins against Polymyxin-Resistant Acinetobacter baumannii and Klebsiella pneumoniae.

Current treatment options for multidrug-resistant (MDR) Gram-negative infections are effectively limited to last-resort antibiotics, such as the polymyxins. Clinical use of polymyxins remains limited due to significant nephrotoxic side effects, and is further limited as a result of increasing polymyxin resistance. The novel polymyxin antibiotic, SPR206, developed in 2019, retains potency against polymyxin-susceptible Gram-negative bacteria while showing no adverse side effects in Phase 1 clinical trials. However, resistance to SPR206 is anticipated through the same mechanisms that drive resist

Molecular Insights into the Bacterial Membrane Interactions of Proline-Stabilized Graphene: Effects of Proline Oligomerization.

Multidrug-resistant bacterial infections pose a major global health challenge. Graphene nanosheets exhibit promising antibacterial activity through membrane disruption, but their practical application is limited by poor aqueous stability and biocompatibility. A recent study [Nature 2025, 645 (8082), 915-921] has suggested that amino acids such as proline can enhance nanomaterial stability and biocompatibility through noncovalent interactions; however, their influence on graphene-membrane interactions remains unclear. Here, all-atom molecular dynamics simulations were performed to investigate t

Antibiotic de-escalation in severe intra-abdominal infections: a single-center retrospective cohort study on safety and prognostic factors.

BACKGROUND: Antibiotic de-escalation is a key antimicrobial stewardship strategy aimed at reducing the ecological and adverse effects of broad-spectrum therapy without compromising efficacy. However, its real-world safety and impact on clinical outcomes in patients with severe intra-abdominal infections (IAIs) remain underexplored. METHODS: A single-center, retrospective cohort study was conducted from June 2022 to June 2025. Adults with severe IAIs who received initial broad-spectrum antibiotics and achieved source control within 48 h were included. Patients were stratified into de-escalatio

BACKGROUND: Antimicrobial resistance (AMR) is a major public health challenge, especially in the pediatric population in low- and middle-income countries, particularly India, where the burden of pediatric infections is high. However, nationally representative, pediatric-specific estimates of AMR with geographic stratification remain lacking. We conducted a systematic review and meta-analysis to define the magnitude and regional variation of AMR among Indian children. METHODS: We conducted a systematic review and meta-analysis in accordance with PRISMA 2020 and the Joanna Briggs Institute guid
